Hip hemi-arthroplasty for neck of femur fracture:What is the current evidence?

This editorial reviews and summarises the current evidence(meta-analyses and Cochrane reviews) relating to the use of hip hemi-arthroplasty for neck of femur fractures. Regarding the optimal surgical approach,two recent meta-analyses have found that posterior approaches are associated with: higher rates of dislocation compared to lateral and anterior approaches; and higher rates of re-operation compared to lateral approaches. Posterior approaches should therefore be avoided when performing hip hemi-arthroplasty procedures. Assessing the optimal prosthesis head component,three recent meta-analyses and one Cochrane review have found that while unipolar hemiarthroplasty can be associated with increased rates of acetabular erosion at short-term follow-up(up to 1 year),there is no significant difference between the unipolar hemi-arthroplasty and bipolar hemi-arthroplasty for surgical outcome,complication profile,functional outcome and acetabular erosion rates at longer-term follow-up(2 to 4 years). With bipolar hemi-arthroplasty being the more expensive prosthesis,unipolar hemi-arthroplasty is the recommended option. With regards to the optimal femoral stem insertion technique,three recent metaanalyses and one Cochrane Review have found that,while cemented hip hemi-arthroplasties are associated with a longer operative time compared to uncemented Hip Hemi-arthroplasties,cemented prostheses have lower rates of implant-related complications(particularly peri-prosthetic femoral fracture) and improved postoperative outcome regarding residual thigh pain and mobility. With no significant difference found between the two techniques for medical complications and mortality,cemented hip hemi-arthroplasty would appear to be the superior technique. On the topic of wound closure,one recent meta-analysis has found that,while staples can result in a quicker closure time,there is no significant difference in post-operative infections rates or wound healing outcomes when comparing staples

Sagittal plane analysis of the spine and pelvis in adult idiopathic scoliosis

Background There has been an increasing recognition of the importance of sagittal spinopelvic alignment in patients with scoliosis as it relates to clinical outcomes. However, the changes seen in sagittal spinopelvic alignment in adult idiopathic scoliosis patients is poorly defined. This study was conducted to evaluate the sagittal alignment of pelvis and spine in adult idiopathic scoliosis *** The sagittal parameters of the spine and pelvis were analyzed in lateral standing radiographs of 124 patients （mean age 47.4 years） with adult idiopathic scoliosis, including thoracic kyphosis （TK）, thoracolumbar junction kyphosis （TLJ）, lumbar lordosis （LL）, pelvic incidence （Pi）, sacrum slope （SS）, pelvic tilt （PT） and C7 plumb line （C7PL）. The patients were divided into three groups according to the age： 20-40 years, 41-64 years, and ≥65 years. The parameters were compared with those in normal adults and adolescent idiopathic scoliosis （AiS） patients. The relationship between all parameters as well as age and sagittal parameters were *** The Pi in patients with adult idiopathic scoliosis was 58.1°±13.0°, which was significantly higher than that in normal adults. The PT （19.9°±10.6°） was also higher than that in both normal adults and AiS patients, while the SS （38.1°±12.0°） was similar or smaller. As age increased, C7PL, PT and TJL increased while LL decreased. There was no relationship between age and both Pi and TK. PT had the strongest statistical association with the *** Pi is higher in adult idiopathic scoliosis than normal subjects. The PT is the most relevant pelvic parameter to the global sagittal alignment of the spine. Age significantly influences sagittal parameters of the spine and pelvis except the Pi and TK.

Stress fractures in sport are becoming increasing more common,comprising up to 10%of all of sporting *** 90%of such injuries are located in the lower *** articles aims to define the optimal management of lower limb stress fractures in the athlete,with a view to maximise return rates and minimise return times to *** planning of this condition is specific to the location of the ***,there remains a clear division of stress fractures by"high"and"low"risk."Low risk"stress fractures are those with a low probability of fracture propagation,delayed union,or non-union,and so can be managed reliably with rest and exercise *** include stress fractures of the PosteroMedial Tibial Diaphysis,Metatarsal Shafts,Distal Fibula,Medial Femoral Neck,Femoral Shaft and Calcaneus."High risk"stress fractures,in contrast,have increased rates of fracture propagation,displacement,delayed and non-union,and so require immediate cessation of activity,with orthopaedic referral,to assess the need for surgical *** include stress fractures of the Anterior Tibial Diaphysis,Fifth Metatarsal Base,Medial Malleolus,Lateral Femoral Neck,Tarsal Navicular and great Toe *** order to establish the optimal methods for managing these injuries,we present and review the current evidence which guides the treatment of stress fractures in *** this,we note an increased role for surgical management of certain high risk stress fractures to improve return times and rates to *** this,key recommendations are provided for the management of the common stress fracture types seen in the *** case reports are also presented to illustrate the application of sportfocussed lower limb stress fracture treatment in the clinical setting.

