Objective: Patients with radioactive iodine-refractory differentiated thyroid cancer(RAIR-DTC) are often diagnosed with delay and constrained to limited treatment options. The correlation between RAI refractoriness and the underlying genetic characteristics has not been extensively ***: Adult patients with distant metastatic DTC were enrolled and assigned to undergo next-generation sequencing of a customized 26-gene panel(Thyro Lead). Patients were classified into RAIR-DTC or non-RAIR groups to determine the differences in clinicopathological and molecular characteristics. Molecular risk stratification(MRS) was constructed based on the association between molecular alterations identified and RAI refractoriness, and the results were classified as high, intermediate or low ***: A total of 220 patients with distant metastases were included, 63.2% of whom were identified as RAIRDTC. Genetic alterations were identified in 90% of all the patients, with BRAF(59.7% vs. 17.3%), TERT promoter(43.9% vs. 7.4%), and TP53 mutations(11.5% vs. 3.7%) being more prevalent in the RAIR-DTC group than in the non-RAIR group, except for RET fusions(15.8% vs. 39.5%), which had the opposite pattern. BRAF and TERT promoter are independent predictors of RAIR-DTC, accounting for 67.6% of patients with RAIR-DTC. MRS was strongly associated with RAI refractoriness(Plinical outcomes and directing precision-based therapeutic interventions.

Efficacy and safety of Liqingtong granules in the treatment of hyperuricemia:A randomized double-blind placebo-controlled pilot clinical trial

Objective:To investigate the efficacy and safety of Liqingtong(LQT)granules in patients with dampness-heat ***:A randomized,double-blind,placebo-controlled pilot trial was conducted at the 983rd Hos-pital of the Joint Logistic Support Force of the People's Liberation Army from March 15,2023,to August 10,*** total,119 participants were enrolled in this trial,and participants were given either LQT granules or placebo for 60 days based on a health *** primary outcome was serum uric acid(SUA)level,and the secondary outcome was the traditional Chinese medicine(TCM)symptom score,measured on days 0,30,and *** indicators,including liver function,kidney function,blood routine,glucose,blood lipid,blood pressure,and heart rate were tested on days 0 and 60 of the *** data were analyzed using Prism 9 software,and the significance level was set at Plinical *** information,including SUA levels,TCM symptom scores,and other clinical characteristics,did not differ between the *** the end of the trial,compared with baseline values,the SUA levels in the LQT granule group decreased(Pline values,the total TCM symptom scores in the LQT granule group decreased(P<0.001),with no change in the placebo group(P=0.136).Safety indicators did not differ significantly between the two ***:The pilot trial demonstrated the potential of LQT granules to lower SUA levels and improve symptoms of dampness and heat.

White Radish polyphenols improve diet-induced obesity by regulating the metabolism of bile acids via gut-liver axis

Natural plant or functional food-derived biologically active ingredients have recently raised considerable interest in their antiobesity effect. However， the antiobesity role of polyphenols of white radish roots remains unknown. This study examined the therapeutic efficacy and underlying mechanisms of polyphenols extract from white radish roots (RPE) on obese mice. RPE administration significantly decreased the weight gain in high-fat diet fed (HFD) mouse models. RPE intervention also increased serum glutathione transferase (GSH-Px) and superoxide dismutase (SOD) levels， and decreased malondialdehyde (MDA)， tumor necrosis factor-α (TNF-α) and lipopolysaccharide (LPS) levels in the HFD-fed mice. The intervention led to an escalation in the proportion of beneficial bacteria (Akkermansia， Clostridia)， and a corresponding decline in the fraction of harmful bacteria (Proteobacteria). According to liquid chromatography-mass spectrometry (LC-MS)-based gut metabolomics， the RPE intervention notably influenced several metabolic pathways， including those involved in protein digestion and absorption， central carbon metabolism related to cancer， and the biosynthesis of steroid hormones. Additionally， RPE treatment led to a reduction in the intestinal microbes possessing bile-salt hydrolase (BSH) activity， whereas increased the expression levels of the bile acid (BA) synthesis enzyme cholesterol 7α-hydroxylase (Cyp7a1). This led to increase hepatic production and fecal excretion of BAs， reduce hepatic cholesterol， and decrease lipogenesis. Treated HDF mice showed mitigation of hepatic steatosis and inflammation through gut-liver axis modulation by RPE interference. These findings together offer a significant understanding of the potential of RPE as an anti-obesity treatment and could assist in developing strategies for weight loss.

