儿童肝外伤的分级和诊治进展

肝外伤是儿童最常见的腹部闭合性损伤,是引起儿童创伤死亡的最主要原因。关于儿童肝外伤的诊断和治疗目前大都根据美国创伤外科协会器官外伤评级委员会(AAST)的评定标准,但在过去30年中肝外伤的诊治理念发生了深刻的变化,在90%以上的患者中,闭合性肝外伤的治疗已从强制性手术转变为非手术性治疗,并且由于儿童肝脏特有的生理解剖特点,在诊断治疗上与成人有一些不同。因此,本文在参照世界急诊外科学会(WSES)关于肝外伤诊治指南的基础上,结合儿童肝脏的生理特点,对儿童肝外伤的诊治进展和远期的治疗结果进行综述,旨在提高儿童肝外伤的诊治水平,降低儿童肝外伤的并发症及死亡率。

Using photodynamic therapy(PDT)to treat deep-seated cancers is limited due to inefficient delivery of photosensitizers and low tissue penetration of *** nanocarriers are widely used for photosensitizer delivery,while the self-quenching of the encapsulated photosensitizers would impair the PDT ***,the generated short-lived reactive oxygen spieces(ROS)can hardly diffuse out of nanocarriers,resulting in low PDT ***,a smart nanocarrier system which can be degraded by light,followed by photosensitizer activation can potentially overcome these limitations and enhance the PDT efficacy.A light-sensitive polymer nanocarrier encapsulating photosensitizer(RB-M)was *** implantable wireless dual wavelength microLED device which delivers the two light wavelengths sequentially was developed to programmatically control the release and activation of the loaded *** transmitter coils with matching resonant frequencies allow activation of the connected LEDs to emit different wavelengths *** irradiation time,dose,and RB-M concentration were determined using an agent-based digital simulation *** vitro and in vivo validation experiments in an orthotopic rat liver hepatocellular carcinoma disease model confirmed that the nanocarrier rupture and sequential low dose light irradiation strategy resulted in successful PDT at reduced photosensitizer and irradiation dose,which is a clinically significant event that enhances treatment safety.

Tumor progression is closely related to tumor tissue metabolism and reshaping of the microenvironment. Oral squamous cell carcinoma (OSCC), a representative hypoxic tumor, has a heterogeneous internal metabolic environment. To clarify the relationship between different metabolic regions and the tumor immune microenvironment (TME) in OSCC, Single cell (SC) and spatial transcriptomics (ST) sequencing of OSCC tissues were performed. The proportion of TME in the ST data was obtained through SPOTlight deconvolution using SC and GSE103322 data. The metabolic activity of each spot was calculated using scMetabolism,and k-means clustering was used to classify all spots into hyper-, normal-, or hypometabolic regions. CD4T cell infiltration and TGF-βexpression is higher in the hypermetabolic regions than in the others. Through CellPhoneDB and NicheNet cell-cell communication analysis, it was found that in the hypermetabolic region, fibroblasts can utilize the lactate produced by glycolysis of epithelial cells to transform into inflammatory cancer-associated fibroblasts (iCAFs), and the increased expression of HIF1A in iCAFs promotes the transcriptional expression of CXCL12. The secretion of CXCL12 recruits regulatory T cells (Tregs), leading to Treg infiltration and increased TGF-β secretion in the microenvironment and promotes the formation of a tumor immunosuppressive microenvironment. This study delineates the coordinate work axis of epithelial cells-iCAFs-Tregs in OSCC using SC, ST and TCGA bulk data, and highlights potential targets for therapy.

晚第四纪以来,因古气候和海平面发生多次变化,南海西北部经历了海岸线变迁和海陆演变多个过程。利用钻孔测年和高分辨率单道地震资料开展对比、划分了7个主要地震反射界面并确定其时代。同时基于现今水深数据,依据全球海平面变化曲线和地层厚度,对海南岛西南海域晚第四纪(100.1~40.7kaBP)以来的古水深和海岸线进行反演。反演结果表明,100.1kaBP时,海南岛西部、西北和南部局部地区海岸线向海方向移动,陆域面积略微增加,海域陆架较窄,最大水深超过550m,海底地形以陆坡深水区为主。78.1kaBP时,海南岛西部海岸线发生较大规模迁移,陆域面积增加,陆架变宽,海底地形平缓。大部分区域水深均小于200m。49.9~40.7kaBP,海南岛西部陆域面积进一步扩大,海底地形平坦,水深变浅,地形以陆架区浅水区为主。研究表明,100.1~40.7kaBP以来研究区的水深变浅,地形变为平缓,海岸线向海迁移,区域古气候、海平面变化以及物源供应对塑造海底地形地貌具有重要的作用。

In spite of the numerous advances in the development of H_(2)and O_(2)evolutions upon water splitting,the separation of H_(2)from O_(2)still remains a severe ***,the novel dual-functional nanocatalysts Pd/carbon nanosphere(CNS),obtained via immobilization of ultrafine Pd nanoparticles onto CNS,are developed and employed for both selective H_(2)generation from HCOOH dehydrogenation and O_(2)evolution from H_(2)O_(2)*** these reactions,the highest activities for Pd/CNS-800(i.e.,calcinated at 800℃)are 2478 h−1 and 993 min^(−1)for H_(2)and O_(2)evolution,*** highly efficient and selective“on-off”switch for selective H_(2)generation from HCOOH is successfully realized by pH *** novel and highly efficient nanocatalyst Pd/CNS-800 not only provides new approaches for the promising application of HCOOH and H_(2)O_(2)as economic and safe H_(2)and O_(2)carriers,respectively,for fuel cells,but also promotes the development of“on-off”switch for on-demand H_(2)evolution.

