AIM: To investigate the incidence and distribution of second primary cancers(SPCs) in early gastric cancer(EGC) patients who underwent endoscopic resection(ER), compared to advanced gastric cancer(AGC) patients who underwent ***: The medical records of 1021 gastric cancer(GC) patients were retrospectively reviewed from January 2006 to December 2010. The characteristics and incidence of SPCs were investigated in those with EGC that underwent curative ER(the EGC group) and those with AGC who underwent curative surgical resection(the AGC group).RESULTS: We ultimately enrolled 184 patients in the EGC group and 229 patients in the AGC group. A total of 38 of the 413(9.2%) GC patients had SPCs; the rate was identical in both groups. Of these 38 patients, 18 had synchronous and 20 had metachronous cancers. The most common SPC was lung cancer(18.4%), followed by colorectal cancer(13.2%) and esophageal cancer(13.2%). No significant risk factors were identified for the development of ***: Endoscopists should provide close surveillance and establish follow-up programs to ensure SPC detection in GC patients undergoing curativeresection regardless of their clinical characteristics.

BACKGROUND The 5-fluorouracil-based chemotherapy combined with oxaliplatin or irinotecan is usually used in colorectal cancer(CRC).The addition of a targeted agent(TA) to this combination chemotherapy is currently the standard treatment for metastatic ***,the efficacy and safety of combination chemotherapy for metastatic CRC in patients aged above 80 years has yet to be *** To assess the clinical outcomes and feasibility of combination chemotherapy using a TA in extremely elderly patients with *** Eligibility criteria were:(1) Age above 80 years;(2) Metastatic colorectal cancer;(3) Palliative chemotherapy na?ve;(4) Eastern Cooperative Oncology Group performance status 0-1;and(5) Adequate organ *** received at least one dose of combination chemotherapy with or without *** was evaluated every 8 *** Of 30 patients,the median age of 15 patients treated with TA was 83.0 years and that of those without TA was 81.3 *** median progression-free survival(PFS) and overall survival(OS) in patients treated with TA were 7.4 mo and 15.4 mo,respectively,compared with 4.4 mo and 15.6 mo,respectively,in patients treated without *** was no significant difference in PFS(P:0.193) and OS(P:0.748) between patients treated with and without *** grade 3/4 hematologic toxicities were anemia(16.7%) and neutropenia(10.0%).After disease progression,the median OS of patients who were treated with and without salvage chemotherapy were 23.5 mo and 7.0 mo,respectively,suggesting significant difference in OS(P = 0.001).CONCLUSION Combination chemotherapy with TA for metastatic CRC may be considered feasible in patients aged above 80 years,when with careful *** chemotherapy can help improve OS in some selected of these elderly patients.

Preparation of human decellularized peripheral nerve allograft using amphoteric detergent and nuclease

Animal studies have shown that amphoteric detergent and nuclease(DNase I and ribonuclease A) is the most reliable decellularization method of the peripheral nerve. However, the optimal combination of chemical reagents for decellularization of human nerve allograft needs further investigation. To find the optimal protocol to remove the immunogenic cellular components of the nerve tissue and preserve the basal lamina and extracellular matrix and whether the optimal protocol can be applied to larger-diameter human peripheral nerves, in this study, we decellularized the median and sural nerves from the cadavers with two different methods: nonionic and anionic detergents(Triton X-100 and sodium deoxycholate) and amphoteric detergent and nuclease(3-[(3-cholamidopropyl)dimethylammonio]-1-propanesulfonate(CHAPS), deoxyribonuclease I, and ribonuclease A). All cellular components were successfully removed from the median and sural nerves by amphoteric detergent and nuclease. Not all cellular components were removed from the median nerve by nonionic and anionic detergent. Both median and sural nerves treated with amphoteric detergent and nuclease maintained a completely intact extracellular matrix. Treatment with nonionic and anionic detergent decreased collagen content in both median and sural nerves, while the amphoteric detergent and nuclease treatment did not reduce collagen content. In addition, a contact cytotoxicity assay revealed that the nerves decellularized by amphoteric detergent and nuclease was biocompatible. Strength failure testing demonstrated that the biomechanical properties of nerves decellularized with amphoteric detergent and nuclease were comparable to those of fresh controls. Decellularization with amphoteric detergent and nuclease better remove cellular components and better preserve extracellular matrix than decellularization with nonionic and anionic detergents, even in large-diameter human peripheral nerves. Cirrhosis occurs as a result of various chronic liver injuries,which may be caused by viral infections,alcohol abuse and the administration of drugs and chemicals. Recently,bone marrow cells(BMCs),hematopoietic stem cells(HSCs) and mesenchymal stem cells(MSCs) have been used for developing treatments for cirrhosis. Clinical trials have investigated the therapeutic potential of BMCs,HSCs and MSCs for the treatment of cirrhosis based on their potential to differentiate into hepatocytes. Although the therapeutic mechanisms of BMC,HSC and MSC treatments are still not fully characterized,the evidence thus far has indicated that the potential therapeutic mechanisms of MSCs are clearer than those of BMCs or HSCs with respect to liver regenerative medicine. MSCs suppress inflammatory responses,reduce hepatocyte apoptosis,increase hepatocyte regeneration,reverse liver fibrosis and enhance liver functionality. This paper summarizes the clinical studies that have used BMCs,HSCs and MSCs in patients with liver failure or cirrhosis. We also present the potential therapeutic mechanisms of BMCs,HSCs and MSCs for the improvement of liver function.