Objective:This study investigated the clinical significance of DICER1 mutations in patients with distant metastatic follicular cell-derived thyroid cancer(FDTC).Methods:This study included 310 Chinese patients with distant metastatic *** analyzed the interactions between DICER1 mutations and other gene alterations and compared the clinicopathological characteristics of patients with pathogenic(P)or likely pathogenic(LP)DICER1 mutations(n=9),other gene alterations(n=253),and no gene alterations(n=37).To compare FDTCs with different drivers,isolated BRAFV600E,RAS mutations,and RET fusions were compared with isolated DICER1 ***:The prevalence of DICER1 mutations was 6.5%(20/310)in the patient *** patients with DICER1 mutations,45%(9/20)harbored P or LP DICER1 variants and 55%(11/20)harbored DICER1 variants of uncertain significance(VUS).The coexistence of DICER1 mutations and other gene alterations was detected in65%(13/20)of *** with VUS,P or LP DICER1 variants were almost mutually exclusive with early driver alterations(such as BRAFV600E)(11.1%vs.81.8%,P=0.002)and more coexisted with late-hit events,particularly TP53 mutations(44.4%vs.27.3%,P=0.642).Clinically,compared with the no alteration and other alteration groups,the DICER1 mutation group exhibited larger primary tumors,higher poorly differentiated thyroid cancer proportion,more extrathyroidal extension,more extrapulmonary metastases,and higher radioactive iodine-refractory proportion(all Plinical cha

The scope and variety of the metabolic intermediates from the mitochondrial tricarboxylic acid(TCA)cycle that are engaged in epigenetic regulation of the chromatin function in the nucleus raise an outstanding question about how timely and precise supply/consumption of these metabolites is achieved in the *** report here the identification of a nonclassical TCA cycle in the nucleus(nTCA cycle).We found that all the TCA cycle-associated enzymes including citrate synthase(CS),aconitase 2(ACO2),isocitrate dehydrogenase 3(IDH3),oxoglutarate dehydrogenase(OGDH),succinyl-CoA synthetase(SCS),fumarate hydratase(FH),and malate dehydrogenase 2(MDH2),except for succinate dehydrogenase(SDH),a component of electron transport chain for generating ATP,exist in the *** showed that these nuclear enzymes catalyze an incomplete TCA cycle similar to that found in *** propose that the nTCA cycle is implemented mainly to generate/consume metabolic intermediates,not for energy *** demonstrated that the nTCA cycle is intrinsically linked to chromatin dynamics and transcription ***,our study uncovers the existence of a nonclassical TCA cycle in the nucleus that links the metabolic pathway to epigenetic regulation.

Dengue, an acute febrile disease transmitted byAedes mosquitoes, is caused by the dengue virus (DENV), presenting a formidable challenge to global public health. By examining clues from ancient Chinese books and conducting a comprehensive review, this study elucidates the characteristics of potential dengue epidemics in China prior to 1978. This evidence indicates that China may not have experience dengue epidemics before 1840. During 1840-1949, however, it experienced a noticeable dengue occurrence and prevalence in the 1870s, 1920s, and 1940s. Then from 1949 to 1978, only sporadic reports were accounted. The disparity in the frequency of dengue occurrences across three time periods suggests that the persistent characteristic of dengue epidemics in China primarily arises from imported cases resulting from international exchanges, subsequently leading to local outbreaks influenced by global epidemic trend. This research offers a novel perspective on retrospectively examining the historical trajectory of dengue epidemics and provides valuable insights into exploration of DENV epidemic patterns.

用Pro/E软件建立喷头内流道的三维实体模型,选择RNGk-ε模型在CFD软件Fluent中模拟了雨鸟30PSH型摇臂式喷头在10种入口压力和4种主喷嘴直径组合下的内流道流场,分析了喷头主副喷嘴的流量、入口压力与出口平均速度等参数的关系。研究结果表明:主喷嘴直径增大时,副喷嘴流量几乎不变;主、副喷嘴的流量分配比例由主喷嘴直径决定,与入口压力无关。入口压力增大,主喷嘴出口平均速度增大,但副喷嘴出口平均速度不变。喷头主、副喷嘴的平均湍动能随入口压力增大而增大,不受主喷嘴直径变化的影响。主喷嘴出口静压力、湍动能和速度的标准差、副喷嘴出口静压力标准差与入口压力近似成正比;而副喷嘴出口湍动能和速度的标准差随主喷嘴直径或入口压力增大产生较大的无规律变化。喷头内流道流场的可视化结果显示喷头副喷嘴与弯管连接处静压力较大,接近喷头入口静压力。
