This study is designed to serve as a reference for the establishment of health security systems for children＇s critical diseases. Through analysis of the operation of Shanghai Children Hospital Care Aid （SCHCA）, this study explored the financing model and management of a children＇s critical disease healthcare system and analyzed the possibility of expanding this system to other areas. It is found that a premium as Iow as RMB 7 per capita per year under SCHCA can provide high-level security for children＇s critical diseases. With the good experience in Shanghai and based on the current basic medical insurance system for urban residents and the new rural cooperative medical scheme （NRCMS）, it is necessary and feasible to build a health security system for children＇s critical diseases at the national level.

Urban development in arid and semi-arid regions is largely constrained by fragile physical environ- ments. The characteristics of an urban settlement are different from those in other regions of China. This paper analyses the coupling characteristics and spatio-temporal variations for oasis urban development and water-land resources at the northern slopes of the Tianshan Mountains by principal component analysis and a coupling degree model. The result shows that the degree and change in regional use of water and land resources are different among the studied cities/counties during their development. The built-up areas of these cities/counties have changed little with increasing populations and urbanization levels, which well reflects that the urban development in arid and semi-arid regions is limited by oasis areas. Per capita amount of water supplied, however, presented a trend of slowed growth with increasing levels of urbanization. Water consumption gradually increased with urban development and the improvement of people＇s living standards, accompanied by enhanced water use efficiency. The level of urbanization can be assessed through the coupling degree between oasis urban development and the use of water and land resources. A high coupling degree represents a high level of comprehensive urban devel- opment and use of water-land resources. Alternatively, a low coupling degree denotes a low level of urban devel- opment and water-land resource use.

目的探索槲皮素抑制乳腺癌细胞恶性生物学行为的分子机制。方法以乳腺癌细胞系MCF-7和MB231作为研究对象,用慢病毒LV-ERα、LV-MALAT-1转染乳腺癌MB231细胞和MCF7细胞,RT-qPCR检测MALAT-1表达,Western blot检测肿瘤细胞中ERα蛋白表达,CCK-8细胞实验、平板克隆形成实验检测细胞增殖能力,PI染色法检测细胞周期,通过mRFP-GFP-LC3荧光双标腺病毒转染检测自噬水平,观察槲皮素和雌二醇对乳腺癌细胞增殖能力的影响。结果17β-雌二醇(E2)可以促进乳腺癌细胞MCF-7的增殖,而5μmol·L^(-1)槲皮素可以明显逆转E2对增殖的促进作用(P<0.05)。槲皮素对不表达雌激素受体α(estrogen receptor-α,ERα)的乳腺癌细胞MB231不表现抑制作用;而过表达ERα后,槲皮素则抑制了E2对MB231-ERα的促进作用。同时槲皮素可以抑制E2激活的MALAT-1表达;并且其抑制作用被过表达MALAT-1所逆转,包括细胞增殖,细胞周期进展以及克隆形成能力。结论槲皮素依赖于ERα的表达对乳腺癌的增殖等恶性行为起抑制作用,并且很可能是通过抑制MALAT-1的表达来发挥作用。

Mountain catchments are prone to flash flooding due to heavy rainfall. Enhanced understanding of the generation and evolution processes of flash floods is essential for effective flood risk management. However, traditional distributed hydrological models based on kinematic and diffusion wave approximations ignore certain physical mechanisms of flash floods and thus bear excessive uncertainty. Here a hydrodynamic model is presented for flash floods based on the full two-dimensional shallow water equations incorporating rainfall and infiltration. Laboratory experiments of overland flows were modelled to illustrate the capability of the model. Then the model was applied to resolve two observed flash floods of distinct magnitudes in the Lengkou catchment in Shanxi Province, China. The present model is shown to be able to reproduce the flood flows fairly well compared to the observed data. The spatial distribution of rainfall is shown to be crucial for the modelling of flash floods. Sensitivity analyses of the model parameters reveal that the stage and discharge hydrographs are more sensitive to the Manning roughness and initial water content in the catchment than to the Green-Ampt head. Most notably, as the flash flood augments due to heavier rainfall, the modelling results agree with observed data better, which clearly characterizes the paramount role of rainfall in dictating the floods. From practical perspectives, the proposed model is more appropriate for modelling large flash floods.

