Successful treatment of hyperglycemia with liraglutide in a hospitalized 27-year-old patient with schizophrenia:A case report

BACKGROUND Antipsychotics are associated with abnormalities in glucose metabolism in patients with schizophrenia. Liraglutide, a GLP-1 receptor agonist, is Food and Drug Administration approved for the treatment of type 2 diabetes mellitus. However, ways to maintain the long-term stability of psychotic symptoms and balance the disadvantages of obesity, diabetes, and other metabolic disorders caused by antipsychotic medications remain unclear. In this study, we present a case of weight gain and hyperglycemia in a schizophrenia patient who received antipsychotic polypharmacy for 6 *** SUMMARY A 27-year-old man with olanzapine and sodium valproate-treated disorganized schizophrenia was admitted to a diabetes outpatient clinic. He was diagnosed with type 2 diabetes(fasting blood glucose, 20 mmol/L) and obesity(body mass index, 38.58 kg/m). The patient had been treated with glargine(40 IU/d) and metformin(1.5 g/d) and showed a poor response for 2 mo. Two years of liraglutide treatment resulted in stable blood glucose levels and weight loss in addition to a maintained stable mental status for a long time. The biological activities of GLP-1 significantly improved glucose levels and body weight in the schizophrenia patient treated with antipsychotic *** Liraglutide administration can be considered an effective alternative treatment for abnormalities in glucose metabolism in schizophrenia patients receiving antipsychotics.

As a complex wall system in flowering plants, the pollen outer wall mainly contains aliphatic sporopollenin; however, the mechanism for synthesizing these lipidic precursors during pollen development remains less well understood. Here, we report on the function of the rice tapetum-expressing TDR （Tapetum Degeneration Retardation） gene in aliphatic metabolism and its regulatory role during rice pollen development. The observations of transmission electron microscopy （TEM） and scanning electron microscopy （SEM） analyses suggested that pollen wall formation was significantly altered in the tdr mutant. The contents of aliphatic compositions of anther were greatly changed in the tdr mutant revealed by GC-MS （gas chromatography-mass spectrometry） testing, particularly less accumulated in fatty acids, primary alcohols, alkanes and alkenes, and an abnormal increase in secondary alcohols with carbon lengths from C29 to C3S in tdr. Microarray data revealed that a group of genes putatively involved in lipid transport and metabolism were significantly altered in the tdr mutant, indicating the critical role of TDR in the formation of the pollen wall. Also, a wide range of genes （236 in total--154 up-regulated and 82 down-regulated） exhibited statistically significant expressional differences between wild-type and tdr. In addition to its function in promoting tapetum PCD, TDR possibly plays crucial regulatory roles in several basic biological processes during rice pollen development.

AIM:To evaluate the predictive value of superficial retinal capillary plexus(SRCP)and radial peripapillary capillary(RPC)for visual field recovery after optic cross decompression and compare them with peripapillary nerve fiber layer(pRNFL)and ganglion cell complex(GCC).METHODS:This prospective longitudinal observational study included patients with chiasmal compression due to sellar region mass scheduled for decompressive *** estimating equations were used to compare retinal vessel density and retinal layer thickness preand post-operatively and with healthy *** regression models were used to assess the relationship between preoperative GCC,pRNFL,SRCP,and RPC parameters and visual field recovery after ***:The study included 43 eyes of 24 patients and 48 eyes of 24 healthy *** RPC and SRCP vessel density and pRNFL and GCC thickness were lower than healthy controls and higher than postoperative *** best predictive GCC and pRNFL models were based on the superior GCC[area under the curve(AUC)=0.866]and the tempo-inferior pRNFL(AUC=0.824),and the best predictive SRCP and RPC models were based on the nasal SRCP(AUC=0.718)and tempo-inferior RPC(AUC=0.825).There was no statistical difference in the predictive value of the superior GCC,tempo-inferior pRNFL,and tempo-inferior RPC(all P>0.05).CONCLUSION:Compression of the optic chiasm by tumors in the saddle area can reduce retinal thickness and blood *** reduction persists despite the recovery of the visual field after decompression ***,pRNFL,and RPC can be used as sensitive predictors of visual field recovery after decompression surgery.

