AIM: To evaluate whether a higher prevalence of giardia lamblia infection is associated with an increase in irritable bowel syndrome (IBS) prescriptions at the county level in Michigan. METHODS: The Michigan Disease Surveillance System (MDSS) was used to ascertain both the numbers of giardia lamblia infections as well as the total number of foodborne illnesses per population by county in Michigan during 2005. This was compared with Blue Cross Blue Shield (BCBS) of Michigan numbers of drug prescriptions for IBS per one thousand members per county in 2005. These data were also analyzed for associations with per capita income by county and the number of refugees entering each county in 2005. RESULTS: There were a total of 786 confirmed cases of giardia lamblia reported to MDSS in 2005. During the same time period, the number of prescriptions for IBS varied from 0.5 per 1000 members up to 6.0 per 1000 members per month. There was no trend towards higher numbers of IBS prescriptions in the counties with more giardia lamblia infections. Per capita income was not associated with either IBS prescriptions or giardiasis. There was a significant linear association between the number of refugees entering each county, and the number of giardia lamblia cases per 100 000 population. CONCLUSION: In this ecological study, there was no association found between BCBS prescriptions for IBS and giardia lamblia infections in Michigan counties. Our findings may have been influenced by the disparate number of refugees admitted per county.

A 67-year-old female patient presented with weight loss, diarrhoea and thrombocytopaenia of unknown aetiology. A computerised tomography (CT) scan demonstrated a mass in the head of the pancreas with liver metastases. A liver biopsy demonstrated a well-differentiated neuroendocrine carcinoma. She was commenced on a somatostatin analogue. Three months later she presented with spontaneous bleeding. Blood test demonstrated results consistent with disseminated intravascular coagulation (DIC). A restaging CT scan showed evidence of disease progression. The DIC was felt to be due to the underlying progressive malignancy. Having considered the potential risks associated with cytotoxic therapy in the context of a consumptive coagulopathy, the patient was commenced on weekly Carboplatin. The patient’s blood counts improved rapidly, and her bruising and bleeding resolved. Following a few weeks of stable blood results and clinical stability, her cytotoxic treatment was changed to a combination of Carboplatin and Etoposide, and to date she remains well on treatment.

gully erosion can account for significant volumes of sediment exiting agricultural landscapes, but is difficult to monitor and quantify its evolution with traditional surveying technology. Scientific investigations of gullies depend on accurate and detailed topographic information to understand and evaluate the complex interactions between field topography and gully evolution. Detailed terrain representations can be produced by new technologies such as terrestrial LiDAR systems. These systems are capable of collecting information with a wide range of ground point sampling densities as a result of operator controlled factors. Increasing point density results in richer datasets at a cost of increased time needed to complete field surveys. In large research watersheds, with hundreds of sites being monitored, data collection can become costly and time consuming. In this study, the effect of point sampling density on the capability to collect topographic information was investigated at individual gully scale. This was performed through the utilization of semi-variograms to produce overall guiding principles for multi-temporal gully surveys based on various levels of laser sampling points and relief variation (low, moderate, and high). Results indicated the existence of a point sampling density threshold that produces little or no additional topographic information when exceeded. A reduced dataset was created using the density thresholds and compared to the original dataset with no major discrepancy. Although variations in relief and soil roughness can lead to different point sampling density requirements, the outcome of this study serves as practical guidance for future field surveys of gully evolution and erosion.

Assessing 10-year coronary heart disease risk in people with Type 2 diabetes mellitus: Framingham versus United Kingdom Prospective Diabetes Study

Aims: Previous studies have suggested that the Framingham coronary heart disease risk prediction equation underestimates risk among people with Type 2 diabetes. We compared the 10-year absolute risks of coronary heart disease (CHD) using a Framingham equation and a United Kingdom Prospective Diabetes Study (UKPDS) equation in adults with Type 2 diabetes. Methods: Participants were from a cross-sectional survey of a randomly selected population. There were 461 people with newly (n = 132) or previously diagnosed (n = 329) diabetes aged 35 to 74 years with no past history of cardiovascular disease or nephropathy. We examined predicted 10-year CHD risk by age, gender, and newly or previously diagnosed diabetes. Results: Overall the mean 10-year CHD risks predicted by the two equations were similar. Among men, the UKPDS and Framingham scores were almost identical below 60 years of age but at older ages, the UKPDS score was 4% - 11% higher than Framingham. For women, the Framingham score was higher than the UKPDS score between ages 40 and 65 years, but the UKPDS score was about 4% - 5% higher for women aged 70 years and over. The UKPDS equation tended to give higher risk estimates in people with a predicted 10-year Framingham CHD risk above 15%. Conclusion: Framingham CHD risk scores tended to be lower than UKPDS scores primarily in people above standard thresholds for drug treatment, so the clinical impact of underestimating risk is likely to be limited. Moreover, the UKPDS equation predicted lower risks than Framingham for women and newly diagnosed diabetes at otherwise low to moderate CHD risk, which could result in later initiation of therapy in these groups if the UKPDS score was used instead of the Framingham score.

在温度423-573 K和应变速率为0.001-1 s^-1下,通过gleeble-3500热机械试验机对双相Mg-9Li-3Al-2Sr合金进行等温热压缩实验,研究其热变形行为并评估其可加工性能。塑性失稳以锯齿形流变的形式存在,锯齿屈服效应归因于Mg和Li原子对移动位错的锁定作用。分析流变应力、应变速率和变形温度的关系,并通过Arrhenius公式计算得到不同应变条件下变形激活能和基本的材料参数。温度和变形速率对合金变形行为的影响可以用Zener–Hollomon指数函数来表示。通过比较不同变形条件下的预测值与实验流变曲线,来验证本构方程的合理性。经计算,得到其相关系数为0.9970,平均相对误差为4.41%。结果表明,该本构模型可以准确地预测双相Mg-9Li-3Al-2Sr合金的高温变形流动行为。
