Vitamin D and Secondary Hyperparathyroidism in HIV Infected Patients Taking Antiretroviral Therapy

Objective: Due to the lack of studies assessing hypovitaminosis D and secondary hyperparathyroidism in Brazilian HIV-infected population, especially in the northeastern population, this study aimed to determine the profile of these conditions in patients infected with HIV and its correlation with immuno-virological, sociodemographic data and associated comorbidities. Methods: Comparison studies were obtained from routine clinical samples of HIV infected patients submitted for 25-OH Vitamin D, PTH and alkaline phosphatase determination. Results: A total of 78 patients were included, 42 (53.8%) males, mean age 45.7 years. Antiretroviral regimens most used in this study were Zidovudine/Lamivudine/Efavirenz 17.9%, Tenofovir/Lamivudine/Efavirenz 17.9%,Tenofovir/Lamivudine/Atazanavir-r 15.4%. The mean value CD4 count was 592.1 ± 247.2 cells/mm3, CD8 cell count was 1026.5 ± 467.3 cells/mm3, mean detectable viral load was 2220 ± 15703 copies and CD4/CD8 ratio was 0.63 ± 0.33. A total of 34 vitamin D dosages were collected with 41.2% representing sufficient amount and 58.8% insufficient. Alkaline Phosphatase (ALP) dosage was elevated in 49.3% (N=35) of the patients. Parathormone (PTH) was elevated in 18% (N = 11). Among patients with elevated PTH levels, 81.9% had elevated levels of ALP (p = 0.01). In the group of patients with high levels of ALP, 45.7% had a CD4 count 3 (p = 0.02). There was no significant difference in vitamin D related to gender (p = 0.21), age (p = 0.23), CD4 count (p = 0.26), suppressed viral load (p = 0.44) or blood glucose (p = 0.45). Conclusions: This study evidenced a high prevalence of Vitamin D insufficiency in Northeast Brazil, which suggests HIV infection correlation. A high prevalence of Hyperparathyroidism was detected and related with inflammatory condition persistence and low CD4 count. We suggest improve vitamin D follow up and measurements in this population with better CD4 count control to avoid future osteoarticular complications of HIV

A combined computational pipeline to detect circular RNAs in human cancer cells under hypoxic stress

Self-reported participation restrictions in normal hearing individuals in thyroid ontogeny: Evidence of subclinical changes

Purpose: To verify evidence of subclinical alterations through self-reported participation restrictions in normal hearing individuals and congenital hypothyroidism ***: An analytical, quantitative, cross-sectional exploratory study with a 1:1 ratio, consisting of a convenience sample of 86 normal hearing individuals with(n = 42) and without(n = 44) congenital hypothyroidism diagnosis. All participants underwent tonal and speech audiometry, immitance and distortion product otoacoustic emissions. The researchers excluded people with hearing loss, genetic syndromes and metabolic diseases. The instrument used for evaluate of self-reported participation restrictions was the HHIE-adapted questionnaire, composed of 25 questions, 12 of which were social domain and 13 emotional domain. Student's ttest and chi-square test were used for statistical analysis at a significance level of 5%.Results: There was a significant(p derate(40.5%)and higher prevalence of association with clinical factors and adherence to ***: The findings indicate that self-reported participation restrictions in normal hearing individuals with congenital hypothyroidism was more significant than in the non-exposed group, suggesting evidence of subclinical auditory abnormalities in this population.

This research investigates the soil conditioning effect of sugar cane fields with biochar produced from sugarcane *** influence on the physicochemical and microbiological conditions of the soil and the agronomic performance of sugarcane seedlings was *** this end,the biochar from sugarcane bagasse was produced by pyrolysis in a double drum oven and mixed with soil at rates of 0,1%,3%,and 5%(in weight).The experiment was installed in a greenhouse with two evaluation periods(30 days and 60 days)and five *** pot received a pre-sprouted cane seedling,and the analysis of colony-forming units(bacteria and fungi),physicochemical characteristics of the soil,and growth and development of sugarcane were performed per *** biochar showed a high fixed carbon content(72%)and a specific surface area estimated by the adsorption of methylene blue of 50 m2 g−***,its structure is porous and contains important nutrients(e.g.,CaO,K2O,and P2O5).In the periods evaluated,the applications of the 3%and 5%(in weight)rates of biochar reduced most of the soil fertility ***,the chemical analyses indicated that the sugarcane field soil collected already possessed high *** microbiota was influenced,but only the application of the 3%(in weight)rate at 60 days after planting showed a significant positive effect on the number of bacteria forming units,with an increase of approximately 385%.On the other hand,no significant positive effect on sugarcane development was ***,biochar application in high-fertility sugarcane plantation soil did not result in gains for sugarcane development in the periods evaluated.

