BACKGROUND Stress hyperglycemia ratio(SHR)could provide accurate information on the acute status of hyperglycemia.The relationship between SHR and acute coronary syndrome(ACS)prognosis remains unclear.This study was conducted to identity the association between SHR and in-hospital outcomes in patients with ACS.METHODS A total of 12,010 patients were eventually enrolled in the study.The relationship between SHR and in-hospital major adverse cardiovascular events(MACEs)was then modeled by restricted cubic spline(RCS)curves,and all patients were divided into three groups according to the results.The multivariate logistic regression analysis was used to determine the associations between the SHR and in-hospital outcomes,described as odds ratios(ORs)and 95%confidence intervals(CIs).Subgroup analyses were also performed on different diseases.RESULTS The median age of this cohort was 63(54,71)years old,and 8942(74.5%)were male.Group 1 was defined as SHR1(n=5763).Compared with Group 2,Group 1(OR=1.891,95%CI:1.028-3.479,P<0.001)and Group 3(OR=1.868,95%CI:1.434-2.434,P<0.001)had higher risks of suffering from in-hospital MACEs.SHR was associated with higher risks of in-hospital MACEs in the subgroups of DM[OR=2.282,95%CI:1.477-3.524).CONCLUSIONS Both low and high SHR levels were independently associated with in-hospital MACEs.Young males with DM,hypertension,and decreased renal function had much higher risks of suffering from SHR-correlated MACEs.

Recent years have witnessed the great success of self-supervised learning(SSL)in recommendation systems.However,SSL recommender models are likely to suffer from spurious correlations,leading to poor generalization.To mitigate spurious correlations,existing work usually pursues ID-based SSL recommendation or utilizes feature engineering to identify spurious features.Nevertheless,ID-based SSL approaches sacrifice the positive impact of invariant features,while feature engineering methods require high-cost human labeling.To address the problems,we aim to automatically mitigate the effect of spurious correlations.This objective requires to 1)automatically mask spurious features without supervision,and 2)block the negative effect transmission from spurious features to other features during SSL.To handle the two challenges,we propose an invariant feature learning framework,which first divides user-item interactions into multiple environments with distribution shifts and then learns a feature mask mechanism to capture invariant features across environments.Based on the mask mechanism,we can remove the spurious features for robust predictions and block the negative effect transmission via mask-guided feature augmentation.Extensive experiments on two datasets demonstrate the effectiveness of the proposed framework in mitigating spurious correlations and improving the generalization abilities of SSL models.

BACKGROUND The prognostic assessment of patients after surgical resection of gastric cancer(GC)patients is critical.However,the role of the circadian clock gene NPAS2 expression in GC remains unknown.AIM To explore the relationship between NPAS2 and the survival prognosis of GC patients and clarify its role in evaluating GC prognosis.METHODS The tumor tissues and clinical data of 101 patients with GC were collected retrospectively.Immunohistochemical staining(IHC)was used to detect the expression of NPAS2 protein in GC and adjacent tissues.Univariate and multivariate Cox regression analysis was used to determine the independent prognostic factors of GC,and a nomogram prediction model was established.The receiver operating characteristic(ROC)curve,the ROC area under the curve,the calibration curve,and C-index were used to evaluate the predictive effectiveness of the model.Kaplan Meier analysis was used to compare the risk stratification of subgroups according to the median score in the nomogram model of each patient.RESULTS Microarray IHC analysis showed that the positive rate of NPAS2 protein expression in GC tissues was 65.35%,which was significantly higher than 30.69%in adjacent tissues.The high expression of NPAS2 was correlated with tumor-node-metastasis(TNM)stage(P<0.05),pN stage(P<0.05),metastasis(P<0.05),venous invasion(P<0.05),lymphatic invasion(P<0.05),and lymph node positive(P<0.05)of GC.Kaplan Meier survival analysis showed that the 3-year overall survival(OS)of patients with high NPAS2 expression was significantly shortened(P<0.0001).Univariate and multivariate COX regression analysis showed that TNM stage(P=0.009),metastasis(P=0.009),and NPAS2 expression(P=0.020)were independent prognostic factors of OS in GC patients for 3 years.The nomogram prediction model based on independent prognostic factors has a C-Index of 0.740(95%CI:0.713-0.767).Furthermore,subgroup analysis showed that the 3-year OS time of the high-risk group was significantly lower than that of the low-risk group(P<0.0001).CONCLUSION NP

