Factors associated with metastasis in superior mesenteric vein lymph node in subtotal gastrectomy for gastric cancer:Retrospective case control study

Objective: The revised Japanese treatment guideline for gastric cancer recommends dissection of the superior mesenteric vein lymph node(No. 14v LN) if there is metastasis in infrapyloric lymph node(No. 6 LN). however,it is still controversial whether LN dissection is necessary. The aim of this study was to investigate the factors associated with metastasis in No. 14v ***: Patients who underwent D2 lymphadenectomy between 2003 and 2010 were included. We excluded patients who underwent total gastrectomy, had multiple lesions, or had missing data about the status of metastasis in the LNs that were included in D2 lymphadenectomy. Clinicopathologic characteristics and the metastasis in regional LNs were compared between patients with No. 14v LN metastasis(14v+) and those without(14v-).Results: Five hundred sixty patients were included in this study. Univariate analysis showed that old age, larger tumor size, tumor location, differentiation, lymphatic invasion, venous invasion, perineural invasion, T classification, and N classification were related to metastasis in No. 14v LN. Multivariate analysis showed differentiation(P=0.027) and N classification(P<0.001) were independent related factors. Metastasis in infrapyloric lymph node(No. 6 LN) and proxiaml splenic lymph node(No. 11p LN) was independently associated with metastasis in No. 14v ***: Differentiation and N classification were independent factors associated with No. 14v LN metastasis, and No. 6 and No. 11p LN metastasis were independent risk factors for No. 14v LN metastasis.

A comparative study of totally laparoscopic distal gastrectomy versus laparoscopic-assisted distal gastrectomy in gastric cancer patients: Short-term operative outcomes at a high-volume center

Objective： Laparoscopic gastrectomy has been established as a standard treatment for early gastric cancer, and its use is increasing recently. Compared with the conventional laparoscopy-assisted distal gastrectomy （LADG）, totally laparoscopic distal gastrectomy （TLDG） involves intracorporeal reconstruction, which can avoid the additional incision, resulting in pain reduction and early recovery. This study aimed to compare the short-term postoperative outcomes of TLDG vs. LADG in gastric cancer in a high-volume ***： A retrospective cohort study was conducted on 1,322 patients who underwent laparoscopic distal gastrectomy from June 2012 to June 2017 at the National Cancer Center, Korea. LAD G was performed in the early period before July 2015, and TLDG was applied in the later period. Postoperative short-term outcomes were compared in terms of complication and clinical course between the two groups. Pain score was measured by rating the pain intensity from 0 to 10 points on postoperative day （POD） 1 and 3. Results： A total of 667 patients underwent LADG and 655 patients underwent TLDG. Clinieopathologic characteristics were not different in both groups. Intraoperative estimated blood loss （EBL） was significantly lower in the TLDG group （P〈0.001）. Postoperative pain scores were significantly lower in the TLDG group than in the LADG group on POD 1 （5.1±1.5 vs. 4.8±1.4, P=0.015）. First flatus passage after operation was significantly earlier in the TLDG group （3.4±0.8 d vs. 3.2±0.6 d, P〈0.001）. There were no differences in postoperative complications and hospital stay between the two groups. Conclusions： Based on the reported short-term postoperative outcomes, TLDG is safe and feasible as well as LADG. Moreover, compared with LADG, TLDG can reduce intraoperative EBL and postoperative pain and enhance the bowel motility in gastric cancer surgery.

Improvement in tensile strength of extruded Mg-5Bi alloy through addition of Sn and its underlying strengthening mechanisms

