Acute venous thromboembolism in acute pancreatitis based on the severity:a retrospective cohort study

Objective: Acute pancreatitis (AP) results in systemic inflammatory responses and activates coagulation pathways. We intend to investigate the risk and hospital outcomes of acute venous thromboembolisms (VTE) in patients with ***: We retrospectively analyzed patients with AP from 2016 to 2019 using the National Inpatient Sample database. Primary outcome was the effect of VTE on the length of stay, inpatient costs, and mortality. Hierarchical multivariate logistic regression models were built using univariate ***: The study included 909,354 weighted discharges with AP. 2.1% of cases had an acute VTE. The length of stay was 5.9 days longer in the hospital of AP patients with VTE compared to AP with no VTE (P <.001). Total hospital charge per patient was $71,914 in patients with VTE compared to AP with no VTE (P < .001). Mortality was higher in AP patients with VTE compared to AP with no VTE (adjusted odds ratio [AOR] 4.2, 95% confidence interval [CI]: 3.4-5.3,P <.001). AP was associated with an increased VTE risk during inpatient stay (AOR 1.06, 95% CI 1.04-1.1,P <.001) There was an increased association of lower and upper extremity deep venous thrombosis with AP without necrosis (AOR 6.9, 95% CI 6.4-7.4,P <.001) and AP with infected necrosis (AOR 12.2, 95% CI 10.6-14.1,P <.001) but not in AP without necrosis (AOR 0.77, 95% CI 0.74-0.81,P <.001).Conclusion: VTE in AP increases length of stay and inpatient costs. The prognosis is poor in such patients, with increased inpatient mortality compared to no VTE. AP with necrosis can increase chances of all VTE subtypes;however, AP without necrosis does not increase upper and lower extremity VTE risk.

Patients presenting for colonoscopy: A great opportunity to screen for sleep apnea

AIMTo discover the prevalence and the feasibility of screening for obstructive sleep apnea (OSA) in patients presenting for routine colonoscopy. METHODSAdult patients having a colonoscopy for routine indications at our outpatient endoscopy center were eligible if they did not carry a diagnosis of OSA or had not had a prior sleep study. All patients were administered the Berlin questionnaire prior to the procedure. Mallampati, neck circumference, height, weight, and BMI were obtained for each patient. Patients were observed for any drops in oxygen saturation 10 s. Patients were determined to be high-risk if they met at least 2 of the 3 symptom categories for the Berlin questionnaire. RESULTSA total of 60 patients were enrolled and completed the study; mean age was 56 years (range 23-72 year). Twenty-six patients had a positive Berlin questionnaire (43.3%), 31 patients had a negative Berlin questionnaire (51.6%) and 3 patients had an equivocal result (5.0%). Patients with a positive Berlin questionnaire were more likely to be of increased weight (mean 210.5 lbs vs mean 169.8 lbs, P = 0.003), increased BMI (33.0 kg/m2vs 26.8 kg/m2, P = 0.0016), and have an increased neck circumference (38.4 cm vs 35.5 cm, P = 0.012). Patients with a positive Berlin questionnaire were more likely to have a drop in oxygen saturation vs 36.4%, P = 0.01). Patients with snoring were more likely to have a positive Berlin questionnaire (8/9 patients vs 1/31 patients with negative Berlin questionnaire; P = 0.0045). CONCLUSIONRisk for OSA is extremely common in a population presenting for a routine colonoscopy, and screening at the time of a colonoscopy offers an excellent opportunity to identify these patients.

The Two Cultures of Science: Implications for University-Industry Relationships in the U.S. Agriculture Biotechnology

Partnerships between U.S. universities and industries have existed for several decades and in recent years have become generally more varied, wider in scope, more aggressive and experimental and higher in public visibility. In addition, in the last few decades, public and private interests have advocated for government policies and laws to globally promote the commercialization of university science. This paper examines the persistence or convergence of the two cultures of science and the implications of this commercialization for university-industry relationships in agriculture biotechnology. The perceptions and values of over 200 U.S. university and industry scientists, managers and administrators who participate in or oversee research collaborations in agricultural biotechnology were analyzed. The findings revealed that the participants in these research relationships continue to perceive very distinct cultures of science and identify a wide range of concerns and disadvantages of these partnerships. Several actions were discussed to ensure that the two cultures serve complementary roles and that they maximize the public benefits from these increasing collaborations.

Interracial differences in prostate cancer progression among patients from the United States, China and Japan

Although previous studies indicate interracial differences in prostate can-cer epidemiology based on gene expression profiles among patients from the United States, China and Japan, evidence at the ge- netic and phenotypic levels that these diffe-rences exist and manifest along ethnic lines has been sparse. Recent studies, however, suggest that genetic differences, such as the lower incidence of Chinese pros-tate cancers harboring TMPRSS2-ERG translocations compared to patients from Western countries, should be carefully con-sidered in the context of genotypic and phenotypic differences among interracial groups. New, more efficient technologies need to be developed to validate genetic, gene expression and/or phenotypic differ-ences associated with prostate cancer tissue specimens obtained from interracial groups, to establish reliable clinical standards that take racial/ethnic data into account to improve the diagnosis, prognosis and treat-ment of patients with prostate cancer.

Overexpression of the promyelocytic leukemia gene suppresses growth of human bladder cancer cells by inducing Gl cell cycle arrest and apoptosis

Objectives To examine the anti-oncogenic effects of promyelocytic leukemia (PML) on bladder cancer and to explore its molecular mechanisms of growth *** Wild-type PML was transfected into bladder cancer cells (5637 cell) and expressed in a replication-deficient adenovirus-mediated gene delivery system and introduced into human bladder cancer cells (5637 cell) in vitro and in vivo. The effect and mechanisms of the PML gene in cell growth, clonogenicity, and tumorigenicity of bladder cancer cells were studied using in vitro and in vivo growth assays, soft agar colony-forming assay, cell cycle analysis, apoptosis assay and in vivo tumorigenicity *** Overexpression of PML in 5637 cells significantly reduced their growth rate and clonogenicity on soft agar. PML suppressed bladder cancer cell growth by inducing G1 cell cycle arrest and apoptosis. Adenovirus-mediated PML ( Ad-PML) significantly suppressed the tumorigenicity and growth of bladder cancer cells. Intratumoral injection of Ad-PML into tumors induced by 5637 cells dramatically suppressed their *** The results indicated that overexpression of PML protein may promote efficient growth inhibition of human bladder cancer cells by inducing G1 cell cycle arrest and apoptosis, and adenovirus-mediated PML (Ad-PML) expression efficiently suppresses human bladder cancer growth.

The discovery of pressure-induced superconductivity in helimagnets(CrAs,MnP)has attracted considerable interest in understanding the relationship between complex magnetism and unconventional ***,the nature of the magnetism and magnetic interactions that drive the unusual double-helical magnetic order in these materials remains ***,we report neutron scattering measurements of magnetic excitations in CrAs single crystals at ambient *** experiments reveal well defined spin wave excitations up to about 150 meV with a pseudogap below 7 meV,which can be effectively described by the Heisenberg model with nearest neighbor exchange *** surprisingly,the spin excitations are largely quenched above the Néel temperature,in contrast to cuprates and iron pnictides where the spectral weight is mostly preserved in the paramagnetic *** results suggest that the helimagnetic order is driven by strongly frustrated exchange interactions,and that CrAs is at the verge of itinerant and correlation-induced localized states,which is therefore highly pressure-tunable and favorable for superconductivity.