自第二次世界大战到20世纪90年代,“慰安妇”作为累积了多重弱势身份的个体叙事几乎处于噤声状态,她们被主流战争史叙述挤压至底层,而无法成为“声音”的主体。在全球化的女性主义运动推动下,“慰安妇”沉默的现状方被打破,相关题材的纪实文学逐渐增多。在这类作品的叙述中,她们的“身体”常处于日军暴力的全面宰制之中、从而导致她们在战后遭受社会及自我贱斥。她们背负日军暴力、男权和父权社会共同作恶造成的身心创伤,在见证的困境中创造言说的可能,以身体话语抵抗污名和见证日军的战争性暴力罪行。

The traits of rural domestic sewage emission are unclear,negatively affecting rural domestic sewage treatment and sewage *** study used data from the Second National Pollution Source Census Bulletin to establish a data *** spatial distribution characteristics and main factors influencing rural sewage discharge in the Northern Region were studied using spatial autocorrelation analysis and structural *** findings demonstrated that(l)a significant Spearman correlation between drainage water volume(DwV),chemical oxygen demand(COD),ammonia nitrogen(NH_(3)-N),total nitrogen(TN),and total phosphorus(TP)and that the correlation coefficients between DWV and COD,NH,-N,TNand TP were 0.87**,1.0**,0.99**,0.99**,respectively;(2)rural sewage discharge showed spatial autocorrelation,and rural domestic sewage discharge in the districts and counties with an administration was significantly higher than in the surrounding areas;and(3)social development was the main driver rural domestic sewage changes(path coefficient was 0.407**),and the main factors influencing rural domestic sewage discharge were the urbanization rate,years of education,and population age *** study obtained the spatial variation law and clarified the main influencing factors of rural domestic sewage to provide data support and a theoretical basis for subsequent rural sewage collection and *** of the Inner Mongolia Autonomous Region in northern China as a typical case,provides a theoretical foundation for scientific decision-making on rural domestic sewage treatment at the national and regional levels and offers new perspectives for managing pollutants.

The oceanic trace metals iron(Fe), nickel(Ni), copper(Cu), zinc(Zn), and cadmium(Cd) are crucial to marine phytoplankton growth and global carbon cycle, and the analysis of their stable isotopes can provide valuable insights into their biogeochemical cycles within the ocean. However, the simultaneous isotopic analysis of multiple elements present in seawater is challenging because of their low concentrations, limited volumes of the test samples, and high salt matrix. In this study, we present the novel method developed for the simultaneous analysis of five isotope systems by 1 L seawater sample. In the developed method, the NOBIAS Chelate-PA1 resin was used to extract metals from seawater, the AG MP-1M anion-exchange resin to purify Cu, Fe, Zn, Cd, and the NOBIAS Chelate-PA1 resin to further extract Ni from the matrix elements. Finally, a multi-collector inductively coupled plasma mass spectroscope(MC-ICPMS) was employed for the isotopic measurements using a doublespike technique or sample-standard bracketing combined with internal normalization. This method exhibited low total procedural blanks(0.04 pg, 0.04 pg, 0.21 pg, 0.15 pg, and 3 pg for Ni, Cu, Fe, Zn, and Cd, respectively) and high extraction efficiencies(100.5% ± 0.3%, 100.2% ± 0.5%, 97.8% ± 1.4%, 99.9% ± 0.8%, and 100.1% ± 0.2% for Ni,Cu, Fe, Zn, and Cd, respectively). The external errors and external precisions of this method could be considered negligible. The proposed method was further tested on the seawater samples obtained from the whole vertical profile of a water column during the Chinese GEOTRACES GP09 cruise in the Northwest Pacific, and the results showed good agreement with previous related data. This innovative method will contribute to the advancement of isotope research and enhance our understanding of the marine biogeochemical cycling of Fe, Ni, Cu, Zn, and Cd.

Balancing electron transfer and intermediate adsorption ability of bifunctional catalysts via tailoring electronic structures is crucial for green hydrogen production,while it still remains challenging due to lacking efficient ***,one efficient and universal strategy is developed to greatly regulate electronic structures of the metallic Ni-Fe-P catalysts via in-situ introducing the rare earth(RE)atoms(Ni-Fe-RE-P,RE=La,Ce,Pr,and Nd).Accordingly,the as-prepared optimal Ni-Fe-Ce-P/CC self-supported bifunctional electrodes exhibited superior electrocatalytic activity and excellent stability with the low overpotentials of 247 and 331 mV at 100 mA cm^(-2) for HER and OER,*** the assembled electrolyzer,the Ni-Fe-Ce-P/CC as bifunctional electrodes displayed low operation potential of 1.49 V to achieve a current density of 10 mA cm^(-2),and the catalytic performance can be maintained for 100 *** results combined with density functional theory(DFT)calculation reveal that Ce doping leads to electron decentralization and crystal structure distortion,which can tailor the band structures and d-band center of Ni-Fe-P,further increasing conductivity and optimizing intermediate adsorption *** work not only proposes a valuable strategy to regulate the electron transfer and intermediate adsorption of electrocatalysts via RE atoms doping,but also provides a deep under-standing of regulation mechanism of metallic electrocatalysts for enhanced water splitting.