Grade 4 febrile neutropenia and Fournier's Syndrome associated with triple therapy for hepatitis C virus:A case report

The use of triple therapy for hepatitis C not only increases the rate of sustained virological responses compared with the use of only interferon and ribavirin(RBV) but also leads to an increased number of side effects. The subject of this study was a 53-year-old male who was cirrhotic with hepatitis C virus genotype 1 A and was a previous null non-responder. We initially attempted retreatment with boceprevir(BOC), Peginterferon and RBV, and a decrease in viral load was observed in the 8th week. In week 12, he presented with disorientation, flapping, fever, tachypnea, arterial hypotension and tachycardia. He also exhibitedleucopenia with neutropenia. Cefepime and filgrastim were initiated, and treatment for hepatitis C was suspended. A myelogram revealed hypoplasia, cytotoxicity and maturational retardation. After 48 h, he developed bilateral inguinal erythema that evolved throughout the perineal area to the root of the thighs, with exulcerations and an outflow of seropurulent secretions. Because we hypothesized that he was suffering from Fournier's Syndrome, treatment was replaced with the antibiotics imipenem, linezolid and clindamycin. After this new treatment paradigm was initiated, his lesions regressed without requiring surgical debridement. Triple therapy requires knowledge regarding the management of adverse effects and drug interactions; it also requires an understanding of the importance of respecting the guidelines for the withdrawal of treatment. In this case report, we observed an adverse event that had not been previously reported in the literature with the use of BOC.

Atrophic gastritis: Risk factor for esophageal squamous cell carcinoma in a Latin-American population

AIM: To study the association between atrophic gastritis (AG) and esophageal squamous cell carcinoma (ESCC) in a Latin-America population. METHODS: A case-control study was performed at two reference Brazilian hospitals including patients diagnosed with advanced ESCC and dyspeptic patients who had been subjected to upper gastrointestinal endoscopy, with biopsies of the gastric antrum and *** cases with ESCC were reviewed by a single pathologist, who applied standard criteria for the diagnosis of mucosal atrophy, intestinal metaplasia, and dysplasia, all classified as AG. The data on the patients' age, sex, smoking status, and alcohol consumption were collected from clinical records, and any missing information was completed by telephone interview. The association between AG and ESCC was assessed by means of univariate and multiple conditional logistic regressions. RESULTS: Most patients were male, and the median age was 59 years (range: 37-79 years) in both the ESCC and control groups. Univariate analysis showed that an intake of ethanol greater than 32 g/d was an independent risk factor that increased the odds of ESCC 7.57 times (P = 0.014); upon multiple analysis, alcohol intake of ethanol greater than 32 g/d exhibited a risk of 4.54 (P = 0.081), as adjusted for AG and smoking. Smoking was shown to be an independent risk factor that increased the odds of ESCC 14.55 times (P = 0.011) for individuals who smoked 0 to 51 packs/year and 21.40 times (P = 0.006) for those who smoked more than 51 packs/year. Upon multiple analyses, those who smoked up to 51 packs/year exhibited a risk of 7.85 (P = 0.058), and those who smoked more than 51 packs/ year had a risk 11.57 times higher (P = 0.04), as adjusted for AG and alcohol consumption. AG proved to be a risk factor that increased the odds of ESCC 5.33 times (95%CI: 1.55-18.30, P = 0.008) according to the results of univariate conditional logistic regression. Drought stress is major abiotic stress that affects soybean ***,it is widely desirable that soybean becomes more tolerant to *** provide insights into regulatory mechanisms of the stress response,we compared the global gene expression profiles from leaves of two soybean genotypes that display different responses to water-deficit(BR 16 and Embrapa 48,drought-sensitive and droughttolerant,respectively).After the RNA-seq analysis,a total of 5335 down-regulated and 3170 up-regulated genes were identified in the *** the other hand,the number of genes differentially expressed was markedly lower in the Embrapa 48,355 up-regulated and 471 down-regulated ***,induction and expression of protein kinases and transcription factors indicated signaling cascades involved in the drought ***,the results suggest that the metabolism of pectin is differently modulated in response to drought stress and may play a role in the soybean defense mechanism against *** occurs via an increase of the cell wall plasticity and crosslink,which contributed to a higher hydraulic conductance(Kf)and relative water content(RWC%).The drought-tolerance mechanism of the Embrapa 48 genotype involves remodeling of the cell wall and increase of the hydraulic conductance to the maintenance of cell turgor and metabolic processes,resulting in the highest leaf RWC,photosynthetic rate(A),transpiration(E)and carboxylation(A/Ci).Thus,we concluded that the cell wall adjustment under drought is important for a more efficient water use which promoted a more active photosynthetic metabolism,maintaining higher plant growth under drought stress.

Microbial metabolites produced during fermentation migrate into the coffee and can influence the decrease in seed viability and coffee *** study evaluated the effects of physiological changes in seed viability on the sensory quality of the beverage using starter cultures through self-induced anaerobic fermentation(SIAF)in Coffea arabica *** natural and pulped *** yeasts were monitored by real-time polymerase chain reaction(qPCR).High-performance liquid chromatography(HPLC)detected citric,malic,and succinic acids in all fermented ***,lactic acid was mainly identified in those coffees processed by the SIAF *** compounds(40)were detected by gas chromatography-mass spectrometry(GC-MS).Alterations in physiological quality were identified,with decreased embryonic viability and cell membrane damage by tetrazolium and electrical conductivity *** fermented coffees obtained scores above 80 *** Torulospora delbrueckii yeast got the best score(86.50)in pulped coffee,and the Candida parapsilosis yeast received the highest score(85.90)in the natural coffee using the SIAF *** coffees were characterized by aromas and flavors of citrus,caramel,honey,chocolate,and *** coffee fermentation process with yeast inoculation affected the coffee bean viability but not the beverage’s sensory quality,indicating that the use of yeast starters by SIAF favors the production of specialty coffees with differentiated sensory characteristics.
