Core muscle functional strength training for reducing the risk of low back pain in military recruits:An open-label randomized controlled trial

Background:Core muscle functional strength training(CMFST)has been reported to reduce injuries to the lower extremity.However,no study has confirmed whether CMFST can reduce the risk of low back pain(LBP).Objective:This study identified the effects of CMFST on the incidence of LBP in military recruits.Design,setting,participants and intervention:We performed a prospective,open-label,randomized,controlled study in a population of young healthy male naval recruits from a Chinese basic combat training program.Participants were randomly assigned to either the core group or the control group.In additional to normal basic combat training,recruits in the core group underwent a CMFST program for 12 weeks,while recruits in the control group received no extra training.Main outcome measures:At the beginning of the study and at the 12 th week,the number of participants with LBP was counted,and lumbar muscle endurance was measured.In addition,when participants complained of LBP,they were assessed using the visual analog scale(VAS)and Roland Morris Disability Questionnaire(RMDQ).Results:A total of 588 participants were included in the final analysis(295 in the core group and 293 in the control group).The incidence of LBP in the control group was about twice that of the core group over the 12-week study(20.8%vs 10.8%,odds ratio:2.161–2.159,P<0.001).The core group had better lumbar muscle endurance at 12 weeks than the control group([200.80±92.98]s vs[147.00±84.51]s,P<0.01).There was no significant difference in VAS score between groups,but the core group had a significantly lower RMDQ score at week 12 than the control group(3.33±0.58 vs 5.47±4.41,P<0.05).Conclusion:This study demonstrated that the CMFST effectively reduced the incidence of LBP,improved lumbar muscle endurance,and relieved the dysfunction of LBP during basic military training.

Effect of uninterrupted dabigatran or rivaroxaban on achieving ideal activated clotting time to heparin response during catheter ablation in patients with atrial fibrillation

BACKGROUND Uninterrupted use of oral anticoagulants before atrial fibrillation(AF)ablation can reduce the incidence of perioperative thromboembolic events.However,the effect of new oral anticoagulants on activated clotting time(ACT)in respo-nse to heparin during AF ablation in Chinese populations remains unknown.The aim of the present retrospective study was to investigate the value of ACTs in response to intraoperative heparin administration in patients using dabigatran or rivaroxaban.METHODS From January 2018 to December 2021,a total of 173 patients undergoing AF ablation were included in the study,in which 101 patients were treated with dabigatran,72 patients were treated with rivaroxaban.The intraoperative ACT values were examined in both groups.The incidence of periprocedural complications was evaluated.RESULTS Initial heparin dosage(88±19 U/kg vs.78±27 U/kg,P<0.05),total heparin dosage(137±41 U/kg vs.106±52 U/kg,P<0.05)during the ablation procedure were higher in the dabigatran group than those in the rivaroxaban group.Mean ACT(280±36 s vs.265±30 s,P<0.05),and the percentage of ACTs within the therapeutic range(250-350 s)(74%±26%vs.60%±29%,P<0.05)were significantly lower in the dabigatran group than those in the rivaroxaban group,particularly in male pati-ents.Furthermore,the average time of achieving the target ACT(250-350 s)was also found longer in the dabigatran group(P<0.05)as compared with the rivaroxaban group.No significant difference was found in the incidence of periprocedural complica-tions between the two groups.CONCLUSIONS The anticoagulant effect of uninterrupted rivaroxaban therapy appears to be more stable and efficient than dabigatran administration during catheter ablation in patients with AF.

Low profile cavity-embedded ultra-wideband UHF array antenna with end-fire beams

A low-profile,vertically polarized,ultra-wideband array antenna with end-fire beams operating in an ultra-high frequency(UHF)band is developed in this paper.The array antenna consists of 1×16 log-periodic top-hat loaded monopole antenna arrays and is feasible to embed into a shallow cavity to further reduce the array height.Capacitance is introduced in the proposed antenna element to reduce profile height and the rectangular top hats are carefully designed to minimize the transverse dimension.Simulated results show that when the antenna array operates in a frequency range of 300 MHz-900 MHz,the end-fire radiation pattern achieves±45°scanning range in the horizontal plane.Then prototypes of the proposed end-fire antenna element and a uniformly spaced linear array(1×2)are fabricated and validated.The end-fire antenna array should be suitable for airborne applications where low-profile and conformal scanning phased antenna arrays with end-fire radiations are required.This design is attractive for airborne platform applications that are used to search,discover,identify,and scout the aerial target with vertically polarized beams.