There is a marked inflammatory and hypermetabolic response following a burn *** inter-linked responses are more pronounced than for other forms of trauma and can persist for≥3 years post-injury in burned *** a burn,patients have an increased risk of diseases of ageing including cancer,diabetes and cardiovascular disease,highlighting the need for effective long-term strategies to ameliorate the stress response *** therapeutic strategies for post-burn recovery include removal of damaged tissue with surgical excision and wound repair,nutritional supplementation and rehabilitative *** strategies aim to minimize the hypermetabolic and inflammatory responses,as well as reducing the loss of lean body *** review briefly summarises the inflammatory and hypermetabolic responses and provides an update on the current therapeutic strategies for burned *** review examines the persistent nutritional challenge of ensuring sufficient energy intake of each macronutrient to fuel the hypermetabolic and counteract the catabolic response of burn injury,whilst reducing periods of hyperglycaemia and *** require individualized treatment options tailored to unique systemic responses following a burn,facilitated by a precision medicine approach to improve clinical and physiological outcomes in burned ***,this review discusses the utility of metabolic flexibility assessment to aid clinical decision making and prescription relating to nutritional supplementation and rehabilitative exercise in the burned patient.

Felsenstein＇s ＂one-allele model＂ of speciation： The role of philopatry in the initial stages of host plant mediated reproductive isolation in Enchenopa binotata

The study of speciation genetics is primarily concerned with identifying the genetic traits that allow divergent selec- tion to overcome the homogenizing effects of gene flow. Felsenstein reviewed this race between gene flow and selection, con- cluding that speciation with gene flow was unlikely under a ＂two-allele model＂（where two traits were necessary for reproductive isolation） but that divergence could occur quite easily under a ＂one-allele model.＂ Despite this finding, much of the sympatric speciation research involving phytophagous insects has relied on a two-trait model, where insects evolve both preferences for and increased performance on novel host plants. Philopatry （a tendency to remain where one was born） is known to occur in phyto- phagous insects and is a single trait isolation mechanism. However, it is traditionally invoked as simply augmenting reproductive isolation. Species in the Enchenopa binotata complex are believed to have speciated in sympatry. They exhibit host plant prefe- rences, host specific performance advantages and strong philopatry. We experimentally shifted E. binotata to evolutionarily novel host plants. Previous research has demonstrated that the experimental population of insects possesses genetic variation in prefe- rence and performance to the novel host. The degree of philopatry at mating and egg-laying was assayed for the first four years under full choice conditions. Host plant preference and performance was assayed after eight years. Philopatry was an immediate and strong isolating mechanism, while preference for and performance on the novel host lagged. We therefore suggest that philo- patry may be a more important mechanism in the early stages of a host shift than previously believed [Current Zoology 59 （5）： 658-666, 2013].

A Hybrid Finite Element-Laplace Transform Method for the Analysis of Transient Electromagnetic Scattering by an Over-Filled Cavity in the ground Plane

A hybrid finite element-Laplace transform method is implemented to analyze the time domain electromagnetic scattering induced by a 2-D overfilled cavity embedded in the infinite ground *** algorithm divides the whole scattering domain into two,interior and exterior,*** the interior sub-domain which covers the cavity,the problem is solved via the finite element *** problem is solved analytically in the exterior sub-domain which slightly overlaps the interior subdomain and extends to the rest of the upper half *** use of the Laplace transform leads to an analytical link condition between the overlapping *** analytical link guides the selection of the overlapping zone and eliminates the need to use the conventional Schwartz *** dramatically improves the efficiency for solving transient scattering *** solutions are tested favorably against analytical ones for a canonical *** perfect link over the artificial boundary between the finite element approximation in the interior and analytical solution in the exterior further indicates the reliability of the *** error analysis is also performed.

Aggressive behavioural interactions between swans(Cygnus spp.)and other waterbirds during winter:a webcam-based study

Background:Our understanding of any impacts of swans on other waterbirds(including other swans),and potential effects on waterbird community structure,remain limited by a paucity of fundamental behavioural and ecological data,including which species swans interact aggressively with and how frequently such interactions ***:Behavioural observations of aggression by swans and other waterbirds in winters 2018/2019 and 2019/2020,were carried out via live-streaming webcams at two wintering sites in the *** occurrence sampling was used to identify all aggressive interactions between conspecific or heterospecifics individuals,whilst focal observations were used to record the total time spent by swans on aggressive interactions with other *** tests were then used to assess whether the proportion of intraspecific aggressive interactions of each species differed from 0.5(which would indicate equal numbers of intraspecific and interspecific interactions).Zero-inflated generalized linear mixed effects models(ZigLMMs)were used to assess between-individual variation in the total time spent by swans on aggressive interactions with other ***:All three swan species were most frequently aggressive towards,and received most aggression from,their *** 10-min focal observations showed that Whooper(Cygnus cygnus)and Bewick’s Swans(*** bewickii)spent 13.8±4.7 s(means±95%Ci)and 1.4±0.3 s,respectively,on aggression with other *** durations were equivalent to 2.3%and 0.2%of the Whooper and Bewick’s Swan time-activity budgets,*** selection indicated that the time spent in aggressive interactions with other swans was best-explained by the number of other swans present for Whooper Swans,and an interactive effect of time of day and winter of observation for Bewick’s ***,the relationship between swan numbers and Whooper Swan aggression times was not strong(R2=19.3%).Conclusions:Whilst swans do exhibit some agg
