Impact of open hepatectomy on postoperative bile leakage in patients with biliary tract cancer

BACKGROUND Bile leakage is a common and serious complication of open hepatectomy for the treatment of biliary tract *** To evaluate the incidence,risk factors,and management of bile leakage after open hepatectomy in patients with biliary tract *** We retrospectively analyzed 120 patients who underwent open hepatectomy for biliary tract cancer from February 2018 to February *** leak was defined as bile drainage from the surgical site or drain or the presence of a biloma on *** incidence,severity,timing,location,and treatment of the bile leaks were *** risk factors for bile leakage were analyzed using univariate and multivariate logistic regression *** The incidence of bile leak was 16.7%(20/120),and most cases were grade A(75%,15/20)according to the International Study Group of liver Surgery *** median time of onset was 5 d(range,1-14 d),and the median duration was 7 d(range,2-28 d).The most common location of bile leakage was the cut surface of the liver(70%,14/20),followed by the anastomosis site(25%,5/20)and the cystic duct stump(5%,1/20).Most bile leaks were treated conservatively with drainage,antibiotics,and nutritional support(85%,17/20),whereas some required endoscopic retrograde cholangiopancreatography with stenting(10%,2/20)or percutaneous transhepatic cholangiography with drainage(5%,1/20).Risk factors for bile leakage include male sex,hepatocellular carcinoma,major hepatectomy,blood loss,and blood *** Bile leakage is a frequent complication of open hepatectomy for biliary tract ***,most cases are mild and can be conservatively *** sex,hepatocellular carcinoma,major hepatectomy,blood loss,and blood transfusion were associated with an increased risk of bile leak.

Coal is of utmost importance for the energy and chemical engi-neering fields of China,as it contributes to 56.8%of primary energy supply in 2020 in the *** the one hand,coal is the major source of energy and resources for its rich endowment in China;However,on the other hand,it is the major source of environmen-tal pollution due to the implementation of outdated ***,according to Professor Kechang Xie in 2004,“The only way out is to develop and promote the clean and efficient coal uti-lization technologies”.In the past 40 years,China has made a sig-nificant progress in research and development related to modern coal chemical engineering,including lab-scale fundamental research and industrial-scale construction of many giant chemical plants.

Regulation of radicals by hydrogen-donor solvent in direct coal liquefaction

Radicals are important intermediates in direct coal *** radicals can cause the cleavage of chemical *** high temperatures,radical fragments can be produced by the splitting of large organic molecules,which can break strong chemical bonds through the induction pyrolysis of *** reaction between the formation and annihilation of coal radical fragments and the effect of hydrogen-donor solvents on the radical fragments are discussed in lignite *** the hydroxyl and ether bonds as indicators,the effects of different radicals on the cleavage of chemical bond were investigated employing density functional theory calculations and lignite hydrogenolysis *** showed that the adjustment of the coal radical fragments could be made by the addition of hydrogendonor *** showed that the transition from coal radical fragment to H radical leads to the variation of product *** synergistic mechanism of hydrogen supply and hydrogenolysis of hydrogen-donor solvent was proposed.

Effect of noble metal nanoparticle size on C–N bond cleavage performance in hydrodenitrogenation: a study of active sites

Breakage of the C-N bond is a structure sensitive process,and the catalyst size significantly affects its *** the active metal nanoparticle scale,the role of catalyst size in C-N bond cleavage has not been clearly ***,Ru catalysts with variable nanoparticle sizes were obtained by modulating the reduction temperature,and the catalytic activity was evaluated using 1,2,3,4-tetrahydroquinoline and o-propylaniline with different C-N bond hybridization patterns as *** showed a 13 times higher reaction rate for sp3-hybridized C-N bond cleavage than sp2-hybridized C-N bond cleavage,while the reaction rate tended to increase first and then decrease as the catalyst nanoparticle size *** concentrations of terrace,step,and corner sites were found in different sizes of Ru *** relationship between catalytic site variation and C-N bond cleavage activity was further investigated by calculating the turnover frequency values for each *** analysis indicates that the variation of different sites on the catalyst is the intrinsic factor of the size dependence of C-N bond cleavage activity,and the step atoms are the active sites for the C-N bond *** Ru nanoparticles are smaller than 1.9 nm,they have a strong adsorption effect on the reactants,which will affect the catalytic performance of the Ru ***,these findings were also confirmed on other metallic Pd/Pt *** role of step sites in C-N bond cleavage was proposed using the density function theory *** reactants have stronger adsorption energies on the step atoms,and step atoms have d-band center nearer to the Fermi *** this case,the interaction with the reactant is stronger,which is beneficial for activating the C-N bond of the reactant.

