胃肿瘤标志物对胃癌新辅助治疗后病理完全缓解的预测价值

目的:探究局部进展期胃癌(locally advanced gastric cancer,LAGC)新辅助治疗后病理完全缓解(pathological complete response,pCR)的影响因素,评估胃肿瘤标志物在LAGC患者pCR的预测价值。方法:回顾性分析2020年1月至2024年4月期间于中国人民解放军总医院第一医学中心接受新辅助治疗后行胃癌根治术及胃肿瘤标志物检查的213例患者临床与病理资料,其中pCR组20例,非pCR组193例。比较pCR与胃肿瘤标志物、临床病理特征的相互关系,探究pCR的独立预测因素,并构建诺曼图预测模型。结果:213例患者中,20例(9.4%)患者术后病理结果提示pCR。单因素分析结果显示,年龄(P=0.067)、肿瘤瘤床直径(P<0.001)、胃泌素-17(P=0.005)、CA72-4(P=0.073)、胃蛋白酶原比值(PG1/PG2)(P=0.024)及新辅助免疫治疗(P=0.022)与LAGC患者新辅助治疗后pCR显著性相关(P<0.1)。将上述指标纳入多因素分析,表明新辅助免疫治疗、CA72-4<2.5 U/mL、胃泌素-17<1.48 pmol/L、肿瘤瘤床直径<2.85 cm是LAGC患者pCR的独立影响因素(P<0.05)。依托多因素指标构建诺曼图预测模型并绘制受试者工作特征曲线(receiver operating characteristic curve,ROC),AUC(95%CI)为0.863(0.785~0.942)。校准曲线及临床决策曲线提示诺曼图校准良好且具有较好的净收益。结论:胃肿瘤标志物可有效预测LAGC患者新辅助治疗后pCR,以胃肿瘤标志物为基础构建的诺曼图具有良好的预测能力,可为临床决策提供参考。

目的探讨5A教学模式在普通外科住院医师规范化培训中的应用效果。方法采用随机对照研究方法,以2020年1月至2022年12月在解放军总医院第一医学中心普通外科医学部接受住院医师规范化培训并进入本研究团队主诊组轮转学习的学员为研究对象,随机分为试验组和对照组。试验组20名,采用5A教学模式;对照组20名,采用传统教学模式。对2组学员进行理论考核、临床技能考核和综合能力考核,记录考核成绩,并比较2组的教学效果。结果相对于对照组,试验组学员在消毒铺巾、切开缝合和腔镜技能考核3个方面的成绩得到提高,差异有统计学意义(P<0.05);试验组学员在团队协作和科学研究2个方面的成绩高于对照组,差异有统计学意义(P<0.05);试验组学员带教满意度高于对照组,差异有统计学意义(P<0.05)。结论5A教学模式可以显著提高学员的临床实践操作、团队协作和科学研究能力,可作为普通外科住院医师规范化培训的有效教学方法。

Effect of preoperative nutrition therapy type and duration on short-time outcomes in gastric cancer patient with gastric outlet obstruction

Objective:To avoid perioperative complications caused malnutrition,nutrition therapy is necessary in gastric outlet obstruction(GOO)*** to parenteral nutrition(PN),enteral nutrition(EN)is associated with many *** study aimed to investigate whether preoperative EN has beneficial clinical effects compared to preoperative PN in gastric cancer patients with GOO undergoing ***:According to the methods of preoperative nutrition therapy,143 patients were divided into EN group(n=42)and PN group(n=101)between January 2013 and December 2017 at the Chinese People’s Liberation Army General *** logistic regression models were used to assess the association between the methods of preoperative nutrition therapy and postoperative day of flatus *** generalized additive model and twopiecewise linear regression model were used to calculate the inflection point of the preoperative nutritional therapy time on the postoperative day of flatus passage in the PN ***:EN shortened the postoperative day of flatus passage in gastric cancer patients with GOO,which is a protective factor,especially in patients who underwent non-radical operations and the postoperative day of flatus passage reduced when the preoperative PN therapy was up to 3 d and a longer PN therapy(>3 d)did not accelerate the postoperative recovery of gastrointestinal ***:Preoperative EN therapy would benefit gastric cancer patients with GOO by accelerating postoperative *** patients with absolute obstruction,no more than 3-day PN therapy is recommended if patients can tolerate general anesthesia and surgery.

However,the feed forward structures have many advantages, so the proposed method is a promising alternative method for the multiple feedback structure.

Numerical simulation of the acoustic wave equation is widely used to theoretically synthesize seismograms and constitutes the basis of reverse-time *** finite-difference methods,the discretization of temporal and spatial derivatives in wave equations introduces numerical grid *** reduce the grid dispersion effect,we propose to satisfy the dispersion relation for a number of uniformly distributed wavenumber points within a wavenumber range with the upper limit determined by the maximum source frequency,the grid spacing and the wave *** new dispersion-relationship-preserving method relatively uniformly reduces the numerical dispersion over a large-frequency *** analysis and seismic numerical simulations demonstrate the effectiveness of the proposed method.

Two extended one-and two-compartment model are Proposed to explain the multiple-peak phenomena of drugs subject to enterohepatic circulaion. The models are formed by adding a storage compartment on the basis of corresponding classic *** that the circulation will be repeated at regular intervals until the effective amount of drugs released to absorption compartment can be neglected. The concentration (c)-time(t) equations of bolus intravenous and oral administration undergoing n circulations are deduced to express the quantitative contribution of each circulation. Furthermore, the concept of total bioavailability is introduced to describe the bioavailability of drugs obeying the mechanism of entcrohepatic circulation.
