Effect of Hemodilution on Fluctuation of Cerebral Oxygen Saturation during Cardiopulmonary Bypass in Children with Cyanotic Congenital Heart Disease

Background:In patients with cyanotic congenital heart disease(CHD),cerebral oxygenation may be maintained by elevations in hematocrit(Hct).Hemodilution accompanying cardiopulmonary bypass(CPB),however,can disrupt cerebral oxygen balance,leading to fluctuations in cerebral oxygen saturation(ScO_(2)).The present study investigated the effects of Hct changes on the fluctuation of ScO_(2)during CPB in cyanotic CHD using performance measurement(PM).Methods:Children with CHD(51 acyanotic and 46 cyanotic)who had undergone cardiac surgery using CPB were *** performance error(MDPE),median absolute performance error(MDAPE),and wobble parameters of ScO_(2)were calculated before(reference value),during,and after *** of PM parameters with Hct and reductions in Hct(ΔHct)were also ***:Before CPB,patients with cyanotic CHD had lower MDPE and larger wobble than those with acyanotic CHD,although mean ScO_(2)did not differ significantly between the two *** CPB,ScO_(2)of acyanotic CHD increased asΔHct increased,but PM variables were not associated withΔ*** cyanotic CHD,MDPE(r=−0.324,p=0.032)and MDAPE(r=0.339,p=0.024)correlated significantly withΔHct during *** CPB,MDPE(r=0.574,p=0.025)and MDAPE(r=−0.543,p=0.036)were significantly correlated with Hct in children with cyanotic CHD who underwent palliative ***:Therefore,ScO_(2)fluctuation during CPB in children with cyanotic CHD may be affected by the decrease in Hct,suggesting that excessive hemodilution can negatively influence the maintenance of cerebral oxygenation in these patients.

Dynamic Changes in Left and Right Cerebral Oxygen Saturation during Selective Cerebral Perfusion in Young Infants

Objectives:We investigated whether the selective cerebral perfusion(SCP)technique causes differences in changes in cerebral perfusion between both hemispheres in young infants,using cerebral oxygen saturation(ScO2)as an ***,we determined the association between the discrepancy in ScO2 and cerebral perfu-sion pressure during ***:The difference in ScO2 between the left and right cerebral hemispheres(ΔScO2 Rt-Lt)was calculated during clamping of the innominate artery(IA)and during ***:In 25 infants(aged 2 to 78 days),the left and right ScO2 were well maintained(median 63.2%and 60.9%during IA clamping,respectively;64.0%and 65.6%during SCP,respectively).During IA clamping,right and left ScO2 decreased(med-ian-1.4%and-1.0%,respectively).During SCP,rightΔScO2 was higher compared to leftΔScO2(median 1.5%vs.0.6%;p<0.001).Eight patients had a higher rightΔScO2 than leftΔScO2 throughout *** had lowerΔScO2 Rt-Lt during IA clamping(median-3.2%vs.0.0%;p<0.001)and higherΔScO2 Rt-Lt during SCP than others(median 5.0%vs.-0.8%;p<0.001).During and after SCP,the correlation coefﬁcient between rightΔScO2 and change in the mean arterial pressure was higher in patients with a discrepancy than in others(r=0.731 vs.r=0.519;p<0.001).Conclusions:This study suggests that SCP permits adequate bilateral cerebral ***,the unilateral cerebral perfusion technique may cause a difference in cerebral perfusion between both hemispheres in young infants;this may depend on the perfusion pressure.

To clarify the trend in colorectal cancer mortality in Asian *** analyzed the colorectal cancer mortality in four Asian countries using the World Health Organization mortality database and the Korea National Statistics Office *** annual age-standardized rates and truncated rated for the three age groups(30-49,50-69 and ≥ 70 years) for Hong Kong of China(1969-2009),Japan(1955-2009),South Korea(1985-2006),and Singapore(1966-2009) were estimated.A joinpoint regression model was used to detect significant trends in mortality *** cancer mortality in men started to decrease in 1992 in Japan followed by Singapore and Hong Kong of China in *** mortality rates in women stared to decrease in 1980 in Singapore,followed by Hong Kong of China and Japan in *** all countries and both genders,except for women in Singapore,the decrease in mortality began in the younger age *** colorectal cancer mortality in the four studied Asian countries has started to decrease,and the decrease occurred first in the younger age groups.

