Changing indications for lamellar keratoplasty in Shandong, 1993-2008

Background With the advancement of microsurgical techniques, lamellar keratoplasty （LK） has been more valued and performed to treat corneal blindness. This study aimed to evaluate the indications and changing trends for LK during the past 16 years in Shandong Eye Institute, an eye center in China. Methods A total of 1529 eyes, predominantly from male （70.7%） patients undergoing LK between January 1993 and December 2008, were enrolled in this study. Data were collected by reviewing patient medical records for demographic characteristics, clinical diagnosis, laboratory findings and risk factors. Surgical times, initial diagnoses and causes of regrafting were recorded. All LKs were sequentially divided into period 1 （from 1993 to 2000） and period 2 （from 2001 to 2008）. Results Rural-dwelling patients in this study numbered 1089 （71.2%）; in all cases of infectious keratitis, rural patients accounted for 90.5%. The leading indications for LK were infectious keratitis （31.0%）, corneal trauma （21.1%）, keratoconus （18.7%）, corneal dystrophy and degeneration （7.3%）, regrafting （7.1%）, immunologic disorders （6.7%）, congenital abnormalities and corneal tumor （4.1%） and corneal scarring （3.1%）. The most common subcategory of infectious keratitis was fungal keratitis （67.5%）. Fusarium solani was the most morbigenous fungi. Among the patients, 73.1% of thermal burns were caused by hot molten metal, and 47.8% of alkali burns by lime. Dermoid was the most common indication for congenital abnormalities. The two main initial diagnoses from 109 regrafting cases were corneal trauma （45.9%） and Mooren＇s ulcer （27.5%）. The most common cause for regrafting was immune rejection （35.8%）. Infectious keratitis was the most common indication for LK from 16.0% in period 1 to 41.1% in period 2, followed by keratoconus （16.8%） and corneal trauma （14.5%）. Genetic characterization of a Chinese family with familial idiopathic pulmonary fibrosis

Background Idiopathic pulmonary fibrosis （IPF） is a chronic inflammatory interstitial lung disease with an unknown cause. Recent studies have shown that genetic factors play an important role in the pathogenesis of IPF. Methods To explore the genetic background of patients with IPF, a candidate gene approach was employed to screen for mutations in seven genes among members with familial IPF in mainland of China. Results Within six of the candidate genes, a total of 31 point mutations were identified. Among the missense mutations, the SFTPA1 exon 6 CAG〉AAG （GIn238Lys） and SFTPB exon 2 CAC〉CCC （His2Pro） mutations caused changes in the physical and chemical properties of amino acids. Each sequence alteration was identified in sporadic IPF patients, control specimens （pneumonia patients and healthy persons）. Genotype frequencies and allele frequencies of codon 238 in exon 6 of SFTPA1 were noted significantly higher in patients with IPF than those in other two control subjects. The computational protein structure prediction by protein homology modeling confirmed differences in three-dimensional structure between mutant SFTPA1 and original SFTPAI. Conclusions Although the functions of the mutant candidate genes vary, these genes may ultimately result in damage to alveolar epithelial cells, initiating the progress of pulmonary fibrosis. In particular, while pathophysiological mechanisms need to be illustrated, the GIn238Lys missense variant of exon 6 in the SFTPA1 may have potential susceptibility in the development of IPF, which was shown in patients with sporadic IPF with a statistically higher frequency.

Quinolones have been used to treat various infectious diseases. The addition of fluorine atoms has allowedfor extensive modifications of structures, which has enhanced the spectrum of activities and oral bioavailability of quinolones and resulted in the creation of fluoroquinolones） Although the adverse effects associated with fluoroquinolone antibiotics have decreased considerably and these drugs are generally well tolerated, adverse effects do sometimes occur.