AIM:To explore the intrinsic brain activity variations in retinal vein occlusion(RVO)subjects by using the voxel-wise degree centrality(DC)***:Twenty-one subjects with RVO and twentyone healthy controls(HCs)were enlisted and underwent the resting-state functional magnetic resonance imaging(rs-f MRI)*** spontaneous cerebrum activity variations were inspected using the DC *** receiver operating characteristic(ROC)curve was implemented to distinguish the DC values of RVOs from *** relationships between DC signal of definite regions of interest and the clinical characteristics in RVO group were evaluated by Pearson’s correlation ***:RVOs showed notably higher DC signals in right superior parietal lobule,middle frontal gyrus and left precuneus,but decreased DC signals in left middle temporal gyrus and bilateral anterior cingulated(BAC)when comparing with *** mean DC value of RVOs in the BAC were negatively correlated with the anxiety and depression ***:RVO is associated aberrant intrinsic brain activity patterns in several brain areas including painrelated as well as visual-related regions,which might assist to reveal the latent neural mechanisms.

Background RNA interference （RNAi） technology is emerging as a very potent tool to generate a cellular knockdown of a desired gene. The aim of this study was to explore whether RNAi targeting vascular endothelial growth factor-C （VEGF-C） could inhibit colorectal tumor lymphangiogenesis and tumor growth. Methods We used vector-based RNAi to inhibit VEGF-C expression in colon cancer in vitro and in vivo. VEGF-C expression was quantified by real-time polymerase chain reaction and Westen blotting. To establish LoVo cell tumor xenografts in mice, we subcutaneously inoculated 1.0×10^6 LoVo cells in nude mice; after injection, tumors were allowed to grow for 4 weeks until the volume reached （75.80± 55.8）mm^3. The mice were then randomly divided into two groups：（1） the VEGF-C-siRNA group （n= 10） received direct injection of ＂therapeutic＂ plasmid 50 IJg of LoVo-VEGF-C-siRNA into the tumor mass; （2） the control group （n= 10） were injected with LoVo-control in 20 IJI of sterile 0.9% NaCI solution into the tumor mass. Tumor growth, microlymphatics and microvessels were compared for mice administered either systemic VEGF-C-siRNA or control over 4 weeks. Results The mRNA and protein expression of VEGF-C in the colon tumor cell line, LoVo, stably transfected with a VEGF-C-siRNA vector, were significantly downregulated 45.3% and 35.3% respectively. In vivo, four weeks after injection, the tumor volume were significantly smaller in VEGF-C-siRNA group than in LoVo-control group （（314.8 ± 54.8） mm3 vs （553.9 ± 90.1） mm3）; the incidences of lymph node metastasis （30%） in VEGF-C-siRNA were significantly inhibited compared with LoVo-control group （70%）; in VEGF-C-siRNA group, the number of microlymphatics per microscopic field was （5.3 ± 0.7） and the number of microvessels per microscopic field was （24.2 ± 6.5） decreased compared with LoVo-control group （12.5 ± 6.9） and （42.1 ± 7.4）（all P〈0.001）. BACKGROUND Coronavirus disease 2019 (COVID-19) is a pandemic disease spreading all overthe world and has aroused global concerns. The increasing mortality has revealedits severity. It is important to distinguish severe patients and provide appropriatetreatment and care to prevent damages. Diabetes is reported to be a commoncomorbidity in COVID-19 patients and associated with higher mortality. Weattempted to clarify the relationship between diabetes and COVID-19 patients’*** determine the role of type 2 diabetes in COVID-19 *** study the relationship between diabetes and COVID-19, we retrospectivelycollected 61 patients’ data from a tertiary medical center in Wuhan. All thepatients were diagnosed with laboratory-confirmed COVID-19 and admitted tothe center from February 13 to March 1, 2020. Patients’ age, sex, laboratory tests,chest computed tomography findings, capillary blood glucose (BG), andtreatments were collected and analyzed. Fisher exact test was used for categoricaldata. Univariate and multivariate logistic regressions were used to explore therelationship between clinical characteristics and patients’ *** the 61 patients, the comorbidity of type 2 diabetes, hypertension, and heartdiseases were 24.6% (15 out of 61), 37.7% (23 out of 61), and 11.5% (7 out of 61),respectively. The diabetic group was related to more invasive treatments (P =0.02) and severe status (P = 0.003). In univariate logistic regression, histories ofdiabetes (OR = 7.13, P = 0.003), hypertension (OR = 3.41, P = 0.039), and hepaticdysfunction (OR = 7.69, P = 0.002) were predictors of patients’ severity while heart disease (OR = 4.21, P = 0.083) and large lung involvement (OR = 2.70, P = 0.093)also slightly exacerbated patients’ conditions. In the multivariate analysis,diabetes (OR = 6.29, P = 0.016) and hepatic dysfunction (OR = 5.88, P = 0.018)were risk factors for severe patients. 目的:观察针刺联合刺血及中药清瘟泄热汤对轻中型新型冠状病毒(COVID-19)感染患者的临床疗效及安全性。方法:将2022年12月至2023年2月收治的100例COVID-19轻中型患者,按照1∶1比例随机分为对照组和观察组,每组50例。对照组予以中药清瘟泄热汤口服,连续服用6 d。观察组在对照组基础上加用针刺联合刺血治疗,针刺每日1次,每次30 min,连续治疗6 d;刺血隔日1次,治疗3次。治疗前和治疗结束当日,记录患者的中医证候评分和肺部CT评分,采用ELISA法检测患者血清C反应蛋白(CRP)、白细胞介素6(IL-6)及肿瘤坏死因子α(TNF-α)水平,采用汉密尔顿抑郁量表(HAMD)及汉密尔顿焦虑量表(HAMA)评价患者抑郁焦虑水平,并评定两组治疗的安全性。结果:与本组治疗前比较,治疗后对照组患者在发热、干咳、咽痛和中医证候评分总分方面有所改善(P<0.01),而肌肉酸痛、乏力未见明显好转;观察组患者中医症候评分总分和各项证候均显著改善(P<0.01);两组患者肺部CT评分、HAMA和HAMD评分、血清CRP和IL-6含量均降低(P<0.01),观察组患者血清TNF-α含量降低(P<0.01)。治疗后观察组各项指标的改善均优于对照组(P<0.01,P<0.05)。两组患者均未出现不良反应。结论:针刺联合刺血及口服清瘟泄热汤治疗轻中型COVID-19感染,可明显改善患者临床症状,缓解肺部炎性损伤,降低炎性因子含量,减轻焦虑和抑郁情绪,且无不良反应发生。