Design Strategies for Aqueous Zinc Metal Batteries with High Zinc Utilization: From Metal Anodes to Anode-Free Structures

Aqueous zinc metal batteries(AZMBs)are promising candidates for next-generation energy storage due to the excellent safety, environmental friendliness, natural abundance, high theoretical specific capacity, and low redox potential of zinc(Zn) metal. However,several issues such as dendrite formation, hydrogen evolution, corrosion, and passivation of Zn metal anodes cause irreversible loss of the active materials. To solve these issues, researchers often use large amounts of excess Zn to ensure a continuous supply of active materials for Zn anodes. This leads to the ultralow utilization of Zn anodes and squanders the high energy density of AZMBs. Herein, the design strategies for AZMBs with high Zn utilization are discussed in depth, from utilizing thinner Zn foils to constructing anode-free structures with theoretical Zn utilization of 100%, which provides comprehensive guidelines for further research. Representative methods for calculating the depth of discharge of Zn anodes with different structures are first summarized. The reasonable modification strategies of Zn foil anodes, current collectors with pre-deposited Zn, and anode-free aqueous Zn metal batteries(AF-AZMBs) to improve Zn utilization are then detailed. In particular, the working mechanism of AF-AZMBs is systematically introduced. Finally, the challenges and perspectives for constructing high-utilization Zn anodes are presented.

BACKGROUND Monomorphic epithelial intestinal T-cell lymphoma(MEITL)is a rare type of peripheral T-cell lymphoma.The clinical manifestations are diarrhea,abdominal pain,perforation and an abdominal mass.CASE SUMMARY We present a 52-year-old female patient who was diagnosed with MEITL.Further disease progression was observed after multiline chemotherapy.Eventually,the patient died of a severe infection.CONCLUSION MEITL is a rare intestinal primary T-cell lymphoma with aggressive behavior,a high risk of severe life-threatening complications,and a poor prognosis.

Does ergogenic effect of caffeine supplementation depend on CYP1A2 genotypes? A systematic review with meta-analysis

Background:The ergogenic effects of caffeine intake on exercise performance are well-established,even if differences exist among individuals in response to caffeine intake.The genetic variation of a specific gene,human cytochrome P450 enzyme 1A2(CYP1A2)(rs762551),may be one reason for this difference.This systematic review and meta-analysis aimed to comprehensively evaluate the influence of CYP1A2 gene types on athletes’exercise performance after caffeine intake.Methods:A literature search through 4 databases(Web of Science,PubMed,Scopus,and China National Knowledge Infrastructure)was conducted until March 2023.The effect size was expressed as the weighted mean difference(WMD)by calculating fixed effects meta-analysis if heterogeneity was not significant(I^(2)≤50%and p≥0.1).Subgroup analyses were performed based on AA and AC/CC genotype of CYP1A2.Results:The final number of studies meeting the inclusion criteria was 12(n=666 participants).The overall analysis showed that the cycling time trial significantly improved after caffeine intake(WMD=-0.48,95%confidence interval(95%CI):-0.83 to-0.13,p=0.007).In subgroup analyses,acute caffeine intake improved cycling time trial only in individuals with the A allele(WMD=-0.90,95%CI:-1.48 to-0.33,p=0.002),but not the C allele(WMD=-0.08,95%CI:-0.32 to 0.17,p=0.53).Caffeine supplementation did not influence the Wingate(WMD=8.07,95%CI:-22.04 to 38.18,p=0.60)or countermovement jump test(CMJ)performance(WMD=1.17,95%CI:-0.02 to 2.36,p=0.05),and these outcomes were not influenced by CYP1A2 genotype.Conclusion:Participants with the CYP1A2 genotype with A allele improved their cycling time trials after caffeine supplementation.However,compared to placebo,acute caffeine supplementation failed to increase the Wingate or CMJ performance,regardless of CYP1A2 genotype.

In smart classrooms, conducting multi-face expression recognition based on existing hardware devices to assessstudents’ group emotions can provide educators with more comprehensive and intuitive classroom effect analysis,thereby continuouslypromotingthe improvementof teaching quality.However,most existingmulti-face expressionrecognition methods adopt a multi-stage approach, with an overall complex process, poor real-time performance,and insufficient generalization ability. In addition, the existing facial expression datasets are mostly single faceimages, which are of low quality and lack specificity, also restricting the development of this research. This paperaims to propose an end-to-end high-performance multi-face expression recognition algorithm model suitable forsmart classrooms, construct a high-quality multi-face expression dataset to support algorithm research, and applythe model to group emotion assessment to expand its application value. To this end, we propose an end-to-endmulti-face expression recognition algorithm model for smart classrooms (E2E-MFERC). In order to provide highqualityand highly targeted data support for model research, we constructed a multi-face expression dataset inreal classrooms (MFED), containing 2,385 images and a total of 18,712 expression labels, collected from smartclassrooms. In constructing E2E-MFERC, by introducing Re-parameterization visual geometry group (RepVGG)block and symmetric positive definite convolution (SPD-Conv) modules to enhance representational capability;combined with the cross stage partial network fusion module optimized by attention mechanism (C2f_Attention),it strengthens the ability to extract key information;adopts asymptotic feature pyramid network (AFPN) featurefusion tailored to classroomscenes and optimizes the head prediction output size;achieves high-performance endto-end multi-face expression detection. Finally, we apply the model to smart classroom group emotion assessmentand provide design references for classroom effect analysis evaluation

