Benefit-Risk Assessment of Chinese Medicine Injections for Primary Liver Cancer Based on Multi-criteria Decision Analysis

ObjectiveTo evaluate the benefit-risk of 3 commonly used Chinese medicine injections, Aidi Injection (ADI), Cinobufagin Injection (CINI) and Compound Kushen Injection (CKI), in the treatment of primary liver cancer (PLC), so as to provide a reference for clinical decision-making.MethodsRandomized controlled trials (RCTs) of ADI, CINI and CKI in the treatment of PLC published in the databases of China National Knowledge Infrastructure, Wanfang, China Science and Technology Journal Database, SinoMed, PubMed, Cochrane Library, and Web of Science were retrieved from January 2020 to October 2022. The data of benefit and risk indicators were combined to obtain the effect value. The multi-criteria decision analysis (MCDA) model was applied to build the decision tree. The benefit value, risk value and benefit risk value of the 3 injections in PLC treatment were calculated. Monte Carlo simulation was carried out to calculate the 95% confidence interval and probability of differences among the 3 injections, so as to optimize the evaluation results.ResultsA total of 71 RCTs were included. The benefit values of ADI, CINI and CKI combined with transcatheter arterial chemoembolization (TACE) were 42, 38 and 36, respectively. The risk values were 42, 25 and 37, respectively. The benefit risk values were 42, 31 and 37, respectively. The benefit risk differences of ADI vs. CINI, ADI vs. CKI, and CKI vs. CINI were 11 (−0.86, 17.75), 5 (−5.01, 11.09), and 6 (−1.87, 12.63), respectively. The probability that ADI superior to CINI, ADI superior to CKI, and CKI superior to CINI was 96.26%, 77.27%, and 92.62%, respectively.ConclusionBased on the results of MCDA model, CINI combined with TACE has the greatest risk in the treatment of the PLC. Considering the efficacy and safety, the possible priority of the 3 Chinese medicine injections combined with TACE in the treatment of PLC is ADI, CKI and CINI.

Electromagnetic fields in ultra-peripheral relativistic heavy-ion collisions

Ultra-peripheral heavy-ion collisions(UPCs)offer unique opportunities to study processes under strong electromagnetic fields.In these collisions,highly charged fast-moving ions carry strong electromagnetic fields that can be effectively treated as photon fluxes.The exchange of photons can induce photonuclear and two-photon interactions and excite ions.This excitation of the ions results in Coulomb dissociation with the emission of photons,neutrons,and other particles.Additionally,the electromagnetic fields generated by the ions can be sufficiently strong to enforce mutual interactions between the two colliding ions.Consequently,the two colliding ions experience an electromagnetic force that pushes them in opposite directions,causing a back-to-back correlation in the emitted neutrons.Using a Monte Carlo simulation,we qualitatively demonstrate that the above electromagnetic effect is large enough to be observed in UPCs,which would provide a clear means to study strong electromagnetic fields and their effects.

Study of deep transportation and plugging performance of deformable gel particles in porous media

Deformable gel particles(DGPs) possess the capability of deep profile control and flooding. However, the deep migration behavior and plugging mechanism along their path remain unclear. Breakage, an inevitable phenomenon during particle migration, significantly impacts the deep plugging effect. Due to the complexity of the process, few studies have been conducted on this subject. In this paper, we conducted DGP flow experiments using a physical model of a multi-point sandpack under various injection rates and particle sizes. Particle size and concentration tests were performed at each measurement point to investigate the transportation behavior of particles in the deep part of the reservoir. The residual resistance coefficient and concentration changes along the porous media were combined to analyze the plugging performance of DGPs. Furthermore, the particle breakage along their path was revealed by analyzing the changes in particle size along the way. A mathematical model of breakage and concentration changes along the path was established. The results showed that the passage after breakage is a significant migration behavior of particles in porous media. The particles were reduced to less than half of their initial size at the front of the porous media. Breakage is an essential reason for the continuous decreases in particle concentration, size, and residual resistance coefficient. However, the particles can remain in porous media after breakage and play a significant role in deep plugging. Higher injection rates or larger particle sizes resulted in faster breakage along the injection direction, higher degrees of breakage, and faster decreases in residual resistance coefficient along the path. These conditions also led to a weaker deep plugging ability. Smaller particles were more evenly retained along the path, but more particles flowed out of the porous media, resulting in a poor deep plugging effect. Urethral stricture is characterized by the chronic formation of fibrous tissue,leading to the narrowing of the urethral lumen.Despite the availability of various endoscopic treatments,the recurrence of urethral strictures remains a common challenge.Postsurgery pharmacotherapy targeting tissue fibrosis is a promising option for reducing recurrence rates.Although drugs cannot replace surgery,they can be used as adjuvant therapies to improve outcomes.In this regard,many drugs have been proposed based on the mechanisms underlying the pathophysiology of urethral stricture.Ongoing studies have obtained substantial progress in treating urethral strictures,highlighting the potential for improved drug effectiveness through appropriate clinical delivery methods.Therefore,this review summarizes the latest researches on the mechanisms related to the pathophysiology of urethral stricture and the drugs to provide a theoretical basis and new insights for the effective use and future advancements in drug therapy for urethral stricture.