该文研究了原花青素(oligomeric proanthocyanidins,OP)对长时间、大强度运动导致的心肌损伤的保护作用及其潜在机制。Wistar大鼠进行连续6周过量游泳运动训练,其中2组大鼠每天分别灌胃50 mg/kg OP(OP50组)和100 mg/kg OP(OP100组),模型组(model,Mod)灌胃等体积生理盐水,正常组(normal,Nor)作为对照。结果表明,Mod组血清中运动性心肌损伤标记物乳酸脱氢酶(lactate dehydrogenase,LDH)、肌酸激酶(creatine kinase,CK)及心肌肌钙蛋白I(cardiac troponin,cTnI)显著升高,心肌组织出现明显病理形态,呈现明显炎性细胞浸润;OP50及OP100组中LDH、CK及cTnI显著下调,心肌组织病理变化明显减轻(P<0.05)。心肌组织中,Mod组丙二醛(malondialdehyde,MDA)明显上升,抗氧化物酶谷胱甘肽(glutathione,GSH)及超氧化物歧化酶(superoxide dismutase,SOD)下降,核因子E2相关因子2(nuclear factor erythroid-2 related factor 2,Nrf2)/血红素加氧酶-1(hemeoxygenase-1,HO-1)(Nrf2/HO-1)抗氧化应激信号通路中关键蛋白的表达量显著下降;OP50及OP100组中MDA含量显著下调,GSH及SOD显著上调,Nrf2/HO-1通路中蛋白表达量显著增多(P<0.05)。Mod组血清及心肌中炎症指标肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)和白细胞介素-1β(interleukin-1β,IL-1β)含量明显上升,硫氧还蛋白互作蛋白/NOD样受体热蛋白结构域相关蛋白3(thioredoxin-interacting protein/NOD-like receptor thermal protein domain associated protein 3,TXNIP/NLRP3)炎症反应信号通路中关键蛋白的表达量显著上升;OP50及OP100组中炎症因子含量均显著下调,OP100组中TXNIP/NLRP3通路中蛋白表达量显著降低(P<0.05)。综上所述,补充OP对大鼠运动性心肌损伤具有明显的保护作用,且这种保护机制与增强心肌Nrf2/HO-1抗氧化通路及抑制TXNIP/NLRP3炎症反应信号通路有关。