ACTIVITY STANDARD OF DONOR ENDOTHEliAL CELLS IN PENETRATING KERATOPLASTY

Penetrating keratoplasty was done in 73 patients with eyeballs enucleated from donors aged l-49 years 2-12 h.ours after death and kept in the human cord serum or in moist condition. The operation started with a grafc of diameter being 68 mm and continuous suture with 10-0 nylon thread under operating micro- scope. B.fore penetrating keratoplasty, Olympus pho- tomicroscope was used to record the number of donor en.dothelial cells but after operation contact specular photomicroscope EB-160 Model III was used Lo observe and photograph donor grafting endothelial cells for evatuating their activity density and activity ratc. After graft was taken, the remained corneal ring schcduled for observation was stained by modi- ficcl stocker’s method with trypan blue. Follow-up for 0.5-6.5 years in our 73 patients showed that using the cndothelial cell activity density as a standard for evaluating donor grafts is of clinical value in eye bank.

Significance of low serum vitamin D for infection risk, disease severity and mortality in critically ill patients

Background Hospitalized patients often have higher rate of vitamin D deficiency than healthy people. Vitamin D levels below normal are associated with hospital stay, increased incidence of adverse prognosis and increased mortality of a number of diseases. Whether there is a relationship between vitamin D levels and infection or sepsis in the critically ill is still unclear. This study will explore the relationship between vitamin D levels and risk of infection, assessment for disease severity, and predictor of mortality. Methods To evaluate the value of vitamin D in intensive care unit （ICU） cases to sepsis, severity and prognosis assessment, high performance liquid chromatography and tandem mass spectrometry were used to measure the concentrations of vitamin D in sera of critically ill patients. The serum samples were drawn within the first 24 hours of ICU admission. Results The study included 206 people, 50 healthy controls, 51 ICU control patients and 105 ICU diagnosed with sepsis. Critically ill ICU patients （ICU sepsis and ICU control group） had lower vitamin D concentration than normal people, but septic patients showed no significant reduction of vitamin D concentration when compared with critically ill patients with no positive etiological evidence. For assessment of disease severity, there were very low negative correlations between APACHE II, SAPS II and SOFA scores and vitamin D level. Additionally, patients of different 25-（OH）D levels showed no difference whether in terms of 28-day survival （χ2=1.78, P=0.776） or 90-day survival （χ2=4.12, P=0.369）. Multivariate Logistic regression demonstrated that APECHE II and SAPS II scores were independent risk factors to deaths caused by sepsis. Conclusion Clinically, serum concentration of vitamin D is not an indicator for diagnosis and assessment in critically ill patients （ClinicalTrial.gov identifier NCT01636232）.

Hypopyon in patients with fungal keratitis

Background Hypopyon is common in eyes with fungal keratitis.The evaluation of the clinical features,culture results and the risk factors for hypopyon and of the possible correlation between hypopyon and the treatment outcome could be helpful for making treatment decisions.Methods The medical records of 1066 inpatients （1069 eyes） with fungal keratitis seen at the Shandong Eye Institute from January 2000 to December 2009 were reviewed retrospectively for demographic features,risk factors,clinical characteristics,laboratory findings and treatment outcomes.The incidence of hypopyon,the fungal culture positivity for hypopyon,risk factors for hypopyon and the effect of hypopyon on the treatment and prognosis were determined.Results We identified 1069 eyes with fungal keratitis.Of the 850 fungal culture-positive eyes,the Fusarium species was the most frequent （73.6%）,followed by Altemaria （10.0%） and Aspergillus （9.0%）.Upon admission,562 （52.6%）eyes with hypopyon were identified.The hypopyon of 66 eyes was evaluated via fungal culturing,and 31 eyes （47.0%）were positive.A total of 194 eyes had ocular hypertension,and 172 （88.7%） of these eyes had hypopyon （P 〈0.001）.Risk factors for incident hypopyon included long duration of symptoms （P 〈0.001）,large lesion size （P 〈0.001） and infection caused by the Fusarium and Aspergillus species （P 〈0.001）.The positivity of fungal culture for hypopyon was associated with duration of symptoms and lesion size.Surgical intervention was more common in cases with hypopyon （P 〈0.001）.Hypopyon was a risk factor for the recurrence of fungal keratitis after corneal transplantation （P=0.002）.Conclusions Hypopyon is common in patients with severe fungal keratitis and can cause ocular hypertension.About half of the hypopyon cases were positive based on fungal culture.Long duration of symptoms,large lesion size and infection with the Fusarium and Aspergillus species were risk factors for hypopyon.The presence of hypopyon increasesthe likelihood of surgical intervention.

