Analysis of Benzene Exposure Levels on Commuters Traveling within the Metropolitan Area of Costa Rica

This study reports the benzene exposure levels of commuters traveling within the metropolitan area of Costa Rica using personal cars, buses, and urban trains. 47 in-vehicle samples were collected in the 2014 wet season under three different driving conditions: rush hour traffic, normal traffic and weekends. Samples were collected in Tedlar bags and analyzed using 75 μm carboxenpolydimethylsiloxane (CAR/PDMS) and gas chromatography with flame ionization detection (GC-FID). Additionally, duplicate samples were collected on adsorption tubes filled with Tenax TA and analyzed by thermal desorption (TD) and GC-FID. Results indicate that travelling in cars and buses under rush hour conditions exposes commuters to statistically equal average benzene concentration of 48.7 and 51.6 μg/m3, respectively. Average benzene levels in urban trains (33.0 μg/m3) were measured only during morning rush hours. Greater benzene levels in buses than personal cars concentrations may be attributable to the immersion of traffic-related emission within the bus cabins. Factors, such as the driving pattern, the number of vehicles on the route, the road infrastructure, and the prevalence of gasoline-fueled vehicles in Costa Rica, may increase ambient benzene concentrations. Benzene levels inside car cabins reported in this study are in the range of those reported in other urban areas;however, the corresponding concentrations inside buses and urban trains (rush hour only) are higher than previously published exposure levels.

The family of voltage-gated （Shaker-like） potassium channels in plants includes both inward-rectifying （Kin） channels that allow plant cells to accumulate K＋ and outward-rectifying （Kout） channels that mediate K＋ efflux. Despite their dose structural similarities, Kin and Kout channels differ in their gating sensitivity towards voltage and the extracellular K＋ concentration. We have carried out a systematic program of domain swapping between the Kout channel SKOR and the Kin channel KAT1 to examine the impacts on gating of the pore regions, the S4, S5, and the S6 helices. We found that, in particular, the N-terminal part of the S5 played a critical role in KAT1 and SKOR gating. Our findings were supported by molecular dynamics of KAT1 and SKOR homology models. In silico analysis revealed that during channel opening and closing, displacement of certain residues, especially in the S5 and S6 segments, is more pronounced in KAT1 than in SKOR. From our analysis of the S4-S6 region, we conclude that gating （and K＋-sensing in SKOR） depend on a number of structural elements that are dispersed over this -145-residue sequence and that these place additional constraints on configurational rearrangement of the channels during gating.

Intraoperative Radiation Therapy(INTRABEAM)Experience at the Mastology Unit Leopoldo Aguerrevere Clinic

Introduction: External radiation therapy has been the fundamental pillar when treating breast cancer. Partial radiation therapy and intraoperative radiation treatment have created modifications that allow the irradiation of the breast to be performed at the surgical act delivering a single large fraction or a “boost” dose directly at the tumor bed. We will discuss patients treated with INTRABEAM (Carl Zeiss Surgical Oberkochen, Germany) at the “Mastology Unit at Leopoldo Aguerrevere Clinic”. Materials and Methods: The selection of patients is crucial for the success of the treatment, same protocol of treatment has been applied to every patient at the surgical act. Since September 2013 until February 2015, we have treated a total of 148 patients with the INTRABEAM unit, we will discuss the 114 patients treated by the team at the “Mastology Unit at Leopoldo Aguerrevere Clinic” with ages between 31 and 87 years in which 46% were single treatments and 54% were treated as a “boost” for external radiation therapy. Results: The procedure has been well tolerated with only a 17% of transient fibrosis and a 12% of seromas. We have had none mayor complications like dehiscence of the wound or necrosis of the borders. Conclusion: With this preliminary presentation, we would like to demonstrate that the technique and protocol used at our mastology unit with the intraoperative radiation treatment is safe and has many advantages to the patients including better comfort, cost-effective and with results comparable to external radiotherapy.

Background: Nursing is considered a science in the making that recognizes the nursing process (NP) as the scientific and methodological tool to provide care. The NP is composed of five stages of which the assessment is the first and fundamental to identify the health problems of the people. This research aimed to evaluate a pedagogical didactic strategy that is based on situated constructivism, the Based-Problem Learning (BPL), in order to improve significant learning. Methods: Quantitative research with an explanatory scope, was transversal, and it was applied to 30 students of the first year of the nursing career at the Faculty of Higher Studies Iztacala, National Autonomous University of Mexico. The sample was chosen considering the inclusion criteria. Data were collected using a Likert-type instrument validated in a previous investigation. Results: The average value obtained from the Likert scale to evaluate the intervention was 103.4, the highest score was 135 and the lowest was 83, which reflects that it was evaluated as good and particularly good. The grade given to the teacher as an agent of the ZPD, was between agree and totally agree, the highest percentages scored at 93%, since it provoked challenge in their thinking, and favored interaction among classmates. Conclusions: The application of the in situ teaching strategy, BPL favored significant learning of the assessment stage, the students were able to carry out the assessment in the hospital, the experience contributed to generate significant learning when students face real scenarios, which showed the effectiveness of the method.

