Background:IgG-class autoantibodies to N-methyl-D-Aspartate(NmDA)-type glutamate receptors define a novel entity of autoimmune *** examining the prevalence of NmDA IgA/Igm antibodies in patients with Parkinson disease with/without dementia produced conflicting *** measured NmDA antibodies in a large,well phenotyped sample of Parkinson patients without and with cognitive impairment(n=296)and controls(n=295)free of neuropsychiatric *** phenotyping and large numbers allowed statistically meaningful correlation of antibody status with diagnostic subgroups as well as quantitative indicators of disease severity and cognitive ***:NmDA antibodies were analysed in the serum of patients and controls using well established validated *** used anti-NmDA antibody positivity as the main independent variable and correlated it with disease status and phenotypic ***:The frequency of NmDA IgA/Igm antibodies was lower in Parkinson patients(13%)than in controls(22%)and higher than in previous studies in both *** IgA/Igm antibodies were neither significantly associated with diagnostic subclasses of Parkinson disease according to cognitive impairment,nor with quantitative indicators of disease severity and cognitive impairment.A positive NmDA antibody status was positively correlated with age in controls but not in Parkinson ***:It is unlikely albeit not impossible that NmDA antibodies play a significant role in the pathogenesis or progression of Parkinson disease *** Parkinson disease with dementia,while NmDA IgG antibodies define a separate disease of its own.

肝转移癌动脉内化疗已有50余年历史,但长期以来仅作为症状治疗。皮下置泵肝动脉插管问题解决后,区域化疗得到重视。氟尿嘧啶脱氧核苷(FUdR)可在肝内维持高浓度,且具良好的溶解性,可经皮下植入泵使用,使病人能在门诊接受2～3周的化疗。其生活质量和有效率(约50％)高于全身化疗组。尽管化学性肝炎、胃十二指肠溃疡、胆管硬化等不良反应只见于动脉内FUdR治疗者,而并不发生于欧洲应用5-氟尿嘧啶(5-Fu)治疗方案者,但肝转移癌的区域治疗还未获公认。

目的：本前瞻性随机研究旨在调查主动脉狭窄（AS）患者中，主动脉瓣设计和患者一人工瓣膜不匹配（PPm）对机械或生物主动脉瓣置换术（AVR）后冠状动脉血流储备（CFR）的影响。背景：CFR可能是AS患者AVR后长期存活的一个重要参数。CFR减少可以导致心血管事件和死亡率的增加。方法：48例行AVR的患者在术前、术后5d和随访6个月时进行磁共振检查。测定冠状动脉血流，并在随访6个月时测定CFR。拟行机械瓣AVR的患者被随机分为侧倾碟瓣（n=12）或双叶瓣组（n=12）。

A 35-year-old haemodialysis-dependent woman with chronic renal failure developed large,very painful necrotic ulcers and necrosis on the thighs,buttocks and the abdomen with signs of fast *** skin biopsy specimens showed a broad necrosis of the epidermis and thrombosed dermal vessels with focal calcium deposits within the *** addition,laboratory findings presented an increased product of serum calcium and phosphate ***,we diagnosed calciphylaxis on the basis of clinical,biochemical and histopathological *** initiated a therapy in which our patient was treated with intravenous sodium thiosulphate 3 times *** after 2 weeks of treatment,no new lesions were detectable and there was a dramatic pain *** the following 4 weeks,a successive decline of the ulcers and the healing of individual tissue defects could be *** months after the start of the therapy,the patient underwent successful renal ***,the intravenoustherapy of calciphylaxis with sodium thiosulphate might be a new effective alternative in the treatment of this life-threatening disease.

Purpose: To report an association between spontaneous subhyaloidal hemorrhage and severe plasminogen activator inhibitor-1 (PAI-1) deficiency. methods: Case report. Results: A 29-year-old woman presented with sudden, painless visual loss to hand motion in her right eye. Ophthalmoscopy showed a massive subhyaloidal hemorrhage. The patients’medical history was negative for cardiovascular risk factors, trauma, infections or bleeding complications. Further investigation into possible causes revealed hyperfibrinolysis secondary to severe PAI-1 deficiency. The non-clearing subhyaloidal hemorrhage was successfully treated by pars plana vitrectomy,and her visual acuity improved to 20/20. Conclusion: When ordering laboratory tests in patients with spontaneous subhyaloidal hemorrhage to rule out fibrinolytic disorders, severe PAI-1 deficiency should be considered in the differential diagnosis. Selective screening may be helpful in identifying ophthalmologic patients with hyperfibrinolysis, especially in young individuals with subhyaloidal hemorrhages in the absence of other recognized risk factors.

The authors report on a case of benign myolipoma(synonym lipoleiomyoma) which was first described in 1991. The benign soft tissue tumor is composed of smooth muscle and adipose tissue and occurs sporadically in different locations. In the available literature cases were described retroperitoneal, spinal, orbital and subcutaneous and mostly have been discovered in females. Characteristically myolipomas are very large at diagnosis and reach diameters of 7 to 30 cm particularly in peritoneal or retroperitoneal localization. The sometimes enormous size leads to a displacing growth pattern which ultimately leads to the clinical symptoms. The patients often complain of nonspecific, mostly painless abdominal or thoracic pressure. Bordered by an intact capsule the tumors show no signs of malignancy and in the available literature there is no evidence of metastatic seeding. To the best of our knowledge the presented case is the first description of a diffuse mesenteric myolipoma in a male individual. In this article, we present the multidetector computed tomographic image characteristics, macroscopic appearance and histopathological findings.

就不同因素对CT引导下经肺活检出现气胸与肺内出血发生率的影响进行回顾性评价。133例病人接受了CT引导下肺内1个病灶的活检，2例病人活检2次。不同影响因素的分析包括病变大小、病变部位、穿刺针通过胸膜的次数、病变边缘、肺内活检径路长度与穿刺时间。活检针采用18G（gauge）切割针（Trucut，Somatex，Tehow，Germany）。135例活检中23例发生气胸（17％），其中3例需要胸部置管治疗（占全部病人的2％）。病变位于肺实质内时气胸的发生率明显高于病变位于胸膜或胸壁（P〈0．05），
