Prevalence, correlates, attitude and treatment seeking of erectile dysfunction among type 2 diabetic Chinese men attending primary care outpatient clinics

To investigate the prevalence, correlates, attitude and treatment seeking behavior of erectile dysfunction （ED） in type 2 diabetes mellitus （T2DM） patients in the primary care setting, a multi-center cross-sectional survey using a structured anonymous self-administered questionnaire was performed in 10 general outpatient clinics. Of the 603 subjects （91% response rate）, the prevalence of ED men, as defined by the International Index of Erectile Function, was 79.1%. Most subjects had mild ED （28.9%）, followed by mild-to-moderate ED （27.9%）, then moderate ED （13.4%） and severe ED （9%）. Nearly 55% of those with ED did not consider themselves as having ED. Less than 10% of them had ever sought medical treatment, although 76.1% of them wished to receive management from doctor（s） should they be diagnosed with ED. They considered the most important management from doctors to be clinical assessment （41.7%）, followed by management of potential underlying cause （37.8%）, referral to specialist （27.5%）, education （23.9%）, prescription of phosphodiesterase type 5 inhibitors （16.9%） and referral to ~：ounseling service （6.7%）. The prevalence of ED was strongly associated with subjects who thought they had ED （odds ratio （OR） = 90.49 （20.00-409.48, P 〈 0.001）） and were from the older age group （OR = 1.043 （1.011-1.076, P = 0.008））. In conclusion, ED is highly prevalent among T2DM men. The majority of them wanted management from doctors should they have ED, but only a minority would actually voice out the request. Screening of ED among T2DM men using structural questionnaire allowed the diagnosis of more than half of the ED cases, which otherwise would have gone undiagnosed.

During pregnancy,a unique and dramatic sequence of events occurs,defining the most remarkable transformation of a single cell into a recognizable human being. Ultrasonographic examination is play a important role for prevention of fetal birth defect. For some years,there have been dramatic advances in ultrasound technology,including improved spatial and contrast resolution,three-dimensional and four-dimenstional imaging,harmonic imaging,new and improved ultrasound sanning probes,and improved digital review workstations,to name a few. Likewise,our knowledge of normal fetal anatomy and pathology,and the pathophysiology of disease have increased substantially. The internet has made communication among researcheres earier. There have been many collaborative studies and refinements of the guidelines for the performance of the obstetric ultrasound examination. But there are still some differences in the approach to the obstetric ultrasound examination from one group to the others. Some issues such as what constitutes a basic ultrasound examination,what structure should be perform and interpret the exa-mination,how safe is ultrasound,how should it be recorded and documented,how should it be reported,and how accurate rate of diagnosis fetal congenital malformation. In conclusion is that,the appeal of the ultrasound examination is that it is a noninvasive,safe procedure that has a high degree of patient accep-tance and can yield a wealth of information. It is always a delight to examine the obstetric patient and reassure her about her pregnancy,when appropriate. However,there are times when an abnormality is strongly suspected but it may be equivoal or may not fit into a specific category. Under these circumstances,the best pathway for the sonologist to is to do a follow-up examination and seek consultation. If time does not allow a follow-up examination,then the sonologist should communicate to the referring physician and the patient that a definiitve answer is not possible

Recurrent giant fibrovascular polyp of the esophagus

Giant fibrovascular polyps of the esophagus and hypopharynx are rare benign esophageal tumors. They arise most commonly in the upper esophagus and may, rarely, originate in the hypopharynx. They can vary significantly in size. Even though they are benign, they may be lethal due to either bleeding or, rarely, asphyxiation if a large polyp is regurgitated. Patients commonly present with dysphagia or hematemesis. The polyps may not be well visualized on endoscopy and imaging plays a vital role in aiding diagnosis as well as providing important information for preoperative planning, such as the location of the pedicle, the vascularity of the polyp and the tissue elements of the mass. They can also be recurrent in rare cases, especially if the resection margins of the base are involved. We review the recent literature and report a case of a 61-year-old man with a recurrent giant esophageal fibrovascular polyp with illustrative contrast barium swallow, CT and intra-operative images, who required several surgeries via a combination of endoscopic, trans-oral, trans-cervical, trans-thoracic and trans-abdominal approaches.

An intelligent reflecting surface(IRS)is proposed to enhance the physical layer security in the Rician fading channel wherein the angular direction of the eavesdropper(ED)is aligned with a legitimate user.A two-phase communication system under active attacks and passive eavesdropping is considered in this *** base station avoids direct transmission to the attacked user in the first phase,whereas other users cooperate in forwarding signals to the attacked user in the second phase,with the help of IRS and energy harvesting *** the occurrence of active attacks,an outage-constrained beamforming design problem is investigated under the statistical cascaded channel error model,which is solved by using the Bernstein-type *** average secrecy rate maximization problem for the passive eavesdropping is formulated,which is then addressed by a low-complexity *** numerical results of this study reveal that the negative effect of the ED’s channel error is larger than that of the legitimate user.

