submaximal oxygen uptake kinetics, functional mobility, and physical activity in older adults with heart failure and reduced ejection fraction

Background submaximal oxygen uptake measures are more feasible and may better predict clinical cardiac outcomes than maximal tests in older adults with heart failure （HF）. We examined relationships between maximal oxygen uptake, submaximal oxygen kinetics, fianctional mobility, and physical activity in older adults with HF and reduced ejection fraction. Methods Older adults with HF and reduced ejection fraction （n = 25, age 75 ：i： 7 years） were compared to 25 healthy ageand gender-matched controls. Assessments included a maximal treadmill test for peak oxygen uptake （VO2p^ak）, oxygen uptake kinetics at onset of and on recovery from a submaximal treadmill test, functional mobility testing [Get Up and Go （GUG）, Comfortable Gait speed （CGs）, Unipedal stance （Us）], and self-reported physical activity （PA）. Results Compared to controls, HF had worse performance on GUG, CGs, and Us, greater delays in submaximal oxygen uptake kinetics, and lower PA. In controls, VO2peak was more strongly associated with functional mobility and PA than submaximal oxygen uptake kinetics. In HF patients, submaximal oxygen uptake kinetics were similarly associated with GUG and CGs as VO2peak, but weakly associated with PA. Conclusions Based on their mobility performance, older HF patients with reduced ejection fraction are at risk for adverse functional outcomes. In this population, submaximal oxygen uptake measures may be equivalent to VO2 peak in predicting functional mobility, and in addition to being more feasible, may provide better insight into how aerobic function relates to mobility in older adults with HF.

Highly enhanced UV absorption and light emission of monolayer Ws_(2)through hybridization with Ti_(2)N MXene quantum dots and g-C_(3)N_(4)quantum dots

Two-dimensional(2D)transition metal dichalcogenides(TMD)are atomically thin semiconductors with promising optoelectronic applications across the visible ***,their intrinsically weak light absorption and the low photoluminescence quantum yield(PLQY)restrict their performance and potential use,especially in ultraviolet(UV)wavelength light *** dots(QD)derived from 2D materials(2D/QD)provide efficient light absorption and emission of which energy can be tuned for desirable light *** this study,we greatly enhanced the photon absorption and PLQY of monolayer(1L)tungsten disulfide(Ws_(2))in the UV range via hybridization with 2D/QD,particularly titanium nitride MXene QD(Ti_(2)N MQD)and graphitic carbon nitride QD(GCNQD).With the hybridization of MQD or GCNQD,1LWs_(2)showed a maximum PL enhancement by 15 times with 300 nm wavelength excitation,while no noticeable enhancement was observed when the excitation photon energy was less than the bandgap of the QD,indicating that UV absorption by the QD played a crucial role in enhancing the light emission of 1L-Ws_(2)in our 0D/2D hybrid *** findings present a convenient method for enhancing the photo-response of 1L-Ws_(2)to UV light and offer exciting possibilities for harvesting UV energy using 1L-TMD.

The sARs-CoV-2 Omicron variants are notorious for their transmissibility,but little is known about their subgenomic RNA(sgRNA)*** study applied RNA-seq to delineate the quantitative and qualitative profiles of canonical sgRNA of 118 respiratory samples collected from patients infected with Omicron BA.2 and compared with 338 patients infected with non-variant of concern(non-VOC)-D614G.A unique characteristic profile depicted by the relative abundance of 9 canonical sgRNAs was reproduced by both BA.2 and non-VOCD614G regardless of host gender,age and presence of ***,such profile was lost in samples with low viral load,suggesting a potential application of sgRNA pattern to indicate viral activity of individual patient at a specific time point.A characteristic qualitative profile of canonical sgRNAs was also reproduced by both BA.2 and *** presence of a full set of canonical sgRNAs carried a coherent correlation with crude viral load(AUC¼0.91,95%CI 0.88–0.94),and sgRNA ORF7b was identified to be the best surrogate marker allowing feasible routine application in characterizing the infection status of individual *** potentials in using sgRNA as a target for vaccine and antiviral development are worth pursuing.

