Hypertension and Heart Failure as Predictors of mortality in an Adult Congenital Heart Defect Population

Early intervention to prevent premature mortality is vital for adults with congenital heart defects(CHD).Anatomic complexity and comorbid conditions are thought to contribute to CHD *** hypertension(HTN)and heart failure(HF)are the comorbid conditions among the most prevalent causes of death in the United States,and commonly accompany CHD,it is crucial to evaluate whether they are reliable predictors of mortality for adults with CHD(ACHD)independent of anatomic CHD complexity.A retrospective cross-sectional analysis of ACHD,aged 18–64,with concomitant HTN and/or HF and at least one health care encounter during 2008–2010 were *** 5,397 ACHD patients(18.3%HTN without HF,4.4%HF without HTN,8.3%with both),3.0%died(n=163)during the study ***,the sample was 45.1%white,61.4%female,and 29.0%had a complex *** those who died,23.3%had HTN without HF,17.2%had HF without HTN,and 42.3%had *** analyses revealed that older age,male gender,black race,and having public health insurance were associated with increased mortality during the three-year study period compared to ACHD patients who were younger,female gender,white race,and covered by private health *** patients diagnosed with non-complex CHD lesions(i.e.,shunts,valves,or shunts+valves)were at greater risk of dying compared to those with severe complex *** CHD type was assessed separately,those with valve lesions were more likely to die compared to those with complex CHD *** adjustment for age,gender,race,insurance and CHD complexity,ACHD patients with HF,with or without HTN,were equally likely to die during the study ***,ACHD patients with HF,without or without HTN,who had valve defects were more likely to die during the three-year study period compared to patients with complex CHDs.

This prospective multicenter phase II study evaluated the clinical efficacy of neoadjuvant nivolumab-exclusive(N)and nivolumab–chemotherapy(N/C)combinations based on PD-L1 *** patients exhibited resectable clinical stage IIA–IIIB(AJCC 8th edition)NSCLC without EGFR/ALK *** received either mono-nivolumab(N)or nivolumab+nabpaclitaxel+carboplatin(N/C)for three cycles based on PD-L1 *** primary endpoint was the major pathological response(mPR).Key secondary endpoints included the pathologic complete response(pCR),objective response rate(ORR),and event-free survival(EFS).Baseline PD-L1 expression and perioperative circulating tumor DNA(ctDNA)status were correlated with pCR and ***-two patients were enrolled,with 46 undergoing *** mPR was 50.0%(26/52),with 25.0%(13/52)achieving pCR,and 16.7%and 66.7%for patients with PD-L1≥50%in N and N/C groups,***(25.0%)patients experienced grade 3 or higher immune-related adverse events during neoadjuvant *** with post-neoadjuvant ctDNA negativity was more likely to have pCR(39.1%)compared with those remained positive(6.7%,odds ratio=6.14,95%CI 0.84-Inf,p=0.077).With a median follow-up of 25.1 months,the 18-month EFS rate was 64.8%(95%CI 51.9–81.0%).For patients with ctDNA–***+,the 18m-EFS rate was 93.8%vs 47.3%(HR,0.15;95%CI 0.04,0.94;p=0.005).Immunochemotherapy may serve as an optimal neoadjuvant treatment even for patients with PD-L1 expression≥50%.ctDNA negativity following neoadjuvant treatment and surgery could help identify superior pathological and survival benefits,which requires further confirmation in a prospective clinical trial(NCT04015778).

multiple Antibiotic Resistance and Heavy metal Resistance Profile of Bacteria Isolated from Giant Freshwater Prawn (macrobrachium rosenbergii) Hatchery