Through an investigation of the microstructure and mechanical properties of extruded Mg–5Bi–x Sn(BT5x, x = 0, 2, 4, and 6 wt%) alloys,this study demonstrates that the addition of Sn to an Mg–5Bi binary alloy significantly improves the tensile strength of the extruded *** the extruded alloys exhibit a typical basal fiber texture and a partially dynamically recrystallized(DRXed) microstructure consisting of fine DRXed grains and coarse un DRXed grains. As the Sn content increases from 0 wt% to 6 wt%, the average size of the DRXed grains decreases from 4.2 to 2.8 μm owing to the increase in the amount of precipitates via their grain-boundary pinning effect. The extruded B5 and BT52 alloys contain numerous Mg_(3)Bi_(2) precipitates, but their size and number density are smaller and higher, respectively, in the latter *** Mg_(2)Sn precipitates as well as Mg_(3)Bi_(2)precipitates are present in the extruded BT54 and BT56 alloys, and the number density of the Mg_(2)Sn precipitates is higher in the latter alloy because of its higher Sn content. The addition of 2 wt% Sn to the B5 alloy significantly improves the yield strength(YS) and ultimate tensile strength(UTS) of the extruded alloy—by 76 and 57 MPa, respectively. This drastic improvement is the combined outcome of enhanced grain-boundary hardening, precipitation hardening, and solid-solution hardening effects induced by the refined DRXed grains, numerous precipitates, and Sn solute atoms, respectively. The further addition of 2 wt% or 4 wt% Sn to the BT52 alloy leads to moderate increments in the YS and UTS of the extruded alloy. Specifically, each addition of 2 wt% Sn increases the YS and UTS by ~26 and ~20 MPa, respectively, which is attributed mainly to the additional precipitation hardening effect induced by the Mg_(2)Sn precipitates.

In this work exergetical performance analysis is carried out based on the second law of thermodynamics for organic flash cycle(OFC) using a two-phase expander instead of throttle expansion in order to recover efficiently finite thermal *** exergy destructions(anergies) at various components of the system are theoretically investigated as well as the exergy *** show that the anergy of heat exchanger or two-phase expander decreases while the anergy of throttle valve increases with increasing flash temperature,and the exergy efficiency has an optimum value with respect to the flash *** the optimal conditions with respect to the flash temperature,exergy efficiency increases with the heating temperature and the component having the largest exergy destruction varies with the flash temperature or heating temperature.

Entransy and Exergy Analyses for Optimizations of Heat-Work Conversion with Carnot Cycle

The concept of entransy has been newly proposed in terms of the analogy between heat and electrical conduction and could bc useful in analyzing and optimizing the heat-work conversion systems. This work presents comparative analyses of entransy and exergy for optimizations of heat-work conversion. The work production and heat transfer processes in Carnot cycle system are investigated with the formulations of exergy destruction, entransy loss, work entransy, entransy dissipation, and cfficiencics for both cases of dumping and non-dumping of used source fluid. The effects of source and condensation temperatures on the system performance arc systematically investigated for optimal condition of producing maximum work or work cntransy.

Effects of ethanol on the tonicity of corporal tissue and the intracellular Ca^2＋ concentration of human corporal smooth muscle cells

Heavy alcohol consumption is associated with an increased risk of erectile dysfunction （ED）; however, the acute effects of ethanol （EtOH） on penile tissue are not fully understood. We sought to investigate the effects of EtOH on corporal tissue tonicity, as well as the intracellular Ca^2＋ concentration （[Ca^2＋]i） and potassium channel activity of corporal smooth muscle. Strips of corpus cavernosum （CC） from rabbits were mounted in organ baths for isometric tension studies. Electrical field stimulation （EFS） was applied to strips precontracted with 10 μmol L^-1 phenylephrine as a control. EtOH was then added to the organ bath and incubated before EFS. The [Ca^2＋]i levels were monitored by the ratio of fura-2 fluorescence intensities using the fura-2 loading method. Single-channel and whole-cell currents were recorded by the conventional patch-clamp technique in short-term cultured smooth muscle cells from human CC tissue. The corpus cavernosal relaxant response of EFS was decreased in proportion to the concentration of EtOH. EtOH induced a sustained increase in [Ca^2＋]i in a dose-dependent manner, Extracellular application of EtOH significantly increased whole-cell K^＋ currents in a concentration-dependent manner （P 〈 0.05）. EtOH also increased the open probability in cell-attached patches; however, in inside-out patches, the application of EtOH to the intracellular aspect of the patches induced slight inhibition of Ca^2＋-activated potassium channel （KCa） activity. EtOH caused a dose-dependent increase in cavemosal tension by alterations to [Ca^2＋]i. Although EtOH did not affect KCa channels directly, it increased the channel activity by increasing [Ca^2＋]i. The increased corpus cavemosal tone caused by EtOH might be one of the mechanisms of ED after heavy drinking.

