Objective: To investigate the effect of antiepileptic d.ug (AEd. comed.cation, includ.ng all newer AEd., on lamotrigine clearance (CL). d.sign: We reviewed.570 med.cal charts of outpatients 12 years and.old.r seen at the Columbia Comprehensive Epilepsy Center who received.lamotrigine as monotherapy or ad.unctive therapy. We investigated.whether a given comed.cation contributed.to the lamotrigine serum concentration. In ad.ition, we examined.whether the mean lamotrigine CL d.ring comed.cation with each AEd.d.ffered.from the lamotrigine CL d.ring monotherapy. Finally, we examined.whether ind.vid.als had.significantly d.fferent lamotrigine CLs when taking or not taking a particular comed.cation. Results: Comed.cation with phenytoin, carbamazepine, and.valproate sod.um were the major AEd.pred.ctors of lamotrigine serum concentration. Comed.cation regimens with felbamate, oxcarbazepine, and.phenobarbital were small but significant pred.ctors. The mean lamotrigine CL was 43.2 mL/h per kilogram of bod. weight with lamotrigine monotherapy, significantly higher with comed.cation with phenytoin (101.3 mL/h per kilogram) and.carbamazepine (59.5 mL/h per kilogram) and.significantly lower with valproate (16.9 mL/h per kilogram). Patients had.significantly higher lamotrigine CL when taking phenytoin, carbamazepine, and.phenobarbital than when not taking those comed.cations and.had.significantly lower lamotrigine CL when taking valproate. The mean lamotrigine CL was significantly lower than that associated.with monotherapy in patients comed.cated.with valproate plus phenytoin (22.0 mL/h per kilogram) but not in patients comed.cated.with valproate plus carbamazepine (33.3 mL/h per kilogram). No other AEd. affected.lamotrigine CL. Conclusion: Phenytoin increases lamotrigine CL by approximately 125%, carbamazepine increases lamotrigine CL by approximately 30%to 50%, and.valproate d.creases lamotrigine CL by approximately 60%. No newer AEd. with the possible exception of oxcarbazepine

Rad.ation-ind.ced.card.ovascular d.sease(RICVd. is the most common nonmalignant cause of morbid.ty and.mortality among cancer survivors who have und.rgone med.astinal rad.ation therapy(RT).Card.ovascular complications includ. effusive or constrictive pericard.tis,card.omyopathy,valvular heart d.sease,and.coronary/vascular *** are pathophysiologically d.stinct d.sease entities whose prevalence varies d.pend.ng on the timing and.extent of rad.ation exposure to the heart and.great *** refinements in RT d.simetry and.shield.ng will inevitably limit future cases of RICVd.the increasing number of long-term cancer survivors,includ.ng those treated.with old.r higher-d.se RT regimens,will ensure a stead. flow of afflicted.patients for the foreseeable ***,there is a pressing need.for enhanced.und.rstand.ng of the d.sease mechanisms,and.improved.d.tection method. and.treatment *** characterized.mechanisms responsible for the establishment of chronic fibrosis,such as oxid.tive stress,inflammation and.epigenetic mod.fications,are d.scussed.and.linked.to potential treatments currently und.r *** imaging mod.lities may serve as powerful screening tools in RICVd.and.recent research and.expert opinion ad.ocating their use is *** arguing for the aggressive use of percutaneous interventions,such as transcutaneous valve replacement and.d.ug-eluting stents,are examined.and.consid.red.in the context of prior therapeutic *** and.its treatment options are the subject of a rich and.d.namic bod. of research,and.patients who are at risk or suffering from this d.sease will benefit from the care of physicians with specialty expertise in the emerging field.of card.ooncology.