In this article, antibiogram and heavy metal resistance profile of bacteria isolated from giant freshwater prawn （macrobrachium rosenbergii） hatchery in malaysia are described. Although giant freshwater prawn was introduced into malaysia since the 1980s, there was no database information on antibiogram and heavy metal resistance profile of bacteria from giant freshwater prawn （34. rosenbergii） hatchery in malaysia. Therefore, this study was carried out to determine the effectiveness of antibiotic and heavy metal resistance profile to control bacterial diseases in m. rosenbergii hatchery. The results can provide valuable information for local m. rosenbergii post-larval producer. Antibiotic sensitivity test was carried out by disk-diffusion method against 15 types of antibiotics as follows： oxolinic acid （2 μg）, ampicillin （10μg）, erythromycin （15μg）, furazolidone （15 μg）, lincomycin （15μg）, amoxicillin （25 μg）, colistin sulphate （25 μg）, doxycycline （30μg）, florfenicol （30 μg）, flumequine （30 lag）, nalidixic acid （30μg）, tetracycline （30μg）, oleandomycin （15μg）, fosfomycin （50μg）, and spiramycin （100 μg）, whereas heavy metal resistance profile of the present bacterial isolates was determined by 2-fold agar dilution technique. In this study, 5 types of bacteria were successfully isolated; they were Aeromonas spp. （n = 77）, Escherichia coli （n = 73）, Edwardsiella spp. （n = 62）, Salmonella spp. （n = 75）, and Vibrio spp. （n = 43）. The result showed that furazolidone was the most effective antibiotic to control the bacteria isolated in this study, approximately 89.7% of the bacterial isolates were sensitive to this antibiotic. multiple antibiotic resistance （mAR） index indicated that the hatchery water source and m. rosenbergii post-larval and sediment tanks were at high-risk exposure to the tested antibiotic. Furthermore, all the tested heavy metals （Cd2＋, Cr6＋, Hg2＋, and Cu2＋） failed to inhibit the growth of the bacterial isolates. Therefore, it indicated that the water

Pre-mRNA modifications and their role in nuclear processing

Background： Cellular non-coding RNAs are extensively modified post-transcriptionally, with more than 100 chemically distinct nucleotides identified to date. In the past five years, new sequencing based methods have revealed widespread decoration of eukaryotic messenger RNA with diverse RNA modifications whose functions in mRNA metabolism are only beginning to be known. Results： Since most of the identified mRNA modifying enzymes are present in the nucleus, these modifications have the potential to function in nuclear pre-mRNA processing including alternative splicing. Here we review recent progress towards illuminating the role of pre-mRNA modifications in splicing and highlight key areas for future investigation in this rapidly growing field. Conclusions： Future studies to identify which modifications are added to nascent pre-mRNA and to interrogate the direct effects of individual modifications are likely to reveal new mechanisms by which nuclear pre-mRNA processing is regulated.

Haplotype phasing after joint estimation of recombination and linkage disequilibrium in breeding populations

A novel method for haplotype phasing in families after joint estimation of recombination fraction and linkage disequilibrium is developed. Results from monte Carlo computer simulations show that the newly developed E.m. algorithm is accurate if true recombination fraction is 0 even for single families of relatively small sizes. Estimates of recombination fraction and linkage disequilibrium were 0.00 （SD 0.00） and 0.19 （SD 0.03） for simulated recombination fraction and linkage disequilibrium of 0.00 and 0.20, respectively. A genome fragmentation phasing strategy was developed and used for phasing haplotypes in a sire and 36 progeny using the 50 k Illumina BeadChip by： a） estimation of the recombination fraction and LD in consecutive SNPs using family information, b） linkage analyses between fragments, c） phasing of haplotypes in parents and progeny and in following generations. Homozygous SNPs in progeny allowed determination of paternal fragment inheritance, and deduction of SNP sequence information of haplotypes from dams. The strategy also allowed detection of genotyping errors. A total of 613 recombination events were detected after linkage analysis was carried out between fragments. Hot and cold spots were identified at the individual （sire level）. SNPs for which the sire and calf were heterozygotes became informative （over 90%） after the phasing of haplotypes. Average of regions of identity between half-sibs when comparing its maternal inherited haplotypes （with at least 20 SNP） in common was 0.11 with a maximum of 0.29 and a minimum of 0.05. A monte-Carlo simulation of BTA1 with the same linkage disequilibrium structure and genetic linkage as the cattle family yielded a 99.98 and 99.94% of correct phases for informative SNPs in sire and calves, respectively.