Risk of sedation for diagnostic esophagogastroduodenoscopy in obstructive sleep apnea patients

AIM: To investigate whether patients with obstructive sleep apnea (OSA) are at risk of sedation-related complications during diagnostic esophagogastroduodenoscopy (EGD). METHODS: A prospective study was performed in consecutive patients with OSA, who were confirmed with full-night polysomnography between July 2010 and April 2011. The occurrence of cardiopulmonary complications related to sedation during diagnostic EGD was compared between OSA and control groups. RESULTS: During the study period, 31 patients with OSA and 65 controls were enrolled. Compared with the control group, a higher dosage of midazolam was administered (P = 0.000) and a higher proportion of deep sedation was performed (P = 0.024) in the OSA group. However, all adverse events, including sedation fail-ure, paradoxical responses, snoring or apnea, hypoxia, hypotension, oxygen or flumazenil administration, and other adverse events were not different between the two groups (all P > 0.1). Patients with OSA were not predisposed to hypoxia with multivariate logistic regression analysis (P = 0.068). CONCLUSION: In patients with OSA, this limited sized study did not disclose an increased risk of cardiopulmonary complications during diagnostic EGD under sedation.

ASL Recognition by the Layered Learning Model Using Clustered Groups

American Sign Language(ASL)images can be used as a communication tool by determining numbers and letters using the shape of the ***,ASL can have an key role in communication for hearing-impaired persons and conveying information to other persons,because sign language is their only channel of *** ASL recognition methods primarily adopt images,sensors,and pose-based recognition techniques,and employ various gestures together with *** study briefly reviews these attempts at ASL recognition and provides an improved ASL classification model that attempts to develop a deep learning method with *** the proposed model,the collected ASL images were clustered based on similarities in shape,and clustered group classification was first performed,followed by reclassification within the *** experiments were conducted with various groups using different learning layers to improve the accuracy of individual image *** selecting the optimized group,we proposed a meta-layered learning model with the highest recognition rate using a deep learning method of image *** proposed model exhibited an improved performance compared with the general classification model.

Arthroscopic treatment of iliopsoas tendinitis after total hip arthroplasty with acetabular cup malposition: Two case reports

BACKGROUND Optimal treatment for iliopsoas tendinitis after total hip arthroplasty(THA)with cup malposition,iliopsoas release alone or with cup revision,is controversial,particularly in young,active ***,arthroscopic iliopsoas tendon(IPT)release in these patients has been rarely described,and midterm effects of this procedure on THA longevity and groin pain recurrence remain *** performed arthroscopic IPT release after THA and report midterm outcomes in two young patients with acetabular cup *** SUMMARY In the two patients,groin pain started early after *** examination revealed nonspecific findings,and laboratory tests showed no evidence of *** and computed tomography showed reduced acetabular component anteversion angle and anterior cup prominence of more than 16 *** therapeutic diagnosis,ultrasonography-guided lidocaine with steroid was injected into the IPT *** both patients,groin pain improved initially but worsened after a few ***,the patients underwent arthroscopic IPT release under spinal *** revealed synovitis with fibrous tissues around the IPT and various lesions related to the implants after *** tenotomy and debridement with biopsy were performed;histopathologic studies showed chronic inflammation with synovial *** patients were encouraged to start walking immediately after surgery,and they returned to complete daily function early after *** experienced no recurrence of groin pain or any implant-related problems 5 years *** Arthroscopic IPT release for cup malposition produced excellent midterm outcomes without recurrence of groin pain and implant-related problems.

Cyberattacks on the Industrial Control System(ICS)have recently been increasing,made more intelligent by advancing *** such,cybersecurity for such systems is attracting *** a core element of control devices,the Programmable Logic Controller(PLC)in an ICS carries out on-site control over the ICS.A cyberattack on the PLC will cause damages on the overall ICS,with Stuxnet and Duqu as the most representative ***,cybersecurity for PLCs is considered essential,and many researchers carry out a variety of analyses on the vulnerabilities of PLCs as part of preemptive efforts against *** this study,a vulnerability analysis was conducted on the XGB *** vulnerabilities were identified by analyzing the network protocols and memory structure of PLCs and were utilized to launch replay attack,memory modulation attack,and FTP/Web service account theft for the verification of the *** on the results,the attacks were proven to be able to cause the PLC to malfunction and disable it,and the identified vulnerabilities were defined.