Effect of early stepwise cardiopulmonary rehabilitation on function and quality of life in sepsis patients

BACKGROUND Sepsis,as a non-limiting host infection disease,can be accompanied by serious complications such as organ failure,which seriously threatens patient quality of life.AIM To investigate the effect of early stepwise cardiopulmonary rehabilitation on cardiopulmonary function and quality of life in patients evacuated from mechanical ventilation with sepsis.METHODS A total of 80 patients with sepsis who were hospitalized in our hospital from January 2021 to January 2022 were selected and divided into the observation group(n=40)and the control group(n=40)according to the random number table method.The observation group was treated with early stepwise cardiopulmonary rehabilitation,and the control group was treated with a conventional treatment regimen.Cardiac function indexes(central venous pressure,cardiac troponin I,B-type brain natriuretic peptide),lung function indicators(diaphragmatic mobility,changes in central venous oxygen saturation,oxygenation index),and quality of life(Quality of Life Evaluation Scale)were compared between the two groups after treatment.RESULTS After treatment,the central venous pressure,diaphragm mobility,central venous oxygen saturation,oxygenation index,and Quality of Life Evaluation Scale scores in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The observation group was less than that of the control group for other parameters,and the differences were statistically significant(P<0.05).CONCLUSION Early stepwise cardiopulmonary rehabilitation can effectively enhance cardiac and pulmonary function and improve the quality of life in patients evacuated from mechanical ventilation with sepsis.

Quantitative characterization of fusion state of virgin and aged asphalt using nano-ferrous ferric oxide tracer

The exact evaluation of the fusion rate of virgin and aged asphalt is essential to optimize the performance of the recycled asphalt mixture. The fusion rate index was developed by tracing the distribution of tracer asphalt, which contains nano-ferrous ferric oxide（nano-Fe3O4） tracer. Laboratory fusion simulation tests were performed on three types of asphalt to obtain different fusion states, and fatigue tests were performed to evaluate the influence of the fusion rate on the fatigue life of the recycled asphalt. It was found that the tracer asphalt containing 4% nano-Fe3O4shows the optimal trace effect, and the developed fusion rate index can quantitatively characterize the influence of different factors on the fusion state of the recycled asphalt. The fusion state is closely related to the fatigue performance of recycled asphalt, and the fusion rate index can fairly evaluate its fatigue performance.The relationship between the fusion rate and the fatigue life of recycled asphalt depends on the factors that cause the fusion rate variation. Under the influence of the amount of rejuvenator, the fatigue life of recycled asphalt grows exponentially with increasing fusion rate due to enhancement of molecular motion as well as a supplement of lightweight components. Under the influence of heat preservation treatment, fatigue life increases linearly with increasing fusion rate. The proposed fusion rate index can be utilized to quantitatively characterize the fusion state of virgin and aged asphalt, thus providing a simple way to guide the practical application of recycled asphalt.

Analysis on the causes of massive stranding of Yellow Sea green tide on Lianyungang and Rizhao coasts in 2022

In 2022,Yellow Sea green tide caused by Ulva prolifera reached a historic minimum and the coastal areas of Shandong Peninsula were less affected.However,the largest amount of seaweed biomass has been washed ashore on Lianyungang and Rizhao coasts since 2015.We studied the development pattern of Yellow Sea green tide in 2022,and analyzed the key environmental factors on the growth and drifting,then discussed the possible reasons that resulted in the massive stranding of green tide biomass in Lianyungang and Rizhao.Results show under the combined influence of the east to southeast winds and currents with shoreward anomalies,green tide drifted to the coastal waters between Shandong and Jiangsu provinces and the distribution areas located westward compared with previous years(2008–2021).Floating U.prolifera rafts from the coastal waters of Binhai and Sheyang drifted continuously into the coastal waters of Lianyungang and Rizhao,providing important supplements for Yellow Sea green tide.Because green tide in 2022 distributed close to the coastal waters,the abundant nutrients might support their continuous high growth rate.In addition,the amount of rainfall around Shandong Peninsula from late June to early July were significantly higher than in previous years,which might promote the development of green tide to some extent.