BACKGROUND survival in patients with autoimmune liver disease overlap syndromes(AILDOs)compared to those with single autoimmune liver disease is *** To investigate the survival of patients with AILDOs and assess the accuracy of non-invasive serum models for predicting liver-related *** Patients with AILDOs were defined as either autoimmune hepatitis and primary biliary cholangitis overlap(AIH-PBC)or autoimmune hepatitis and primary sclerosing cholangitis overlap(AIH-PsC)and were identified from three tertiary centres for this cohort ***-related death or transplantation(liver-related mortality)was determined using a population-based data linkage *** scores for liver-related death were compared for accuracy[including liver outcome score(LOs),Hepascore,Mayo score,model for end-stage liver disease(MELD)score and MELD incorporated with serum sodium(MELD-Na)score].REsULTs Twenty-two AILDOs patients were followed for a median of 3.1 years(range,0.35-7.7).Fourteen were female,the median age was 46.7 years(range,17.8 to 82.1)and median Hepascore was 1(range,0.07-1).At five years post enrolment,57%of patients remained free from liver-related mortality(74%AIH-PBC,27%AIH-PsC).There was no significant difference in survival between AIH-PBC and *** was a significant predictor of liver-related mortality(Ps with AIH-PBC(n=14)but not AIH-PsC(n=8).A LOs cut-point of 6 discriminated liver-related mortality in AIH-PBC patients(P=0.012,log-rank test,100%sensitivity,77.8%specificity)(Harrell's C-statistic 0.867).The MELD score,MELD-Na score and Mayo score were not predictive of liver-related mortality in any *** survival in the rare,AILDOs is *** current study supports the LOs as a predictor of liver-related mortality in AIH-PBC *** trials investigating predictors of survival in AILDOs are required.

This prospective multicenter phase II study evaluated the clinical efficacy of neoadjuvant nivolumab-exclusive(N)and nivolumab–chemotherapy(N/C)combinations based on PD-L1 *** patients exhibited resectable clinical stage IIA–IIIB(AJCC 8th edition)NsCLC without EGFR/ALK *** received either mono-nivolumab(N)or nivolumab+nabpaclitaxel+carboplatin(N/C)for three cycles based on PD-L1 *** primary endpoint was the major pathological response(MPR).Key secondary endpoints included the pathologic complete response(pCR),objective response rate(ORR),and event-free survival(EFs).Baseline PD-L1 expression and perioperative circulating tumor DNA(ctDNA)status were correlated with pCR and ***-two patients were enrolled,with 46 undergoing *** MPR was 50.0%(26/52),with 25.0%(13/52)achieving pCR,and 16.7%and 66.7%for patients with PD-L1≥50%in N and N/C groups,***(25.0%)patients experienced grade 3 or higher immune-related adverse events during neoadjuvant *** with post-neoadjuvant ctDNA negativity was more likely to have pCR(39.1%)compared with those remained positive(6.7%,odds ratio=6.14,95%CI 0.84-Inf,p=0.077).With a median follow-up of 25.1 months,the 18-month EFs rate was 64.8%(95%CI 51.9–81.0%).For patients with ctDNA–***+,the 18m-EFs rate was 93.8%vs 47.3%(HR,0.15;95%CI 0.04,0.94;p=0.005).Immunochemotherapy may serve as an optimal neoadjuvant treatment even for patients with PD-L1 expression≥50%.ctDNA negativity following neoadjuvant treatment and surgery could help identify superior pathological and survival benefits,which requires further confirmation in a prospective clinical trial(NCT04015778).

Multiple Antibiotic Resistance and Heavy Metal Resistance Profile of Bacteria Isolated from Giant Freshwater Prawn (Macrobrachium rosenbergii) Hatchery

In this article, antibiogram and heavy metal resistance profile of bacteria isolated from giant freshwater prawn （Macrobrachium rosenbergii） hatchery in Malaysia are described. Although giant freshwater prawn was introduced into Malaysia since the 1980s, there was no database information on antibiogram and heavy metal resistance profile of bacteria from giant freshwater prawn （34. rosenbergii） hatchery in Malaysia. Therefore, this study was carried out to determine the effectiveness of antibiotic and heavy metal resistance profile to control bacterial diseases in M. rosenbergii hatchery. The results can provide valuable information for local M. rosenbergii post-larval producer. Antibiotic sensitivity test was carried out by disk-diffusion method against 15 types of antibiotics as follows： oxolinic acid （2 μg）, ampicillin （10μg）, erythromycin （15μg）, furazolidone （15 μg）, lincomycin （15μg）, amoxicillin （25 μg）, colistin sulphate （25 μg）, doxycycline （30μg）, florfenicol （30 μg）, flumequine （30 lag）, nalidixic acid （30μg）, tetracycline （30μg）, oleandomycin （15μg）, fosfomycin （50μg）, and spiramycin （100 μg）, whereas heavy metal resistance profile of the present bacterial isolates was determined by 2-fold agar dilution technique. In this study, 5 types of bacteria were successfully isolated; they were Aeromonas spp. （n = 77）, Escherichia coli （n = 73）, Edwardsiella spp. （n = 62）, salmonella spp. （n = 75）, and Vibrio spp. （n = 43）. The result showed that furazolidone was the most effective antibiotic to control the bacteria isolated in this study, approximately 89.7% of the bacterial isolates were sensitive to this antibiotic. Multiple antibiotic resistance （MAR） index indicated that the hatchery water source and M. rosenbergii post-larval and sediment tanks were at high-risk exposure to the tested antibiotic. Furthermore, all the tested heavy metals （Cd2＋, Cr6＋, Hg2＋, and Cu2＋） failed to inhibit the growth of the bacterial isolates. Therefore, it indicated that the water
