Influence of initial check, information exchange, final accuracy check, reaction information nursing on the psychology of elderly with lung cancer

BACKGROUND As one of the fatal diseases with high incidence,lung cancer has seriously endangered public health and safety.Elderly patients usually have poor self-care and are more likely to show a series of psychological problems.AIM To investigate the effectiveness of the initial check,information exchange,final accuracy check,reaction(IIFAR)information care model on the mental health status of elderly patients with lung cancer.METHODS This study is a single-centre study.We randomly recruited 60 elderly patients with lung cancer who attended our hospital from January 2021 to January 2022.These elderly patients with lung cancer were randomly divided into two groups,with the control group taking the conventional propaganda and education and the observation group taking the IIFAR information care model based on the conventional care protocol.The differences in psychological distress,anxiety and depression,life quality,fatigue,and the locus of control in psychology were compared between these two groups,and the causes of psychological distress were analyzed.RESULTS After the intervention,Distress Thermometer,Hospital Anxiety and Depression Scale(HADS)for anxiety and the HADS for depression,Revised Piper’s Fatigue Scale,and Chance Health Locus of Control scores were lower in the observation group compared to the pre-intervention period in the same group and were significantly lower in the observation group compared to those of the control group(Pmation care model can help elderly patients with lung cancer by reducing their anxiety and depression,psychological distress,and fatigue,improving the

High-Precision Doppler Frequency Estimation Based Positioning Using OTFS Modulations by Red and Blue Frequency Shift Discriminator

Orthogonal Time Frequency and Space(OTFS) modulation is expected to provide high-speed and ultra-reliable communications for emerging mobile applications, including low-orbit satellite communications. Using the Doppler frequency for positioning is a promising research direction on communication and navigation integration. To tackle the high Doppler frequency and low signal-to-noise ratio(SNR) in satellite communication, this paper proposes a Red and Blue Frequency Shift Discriminator(RBFSD) based on the pseudo-noise(PN) sequence.The paper derives that the cross-correlation function on the Doppler domain exhibits the characteristic of a Sinc function. Therefore, it applies modulation onto the Delay-Doppler domain using PN sequence and adjusts Doppler frequency estimation by red-shifting or blue-shifting. Simulation results show that the performance of Doppler frequency estimation is close to the Cramér-Rao Lower Bound when the SNR is greater than -15dB. The proposed algorithm is about 1/D times less complex than the existing PN pilot sequence algorithm, where D is the resolution of the fractional Doppler.

Coexistence of liver abscess, hepatic cystic echinococcosis and hepatocellular carcinoma: A case report

BACKGROUND Human cystic echinococcosis(CE)is a life-threatening zoonosis caused by the Echinococcus granulosus(sensu lato).Hepatocellular carcinoma(HCC)is a leading cause of cancer-related mortality in the world.The coexistence of CE and HCC is exceedingly rare,and only several well-documented cases have been reported.In addition to this coexistence,there is no report of the coexistence of CE,HCC,and liver abscess to date.Herein,we aimed to report a case of coexistence of liver abscess,hepatic CE,and HCC.CASE SUMmaRY A 65-year-old herdsman presented to the department of interventional therapy with jaundice,right upper abdominal distension and pain for 10 d.Laboratory test showed that he had positive results for HBsAg,HBeAb,HBcAb,and echino-coccosis IgG antibody.The test also showed an increased level of alpha fetopro-tein of 3400 ng/mL.An abdominal computed tomography(CT)scan revealed an uneven enhanced lesion of the liver at the arterial phase with enhancement and was located S4/8 segment of the liver.In addition,CT scan also revealed a mass in the S6 segment of the liver with a thick calcified wall and according to current guideline and medical images,the diagnoses of hepatic CE(CE4 subtype)and HCC were established.Initially,transarterial chemoembolization was performed for HCC.In the follow-up,liver abscess occurred in addition to CE and HCC;thus,percutaneous liver puncture drainage was performed.In the next follow-up,CE and HCC were stable.The liver abscess was completely resolved,and the patient was discharged with no evidence of recurrence.CONCLUSION This is the first reported case on the coexistence of liver abscess,hepatic CE,and HCC.Individualized treatment and multidisciplinary discussions should be performed in this setting.Therefore,treatment and diagnosis should be based on the characteristics of liver abscess,hepatic CE,and HCC,and in future clinical work,it is necessary to be aware of the possibility of this complex composition of liver diseases.

