Treatment Satisfaction with Insulin Glargine in Insulin-Naïve Type 2 Diabetes Patients—A Hong Kong Based Registry

Objective: To evaluate patient satisfaction with insulin glargine. Design: Multicentre observational registry. Data were collected at baseline/inclusion visit, and 12 and 24 weeks. Setting: Physicians in Hong Kong, who managed type 2 diabetes patients and had >5 years’ experience in using insulin glargine. Patients: People with type 2 diabetes, new to insulin, aged 18 - 75 years, who were previously being treated with ≤3 oral antidiabetes drugs (OAD) and had HbA1c > 7%, and in whom the?physicians had chosen to prescribe glargine for the first time. Main outcome measures: Treatment satisfaction assessed by Diabetes Treatment Satisfaction Questionnaire (DTSQs), glycaemic control (fasting blood glucose and HbA1c) and adverse events. Results: Between April 2010-October 2011, 41 patients completed the study. Average duration of diabetes and OAD therapy was 7.8 ± 8.0 years and 6.7 ± 7.4 years, respectively. The global DTSQs treatment satisfaction scores improved from 20.9 at baseline to 28.4 (p < 0.05) at the end of 24 weeks insulin glargine treatment. Analysis of DTSQs scores showed a decrease in perceived frequency of hyperglycaemia (4.1 to 1.9, p < 0.001) and hypoglycemia (2.2 to 1.5, p = 0.079). Perceived convenience (0.60, p < 0.025) and flexibility (0.9, p < 0.009) were also improved from baseline. Reduction in mean HbA1c (10.2% ± 2.2% to 7.0% ± 1.0%) and fasting blood glucose (10.9 ± 4.0 mmol/L to 6.4 ± 1.8 mmol/L) from baseline to study termination was significant (p < 0.05). Almost half (48.7%) of patients achieved HbA1c ≤ 7.0%, while 26.0% patients had FBG < 5.6 mmol/L. In total, 9 (22.0%) patients experienced at least one hypoglycemia event;there were no reports of severe hypoglycaemia. Fabrication of Microlens Array and Its Application:A Review

Microlens arrays are the key component in the next generation of 3D imaging system, for it exhibits some good optical properties such as extremely large field of view angles, low aberration and distortion, high temporal resolution and infinite depth of field. Although many fabrication methods or processes are proposed for manufacturing such precision component, however, those methods still need to be improved. In this review, those fabrication methods are categorized into direct and indirect method and compared in detail. Two main challenges in manufacturing microlens array are identified: how to obtain a microlens array with good uniformity in a large area and how to produce the microlens array on a curved surface? In order to effectively achieve control of the geometry of a microlens,indirect methods involving the use of 3D molds and replication technologies are suggested. Further development of ultraprecision machining technology is needed to reduce the surface fluctuation by considering the dynamics of machine tool in tool path planning. Finally, the challenges and opportunities of manufacturing microlens array in industry and academic research are discussed and several principle conclusions are drawn.

A Dynamic Forecasting System with Applications in Production Logistics

Production logistics involve the co-ordination of ac tivities such as production and materials control (PMC), inventory management, p roduct life cycle management, etc. Those activities demand for an accurate forec asting model. However, the conventional methods of making sell and buy decision based on human forecast or conventional moving average and exponential smoothing methods is no longer be sufficient to meet the future need. Furthermore, the un derlying statistics of the market information change from time to time due to a number of reasons such as change of global economic environment, government poli cies and business risks. This demands for highly adaptive forecasting model which is robust enough to response and adapt well to the fast changes in the dat a characteristics, in other words, the trajectory of the "dynamic characteristic s" of the data. In this paper, an adaptive time-series modelling method was proposed for short -term dynamic forecasting. The method employs an autoregressive (AR) time-seri es model to carry out the forecasting process. A modified least mean square (MLM S) adaptive filter algorithm was established for adjusting the AR model coeffici ents so as to minimise the sum of squared of forecasting errors. A prototype dyn amic forecasting system was built based on the adaptive time-series modelling m ethod. Basically, the dynamic forecasting system can be divided into two phases, i.e. the Learning Phase and the Application Phase. The learning procedures star t with the determination of upper limit of the adaptation gain based on the conv ergence in the mean square criterion. Hence, the optimum ELMS filter parameters are determined using an iteration algorithm which changes each filter parameter i.e. the order, the adaptation gain andthe values initial coefficient vector on e by one inside a predetermined iteration range. High-Q lithium niobate microring resonators using lift-off metallic masks [Invited]

