BACKGROUND Rearrangements of the anaplastic lymphoma kinase(ALK)gene(ALK-positive)represent an oncogenic driver in approximately 3%-5%of non-small-lung cancer(NSCLC)*** is a multisystem disease,and its reported incidence in Asia is 1 or less per 100000 people per *** co-occurrence of sarcoidosis and ALK-positive NSCLC is rare,and ALK-positive lung cancer is likely to spread ***,the co-occurrence of sarcoidosis is more easily misdiagnosed as metastatic lung cancer by radiological *** SUMMARY A 50-year-old man had a nodule in the left superior lobe,many small nodules in left superior and right lungs,and enlarged bilateral hilar,mediastinal,and right supraclavicular lymph *** tomography-guided pulmonary biopsy of the nodule in the left superior lobe revealed echinoderm microtubuleassociated protein-like 4 gene-ALK positive NSCLC with concomitant noncaseating *** patient was treated with *** days later,a chest computed tomography scan revealed a dramatic decrease in the size of the left superior lobe nodule;however,the lesions in the right lung *** right supraclavicular lymph nodes showed granulomas,and no tumor cells were identified in the specimens. The angiotensin-converting enzyme level was *** 1 wk of methylprednisolone treatment, a significant response of all lesionswas revealed. Hepatocellular carcinoma(HCC),a prevalent solid carcinoma of significant concern,is an aggressive and often fatal disease with increasing global incidence rates and poor therapeutic *** etiology and pathological progression of non-alcoholic steatohepatitis(NASH)-related HCC is multifactorial and ***,no single animal model can accurately mimic the full NASH-related HCC pathological progression,posing considerable challenges to transition and mechanistic ***,a novel conditional inducible wild-type human HRAS overexpressed mouse model(HRAS-HCC)was established,demonstrating 100%morbidity and mortality within approximately one month under normal dietary and lifestyle *** symptoms of HCC such as ascites,thrombus,internal hemorrhage,jaundice,and lung metastasis were successfully replicated in ***-depth pathological features of NASH-related HCC were demonstrated by pathological staining,biochemical analyses,and typical marker gene *** murine anti-PD-1 and sorafenib treatment effectively prolonged mouse survival,further confirming the accuracy and reliability of the *** on protein-protein interaction(PPI)network and RNA sequencing analyses,we speculated that overexpression of HRAS may initiate the THBS1-COL4A3 axis to induce NASH with severe fibrosis,with subsequent progression to ***,our study successfully duplicated natural sequential progression in a single murine model over a very short period,providing an accurate and reliable preclinical tool for therapeutic evaluations targeting the NASH to HCC continuum.

Research progress on refractory composition and deformability of shell molds for TiAl alloy castings

At present, most TiAl components are produced by an investment casting process. Environmental and economic pressures have, however, resulted in a need for the industry to improve the current casting quality, reduce manufacturing costs and explore new markets for the process. Currently, the main problems for investment casting of TiAl alloys are cracks, porosities, and surface defects. To solve these problems, many studies have been conducted around the world, and it is found that casting defects can be reduced by improving composition and properties of the shell molds. It is important to make a summary for the related research progress for quality improvement of TiAl castings. So, the development on refractory composition of shell molds for TiAl alloy investment castings was reviewed, and research progress on deformability of shell mold for TiAl alloy castings both at home and abroad in recent years was introduced. The existing methods for deformability characterization and methods for improving the deformability of shell molds were summarized and discussed. The updated advancement in numerical simulation of TiAl alloy investment casting was presented, showing the necessity for considering the deformability of shell mold during simulation. Finally, possible research points for future studies on deformability of shell mold for TiAl alloy investment casting were proposed.

In order to consider the influence of the continuous rotary motor electro-hydraulic servo system parameters change on its performance,the design method of backstepping adaptive controller is put *** mathematical model of electro-hydraulic servo system of continuous rotary motor is established,and the whole system is decomposed into several lower order subsystems,and the virtual control signal is designed for each subsystem from the final subsystem with motor angular displacement to the subsystem with system control input voltage. Based on Lyapunov method and the backstepping theory,an adaptive backstepping controller is designed with the changed parameters adaptive law. It is proved that the system reaches the global asymptotic stability,and the system tracking error asymptotically tends to zero. The simulation results show that the backstepping adaptive controller based on the adaptive law of the changed parameters can improve the performance of continuous rotary motor,and the proposed control strategy is feasible.