KAT7/HMGN1 signaling epigenetically induces tyrosine phosphorylation-regulated kinase 1A expression to ameliorate insulin resistance in Alzheimer’s disease

BACKGROUND Epidemiological studies have revealed a correlation between Alzheimer’s disease(AD)and type 2 diabetes mellitus(T2D).Insulin resistance in the brain is a common feature in patients with T2D and AD.KAT7 is a histone acetyltransferase that participates in the modulation of various genes.AIM To determine the effects of KAT7 on insulin patients with AD.METHODS APPswe/PS1-dE9 double-transgenic and db/db mice were used to mimic AD and diabetes,respectively.An in vitro model of AD was established by Aβstimulation.Insulin resistance was induced by chronic stimulation with high insulin levels.The expression of microtubule-associated protein 2(maP2)was assessed using immunofluorescence.The protein levels of maP2,Aβ,dual-specificity tyrosine phosphorylation-regulated kinase-1A(DYRK1A),IRS-1,p-AKT,total AKT,p-GSK3β,total GSK3β,DYRK1A,and KAT7 were measured via western blotting.Accumulation of reactive oxygen species(ROS),malondialdehyde(MDA),and SOD activity was measured to determine cellular oxidative stress.Flow cytometry and CCK-8 assay were performed to evaluate neuronal cell death and proliferation,respectively.Relative RNA levels of KAT7 and DYRK1A were examined using quantitative PCR.A chromatin immunoprecipitation assay was conducted to detect H3K14ac in DYRK1A.RESULTS KAT7 expression was suppressed in the AD mice.Overexpression of KAT7 decreased Aβaccumulation and maP2 expression in AD brains.KAT7 overexpression decreased ROS and MDA levels,elevated SOD activity in brain tissues and neurons,and simultaneously suppressed neuronal apoptosis.KAT7 upregulated levels of p-AKT and p-GSK3βto alleviate insulin resistance,along with elevated expression of DYRK1A.KAT7 depletion suppressed DYRK1A expression and impaired H3K14ac of DYRK1A.HMGN1 overexpression recovered DYRK1A levels and reversed insulin resistance caused by KAT7 depletion.CONCLUSION We determined that KAT7 overexpression recovered insulin sensitivity in AD by recruiting HMGN1 to enhance DYRK1A acetylation.Our findings suggest that KAT7 is a novel and

Enrichment of anammox biomass during mainstream wastewater treatment driven by achievement of partial denitrification through the addition of bio-carriers

Anammox is widely considered as the most cost-effective and sustainable process for nitrogen removal.However,how to achieve the enrichment of anammox biomass remains a challenge for its large-scale application,especially in mainstream wastewater treatment.In this study,the feasibility of enrichment of anammox biomass was explored through the realization of partial denitrification and the addition of bio-carriers.By using ordinary activated sludge,a sequencing batch reactor(SBR)followed by an up-fow anaerobic sludge bed(UASB)was operated at 25±2℃ for 214 days.The long-term operation was divided into five phases,in which SBR and UASB were started-up in Phases I and II,respectively.By eliminating oxygen and adjusting the infow ratios in Phases III-V,advanced nitrogen removal was achieved with the effuent total nitrogen being 4.7 mg/L and the nitrogen removal efficiency being 90.5%in Phase V.Both in-situ and ex-situ activity tests demonstrated the occurrence of partial denitrification and anammox.Moreover,16S rRNA high-throughput sequencing analysis revealed that Candidatus Brocadia was enriched from below the detection limit to in biofilms(0.4%in SBR,2.2%in UASB)and the foc sludge(0.2%in SBR,1.3%in UASB),while Thauera was mainly detected in the foc sludge(8.1%in SBR,8.8%in UASB),which might play a key role in partial denitrification.Overall,this study provides a novel strategy to enrich anammox biomass driven by rapid achievement of partial denitrification through the addition of bio-carriers,which will improve large-scale application of anammox processes in mainstream wastewater treatment.

