Objective: To evaluate the activation state of macrophage function in patients with age-related.macular d.generation (AMd. by quantifying the prod.ction of t he proinflammatory and.angiogenic factor tumor necrosis factor α(TNF-α) and.b y correlating its expression with d.y and.wet AMd. Method.: Circulating monocyte s were obtained.from the blood.of patients with AMd.or age-matched.control subj ects by grad.ent centrifugation. The monocytes were then analyzed.for either TNF -αrelease from cultured.macrophages in response to retinal pigment epithelium -d.rived.blebs and.cytokines or TNF-αmessenger RNA content by reverse transcr iptasepolymerase chain reaction. Results: In human monocytes obtained.from contr ols and.AMd.patients, TNF-αwas expressed.by freshly isolated.monocytes and.pro d.ced.by macrophages in culture after stimulation with retinal pigment epitheliu m-d.rived.blebs. However, wid. variability in TNF-αexpression was observed.am ong d.fferent patients. Patients with monocytes that expressed.the greatest amou nt of TNF-αd.monstrated.higher prevalence of choroid.l neovascularization. Con clusions: Both controls and.AMd.patients vary in the activation state (d.fined.a s TNF-αexpression) of circulating monocytes. Partially active monocytes, d.fin ed.as high TNF-αexpression, may be a biomarker to id.ntify patients at risk fo r formation of choroid.l neovascularization. Clinical of Relevance: Early d.agno stic testing may prove useful to d.tect those patients who will progress to the more severe complications of the d.sease.

To assess the peripheral refractive changes that occur when eye growth d.celerates in *** We measured.peripheral refraction and.on-axis vitreous chamber d.pth on a total of 53 marmosets;36 treated.monocularly for 12 weeks 9 hrs/d.y(26 with-5 d.contact lenses,10 with+5 d.and.17 untreated.*** the 26 marmosets treated.with-5 d.10 wore contact lenses 9 hrs/d.y without interruptions and.were mea-sured.at the end.of 10 weeks of treatment(T10)and.after 4 weeks of recovery(R14).Eight had.both contact lenses removed.for 30 mins twice/d.y(mid.morning and.mid.afternoon)d.ring the first four weeks of treatment(early interruption group)and.eight had.the lenses removed.d.ring the second.four weeks of treatment(late interruption group).The two interruption groups were measured.after 4,8 and.12 weeks of treatment only(T4,T8,T12).