The present study was carried out on natural Korean fir forests （Abies koreana） growing in Mount Halla in Jeju Island, Korea （33° 13-36＇ N and 126° 12-57＇ E）. Mount Halla is the highest mountain （1 950 m a.s.h） in South Korea. On the Korean fir forests near the top of Mount Halla in Korea, we established permanent plots between dieback and healthy population. Each permanent plot includes both dieback and relatively healthy Korean fir individuals. Three sites in this study showed similar altitude, topographic position, aspects, slope, diameter at breast height, average height and ages. Net photosynthetic rates （PN） on different temperature regimes were evaluated to explain the forest dieback phenomenon on Korean fir populations. Light response curves were determined on three different temperature regimes： 15 ℃, 20℃ and 25℃. The irradiance response curve showed higher values in lower air temperatures. Generally, irradiance response curves of healthy Korean fir populations were higher than the dieback population at all sites.

Volume Estimation of Small Scale Debris Flows Based on Observations of Topographic Changes Using Airborne LiDAR DEMs

This paper describes a geographic information system(GIS)-based method for observing changes in topography caused by the initiation, transport, and deposition of debris flows using highresolution light detection and ranging(LiDAR) digital elevation models(DEMs) obtained before and after the debris flow events. The paper also describes a method for estimating the volume of debris flows using the differences between the LiDAR DEMs. The relative and absolute positioning accuracies of the LiDAR DEMs were evaluated using a real-time precise global navigation satellite system(GNSS) positioning method. In addition, longitudinal and cross-sectional profiles of the study area were constructed to determine the topographic changes caused by the debris flows. The volume of the debris flows was estimated based on the difference between the LiDAR DEMs. The accuracies of the relative and absolute positioning of the two LiDAR DEMs were determined to be ±10 cm and ±11 cm RMSE, respectively, which demonstrates the efficiency of the method for determining topographic changes at an scale equivalent to that of field investigations. Based on the topographic changes, the volume of the debris flows in the study area was estimated to be 3747 m3, which is comparable with the volume estimated based on the data from field investigations.

Endoscopic submucosal dissection(ESD) is becoming a popular procedure for the diagnosis and treatment of superficial mucosal lesions,and has the advantage of en bloc resection which yields a higher complete resection and remission rate compared to endoscopic mucosal resection(EMR).However,the learning process of this advanced endoscopic procedure requires a lengthy training period and considerable experience to be proficient.A well framed training protocol which is safe,effective,easily reproducible and cost-effective is desirable to teach *** addition,the training course may need to be tailored around settings such as ethnicity,culture,workload,and disease *** Asian countries with a large volume of early gastric lesions which need endoscopic treatment,endoscopists would be able to learn ESD expanding their skills from EMR to ESD under the supervision of ***,in Western countries due to the low incidence of superficial gastric tumors,trials have utilized simulator models to improve *** Korea,the Korean Society of Gastrointestinal Endoscopy(KSGE) is playing an important role in training many gastroenterologists who have shown an interest in performing ESD by providing an annual live demonstration and a nationwide tutoring *** purpose of this article is to introduce our ESD tutoring experience,review the published papers related to this topic,and propose several suggestions for future directions in teaching and learning ESD.