Hydroxysafflor Yellow A Promotes HaCaT Cell Proliferation and Migration by Regulating HBEGF/EGFR and PI3K/AKT Pathways and Circ_0084443

Objective:To investigate the effect and possible mechanism of hydroxysafflor yellow A(HSYA) on human immortalized keratinocyte cell proliferation and migration.Methods:HaCaT cells were treated with HSYA.Cell proliferation was detected by the cell counting kit-8 assay,and cell migration was measured using wound healing assay and Transwell migration assay.The mRNA and protein expression levels of heparin-binding epidermal growth factor(EGF)-like growth factor(HBEGF),EGF receptor(EGFR),phosphatidylinositol 3-kinase(PI3K),protein kinase B(AKT),mammalian target of rapamycin(mTOR),and hypoxia-inducible factor-1α(HIF-1α) were detected by quantitative real-time polymerase chain reaction(qRT-PCR) and Western blot,respectively.Circ_0084443-overexpressing HaCaT cells and empty plasmid HaCaT cells were constructed using the lentiviral stable transfection and treated with HSYA.The expression of circ_0084443 was detected by qRT-PCR.Results:HSYA(800 μmol/L) significantly promoted HaCaT cell proliferation and migration(P0.05).Conclusion:HSYA played an accelerative role in HaCaT cell proliferation and migration,which may be attributable to activating HBEGF/EGFR and PI3K/AKT signaling pathways,and had a particular inhibitory effect on the keratinocyte negative regulator circ_0084443.

Interdecadal Variations of ENSO Impacts over the Indo-Northwest Pacific Region and the Related Mechanisms

Owing to limited observations,it remains unknown whether the impact of El Niño-Southern Oscillation(ENSO)on the Indian Ocean-Northwest Pacific(IO-NWP)climate showed decadal changes in the early 20th century.Using multi-source reanalysis and hindcast datasets from the ECMWF and NOAA extending back to 1901,this study investigates interdecadal variations of the impact of ENSO on the IO-NWP climate from 1901 to 2009.It is found that the influence of ENSO on the IO-NWP climate shows“strong-weak-strong”interdecadal change during 1901-2009.This is characterized by much weaker Indian Ocean sea surface temperature(SST)warming and a weaker NWP subtropical anticyclone(NWPSA)in the following summer of El Ni?o during 1946-1967,compared with those in the other two periods(1901-1945 and 1968-2009).Analyses of the datasets indicate that the interdecadal variation is mainly associated with the change in ENSO amplitude.In contrast to the period of 1946-1967,a greater SST variance occurred in the central-eastern equatorial Pacific during 1901-1945 and 1968-2009.A stronger El Ni?o tends to generate more significant anticyclonic anomalies over the southeast Indian Ocean through teleconnection.The northwesterly anomalies to the south of the anticyclone weaken the southeast trade winds and warm the south Indian Ocean SST via wind-evaporation-SST feedback,and the positive south Indian Ocean SST anomalies trigger westward-propagating oceanic Rossby waves to induce stronger warming of the southwest Indian Ocean,leading to a significant asymmetric wind pattern across the equator in spring.The profound northeastward winds on the north side weaken the southwest monsoon,leading to a“second warming”over the north Indian Ocean in summer,which anchors the eastward-propagating warm Kelvin waves and results in a stronger NWPSA by inducing surface divergence and suppressing deep convection.