Recent advances in polymeric membranes for CO_2 capture

Membrane and membrane process have been considered as one of the most promising technologies for mitigating CO_2 emissions from the use of fossil fuels. In this paper, recent advances in polymeric membranes for CO_2 capture are reviewed in terms of material design and membrane formation. The selected polymeric materials are grouped based on their gas transport mechanisms, i.e., solution‐diffusion and facilitated transport. The discussion of solution‐diffusion membranes encompasses the recent efforts to shift the upper bound barrier, including the enhanced CO_2 solubility in several rubbery polymers and novel methods to construct shape-persisting macromolecules with unprecedented sieving ability. The carrier-bearing facilitated transport membranes are categorized based on the specific CO_2-carrier chemistry. Finally, opportunities and challenges in practical applications are also discussed, including post-combustion carbon capture(CO_2/N2), hydrogen purification(CO_2/H2), and natural gas sweetening(CO_2/CH4).

GnRH impairs diabetic wound healing through enhanced NETosis

It has been reported that neutrophil extracellular traps(NETs)impair wound healing in diabetes and that inhibiting NET generation(NETosis)improves wound healing in diabetic ***-releasing hormone(GnRH)agonists are associated with a greater risk of ***,the role of GnRH in diabetic wound healing is *** determined whether GnRH-promoted NETosis and induced more severe and delayed diabetic wound healing.A mouse model of diabetes was established using five injections with *** with blood glucose levels>250 mg/dL were then used in the *** agonist treatment induced delayed wound healing and increased NETosis at the skin wounds of diabetic *** contrast,GnRH antagonist treatment inhibited GnRH agonist-induced delayed wound *** expression of NETosis markers PAD4 and citrullinated histone H3 were increased in the GnRH-treated diabetic skin wounds in diabetic mice and *** vitro experiments also showed that neutrophils expressed a GnRH receptor and that GnRH agonist treatment increased NETosis markers and promoted phorbol myristate acetate(PMA)-induced NETosis in mouse and human ***,GnRH antagonist treatment suppressed the expression of NETosis markers and PMA-induced NETosis,which were increased by GnRH *** results indicated that GnRH-promoted NETosis and that increased NETosis induced delayed wound healing in diabetic skin ***,inhibition of GnRH might be a novel treatment of diabetic foot ulcers.

Traumatic axonal injury of the cingulum in patients with mild traumatic brain injury: a diffusion tensor tractography study

The cingulum,connecting the orbitofrontal cortex to the medial temporal lobe,involves in diverse cognition functions including attention,memory,and *** investigate the relationship between the cingulum injury and cognitive impairment in patients with chronic mild traumatic brain injury,we evaluated the integrity between the anterior cingulum and the basal forebrain using diffusion tensor tractography in 73 patients with chronic mild traumatic brain injury(39 males,34 females,age 43.29±11.42 years)and 40 healthy controls(22 males,18 females,age 40.11±16.81 years).The patients were divided into three subgroups based on the integrity between the anterior cingulum and the basal forebrain on diffusion tensor tractography:subgroup A(n=19 patients)– both sides of the anterior cingulum were intact;subgroup B(n=36 patients)– either side of the anterior cingulum was intact;and subgroup C(18 patients)– both sides of the anterior cingulum were *** were significant differences in total Memory Assessment Scale score between subgroups A and B and between subgroups A and *** were no significant differences in diffusion tensor tractography parameters(fractional anisotropy,apparent diffusion coefficient,and fiber volume)between patients and *** findings suggest that the integrity between the anterior cingulum and the basal forebrain,but not diffusion tensor tractography parameter,can be used to predict the cognitive function of patients with chronic mild traumatic brain *** study was approved by Yeungnam University hospital Institutional Review Board(approval ***-2014-01-425-010)on August 16,2017.