Solubility study of hydrogen in direct coal liquefaction solvent based on quantitative structure–property relationships model

Direct coal liquefaction(DCL)is an important and effective method of converting coal into high-valueadded chemicals and fuel *** DCL,heating the direct coal liquefaction solvent(DCLS)from low to high temperature and pre-hydrogenation of the DCLS are critical ***,studying the dissolution of hydrogen in DCLS under liquefaction conditions gains ***,it is difficult to precisely determine hydrogen solubility only by experiments,especially under the actual DCL *** address this issue,we developed a prediction model of hydrogen solubility in a single solvent based on the machine-learning quantitative structure–property relationship(ML-QSPR)*** results showed that the squared correlation coefficient R^(2)=0.92 and root mean square error RMSE=0.095,indicating the model’s good statistical *** external validation of the model also reveals excellent accuracy and predictive *** polarization(a)is the main factor affecting the dissolution of hydrogen in *** hydrogen solubility in acyclic alkanes increases with increasing carbon *** in polycyclic aromatics,it decreases with increasing ring number,and in hydrogenated aromatics,it increases with hydrogenation *** work provides a new reference for the selection and proportioning of DCLS,i.e.,a solvent with higher hydrogen solubility can be added to provide active hydrogen for the reaction and thus reduce the hydrogen ***,it brings important insight into the theoretical significance and practical value of the DCL.

Enhancement and selective production of avermectin B by recombinants of Streptomyces avermitilis via intraspecific protoplast fusion

Among eight components of avermectin, B1 fractions have the most effective antiparasitic activities and the lowest level of toxic side-effects and are used widely in veterinary and agricultural fields. In-traspecific protoplast fusion between two strains of Streptomyces avermitilis, one an avermectin high producer (strain 76-05) and the other a genetically engineered strain containing the mutations aveDˉ and olmAˉ (strain 73-12) was performed for enhancement and selective production of avermectin B in the absence of oligomycin. Two recombinant strains (F23 and F29) were isolated and characterized with regards to the parental merits. F23 and F29 produced only the four avermectin B components with high yield and produced no oligomycin. The avermectin production of F23 and F29 was about 84.20% and 103.45% of the parental strain 76-05, respectively, and increased about 2.66-fold and 3.50-fold, re-spectively, compared to that of parental strain 73-12. F23 and F29 were genetically stable prototrophic recombinants and F29 was quite tolerant of fermentation conditions compared to avermectin high producer parental strain 76-05. The ability to produce avermectin B with high yield without the produc-tion of other avermectin components and oligomycin will make F23 and F29 useful strains for aver-mectin production. Strain F29's tolerance of fermentation conditions will also make it suitable for in-dustrial applications.

Purification of carbazole by solvent crystallization under two forced cooling modes

To identify the effect of solvents and anthracene on the purification of carbazole,the solvent crystallization of carbazole was investigated with xylene,chlorobenzene and tetrachloroethylene(TCE)as solvents under two forced circulation cooling(FCC)*** co-crystalline experimental data were obtained from runs carried out at different anthracene levels between 1%(mass)and 10%(mass).The results showed that a uniform flake carbazole crystal obtained when using xylene and chlorobenzene under the FCC-1 mode with gradual cooling ***,fine flake crystals grown under shock cooling of FCC-2 *** is beneficial to improving the purity of carbazole with chlorobenzene as solvent under cooling mode of *** could promote the growth of carbazole in solution,and it has a significant influence on the purification of carbazole.

