Relationship of demoralization with anxiety, depression, and demographics of Chinese dialysis patients

Objective:The purpose of this study is to investigate the demoralization level of dialysis patients in China and analyze the influencing factors.Materials and Methods:A convenience sample of 278 dialysis patients from a tertiary hospital participated in a cross-sectional descriptive study using a structured questionnaire including assessments of demographic data,Demoralization Scale-II(DS-II),and Hospital Anxiety and Depression Scale.The main analysis consisted of descriptive statistics,correlation coefficients,and multiple linear regression.Results:The mean standard deviation DS-II score was 11.87(7.72).Dialysis patients had a high level of demoralization.Findings from multiple linear regression indicated that demoralization was positively correlated with marital status(β=0.141;P=0.001),employment status(β=0.113;P=0.006),anxiety(β=0.393;P<0.001),and depression(β=0.224;P<0.001).These variables explained 57.1%of the variance in patient demoralization.Conclusions:Demoralization is common in dialysis patients and is associated with marital status,employment status,anxiety,and depression.Health-care providers might regularly assess demoralization in dialysis patients,and develop related interventions to improve the quality of life.

目的观察沙美特罗替卡松粉吸入剂治疗慢性阻塞性肺疾病(COPD)的临床疗效。方法 108例COPD患者,随机分为实验组和对照组,各54例。对照组予口服盐酸氨溴索片、茶碱缓释片等进行常规治疗,实验组在常规治疗的基础上同时给予沙美特罗替卡松粉吸入治疗。比较两组患者治疗前后的肺功能指标[第1秒用力呼气容积(FEV1)、第1秒用力呼气容积占用力肺活量的比值(FEV1/FVC)]、动脉血气分析指标[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)]、COPD评估测试(CAT)评分及不良反应发生情况。结果治疗后,对照组患者的FEV1为(1.68±0.13)L、FEV1/FVC为(52.44±6.81)%、PaO2为(64.40±7.89)mm Hg(1 mm Hg=0.133 kPa)、PaCO2为(51.60±6.50)mm Hg及CAT评分为(32.6±6.5)分,与本组治疗前的(1.61±0.13)L、(51.10±6.08)%、(62.00±7.15)mm Hg、(52.20±6.42)mm Hg及(33.1±6.7)分比较,差异均无统计学意义(P>0.05)。实验组患者的FEV1(1.94±0.11)L、FEV1/FVC (63.48±6.52)%、PaO2 (74.80±8.21)mm Hg均高于本组治疗前的(1.58±0.16)L、(50.90±5.96)%、(61.60±7.08)mm Hg, PaCO2 (40.80±6.35)mm Hg、CAT评分(21.8±5.8)分低于本组治疗前的(52.50±6.36)mm Hg、(32.4±6.5)分,差异均具有统计学意义(P<0.05)。实验组患者治疗后的FEV1、FEV1/FVC、PaO2水平均高于对照组, PaCO2、CAT评分低于对照组,差异均具有统计学意义(P<0.05)。两组患者均未出现明显的不良反应。结论采用沙美特罗替卡松粉吸入剂治疗COPD,能够显著改善患者的临床症状和肺功能。

The minor precipitations caused by B and Zr which are the normal constituents of U720 Li alloy have been studied by analyzing the solidification process and the composition evolution. The present study aims to supply the elementary information about the existing form of B and Zr in the as-cast microstructure, which is helpful for the subsequent processing, such as homogenization treatment. The M_3B_2 and Ni_5Zr phases were observed in the U720 Li alloy in as-cast state, which were usually accompanying with each other together with g-Ni_3 Ti phase at the edge of eutectic（γ＋γ＇）. Combining the DTA analysis and heating and quenching tests, the solidification sequence was determined to be the following： c matrix, eutectic（γ＋γ＇）, g-Ni_3Ti, M_3B_2 and Ni_5Zr. The in situ composition analysis by EDS and EPMA revealed that the precipitation and microstructure were governed by the composition evolution in the liquids. The solidification of c matrix increased the Ti concentration in the residual liquids and resulted in the eutectic（γ＋γ＇） formation; the（γ＋γ＇） formation increased the Ti/Al radio in the liquids and the g-Ni_3Ti was formed in front of the eutectic（γ＋γ＇）; the g-Ni_3Ti precipitation consumed up Al and Ti and increased the concentration of B, Mo and Cr, and M_3B_2 boride is formed;the previous precipitation of the phases consumed up most of the elements other than Ni and Zr, and Ni_5Zr is formed finally. The melting points are in the ranges of 1130–1140 °C for Ni_5Zr phase, 1180–1190 °C for M_3B_2 boride and1190–1200 °C for g-Ni_3Ti phase.

目的探讨血清降钙素原(PCT)在泌尿系感染并发脓毒血症早期诊断中的应用价值。方法78例确诊为泌尿系感染患者,根据病理诊断是否并发脓毒血症分为脓毒血症组(32例)和非脓毒血症组(46例)。检测并比较两组患者的血清PCT、超敏C反应蛋白(hs-CRP)、外周血白细胞计数(WBC)水平,根据受试者工作曲线(ROC)分析比较血清PCT、hs-CRP、外周血WBC检测脓毒血症的敏感度、特异度。结果脓毒血症组血清PCT、hs-CRP分别为(9.37±3.48)μg/L、(128.59±45.39)mg/L,均高于非脓毒血症组的(1.06±0.93)μg/L、(101.38±34.93)mg/L,差异有统计学意义(P0.05)。PCT的曲线下面积(AUC)为0.938[95%CI(0.84,0.96)],hs-CRP和WBC的AUC分别为0.695和0.621;PCT的最佳截断点为3.52μg/L,此时的敏感度为87.1%,特异度为91.7%,hs-CRP、WBC在最佳截断点的敏感度和特异度均<70.0%。结论PCT对泌尿系感染患者并发脓毒血症有较高的敏感度和特异度,可作为怀疑泌尿系感染并发脓毒血症时的常规检测指标,有利于早期诊断。