Background Hyperprolactinemia is a common adverse reaction in patients with schizophrenia who take antipsychotic drugs;it often leads to treatment noncompliance in patients and has an adverse effect on their *** This study aimed to explore the risk factors of elevated prolactin(PRL)caused by risperidone(RIS)and olanzapine(OLZ)and the relationship between PRL and fasting plasma glucose and *** Patients with schizophrenia were divided into two groups:264 patients who were taking RIS and 175 patients who were taking *** two groups were further divided according to serum PRL levels:an elevated PRL group(>30ng/mL)and a normal PRL group(PRL<30ng/mL).The demographics,medication dosage,fasting plasma glucose,total cholesterol and triglycerides were compared in the two *** regression analysis was performed to explore the risk factors of elevated PRL *** Compared with the OLZ group,the RIS group had a greater number of patients with elevated PRL(155/264 vs 58/175).Either the RIS or the OLZ group,the proportion of elevated PRL was greater in female patients(RIS:x^2=6.76,p=0.009;OLZ:x^2=12.98,p<0.001)and with higher doses of the related drugs(RIS:U=-3.73,p<0.001;OLZ:U=-2.31,p=0.021).In patients taking RIS,the elevated PRL subgroup took the drug for a longer period(U=-2.76,p=0.006)and had lower triglyceride levels(U=2.76,p=0.006).In patients taking OLZ,the elevated PRL subgroup had lower fasting plasma glucose levels(U=2.29,p=0.022).Logistic regression analysis showed that gender,dose and fasting glucose levels were significantly associated with elevated PRL levels(RIS:p=0.001,OLZ:p<0.001;RIS:p<0.001;OLZ:p=0.003;RIS:p=0.020,OLZ:p=0.001,respectively).Conclusion Compared with OLZ,RIS had a greater effect on PRL in patients with schizophrenia,and in patients with schizophrenia taking RIS or OLZ,gender and dose were significantly correlated with the PRL ***,the plasma glucose level of the group with elevated PRL was lower

Acute lymphocytic leukemia(ALL)is one of the most common malignancies,especially in young *** chemotherapy for ALL typically includes corticosteroids(Kantaijian et al.,2000).Hyperglycemia is a well-recognized complication of corticosteroids,and chemotherapy-induced diabetes(CID)is not uncommon(27.5%-37.0%)during the treatment of ALL(Hsu et al.,2002;Weiser et al.,2004;Alves et al.,2007).Besides the effect of corticosteroids,potential factors triggering hyperglycemia in ALL also include direct infiltration of the pancreas by leukemia cells and β cell dysfunction induced by chemotherapeutic agents such as L-asparagine(Mohn et al.,2004).

目的:探讨胃癌组织中微淋巴管的生成特点及与临床病理特征之间的关系.方法:对胃癌56例及相应正常胃组织12例进行双重免疫组织化学染色,采用微淋巴管特异标记物Podoplanin标记微淋巴管,同时采用Ki-67来检测微淋巴管内皮细胞的增殖活性,并观察微淋巴管分布特点,分析其与临床病理特征之间的关系.结果:胃癌组织中存在微淋巴管,但分布不一致.癌内微淋巴管数目少,并且多呈闭锁条索状,癌周微淋巴管数目多且多扩张,呈管腔样,有时可在其中发现癌栓.胃癌癌周微淋巴管密度(11.89±3.95)与癌内微淋巴管密度(5.83±3.26)相比较,差别有显著统计学意义(P<0.01),与相应正常组织微淋巴管密度(6.93±1.32)相比较,差别亦有统计学意义(P<0.05).胃癌癌周微淋巴管内皮细胞Ki-67指数(0.04±0.02)分别与癌内微淋巴管内皮细胞Ki-67指数(0)及相应正常胃组织微淋巴管内皮细胞Ki-67指数(0)相比较,差别均有统计学意义(P<0.05).胃癌癌周微淋巴管密度与肿瘤的分化程度、淋巴结转移以及淋巴管受累相关(P<0.05).胃癌癌周微淋巴管内皮细胞Ki-67指数仅与淋巴管受累相关(P<0.05).结论:胃癌组织中存在新生淋巴管,且主要存在于癌周,癌周淋巴管是发生淋巴结转移的主要途径.