BACKGROUND Morgagni hernias are rare anomalies that are easily misdiagnosed or missed.AIM To summarize the ultrasound(US)imaging characteristics of Morgagni hernias through a comparison of imaging and surgical results.METHODS The records of children with Morgagni hernias who were hospitalized at two hospitals between January 2013 and November 2023 were retrospectively re-viewed in terms of clinical findings,US features,and operative details.RESULTS Between 2013 and 2023,we observed nine(five male and four female)children with Morgagni hernias.Upper abdominal scanning revealed a widening of the prehepatic space,with an abnormal channel extending from the xiphoid process to the right or left side of the thoracic cavity.The channel had intestinal duct and intestinal gas echoes.Hernia contents were found in the transverse colon(n=6),the colon and small intestine(n=2),and the colon and stomach(n=1).Among the patients,seven had a right-sided lesion,two had a left-sided lesion,and all of them had hernial sacs.CONCLUSION US imaging can accurately determine the location,extent,and content of Morgagni hernias.For suspected Mor-gagni hernias,we recommend performing sonographic screening first.

Influence of varied drought types on soil conservation service within the framework of climate change:insights from the Jinghe River Basin,China

Severe soil erosion and drought are the two main factors affecting the ecological security of the Loess Plateau,China.Investigating the influence of drought on soil conservation service is of great importance to regional environmental protection and sustainable development.However,there is little research on the coupling relationship between them.In this study,focusing on the Jinghe River Basin,China as a case study,we conducted a quantitative evaluation on meteorological,hydrological,and agricultural droughts(represented by the Standardized Precipitation Index(SPI),Standardized Runoff Index(SRI),and Standardized Soil Moisture Index(SSMI),respectively)using the Variable Infiltration Capacity(VIC)model,and quantified the soil conservation service using the Revised Universal Soil Loss Equation(RUSLE)in the historical period(2000-2019)and future period(2026-2060)under two Representative Concentration Pathways(RCPs)(RCP4.5 and RCP8.5).We further examined the influence of the three types of drought on soil conservation service at annual and seasonal scales.The NASA Earth Exchange Global Daily Downscaled Projections(NEX-GDDP)dataset was used to predict and model the hydrometeorological elements in the future period under the RCP4.5 and RCP8.5 scenarios.The results showed that in the historical period,annual-scale meteorological drought exhibited the highest intensity,while seasonal-scale drought was generally weakest in autumn and most severe in summer.Drought intensity of all three types of drought will increase over the next 40 years,with a greater increase under the RCP4.5 scenario than under the RCP8.5 scenario.Furthermore,the intra-annual variation in the drought intensity of the three types of drought becomes smaller under the two future scenarios relative to the historical period(2000-2019).Soil conservation service exhibits a distribution pattern characterized by high levels in the southwest and southeast and lower levels in the north,and this pattern has remained consistent both in the historical and future pe

Fully polarized Compton scattering in plane waves and its polarization transfer

Fully polarized Compton scattering from a beam of spin-polarized electrons is investigated in plane-wave backgrounds in a broad intensity region from the perturbative to the nonperturbative regimes.In the perturbative regime,polarized linear Compton scattering is considered for investigating polarization transfer from a single laser photon to a scattered photon,and in the high-intensity region,the polarized locally monochromatic approximation and locally constant field approximation are established and are employed to study polarization transfer from an incoming electron to a scattered photon.The numerical results suggest an appreciable improvement of about 10%in the scattering probability in the intermediate-intensity region if the electron’s longitudinal spin is parallel to the laser rotation.The longitudinal spin of the incoming electron can be transferred to the scattered photon with an efficiency that increases with laser intensity and collisional energy.For collision between an optical laser with frequency1 eV and a 10 GeV electron,this polarization transfer efficiency can increase from about 20%in the perturbative regime to about 50%in the nonperturbative regime for scattered photons with relatively high energy.