A colorimetric and fluorescent sensor for trace water in methanol has been designed and characterized, which is a kind of aluminium-Schiff base coordination complex derived from diaminomaleonitrile. The fluorescence properties of this complex have been studied based on excited state intramolecular proton transfer （ESIPT） and intramolecular charge transfer （ICT） mechanism. The fluorescent emission of AI-L complex can be quenched by 0.1% water and distinguished by naked eyes. The detection limitation is 0.01% recognized by fluorescence spectra.

Here,we developed a prostate cancer(PCa)risk nomogram including lymphocyte-to-monocyte ratio(LMR)for initial prostate biopsy,and internal and external validation were further conducted.A prediction model was developed on a training set.Significant risk factors with P4%and<99%.The nomogram predicting PCa risk in patients with PSA 4-10 ng ml^(-1)also displayed good calibration and discrimination performance(C-index:0.734,95%Cl:0.708-0.760).This nomogram incorporating age,PSA,digital rectal examination,abnormal imaging signals,PSA density,and LMR could be used to facilitate individual PCa risk prediction in initial prostate biopsy.

Experiments simulating the effect of coal mine stopping through a fault zone were designed based on a working face of the Qianqiu coal mine in Yima, China. Through simulation of the physical process of fault reactivation and coal bumps, the displacement of the surrounding strata and evolution characteristics of fault stress under the effect of mining were studied. The mechanism of fault reactivation induced by coal mining was analyzed. The results show that shortly before fault reactivation, the normal stress and shear stress increased rapidly and the risk of a fault slip occurring was also increased. The fault reactivation, caused by the mining activity, occurred when the working face was 25–35 m from the fault along the hanging wall. The influence of mining increased the possibility of fault reactivation, while the local failure of the bearing capacity of the working face was the direct cause of the fault slip. Our results indicate that the influence of fault slip on the coal of the working face had a transient impact and acted as a loading-unloading function.

Bevacizumab versus ranibizumab for neovascular agerelated macular degeneration: a Meta-analysis

AIM: To systematically compare the efficacy and safety of off-label bevacizumab versus licensed ranibizumab intravitreal injections as well as monthly regimen versus pro re nata [PRN(as needed)] regimen in the treatment of neovascular age-related macular degeneration(n AMD).METHODS: Relevant publications were identified through automatically retrieve of database and manually retrieving. The methodological quality of studies included was assessed using the Jadad score and the risk-of-bias assessment. The efficacy estimates were measured by the weight mean difference(WMD) for the improvement of best-corrected visual acuity(BCVA) and central retinal thickness(CRT) reduction. The safety estimates were measured by odds ratios(OR) for adverse events rates. Statistical analysis was conducted by Revman 5.2.7.RESULTS: Seven studies were included in the Metaanalysis. There were no statistically significant differences between bevacizumab and ranibizumab in BCVA at 1 and 2y(P =0.37, P =0.18, respectively),However, both drugs has better BCVA given monthly than given as needed at 1 and 2y(P <0.05). The results demonstrated the mean decrease in CRT was less in bevacizumab group than ranibizumab group at 1y(P <0.05),while the difference was not significant at 2y(P =0.24).Treatment monthly gained much more decrease in CRT at 1 and 2y(P <0.005).There were no differences between drugs in the rates of death, arterial thrombotic events and venous thrombotic events(P =0.41, P =0.55, P =0.10,respectively), while the rates of medical dictionary for regulatory activities(Med DAR) system organ class events and ≥1 systemic serious adverse events were higher in bevacizumab group than ranibizumab group(P <0.05).But the incidences of death, arterial thrombotic events,venous thrombotic events, Med DAR system organ class events as well as ≥1 systemic serious adverse events were not statistically different between both treatment regimens of monthly and as needed(P =0.14, P =0.76,P =0.73, P =0.12, P =0.11, respectively).· CONCLUSION:

Effects of resveratrol on ARPE-19 cell proliferation and migration via regulating the expression of proliferating cell nuclear antigen, P21,P27 and p38MAPK/MMP-9

AIM： To explore whether resveratrol （Res） can inhibit human retinal pigment epithelial cell （ARPE-19 cell） proliferation and migration, and to research the molecular mechanisms.METHODS： ARPE-19 cells were pretreated with various concentrations at 0, 50, 100, 150, 200 and 300 μmol/L of Res, and with 0 μmol/L Res as the control for 24, 48 and 72h. The cell proliferation, apoptosis and migration were measured with cell counting kit-8 （CCK-8）, flow cytometry, and wound-healing and Transwell assays, respectively. The expression of proliferating cell nuclear antigen （PCNA）, P21 and P27, as well as matrix metalloproteinase-9 （MMP-9） and p38 mitogen-activated protein kinases （p38MAPK） was identified by Western blot.RESULTS： Cell proliferation was effectively inhibited by Res （P〈0.05）. When pretreated with Res, cells arrested in S-phase increased remarkably （P〈0.05）, but the apoptosis ratios showed no significant difference between the treatment and control groups （P〉0.05）. Cell migration was suppressed by Res both in wound-healing assay and Transwell migration assay （P〈0.05）. Decreases of PCNA, MMP-9 and p38MAPK, as well as increases of P21 and P27 were detected by Western blot （P〈0.05）. CONCLUSION： Res can inhibit APRE-19 cell proliferation and migration in a concentration-dependent manner with up-regulation of the expression of P21 and P27, and down-regulation of PCNA, MMP-9 and p38MAPK.

Identification of Factors Influencing Out-of-county Hospitalizations in the New Cooperative Medical Scheme

Summary:Throughout the duration of the New Cooperative Medical Scheme(NCMS),it was found that an increasing number of rural patients were seeking out-of^county medical treatment,which posed a great burden on the NCMS fund.Our study was conducted to examine the prevalence of out-of^county hospitalizations and its related factors,and to provide a scientific basis for follow?up health insurance policies.A total of 215 counties in central and western China from 2008 to 2016 were selected.The total out-of-county hospitalization rate in nine years was 16.95%,which increased from 12.37%in 2008 to 19.21%in 2016 with an average annual growth rate of 5.66%.Its related expenses and compensations were shown to increase each year,with those in the central region being higher than those in the western region.Stepwise logistic regression reveals that the increase in out-of-county hospitalization rate was associated with region(XI),rural population(X2),per capita per year net income(X3),per capita gross domestic product(GDP)(X4),per capita funding amount of NCMS(X5),compensation ratio of out-of^county hospitalization cost(X6),per time average in-county(X7)and out-of-county hospitalization cost(X8).According to Bayesian network(BN),the marginal probability of high out-of^county hospitalization rate was as high as 81.7%.Out-of^county hospitalizations were directly related to X8,X3,X4 and X6.The probability of high out-of-county hospitalization obtained based on hospitalization expenses factors,economy factors,regional characteristics and NCMS policy factors was 95.7%,91.1%,93.0% and 88.8%,respectively.And how these factors affect out-of-county hospitalization and their interrelationships were found out.Our findings suggest that more attention should be paid to the influence mechanism of these factors on out-of-county hospitalizations,and the increase of hospitalizations outside the county should be reasonably supervised and controlled and our results will be used to help guide the formulation of proper intervention policies.