Growth and photosynthesis of Chlorella strains from polar,temperate and tropical freshwater environments under temperature stress

Elevated temperatures as a consequence of global warming have significant impacts on the adaptation and survival of microalgae which are important primary producers in many ecosystems. The impact of temperature on the photosynthesis of microalgae is of great interest as the primary production of algal biomass is strongly dependent on the photosynthetic rates in a dynamic environment. Here, we examine the effects of elevated temperature on Chlorella strains originating from different latitudes, namely Antarctic, Arctic, temperate and tropical regions. Chlorophyll fluorescence was used to assess the photosynthetic responses of the microalgae. Rapid light curves(RLCs) and maximum quantum yield( F_v/F_m) were recorded. The results showed that Chlorella originating from different latitudes portrayed different growth trends and photosynthetic performance. The Chlorella genus is eurythermal, with a broad temperature tolerance range, but with strain-specific characteristics. However, there was a large overlap between the tolerance range of the four strains due to their "eurythermal adaptivity". Changes in the photosynthetic parameters indicated temperature stress. The ability of the four strains to reactivate photosynthesis after inhibition of photosynthesis under high temperatures was also studied. The Chlorella strains were shown to recover in terms of photosynthesis and growth(measured as Chl a) when they were returned to their ambient temperatures. Polar strains showed faster recovery in their optimal temperature compared to that under the ambient temperature from which they were isolated.

Dengue virus(DENV) has emerged as a major virus that is spread by mosquitoes. Recently, it has spread to more than a hundred nations but continues to lack specific treatable medication. Many traditional Chinese medicinal(TCM) plants are in practice for dengue fever in dengue endemic regions. These traditional medicines persevere with treatments, which modern medicines lack. The study aims to substantiate the anti-dengue potential of some traditional herbs and make them available for further studies to facilitate TCM users. Twelve TCM plants aqueous extracts were evaluated, which are described as cool herbs used for the diseases with high fever. Lead plants were established through detailed in vitro foci forming unit reduction analysis(FFURA) against all four serotypes and validated through quantitative real-time RT-PCR(q RT-PCR). Four plants potentially inhibited the virus in primary phenotypic in vitro evaluation. Two lead plants Dryopteris crassirhizoma(DC) and Morus alba(MA) were identified with half minimal inhibitory concentration(IC50) 130 and 221 μg m L^-1, respectively, while the selectivity indices(SI) were 4.21 and 4.62, respectively. Lead plants equally inhibited all four serotypes of DENV. Time-of-addition analysis suggested that, DC was active at later stages of viral replication, whereas MA was active during the early stages and even showed some prophylactic activity. Liquid chromatography-mass spectrometry(HPLC/MS) analysis revealed presence of flavonoids. DC and MA are identified as potential anti-dengue plants, active against varied stages of dengue virus replication cycle. These results may serve as the base knowledge for further investigation on their combined treatments or integrative treatment with western medicines, which may improve the overall anti-dengue activity in future.

Hepatocellular carcinoma masquerading as a bleeding gastric ulcer: A case report and a review of the surgical management

Hepatocellular Carcinoma (HCC) is a common malignancyworldwide. While bleeding from the gastrointestinal tract(BGIT) has a well known association with HCC, suchcases are mainly due to gastric and esophageal *** as a result of invasion of the gastrointestinal tractby HCC is extremely rare and is reportedly associatedwith very poor prognosis. We describe a 67-year-oldmale who presented with BGIT. Endoscopy showed thesite of bleeding to be from a gastric ulcer, but endoscopictherapy failed to control the bleeding and emergencysurgery was required. At surgery, the ulcer was found tohave arisen from direct invasion of the gastrointestinaltract by HCC of the left lobe. Control of the bleedingwas achieved by surgical resection of the HCC en-bloc with the lesser curve of the stomach. The patientremains alive 33 mo after surgery. Direct invasion of thegastrointestinal tract by HCC giving rise to BGIT is veryuncommon. Surgical resection may offer significantlybetter survival over non-surgical therapy, especially if thepatient is a good surgical candidate and has adequatefunctional liver reserves. Prognosis is not uniformly grave.

Antralization at the edge of proximal gastric ulcers:Does Helicobacter pylori infection play a role?

AIM: To determine the prevalence of antralization at the edge of proximal gastric ulcers, and the effect ofH. pylori eradication on the mucosal appearances.METHODS: Biopsies were taken from the antrum, body and the ulcer edge of patients with benign proximal gastric ulcers before and one year after treatment. Gastric mucosa was classified as antral, transitional or body type.H. pylori positive patients receivedeither triple therapy, or omeprazole.RESULTS: Patients with index ulcers in the incisura, body or fundus (n=116) were analyzed. Antral-type mucosa was more prevalent at the ulcer edge inH. pylori-positive patients ***-negative patients (93% vs 60%, OR=8.95,95%CI: 2.47-32.4, P=0.001). At one year, there was a significant reduction in the prevalence of antralization (from 93 % to 61%, P=0.004) at the ulcer edge in patients with H. pyloribeing eradicated. However, there was no difference in the prevalence of antralization at the ulcer edge in those with persistent infection.CONCLUSION: H. pylori infection is associated with antralization at the edge of proximal gastric ulcers, which may be reversible in some patients after eradication of the infection.

Trabecular bone deterioration at the greater trochanter ol mice with unilateral obstructive nephropathy

Our previous study showed the early molecular responses of bone in response to obstructive nephropathy in a unilateral ureteral obstruction （UUO） mouse model. Here, we addressed the changes in trabecular bone properties at greater trochanter, the proximal and the distal metaphysis of femur in UUO mice. The male mice were subjected to UUO （n= 10） or sham operation （n= 10）. All mice were killed on day 7 after the surgical operation. The micro-computed tomography （micro-CT） analysis for different femoral trabecular bone sites demonstrated pathological alterations of trabecular bone mass and micro-networks at greater trochanter as shown by decreases in bone mineral density/bone volume （P〈O.05） and trabecular number （P〈O.05） and increases in trabecular separation （P〈O.01） and bone surface/bone volume （P〈O.05） in UUO mice. The present study demonstrates that UUO-induced unilateral obstructivenephropathy has markedly detrimental effects on the trabecular trochanter of the femur.