Introduction: Congenitally corrected transposition of the great arteries (CCTGA) is a rare heart disease that encompasses an atrioventricular and ventriculoarterial discordance and accounts for less than 1 percent of congenital heart diseases. Objective: To present an atypical case of a man with complex congenital heart disease and conduction anomalies. Case Presentation: This is a case of a 34-year-old patient who came to the hospital with 1 week of dyspnea on exertion and episodes of lipothymia. The patient was referred to our hospital after an electrocardiogram from his primary care with evidence of blocked atrial fibrillation. During the initial evaluation blocked atrial fibrillation was confirmed with a rescue ventricular rate of 38 bpm. A magnetic resonance confirmed the presence of the atrioventricular and ventriculoarterial discordance, the aorta had a left anterior position, perimembranous ventricular septal defect with a right to left shunt, biventricular systolic dysfunction, moderate tricuspid, and mitral regurgitation, interventricular septal intramyocardial and biatrial fibrosis, left atrial dilation, and dilation of the pulmonary artery. After pacemaker placement, the patient has an improvement in his clinical symptoms and quality of life. Immersion Tools as a Strategy for Adolescents Mental Health: A Qualitative Study

Background: Information and communication technologies have revolutionized education, due to development of didactic strategies to take advantage of them for maximizing learning. The aim was to analyze the influence of immersion tools as a strategy to improve adolescents’ mental health. Methods: Qualitative research was employed, and participants’ experiences, opinions, values, behaviors, emotions, feelings were recorded. Twenty six students of both sexes, aged 13 to 15 years, from the three grades of secondary school were selected by convenience, and supported by the school’s psychologist. A workshop on emotional intelligence (EI) based on educational technology was implemented using a video;students practiced a reflexive process to understand themselves and their surroundings, creating a video + audio as a product of the workshop. In it, students analyzed several problems they face and their relationship with their contextual situation. Data collection was through semi-structured interviews before and after the workshop on EI. Data analysis was according to Souza Minayo, and the theoretical framework according to Goleman, Tapia et al., Gorostieta et al., and Vigotsky. Results: Two categories emerged: Category I, EI as a risk factors transformer from living with addictions to protection factors in adolescents. Category II, EI development through immersion tools to improve mental health in adolescents. Conclusions: Immersion tools favor user to be closer, with a humanist focus where real necessities are studied after the vivid experience of the problem to solve or get a better situation. The goal is that students experience real situations inside controlled surroundings, in such a way that they learn from their experiences and acquire knowledge through self-learning. Characterizing the diversity and structure of host–parasite communities is crucial to understandingtheir eco-evolutionary *** and related haemosporidian parasites are responsible forfitness loss and mortality in bird species ***,despite exhibiting the greatest ornithologicalbiodiversity,avian haemosporidians from Neotropical regions are quite ***,we analyze the genetic diversity of bird haemosporidian parasites(Plasmodium andHaemoproteus)in 1,336 individuals belonging to 206 bird species to explore for differences in diversityof parasite lineages and bird species across 5 well-differentiated Peruvian *** 70 different haemosporidian lineages infecting 74 bird *** showed that 25 out ofthe 70 haplotypes had not been previously ***,we also identified 81 new host–parasite interactions representing new host records for these haemosporidian *** outcomesrevealed that the effective diversity(as well as the richness,abundance,and Shannon–Weaver index)for both birds and parasite lineages was higher in Amazon basin ***,we also showed that ecoregions with greater diversity of bird species also had highparasite richness,hence suggesting that host community is crucial in explaining parasite *** parasites were found in ecoregions with lower bird diversity,implying that the abundanceand richness of hosts may shape the exploitation strategy followed by haemosporidian *** outcomes reveal that Neotropical region is a major reservoir of unidentified *** studies analyzing host distribution and specificity of these parasites inthe tropics will provide important knowledge about phylogenetic relationships,phylogeography,and patterns of evolution and distribution of haemosporidian parasites.
