New discoveries in obesity and related metabolic diseases:from bench to bed

Obesity,and other metabolic diseases including non-alcoholic fatty liver disease(NAFLD),metabolic syndrome(MetS),type 2 diabetes mellitus(T2DM)and its related complications like chronic kidney disease(CKD),and hepatocellular carcinoma(HCC)have led to alarming health and social burdens globally and in China as *** strong links exist among those multifactorial metabolic disorders,there are limited researches on better understanding of their pathogenesis and the provision of effective interventional measures in *** this special issue,we have five researches including three research articles providing new insights into potential mechanisms in the development and possible prevention and treatment therapies of NAFLD,HCC,and CKD,as well as two review articles summarizing multi-omics profiling and future strategies for precision medicine in T2DM,NAFLD,and MetS,and the rapid advances in the field of metabolic surgery in China.

Although type 2 diabetes is a disease often associated with aging, the global prevalence of early-onset diabetes has been increasing due to man's sedentary lifestyle, low-physical activity, obesity, and some nonmodifiable risk factors. Many studies have found that individuals with early-onset type 2 diabetes were at higher risk of developing vascular complications than those with late-onset diabetes. Individuals with early-onset diabetes are usually unwilling to visit hospital and have more confidence in their health, which results in poor glycemic control and the delayed detection of diabetes-related complications. Few studies have focused on the treatment and prevention of complications in specific population of individuals with early-onset type 2 diabetes. Therefore, focusing on this particular population is critical for the government and academic societies. Screening for T2DM is imminent for young adults with a family history of diabetes, obesity, markers of insulin resistance, or alcohol consumption. More data are definitely required to establish a reasonable risk model to screen for early-onset diabetes.

The high prevalence of diabetes and diabetic complications has caused a huge burden on the modern *** scientific advances have led to effective strategies for preventing and treating diabetes over the past several decades,little progress has been made toward curing the disease or even getting it under control,from a public health and overall societal *** is still a lack of reliable biomarkers indicative of metabolic alterations associated with diabetes and different drug responses,highlighting the need for the development of early diagnostic and prognostic markers for diabetes and diabetic *** emergence of metabolomics has allowed researchers to systemically measure the small molecule metabolites,which are sensitive to the changes of both environmental and genetic factors and therefore,could be regarded as the link between genotypes and *** the last decade,the progression made in metabolomics has provided insightful information on disease development and disease onset *** studies using metabolomics approach coupled with statistical tools to predict incident diabetes revealed a number of metabolites that are significantly altered,including branched-chain and aromatic amino acids,such as isoleucine,leucine,valine,tyrosine and phenylalanine,as diagnostic or highly-significant predictors of future *** review summarizes the current findings of metabolomic studies in human investigations with the most common form of diabetes,type 2 diabetes.

Obesity prevalence has increased in recent years. Lifestyle change fuels obesity, but genetic factors cause more than 50% of average variations in obesity. The advent of genome-wide association studies （GWAS） has hastened the progress of polygenic obesity research. As of this writing, more than 73 obesity susceptibility loci have been identified in ethnic groups through GWAS. The identified loci explain only 2% to 4% of obesity heritability, thereby indicating that a large proportion of loci remain undiscovered. Thus, the next step is to identify and confirm novel loci, which may exhibit smaller effects and lower allele frequencies than established loci. However, achieving these tasks has been difficult for researchers. GWAS help researchers discover the causal loci. Moreover, numerous biological studies have been performed on the polygenic effects on obesity, such as studies on fat mass- and obesity-associated gene （FTO）, but the role of these polygenic effects in the mechanism of obesity remains unclear. Thus, obesity-causing variations should be identified, and insights into the biology of polygenic effects on obesity are needed.