Recent cumulative findings signify the adaptive immunity of materials as a key agenda in tissue healing that can improve regenerative events and outcomes. Modulating immune responses, mainly the recruitment and functions of T and B cells and their further interplay with innate immune cells (e.g., dendritic cells, macrophages) can be orchestrated by materials. For instance, decellularized matrices have been shown to promote muscle healing by inducing T helper 2 (Th2) cell immunity, while synthetic biopolymers exhibit differential effects on B cell responses and fibrosis compared decellularized matrices. We discuss the recent findings on how implantable materials instruct the adaptive immune events and the subsequent tissue healing process. In particular, we dissect the materials’ physicochemical properties (shape, size, topology, degradation, rigidity, and matrix dynamic mechanics) to demonstrate the relations of these parameters with the adaptive immune responses in vitro and the underlying biological mechanisms. Furthermore, we present evidence of recent in vivo phenomena, including tissue healing, cancer progression, and fibrosis, wherein biomaterials potentially shape adaptive immune cell functions and in vivo outcomes. Our discussion will help understand the materials-regulated immunology events more deeply, and offer the design rationale of materials with tunable matrix properties for accelerated tissue repair and regeneration.

Formation Mechanism of Chunky Graphite and Its Preventive Measures

The formation mechanism of chunky graphite has been reviewed and studied. The study consisted of a unidirectional solidification method, a small droplet method and a furnace cooling method. Four kinds of iron samples were prepared, namely, the pure Fe-C, Fe-C-S, Fe-C-Ce and Fe-C-Si-Ce alloys, and three kinds of nickel samples, namely the Ni-C, Ni-C-S and Ni-C-Mg alloys. The results of the unidirectional solidification of the Ni-C alloys showed that spheroidal graphite is not observed in the continuous solidified region, in which only flake-like graphite is observed, while spheroidal graphite is usually observed in the quenched liquid region. The existence of spheroidal graphite in the solidified phase is recognized only in the discontinuous growth mode of the Ni-C-Mg alloy solidified at 150 mm/h. This means that the spheroidal graphite is directly crystallized from the melt and entrapped by the flake-like chunky graphite that is formed by the continuous growth mode. In the small droplet method, a small piece of the Fe-C or Fe-C-Ce sample was melted on a pure graphite plate then cooled at a different cooling rate in a He-3%H2 atmosphere. The graphite in the Fe-C-Ce alloy is usually spherical. Nevertheless, the graphite morphology of the final solidified area changed from spherical to chunky and chunky to ledeburite with an increase in the cooling rate. This means that the chunky graphite is formed in the residual liquid region by the solidification into Fe-graphite system. The sample was cooled in a furnace, and the graphite morphology changes from spherical to chunky and chunky to ledeburite with the decrease in the Si content. These phenomena can be confirmed by the cooling curves of these samples.

BACKGROUND： Thyroid hormones modulate proliferation of astrocytes and microglia depending on maturation stage and localization. Studies have demonstrated that triiodothyronine treatment or thyroidectomy during developmental stages results in morphological alterations and changes in the number of astrocytes and microglia. Little is known about the effects of hypothyroidism on astrocytes and microglia in adults. OBJECTIVE： To investigate the effects of hypothyroidism on morphology and number of astrocytes and microglia in the adult rat hippocampus. DESIGN, TIME AND SETTING： A randomized, controlled, neuroendocrinological, animal study was performed at the College of Medicine, Hallym University, South Korea between May 2008 and April 2009. MATERIALS： Methimazole, rabbit anti-glial fibrillary acidic protein （GFAP） antiserum, and rabbit anti-lba-1 antiserum were purchased from Sigma, USA. Rabbit anti-GFAP polyclonal antibody was provided by Chemicon, USA. Rabbit anti-lba-1 polyclonal antibody was purchased from Wako, Japan. Terminal deoxynucleotidyl transferase dUTP-biotin nick-end-labeling （TUNEL） kit was provided by Roche Molecular Biochemicals, Mannheim, Germany. METHODS： Hypothyroidism was induced in Wistar rats via methimazole administration （0.025%） in drinking water for 5 weeks, starting at 6 months of age. MAIN OUTCOME MEASURES： Following methimazole treatment, hippocampai neuronal death was determined using TUNEL staining. The morphology and number of GFAP and lba-1 immunoreactive cells were detected by immunohistochemistry. Hippocampal GFAP and lba-1 protein levels were detected by Western blot analysis. Serum-free triiodothyronine and thyroxine levels were quantified. RESULTS： TUNEL-positive neurons were not observed in the hippocampus of euthyroid and hypothyroid rats.
