Comprehensive review of hepatocellular carcinoma with portal vein tumor thrombus:State of art and future perspectives

Background:Despite advances in the diagnosis of patients with hepatocellular carcinoma(HCC),70%-80%of patients are diagnosed with advanced stage *** vein tumor thrombus(PVTT)is among the most ominous signs of advanced stage disease and has been associated with poor survival if *** sources:A systematic search of MEDLINE(PubMed),Embase,Cochrane Library and Database for Systematic Reviews(CDSR),Google Scholar,and National Institute for Health and Clinical Excellence(NICE)databases until December 2022 was conducted using free text and MeSH terms:hepatocellular carcinoma,portal vein tumor thrombus,portal vein thrombosis,vascular invasion,liver and/or hepatic resection,liver transplantation,and systematic ***:Centers of surgical excellence have reported promising results related to the individualized surgical management of portal thrombus versus arterial chemoembolization or systemic *** elements to the individualized surgical management of HCC and portal thrombus include precise classification of the portal vein tumor thrombus,accurate identification of the subgroups of patients who may benefit from resection,as well as meticulous surgical *** review addressed five specific areas:(a)formation of PVTT;(b)classifications of PVTT;(c)controversies related to clinical guidelines;(d)surgical treatments versus non-surgical approaches;and(e)characterization of surgical techniques correlated with classifications of ***:Current evidence from Chinese and Japanese high-volume centers demonstrated that patients with HCC and associated PVTT can be managed with surgical resection with acceptable results.

Second-harmonic generation in periodic fork-shaped χ^((2)) gratings at oblique incidence

Three-dimensional(3D) nonlinear photonic crystals have received intensive interest as an ideal platform to study nonlinear wave interactions and explore their applications. Periodic fork-shaped gratings are extremely important in this context because they are capable of generating second-harmonic vortex beams from a fundamental Gaussian wave, which has versatile applications in optical trapping and materials engineering. However, previous studies mainly focused on the normal incidence of the fundamental Gaussian beam, resulting in symmetric emissions of the second-harmonic vortices. Here we present an experimental study on second-harmonic vortex generation in periodic fork-shaped gratings at oblique incidence, in comparison with the case of normal incidence. More quasi-phase-matching resonant wavelengths have been observed at oblique incidence, and the second-harmonic emissions become asymmetric against the incident *** results agree well with theoretic explanations. The oblique incidence of the fundamental wave is also used for the generation of second-harmonic Bessel beams with uniform azimuthal intensity distributions. Our study is important for a deeper understanding of nonlinear interactions in a 3D periodic medium. It also paves the way toward achieving highquality structured beams at new frequencies, which is important for manipulation of the orbital angular momentum of light.

Composite ceramic phosphor(CCP)is a candidate light-conversion material to obtain the high-quality laser lighting *** based on the transmissive configuration model could not simultaneously meet the requirements of angular color uniformity and high thermal *** this study,a novel composite structure ceramic was designed,including Al_(2)O_(3)-YAG:Ce/YAG layered ceramic with a size of 1 mm×1 mm for lighting,and Al_(2)O_(3) ceramic(φ=16.0 mm)was used as the wrapping material due to its outstanding thermal *** prepared ceramics exhibited excellent thermal performance and no yellow ring *** this design,we achieved the match of the intensity distribution of the blue and yellow lights,resulting in a high angular color uniformity of 0.9 with a view angle of±80°.All ceramics showed no luminous saturation phenomenon,even the laser power density was increased up to 47.51 w/mm^(2).A high-brightness white-light source with a luminous flux of 618 lm,a luminous efficiency of 126 lm/w,a CCT of 6615 K,and a CRI of 69.9 was obtained in the transmissive *** particular,the surface temperature of the ceramic was as low as 74.1℃ under a high laser radiation(47.51 w/mm^(2)).These results indicate that Al_(2)O_(3)/Al_(2)O_(3)-YAG:Ce/YAG composite structure ceramic is a promising luminescent material in the high-power laser lighting applications.

Grapefruit-seed extract attenuates ethanol-and stress-induced gastric lesions via activation of prostaglandin, nitric oxide and sensory nerve pathways