High-Q lithium niobate(LN) optical micro-resonators are an excellent platform for future applications in optical communications, nonlinear optics, and quantum optics. To date, high-Q factors are typically achieved in LN using either dielectric masks or femtosecond laser ablation, while the more standard and commonly available lift-off metallic masks are often believed to lead to rough sidewalls and lowered Q factors. Here, we show that LN microring resonators with strong light confinement and intrinsic Q factors over 1 million can be fabricated using optimized lift-off metallic masks and dry etching processes, corresponding to a waveguide propagation loss of ~0.3 d B=cm. The entire process is fully compatible with wafer-scale production and could be transferred to other photonic materials.

Life skill-based training is essential for intellectual disabled individuals to regain autonomy and social *** reality(VR)could deliver a nascent application that is more engaging and *** objective of this study was to evaluate the training effects of the VR-based multiple life skill training program on life skill perfor-mance,self-efficacy,memory,cognitive and behavioral functions via a multicenter randomized controlled trial.A total of 145 intellectual disabled participants were recruited for a randomized controlled trial with three in-tervention arms:VR(n=42);traditional(n=53);and control(n=50).The life skill tasks for the interventions included 1)grocery shopping,2)cooking,and 3)kitchen *** measures were performance scores for the three tasks,self-efficacy scale,digit span score,and Frontal Assessment Battery score,graded by blinded ***-after effects of each group were evaluated using separated Wilcoxon Signed-rank *** sig-nificantly improved cooking,cleaning performance,and memory span,while traditional training significantly improved shopping,cooking,and cleaning *** generalized estimating equation evaluated effects between groups adjusted for age,gender and intelligence quotient(IQ).VR had significantly larger improvement effects in cooking and cleaning than the control group and memory span compared to traditional training and controls *** appeared to be a significant confounder on the training *** work may consider developing artificial intelligence to customize programs depending on IQ level.

Study on Injection Moulding of High Precision Aspheric Plastic Lens

With the rapid development of information and multi me dia technologies, the demand for the optical plastic aspheric elements used in o pto-electronic devices, camera, optical disc and projector lens etc. has been i ncreased rapidly in the recent years. The key technologies of fabrication of asp heric plastic lens are the design and manufacturing moulds, selection of proper injection moulding equipment, and optimization of injection moulding parameters etc. In this paper, the effect of injection pressure, moulding temperature, cool ing time and injection speed on the surface profile of the lenses during injecti on and holding process is investigated. Surface quality of plastic lenses is mea sured by Talysurf Texture Measuring System. The experimental results showed that the injection pressure and moulding temperature are important parameters compar ing to cooling time and injection speed. A bit change of injection pressure or m oulding temperature will affect the property of the surface profile. Either incr easing injection pressure or mould temperature can achieve less shrinkage. Other wise, a lower injection pressure will produce more shrinkage, more air traps and a lower mould temperature results greater warp and higher shrinkage. The dynami c process of injection for optical plastic lenses is simulated by 3D Moldflow pl astic Insight software (MPI). The MPI will help us to optimize injection mouldin g parameters.

m.3243A>G MT-TL1 mutation is the most common mitochondrial DNA mutation that results in a wide spectrum of disorders in a maternally inherited pedigree. In adult patients, many present with symptoms and signs indistinguishable from acquired diseases and the correct diagnosis is often delayed after many years. Nevertheless, clues suggesting m.3243A>G usually exist early in the disease course but are only realized late. These hints, from the evolution of symptoms and signs, family background, investigation results, or a combination of these, enable the physician to make the correct diagnosis early, which is important for appropriate treatment and better patient care. As with other inheritable diseases, genetic counselling should be offered regarding the disease management, inheritance mode, recurrence risk, usefulness and limitations of genetic testing and reproductive options.