In this experimental prospective study, we aimed to analyze the effect of transient scrotal hyperthermia on the male reproductive organs, from the perspective of sperm parameters, semen plasma biochemical markers, and oxidative stress, to evaluate whether different frequencies of heat exposure cause different degrees of damage to spermatogenesis. Two groups of volunteers （10 per group） received testicular warming in a 43~C water bath 10 times, for 30 min each time： group 1：10 consecutive days; group 2： once every 3 days. Sperm parameters, epididymis and accessory sex gland function, semen plasma oxidative stress and serum sex hormones were tested before treatment and in the 16-week recovery period after treatment. At last, we found an obvious reversible decrease in sperm concentration （P = 0.005 for Group 1 and P = 0.008 for Group 2 when the minimums were compared with baseline levels, the same below）, motility （P= 0.009 and 0.021, respectively）, the hypoosmotic swelling test score （P-- 0.007 and 0.008, respectively）, total acrosin activity （P = 0.018 and 0.009, respectively）, and an increase in the seminal plasma malondialdehyde concentration （P = 0.005 and 0.017, respectively）. The decrease of sperm concentration was greater for Group 2 than for Group 1 （P = 0.031）. We concluded that transient scrotal hyperthermia seriously, but reversibly, negatively affected the spermatogenesis, oxidative stress may be involved in this process. In addition, intermittent heat exposure more seriously suppresses the spermatogenesis compared to consecutive heat exposure. This may be indicative for clinical infertility etiology analysis and the design of contraceptive methods based on heat stress.

Histopathological characteristics of needle core biopsy and surgical specimens from patients with solitary hepatocellular carcinoma or intrahepatic cholangiocarcinoma

BACKGROUND Pathological manifestations of hepatic tumours are often associated with prognosis. Although surgical specimens(SS) can provide more information,currently, pre-treatment needle core biopsy(NCB) is increasingly showing important value in understanding the nature of liver tumors and even in diagnosis and treatment decisions. However, the concordance of the clinicopathological characteristics and immunohistochemical(IHC) staining between NCB and SS from patients with hepatic tumours were less *** To introduce a more accurate method for interpreting the IHC staining results in order to improve the diagnostic value of hepatic malignancy in NCB *** A total of 208 patients who underwent both preoperative NCB and surgical resection for hepatocellular carcinoma(HCC) or intrahepatic cholangiocarcinomaRESULTS Morphologically, the presence of compact tumour nests or a cord-like structure in NCB was considered the primary cause of misdiagnosis of HCC from ICC. The kappa statistic showed a moderate agreement in histomorphology(k = 0.504) and histological grade(k = 0.488) between NCB and SS of the tumours. A 4-tier(+++,++, +, and-) scoring scheme that emphasized the focal neoplastic cell immunoreactivity of tumour cells revealed perfect concordance of CK19, GPC3 and HepPar1 between NCB and SS(k = 0.717; k = 0.768; k = 0.633). Furthermore,with the aid of a binary classification derived from the 4-tier score, a high concordance was achieved in interpreting the IHC staining of the three markers between NCB and final SS(k = 0.931; k = 0.907; k = 0.803), increasing the accuracy of NCB diagnosis C(k = 0.987; area under the curve = 0.997, 95%CI: 0.990-1.000; P< 0.001).CONCLUSION These findings imply that reasonable interpretation of IHC results in NCB is vital for improving the accuracy of tumour diagnosis. The simplified binary classification provides an easy and applicable approach.

Up to now,how to control the durability of concrete structures is still a pending research topic in civil *** to the fact that concrete durability can be affected by many non-mechanical factors,it is very difficult to research durability only based on mechanics. However,the phenomenon of failure caused by non-mechanical factors is similar to that caused by *** frost durability reflects the comprehensive durability of concrete in a certain sense, the specific failure process is analyzed and a damage model is introduced,which can describe the durability degradation process of concrete in the whole freeze-thawing cycle.