Relationship between alanine aminotransferase levels and metabolic syndrome in nonalcoholic fatty liver disease

Objective： To investigate the relationship between alanine aminotransferase （ALT） levels and metabolic syndrome （MS） in nonalcoholic fatty liver disease （NAFLD）. Methods： A total of 26527 subjects who received medical health checkup in our hospital from January 2005 to July 2007 were enrolled in the study. The diagnosis of fatty liver was based on ultrasound imaging. MS was defined according to the criteria of the Adult Treatment Panel III. ALT, triglyceride （TG）, high density lipoprotein cholesterol （HDL-c）, fasting plasma glucose （FPG）, height, weight, waist circumference （WC）, systolic blood pressure （SBP） and diastolic blood pressure （DBP） were measured in each subject to analyze the relationship between MS and ALT activity Results：（1） The prevalence of NAFLD in men （30.94%） was significantly higher than that in women （15.65%）; （2） The incidence of MS in NAFLD （33.83%） was significantly greater than that in non-NAFLD （10.62%）; （3） Of the 6470 subjects with NAFLD, in the age-adjusted partial correlation analysis, there were statistically significant correlations between the ALT levels and most metabolic risk factors in each sex （P〈0.01）, except that ALT levels multiple stepwise regression analysis, SBP lost its significance, and had no correlation with HDL-c in women. Moreover, in the WC, body mass index （BMI）, age, DBP, TG and FPG were independently associated with ALT levels in both sexes （P〈0.05）. HDL-c remained significant and was independently related to ALT levels in men; （4） ALT levels were significantly higher in subjects with MS compared to those without MS （P〈0.001）. Mean ALT levels increased with the number of MS components in each sex （P〈0.05 for trend）. Conclusion： We found a strong relationship between ALT levels and MS in NAFLD and revealed that the cluster of MS components might be the predictor for ALT elevations.

Mechanism of dibenzofuran hydrodeoxygenation on the Ni(1 1 1)surface

The low-temperature coal tar contains a considerable number of oxygen-containing compounds,which results in poor *** catalytic hydrodeoxygenation of oxygen-containing compound to an added-value chemical compound is one of the most efficient methods to upgrade coal *** this study,density functional theory calculations are employed to assess and analyze in detail the hydrodeoxygenation of dibenzofuran,as a model compound of coal tar,on the Ni(111)*** obtained results indicate that dibenzofuran can be firstly hydrogenated to tetra hy d rod i be nzofura n and *** five-membered-ring opening reaction of tetrahydrodibenzofuran is more straightforward than that of hexahydrodibenzofuran(Ea=0.71 eV vs.1.66 eV).Then,both pathways generate an intermediate 2-cyclohexylphenoxy *** part of 2-cyclohexylphenoxy is hydrogenated to 2-cyclohexylphenol and consecutively hydrogenated to cyclohexylcyclohexanol,and another part is directly hydrogenated to *** hydrogenated intermediates of2-cyclohexylphenol have higher deoxygenation barriers than 2-cyclohexylphenol and cyclohexylcy *** the hydrogenation process of cyclohexylcyclohexanone to cyclohexylcyclohexanol,the intermediate 26,formed by adding H to O atom of cyclohexylcyclohexanone,exhibits the lowest deoxygenation barrier of 1.08 *** hydrogen coverage may promote the hydrogenation of tetrahydrodibenzofuran,hexahydrodibenzofuran,and intermediate 26 to generate dodecahydrodibenzofuran and *** dibenzofuran hydrodeoxygenation reaction mechanism corroborates well with previous experimental results and provides a theoretical basis for further optimization of the design of nickel-based catalysts.

Family Status and Dilemma of HIV Positive Subjects

Objective To understand the family status and dilemma of HIV positive subjects and provide some related suggestions. Methods Constructured questionnaire and in-depth interview were used to survey their psychological needs of the 111 HIV positive subjects and AIDS patients from 4 provinces in China. Results HIV positive subjects and AIDS patients had strong mental stress. All of them hope to get support from their families; the most difficulty the subjects faced was economic poor. Conclusion It is a highlight to promote AIDS education to establish a good social environment in which HIV positive subjects and AIDS patients can live quietly and happily. And psychological support from their families and the society is very important to HIV positive subjects and AIDS patients.