Effect of low-degree astigmatism on the objective visual quality

AIM:To evaluate the effect of low-degree astigmatism on objective visual quality through the Optical Quality Analysis System(OQAS).METHODS:This study enrolled 46 participants(aged 23 to 30y,90 eyes)with normal or corrected-to-normal vision.The cylindrical lenses(0,0.5,0.75,1.0,and 1.25 D)were placed at the axial direction(180°,45°,90°,and 135°)in front of the eyes with the best correction to form 16 types of regular low-degree astigmatism.OQAS was used to detect the objective visual quality,recorded as the objective scattering index(OSI),OQAS values at contrasts of 100%,20%,and 9%predictive visual acuity(OV100%,OV20%,and OV9%),modulation transfer function cut-off(MTFcut-off)and Strehl ratio(SR).The mixed effect linear model was used to compare objective visual quality differences between groups and examine associations between astigmatic magnitude and objective visual quality parameters.RESULTS:Apparent negative relationships between the magnitude of low astigmatism and objective visual quality were observed.The increase of OSI per degree of astigmatism at 180°,45°,90°,and 135°axis were 0.38(95%CI:0.35,0.42),0.50(95%CI:0.46,0.53),0.49(95%CI:0.45,0.54)and 0.37(95%CI:0.34,0.41),respectively.The decrease of MTFcut-off per degree of astigmatism at 180°,45°,90°,and 135°axis were-10.30(95%CI:-11.43,-9.16),-12.73(95%CI:-13.62,-11.86),-12.75(95%CI:-13.79,-11.70),and-9.97(95%CI:-10.92,-9.03),respectively.At the same astigmatism degree,OSI at 45°and 90°axis were higher than that at 0°and 135°axis,while MTFcut-off were lower.CONCLUSION:Low astigmatism of only 0.50 D can significantly reduce the objective visual quality.

The Multi-parameter Test of Gravitational Wave Dispersion with Principal Component Analysis

In this work, we consider a conventional test of gravitational wave(GW) propagation which is based on the phenomenological parameterized dispersion relation to describe potential departures from General Relativity(GR)along the propagation of GWs. But different from tests conventionally performed previously, we vary multiple deformation coefficients simultaneously and employ the principal component analysis(PCA) method to remedy the strong degeneracy among deformation coefficients and obtain informative posteriors. The dominant PCA components can be better measured and constrained, and thus are expected to be more sensitive to potential departures from the waveform model. Using this method we analyze ten selected events and get the result that the combined posteriors of the dominant PCA parameters are consistent with GR within 99.7% credible intervals. The standard deviation of the first dominant PCA parameter is three times smaller than that of the original dispersion parameter of the leading order. However, the multi-parameter test with PCA is more sensitive to not only potential deviations from GR but also systematic errors of waveform models. The difference in results obtained by using different waveform templates hints that the demands of waveform accuracy are higher to perform the multiparameter test with PCA. Whereas, it cannot be strictly proven that the deviation is indeed and only induced by systematic errors. It requires more thorough research in the future to exclude other possible reasons in parameter estimation and data processing.

The"4+4"Medical Doctor(MD)Pilot Program at PUMC:Implementation,Performance,and Prospects

Peking Union Medical College(PUMC)launched the"4+4"Medical Doctor(MD)pilot program in 2018,admitting students with non-medical backgrounds from top universities,aligning with national medical talent training policies to foster diverse and eager learners in medicine.On the occasion of the graduation of the first class of the"4+4"MD pilot class at PUMC in 2023,we reviewed the teaching reform in the pilot program and carried out a systematic survey and interviews with students,faculties,and management staff of the pilot class.This article reports on the measures taken by the pilot class at PUMC in enrollment and curriculum setting,and demonstrates the achievements of the pilot class in terms of student academic background structure,knowledge acquisition and skill learning,scientific research ability,and course evaluation.The results indicated that the pilot class had met the national demand for the"Medicine+X"talent training model.More specifically,with a diverse academic backgrounds,the pilot class graduates had academic levels comparable to the eight-year medical education graduates,and their scientific research abilities were satisfactory.The pilot program at PUMC will optimize the curriculum setting,strengthen the construction of faculty,learning resources,and teaching facilities,and reform the academic evaluation methods,thus deepening the reform of medical education and improving the"4+4"MD program as a novel medical education model.