AIM: Grapefruit-seed extract (GSE) containing flavonoids, possesses antibacterial and antioxidative properties but whether it influences the gastric defense mechanism and gastroprotection against ethanol- and stress-induced gastric lesions remains unknown. METHODS: we compared the effects of GSE on gastric mucosal lesions induced in rats by topical application of 100% ethanol or 3.5 h of water immersion and restraint stress (wRS) with or without (A) inhibition of cyclooxygenase (COX)-1 activity by indomethacin and rofecoxib, the selective COX-2 inhibitor, (B) suppression of NO-synthase with L-NNA (20 mg/kg ip), and (C) inactivation by capsaicin (125 mg/kg sc) of sensory nerves with or without intragastric (ig) pretreatment with GSE applied 30 min prior to ethanol or wRS. One hour after ethanol and 3.5 h after the end of wRS, the number and area of gastric lesions were measured by planimetry, the gastric blood flow (GBF) was assessed by H2-gas clearance technique and plasma gastrin levels and the gastric mucosal generation of PGE2, superoxide dismutase (SOD) activity and malonyldialdehyde (MDA) concentration, as an index of lipid peroxidation were determined. RESULTS: Ethanol and wRS caused gastric lesions accompanied by the significant fall in the GBF and SOD activity and the rise in the mucosal MDA content. Pretreatment with GSE (8-64 mg/kg i g) dose-dependently attenuated gastric lesions induced by 100% ethanol and wRS; the dose reducing these lesions by 50% (ID50) was 25 and 36 mg/kg, respectively, and this protective effect was similar to that obtained with methyl PGE2 analog (5 μg/kg i g). GSE significantly raised the GBF, mucosal generation of PGE2, SOD activity and plasma gastrin levels while attenuating MDA content. Inhibition of PGE2 generation with indomethacin or rofecoxib and suppression of NO synthase by L-NNA or capsaicin denervation reversed the GSE-induced protection and the accompanying hyperemia. Ischemic preconditioning inhibits development of edematous cerulein-induced pancreatitis: Involvement of cyclooxygenases and heat shock protein 70

AIM: To determine whether ischemic preconditioning (IP)affects the development of edematous cerulein-induced pancreatitis and to assess the role of cyclooxygenase-1 (COX-1), COX-2, and heat shock protein 70 (HSP 70) in this ***: In male wistar rats, IP was performed by damping of celiac artery (twice for 5 min at 5-min intervals).Thirty minutes after IP or sham operation, acute pancreatitis was induced by cerulein. Activity of COX-1 or COX-2 was inhibited by resveratrol or rofecoxib, respectively (10 mg/kg).RESULTS: IP significantly reduced pancreatic damage in cerulein-induced pancreatitis as demonstrated by the improvement of pancreas histology, reduction in serum lipase and poly-C ribonuclease activity, and serum concentration of pro-inflammatory interleukin (IL)-1β.Also, IP attenuated the pancreatitis-evoked fall in pancreatic blood flow and pancreatic DNA *** level of anti-inflammatory IL-10 was not affected by IP. Cerulein-induced pancreatitis and IP increased the content of HSP 70 in the pancreas. Maximal increase in HSP 70 was observed when IP was combined with cerulein-induced pancreatitis. Inhibition of COXs, especially COX-2, reduced the protective effect of IP in edematous ***: Our results indicate that IP reduces pancreatic damage in cerulein-induced pancreatitis and this effect, at least in part, depends on the activity of COXs and pancreatic production of HSP 70.

Achieving a high color rendering index(CRI)and luminous stability in single-structured Ce:Y_(3)Al_(5)O_(12)(Ce:YAG)phosphor ceramics(PCs)is crucial for high-power white light-emitting diodes or laser diodes(LEDs/LDs).However,cyan valleys and insufficient amounts of the red component in the Ce:YAG emission spectra significantly limit their real *** this work,a series of Ce,Mn:Y_(3)(Al,Sc)_(2)Al_(3)O_(12)(Ce,Mn:YSAG)PCs were fabricated by vacuum sintering,and efficient spectral regulation was realized for full-color *** cyan valley was filled by the blueshifted emission peak of Ce^(3+)via Sc^(3+)*** orange‒red emission at approximately 580 nm was effectively supplemented via Mn^(2+)*** particular,CRI of Ce,Mn:YSAG increased from 56.4 to 85.8,a 52%increase compared with that of Ce:YAG under high-power LED excitation,and the operating temperature was stable at approximately 50℃for long working ***,CRI of 80.9 could still be obtained for PC-based white *** results indicated that Ce,Mn:YSAG PC,which has excellent CRI and luminous stability,is an extremely promising color convertor for high-power white LEDs/LDs.

Superplasticity of fine-grained Mg-10Li alloy prepared by severe plastic deformation and understanding its deformation mechanisms

The superplastic behavior and associated deformation mechanisms of a fine-grained Mg-10.1 Li-0.8Al-0.6Zn alloy(LAZ1011)with a grain size of 3.2μm,primarily composed of the BCCβphase and a small amount of the HCPαphase,were examined in a temperature range of 473 K to 623 *** microstructural refinement of this alloy was achieved by employing high-ratio differential speed *** best superplasticity was achieved at 523 K and at strain rates of 10^(-4)-5×10^(-4)s^(-1),where tensile elongations of 550±600%were *** the heating and holding stage of the tensile samples prior to tensile loading,a significant increase in grain size was observed at temperatures above 573 ***,it was important to consider this effect when analyzing and understanding the superplastic deformation behavior and *** the investigated strain rate range,the superplastic flow at low strain rates was governed by lattice diffusion-controlled grain boundary sliding,while at high strain rates,lattice diffusion-controlled dislocation climb creep was the rate-controlling deformation *** was concluded that solute drag creep is unlikely to *** the late stages of deformation at 523 K,it was observed that grain boundary sliding led to the agglomeration of theαphase,resulting in significant strain *** mechanism maps were constructed forβ-Mg-Li alloys in the form of 2D and 3D formats as a function of strain rate,stress,temperature,and grain size,using the constitutive equations for various deformation mechanisms derived based on the data of the current tests.