UAV-assisted cooperative offloading energy efficiency system for mobile edge computing

Reliable communication and intensive computing power cannot be provided effectively by temporary hot spots in disaster areas and complex terrain ground infrastructure.Mitigating this has greatly developed the application and integration of UAV and Mobile Edge Computing(MEC)to the Internet of Things(loT).However,problems such as multi-user and huge data flow in large areas,which contradict the reality that a single UAV is constrained by limited computing power,still exist.Due to allowing UAV collaboration to accomplish complex tasks,cooperative task offloading between multiple UAVs must meet the interdependence of tasks and realize parallel processing,which reduces the computing power consumption and endurance pressure of terminals.Considering the computing requirements of the user terminal,delay constraint of a computing task,energy constraint,and safe distance of UAV,we constructed a UAV-Assisted cooperative offloading energy efficiency system for mobile edge computing to minimize user terminal energy consumption.However,the resulting optimization problem is originally nonconvex and thus,difficult to solve optimally.To tackle this problem,we developed an energy efficiency optimization algorithm using Block Coordinate Descent(BCD)that decomposes the problem into three convex subproblems.Furthermore,we jointly optimized the number of local computing tasks,number of computing offloaded tasks,trajectories of UAV,and offloading matching relationship between multi-UAVs and multiuser terminals.Simulation results show that the proposed approach is suitable for different channel conditions and significantly saves the user terminal energy consumption compared with other benchmark schemes.

Omental fibroma combined with right indirect inguinal hernia masquerades as a scrotal tumor: A case report

BACKGROUND The most common causes of scrotal enlargement in patients include primary tumor of the scrotum,inflammation,hydrocele of the tunica vaginalis,and indirect inguinal hernia;scrotal enlargement caused by external tumors of the scrotum is rare.The patient had both a greater omentum tumor and an inguinal hernia,and the tumor protruded into the scrotum through the hernia sac,which is even rarer.Moreover,omental tumors are mostly metastatic,and primary omental fibroma is rare.CASE SUMMARY Here,we report a rare case of a 25-year-old young man with scrotal enlargement and pain for 3 months.Preoperative examination and multidisciplinary discu-ssions considered intra-abdominal tumor displacement and inguinal hernia,and intraoperative exploration confirmed that the greater omentum tumor protruded into the scrotum.Therefore,tumor resection and tension-free inguinal hernia repair were performed.The final diagnosis was benign fibroma of the greater omentum accompanied by an indirect inguinal hernia.CONCLUSION This unusual presentation of a common inguinal hernia disease illustrates the necessity of performing detailed history taking,physical examination,and imaging before surgery.

Salivary metabolites are promising noninvasive biomarkers of druginduced liver injury

BACKGROUND Drug-induced liver injury(DILI)is one of the most common adverse events of medication use,and its incidence is increasing.However,early detection of DILI is a crucial challenge due to a lack of biomarkers and noninvasive tests.AIM To identify salivary metabolic biomarkers of DILI for the future development of noninvasive diagnostic tools.METHODS Saliva samples from 31 DILI patients and 35 healthy controls(HCs)were subjected to untargeted metabolomics using ultrahigh-pressure liquid chromatography coupled with tandem mass spectrometry.Subsequent analyses,including partial least squares-discriminant analysis modeling,t tests and weighted metabolite coexpression network analysis(WMCNA),were conducted to identify key differentially expressed metabolites(DEMs)and metabolite sets.Furthermore we utilized least absolute shrinkage and selection operato and random fores analyses for biomarker prediction.The use of each metabolite and metabolite set to detect DILI was evaluated with area under the receiver operating characteristic curves.RESULTS We found 247 differentially expressed salivary metabolites between the DILI group and the HC group.Using WMCNA,we identified a set of 8 DEMs closely related to liver injury for further prediction testing.Interestingly,the distinct separation of DILI patients and HCs was achieved with five metabolites,namely,12-hydroxydodecanoic acid,3-hydroxydecanoic acid,tetradecanedioic acid,hypoxanthine,and inosine(area under the curve:0.733-1).CONCLUSION Salivary metabolomics revealed previously unreported metabolic alterations and diagnostic biomarkers in the saliva of DILI patients.Our study may provide a potentially feasible and noninvasive diagnostic method for DILI,but further validation is needed.