Certain amino acids changes in the human Na^(+)/K^(+)-ATPase pump,ATPase Na^(+)/K^(+)transporting subunit alpha 1(ATP1A1),cause Charcot-Marie-Tooth disease type 2(CMT2)disease and refractory seizures.To develop in vivo models to study the role of Na^(+)/K^(+)-ATPase in these diseases,we modified the Drosophila gene homolog,Atpα,to mimic the human ATP1A1 gene mutations that cause CMT2.Mutations located within the helical linker region of human ATP1A1(I592T,A597T,P600T,and D601F)were simultaneously introduced into endogenous Drosophila Atpαby CRISPR/Cas9-mediated genome editing,generating the Atpα^(TTTF)model.In addition,the same strategy was used to generate the corresponding single point mutations in flies(Atpα^(I571T),Atpα^(A576T),Atpα^(P579T),and Atpα^(D580F)).Moreover,a deletion mutation(Atpα^(mut))that causes premature termination of translation was generated as a positive control.Of these alleles,we found two that could be maintained as homozygotes(Atpα^(I571T)and Atpα^(P579T)).Three alleles(Atpα^(A576T),Atpα^(P579)and Atpα^(D580F))can form heterozygotes with the Atpαmut allele.We found that the Atpαallele carrying these CMT2-associated mutations showed differential phenotypes in Drosophila.Flies heterozygous for Atpα^(TTTF)mutations have motor performance defects,a reduced lifespan,seizures,and an abnormal neuronal morphology.These Drosophila models will provide a new platform for studying the function and regulation of the sodium-potassium pump.

BACKGROUND The mechanisms underlying diabetes remission after duodenal-jejunal bypass(DJB) remain elusive. In DJB surgery, the duodenum is excluded. However, the duodenum has emerged as an important regulator of glucose homeostasis, and elevated duodenal SIRT1 leads to improved hepatic insulin sensitivity. After DJB, bile acids(BAs) in the duodenum are not mixed and diluted by the ingested food. And activation of BA receptors promotes SIRT1 expression in many tissues. We hypothesized that BA-mediated upregulation of SIRT1 may contribute to diabetic control after DJB.AIM To investigate the surgical effects of DJB on duodenal SIRT1 expression and uncover the potential crosslinks between BAs and SIRT1.METHODS Twenty diabetic rats were randomly allocated to the sham(n = 10) and DJB(n = 10) groups. Body weight, food intake, fasting blood glucose(FBG), serum and intraduodenal total BA(TBA) levels were measured accordingly. Oral glucose tolerance test(OGTT) and intraperitoneal pyruvate tolerance test(ip PTT) were performed to evaluate the effects of surgeries on systemic glucose disposal and hepatic gluconeogenesis. The key genes of BA signaling pathway in the duodenal mucosa, including farnesoid X receptor(FXR), small heterodimer partner(SHP), and Takeda G-protein-coupled receptor 5(TGR5) were evaluated by real-time quantitative polymerase chain reaction 8 wk postoperatively. The duodenal SIRT1, AMPK, and phosphorylated AMPK(p-AMPK) levels were evaluated by western blotting. Rat small intestine epithelial IEC-6 cells were treated with GW4064 and INT-777 to verify the effects of BAs on SIRT1 expression in enterocytes.RESULTS The DJB group exhibited body weight and food intake comparable to those of the sham group at all postoperative time points. The FBG level and area under the curve for the OGTT and ip PTT were significantly lower in the DJB group. BACKGROUND Growing evidence have demonstrated that thyroid hormones have been involved in the processes of cardiovascular metabolism.However,the causal relationship of thyroid function and cardiometabolic health remains partly unknown.METHODS The Mendelian randomization(MR)was used to test genetic,potentially causal relationships between instrumental variables and cardiometabolic traits.Genetic variants of free thyroxine(FT4)and thyrotropin(TSH)levels within the reference range were used as instrumental variables.Data for genetic associations with cardiometabolic diseases were acquired from the genome-wide association studies of the FinnGen,CARDIoGRAM and CARDIoGRAMplusC4D,CHARGE,and MEGASTROKE.This study was conducted using summary statistic data from large,previously described cohorts.Association between thyroid function and essential hypertension(EHTN),secondary hypertension(SHTN),hyperlipidemia(HPL),type 2 diabetes mellitus(T2DM),ischemic heart disease(IHD),myocardial infarction(MI),heart failure(HF),pulmonary heart disease(PHD),stroke,and non-rheumatic valve disease(NRVD)were examined.RESULTS Genetically predicted FT4 levels were associated with SHTN(odds ratio=0.48;95%CI=0.04−0.82,P=0.027),HPL(odds ratio=0.67;95%CI=0.18−0.88,P=0.023),T2DM(odds ratio=0.80;95%CI=0.42−0.86,P=0.005),IHD(odds ratio=0.85;95%CI=0.49−0.98,P=0.039),NRVD(odds ratio=0.75;95%CI=0.27−0.97,P=0.039).Additionally,genetically predicted TSH levels were associated with HF(odds ratio=0.82;95%CI=0.68−0.99,P=0.042),PHD(odds ratio=0.75;95%CI=0.32−0.82,P=0.006),stroke(odds ratio=0.95;95%CI=0.81−0.97,P=0.007).However,genetically predicted thyroid function traits were not associated with EHTN and MI.CONCLUSIONS Our study suggests FT4 and TSH are associated with cardiometabolic diseases,underscoring the importance of the pituitary-thyroid-cardiac axis in cardiometabolic health susceptibility.