Pulmonary alveolar proteinosis （PAP） IS an uncommon disease first reported by Rosen et al^1 in 1958, and characterized by the accumulation of surtactant proteins and phospholipids within the alveolar spaces. Acquired PAP is divided into two forms based on clinical features： idiopathic PAP and secondary PAE Secondary PAP is reported to be associated with haematological malignancies, Pneumocystis carinii pneumonia and inhalation of silica or titanium, and the most frequent underlying disease of secondary PAP is haematological malignancy. The exact incidence of PAP in haematological malignancies is still obscure, since there have been only sporadic reports of secondary PAP.^ 2, 3

The severe acute respiratory syndrome （SARS） is an emerging and highly contagious infection caused by a newly discovered strain of coronavirus. Since the clinical case definition of SARS is similar to other severe atypical pneumonias, specific laboratory tests that can accurately diagnose SARS-associated coronavirus （SARS-CoV） infection are important. However, published data are insufficient to investigate whether clinically diagnosed SARS patients may include some non-SARS pneumonia. Therefore, we aimed to determine clinical and laboratory features to differentiate SARS patients from non-SARS pneumonias that could reduce misdiagnosis of SARS. A retrospective analysis of clinical and laboratory characteristics after the initial onset of SARS, as well as its convalescent-phase, was examined from clinically diagnosed 197 SARS patients.

目的研究不同地域支气管扩张症(简称支扩)患者下呼吸道背景微生物差异。方法收集2021年9月-2022年9月解放军总医院海南医院及解放军总医院第八医学中心收治的稳定期支扩患者75例的肺泡灌洗液进行宏基因组测序,并对结果进行对比分析。结果南、北支扩患者下呼吸道丰度最高的菌属是普氏菌属、奈瑟菌属和罗斯氏菌属,菌种是产黑色普雷沃菌、中间普雷沃菌和龋齿罗氏菌;不同地域支扩患者下呼吸道背景微生物存在差异,北方组丰度最高的菌属是普氏菌属、奈瑟菌属和罗斯氏菌属,菌种是产黑色普雷沃菌、黏滑罗氏菌和龋齿罗氏菌;南方组丰度最高的菌属是普氏菌属、奈瑟菌属和罗斯氏菌属,菌种是中间普雷沃菌、产黑色普雷沃菌和龋齿罗氏菌;以铜绿假单胞菌和流感嗜血杆菌为致病菌的背景菌群其α多样性无统计学差异。结论我国南北方稳定期支扩患者下呼吸道背景微生物存在差异,北方组产黑色普雷沃菌、黏滑罗氏菌、龋齿罗氏菌多见,南方组中间普雷沃菌、产黑色普雷沃菌、龋齿罗氏菌多见,该研究对支扩的治疗提供新思路。

Classification and spectrum optimization method of grease based on infrared spectrum

The infrared(IR)absorption spectral data of 63 kinds of lubricating greases containing six different types of thickeners were obtained using the IR spectroscopy.The Kohonen neural network algorithm was used to identify the type of the lubricating grease.The results show that this machine learning method can effectively eliminate the interference fringes in the IR spectrum,and complete the feature selection and dimensionality reduction of the high-dimensional spectral data.The 63 kinds of greases exhibit spatial clustering under certain IR spectrum recognition spectral bands,which are linked to characteristic peaks of lubricating greases and improve the recognition accuracy of these greases.The model achieved recognition accuracy of 100.00%,96.08%,94.87%,100.00%,and 87.50%for polyurea grease,calcium sulfonate composite grease,aluminum(Al)-based grease,bentonite grease,and lithium-based grease,respectively.Based on the different IR absorption spectrum bands produced by each kind of lubricating grease,the three-dimensional spatial distribution map of the lubricating grease drawn also verifies the accuracy of classification while recognizing the accuracy.This paper demonstrates fast recognition speed and high accuracy,proving that the Kohonen neural network algorithm has an efficient recognition ability for identifying the types of the lubricating grease.
