Accelerated Infliximab Induction for Severe Lower Gastrointestinal Bleeding in a Young Patient with Crohn’s Disease:A Case Report

BACKGROUND Severe lower gastrointestinal bleeding(SLGIB)is a rare complication of Crohn's disease(CD).The treatment of these patients is a clinical *** anti-TNFαantibody(IFX)can induce relatively fast mucosal *** has been reported for the treatment of SLGIB,but there are few reports on accelerated IFX induction in CD patients with *** SUMMARY A 16-year-old boy with a history of recurrent oral ulcers for nearly 1 year presented to the Gastroenterology Department of our hospital complaining of recurrent periumbilical pain for more than 1 mo and having bloody stool 4 times within 2 *** showed multiple areas of inflammation of the colon and a sigmoid colon ulcer with active *** was immediately performed under *** physical examination of the patient showed scattered small ulcers in the lower lip of the mouth and small cracks in the perianal *** with his medical history,physical examination,laboratory examinations with high C-reactive protein(CRP),platelet count(PLT),erythrocyte sedimentation rate(ESR)and fecal calprotectin levels,imaging examinations and pathology,a diagnosis of CD was taken into *** to the pediatric CD activity index 47.5,methylprednisolone(40 mg QD)was given *** abdominal pain disappeared,and CRP,PLT,and ESR levels decreased significantly after the ***,he had a large amount of bloody stool again after 1 wk of methylprednisolone treatment,and his hemoglobin level decreased *** infliximab(IFX)(5 mg/kg)was given as a combination therapy regimen,he still had bloody stool with his hemoglobin level decreasing from 112 g/L to 80 g/L in a short time,so-called *** informed consent,accelerated IFX(5 mg/kg)induction was given 7 days after initial *** bleeding then *** weeks after the treatment,repeat colonoscopy showed mucosal healing;thus far,no recurrent bleeding has occurred,and the patie

Integrin antagonists are effective and safe for Crohn's disease:a meta-analysis

AIM: To evaluate the efficacy and safety of integrin antagonists, including natalizumab and vedolizumab, in Crohn's disease(CD).METHODS: We carried out a literature search in Pub Med, MEDLINE, EMBASE and the Cochrane Library to screen for citations from January 1990 to August2014. Data analysis was performed using Review Manager version ***: A total of 1340 patients from five studies were involved in this meta-analysis. During 6-12 wk treatment, integrin antagonists increased the rate of clinical response and remission with OR = 1.69, 95%CI: 1.37-2.09 and 1.84, 95%CI: 1.44-2.34, respectively. No significant difference was found between integrin antagonists and placebo treatments regarding their adverse reactions(OR = 1.07, 95%CI: 0.83-1.38) and serious adverse reactions(OR = 0.81, 95%CI: 0.57-1.15).CONCLUSION: The results prove the efficacy and safety of integrin antagonists for CD treatment, although the treatment strategies varied.

Ovarian endometrioma(OE),also known as“chocolate cysts,”is a cystic mass that develops in the ovaries due to endometriosis and is a common gynecological condition characterized by the growth of endometrial tissue outside the uterus,leading to symptoms such as dysmenorrhea,pelvic pain,and ***,the precise molecular and cellular mechanisms driving this pathophysiology remain largely unknown,posing challenges for diagnosis and ***,we employed integrated single-cell transcriptomic profiling of over 52,000 individual cells from endometrial tissues of OE patients and healthy donors and identified twelve major cell *** identified notable alterations in cell type-specific proportions and molecular signatures associated with ***,the activation of IGFBP5^(+) macrophages with pro-inflammatory properties,NK cell exhaustion,and aberrant proliferation of IQCG^(+) and KLF2^(+) epithelium are key features and may be the potential mechanisms underlying the pathogenesis of ***,our data contribute to a better understanding of OE at the single cell level and may pave the way for the development of novel therapeutic strategies.

Biomass-derived heteroatom self-doped cathode catalysts has attracted considerable interest for electrochemical advanced oxidation processes(EAOPs)due to its high performance and sustainable ***,we illustrated the morphological fates of waste leaf-derived graphitic carbon(WLGC)produced from waste ginkgo leaves via pyrolysis temperature regulation and used as bifunctional cathode catalyst for simultaneous H_(2)O_(2) electrochemical generation and organic pollutant degradation,discovering S/N-self-doping shown to facilitate a synergistic effect on reactive oxygen species(ROS)*** the optimum temperature of 800℃,the WLGC exhibited a H_(2)O_(2) selectivity of 94.2%and tetracycline removal of 99.3%within 60 *** functional theory calculations and in-situ Fourier transformed infrared spectroscopy verified that graphitic N was the critical site for H_(2)O_(2) *** pyridinic N and thiophene S were the main active sites responsible for OH generation,N vacancies were the active sites to produce ^(1)O_(2) from O_(2).The performance of the novel cathode for tetracycline degradation remains well under a wide pH range(3–11),maintaining excellent stability in 10 *** is also industrially applicable,achieving satisfactory performance treating in real water *** system facilitates both radical and non-radical degradation,offering valuable advances in the preparation of cost-effective and sustainable electrocatalysts and hold strong potentials in metal-free EAOPs for organic pollutant degradation.

Accumulation characteristics and fate modeling of phthalic acid esters in surface water from the Three Gorges Reservoir area,China

Phthalic acid esters(PAEs)are a group of compounds widespread in the *** investigate the occurrence and accumulation characteristics of PAEs,surface water samples were collected from the Three Gorges Reservoir area,*** total concentrations of∑_(11)analyzed PAEs(11PAEs)in the collected water samples ranging from 197.7 to 1,409.3 ng/L(mean±IQR:583.1±308.4 ng/L).While DEHP was the most frequently detected PAE,DnBP and DnNP were the most predominant PAEs in the analyzed water samples with a mean contribution of 63.3%of the∑_(11)*** concentrations of the∑_(11)PAEs in the water samples from the upper reaches of the Yangtze River were significantly higher than those from themiddle *** better understand the transport and fate of the PAEs,seven detected PAEs were modeled by Quantitative Water Air Sediment Interaction(QWASI).The simulated and measured values were close for most PAEs,and differences are within one order of magnitude even for the worst *** all simulated PAEs,water and particle inflow were main sources in the reservoir,whereas water outflow and degradation in water were important removal *** contribution ratios of different sources/losses varied fromPAEs,depending on their *** calculated risk quotients of DnNP in the Three Gorges Reservoir area whether based onmonitoring or simulating results were all far exceeded the safety threshold value,implying the occurrence of this PAE compound may cause potential adverse effects for the aquatic ecology of the Three Gorges Reservoir area.

以2009～2011年中国A股上市公司为研究样本,实证研究了董事会特征与内部控制各目标之间的关系。研究发现,董事会特征与财务报告可靠性、经营效率效果以及法律遵循性之间的相关关系与假设并不完全契合。独立董事比例和审计委员会的设置与内部控制目标的实现显著正相关,在各指标间无差异。董事会会议频率对内部控制目标的实现作用不显著。董事长与CEO两职合一一定程度上能够提高企业的经营效益,但与财务报告可靠性、法律法规遵循性却呈负相关关系。