Tropomyosin receptor kinase B(TrkB)signaling plays a pivotal role in dendritic growth and dendritic spine formation to promote learning and *** activity-dependent release of brain-derived neurotrophic factor at synapses binds to pre-or postsynaptic TrkB resulting in the strengthening of synapses,reflected by long-term ***,the association of postsynaptic density protein-95 with TrkB enhances phospholipase Cγ-Ca^(2+)/calmodulin-dependent protein kinaseⅡand phosphatidylinositol 3-kinase-mechanistic target of rapamycin signaling required for long-term *** this review,we discuss TrkB-postsynaptic density protein-95 coupling as a promising strategy to magnify brain-derived neurotrophic factor signaling towards the development of novel therapeutics for specific neurological disorders.A reduction of TrkB signaling has been observed in neurodegenerative disorders,such as Alzheimer's disease and Huntington's disease,and enhancement of postsynaptic density protein-95 association with TrkB signaling could mitigate the observed deficiency of neuronal connectivity in schizophrenia and *** with brain-derived neurotrophic factor is problematic,due to poor pharmacokinetics,low brain penetration,and side effects resulting from activation of the p75 neurotrophin receptor or the truncated TrkB.T1 *** TrkB agonists and antibodies that activate TrkB are being intensively investigated,they cannot distinguish the multiple human TrkB splicing isoforms or cell type-specific *** TrkB–postsynaptic density protein-95 coupling provides an alternative approach to specifically boost TrkB signaling at localized synaptic sites versus global stimulation that risks many adverse side effects.

A novel polysaccharide FVPB2 was purified from fruiting bodies of Flammulina velutipes. Its structure was elucidated by monosaccharide composition and methylation analyses, UV-Visible and FTIR spectroscopy as well as NMR. FVPB2 was a homogeneous heteropolysaccharide（molecular weight ～1.50×104 Da） containing D-galactose, D-mannose, L-fucose, and D-glucose at molar ratio of 1.9:1.2:1:2.5. In vitro immunomodulatory studies showed FVPB2 induced proliferation of mouse spleen lymphocytes in a dose-dependent manner. The levels of Ig M and IgG, secreted by B cells, increased after FVPB2 treatment. So FVPB2 has potential to be a new important immunomodulatory nutraceutical.

BACKGROUND Return to work(RTW)serves as an indication for young and middle-aged colorectal cancer(CRC)survivors to resume their normal social ***,these survivors encounter significant challenges during their RTW ***,scientific research is necessary to explore the barriers and facilitating factors of returning to work for young and middle-aged CRC *** To examine the current RTW status among young and middle-aged CRC survivors and to analyze the impact of RTW self-efficacy(RTW-SE),fear of progression(FoP),eHealth literacy(eHL),family resilience(FR),and financial toxicity(FT)on their RTW *** A cross-sectional investigation was adopted in this *** September 2022 to February 2023,a total of 209 participants were recruited through a convenience sampling method from the gastrointestinal surgery department of a class A tertiary hospital in *** investigation utilized a general information questionnaire alongside scales assessing RTW-SE,FoP,eHL,FR,and *** analyze the factors that influence RTW outcomes among young and middle-aged CRC survivors,Cox regression modeling and Kaplan-Meier survival analysis were *** A total of 43.54%of the participants successfully returned to work,with an average RTW time of 100 *** regression univariate analysis revealed that RTW-SE,FoP,eHL,FR,and FT were significantly different between the non-RTW and RTW groups(P<0.05).Furthermore,Cox regression multivariate analysis identified per capita family monthly income,job type,RTW-SE,and FR as independent influencing factors for RTW(P<0.05).CONCLUSION The RTW rate requires further *** levels of RTW-SE and FR were found to significantly increase RTW among young and middle-aged CRC *** professionals should focus on modifiable factors,such as RTW-SE and FR,to design targeted RTW support programs,thereby facilitating their timely reintegration into mainstream society.
