Enhanced recovery after surgery protocols in functional endoscopic sinus surgery for patients with chronic rhinosinusitis with nasal polyps: a randomized clinical trial

Background:Enhanced recovery after surgery (ERAS) protocols are a series of perioperative care to optimize preoperative preparation, prevent postoperative complications, minimize stress, and speed up recovery. This study aimed to assess the impact of ERAS protocols for functional endoscopic sinus surgery (FESS) in patients with chronic rhinosinusitis with nasal polyps (CRSwNP).Methods:One hundred and two patients with CRSwNP undergoing FESS were randomly divided into the ERAS group and the control group. The outcomes of the Self-Rating Anxiety Scale (SAS), Visual Analogue Scale (VAS), Medical Outcomes Study Sleep Scale (MOS-SS) and Kolcaba Comfort Scale Questionnaire (GCQ) were determined in both groups. The serum levels of C-reactive protein (CRP) were compared preoperatively and 24 hours postoperatively.Results:The ERAS group had a significantly better SAS scores than did the control group (28 [24, 35] vs. 43 [42, 47], Z=5.968, P 0.05);However, the CRP level in 24 hours postoperatively was significantly lower in the ERAS group than that in the control group (2.5 [1.4, 3.9] vs. 6.6 [3.8, 9.0], Z = 5.027, P 0.05), hemorrhage, aspiration and tumble, were not increased in the ERAS group compared with those in the control group. The ERAS group had a significantly shorter length of hospital stay (5 [4, 5] days vs. 8 [8,9] days, Z=8.939, P<0.001) and hospitalization expenses ($ 2670 [2375, 2740] vs. $3129 [3116, 3456], Z=8.514, P<0.001).Conclusions:ERAS pro

A Low Cost, Green Sustainable and Biodegradable Biomass-based Fluorine-free Water and Oil Repellent

With the global ban on plastics intensifying, the substitution of plastic with paper has garnered increasing attention. However, the inadequate water and oil repellency of pulp molding hinders its practical applications. Currently, the common method to enhance the oil and water repellency of pulp molding is by adding fluorinated water and oil repellents. Nevertheless, fluorinated compounds are environmentally and physiologically harmful. Therefore, the development of fluorine-free, water and oil repellent alternatives is crucial. In this study, chitosan and stearic acid were utilized as the first and second layers of the oil and water repellent coatings, respectively. The coated samples exhibited favorable water repellency, with a water contact angle of 116.4°, and excellent oil repellency, achieving a 12/12 rating on the kit scale. Importantly, the samples did not exhibit any leakage after being soaked in hot water and hot oil at 95±5℃ for 30 min, demonstrating remarkable performance as a barrier against hot water and oil. Moreover, the coated samples displayed outstanding mechanical properties, thermal stability, biodegradability, and recyclability. The approach presented in this study is simple, cost-effective, environmentally friendly, and represents a promising technique for producing fluoride-free, oil-and water-resistant pulp molding products.

Al-modified yolk-shell silica particle-supported NiMo catalysts for ultradeep hydrodesulfurization of dibenzothiophene and 4,6-dimethyldibenzothiophene:Efficient accessibility of active sites and suitable acidity

Yolk-shell SiO2 particles(YP)with center-radial meso-channels were fabricated through a simple and effective method.Al-containing YP-supported NiMo catalysts with different Al amounts(NiMo/AYP-x,x=Si/Al molar proportion)were prepared and dibenzothiophene(DBT)and 4,6-dimethyl-dibenzothiophene(4,6-DMDBT)were employed as the probes to evaluate the hydrodesulfurization(HDS)catalytic performance.The as-prepared AYP-x carriers and corresponding catalysts were characterized by some advanced characterizations to obtain deeper correlations between physicochemical properties and the HDS performance.The average pore sizes of series AYP-x supports are above 6.0 nm,which favors the mass transfer of organic sulfides.The cavity between the yolk and the shell is beneficial for the enrichment of S-containing compounds and the accessibility between reactants and active metals.Aluminum embedded into the silica framework could facilitate the formation of Lewis(L)and Brønsted(B)acid sites and adjust the metal-support interaction(MSI).Among all the as-synthesized catalysts,NiMo/AYP-20 catalyst shows the highest HDS activities.The improved HDS activity of NiMo/AYP-20 catalyst is attributed to the perfect combination of excellent structural properties of the yolk-shell mesoporous silica,enhanced acidity,moderate MSI,and good accessibility/dispersion of active components.

Repeated inoculation with rumen fluid accelerates the rumen bacterial transition with no benefit on production performance in postpartum Holstein dairy cows

Background The dairy cow’s postpartum period is characterized by dramatic physiological changes,therefore imposing severe challenges on the animal for maintaining health and milk output.The dynamics of the ruminal microbiota are also tremendous and may play a crucial role in lactation launch.We aim to investigate the potential benefits of early microbial intervention by fresh rumen microbiota transplantation(RMT)and sterile RMT in postpartum dairy cows.Twelve fistulated peak-lactation dairy cows were selected to be the donors for rumen fluid collection.Thirty postpartum cows were divided into 3 groups as the transplantation receptors respectively receiving 10 L fresh rumen fluid(FR),10 L sterile rumen fluid(SR),or 10 L saline(CON)during 3 d after calving.Results Production performance,plasma indices,plasma lipidome,ruminal microbiome,and liver transcriptome were recorded.After fresh and sterile RMT,we found that the molar proportion of propionic acid was increased on d 7 in the FR and SR groups and the bacterial composition was also significantly changed when compared with the CON group.A similarity analysis showed that the similarities between the CON group and FR or SR group on d 7 were 48.40%or 47.85%,whereas the similarities between microbiota on d 7 and 21 in the FR and SR groups were 68.34%or 66.85%.Dry matter intake and feed efficiency were not affected by treatments.Plasmaβ-hydroxybutyrate concentration in the FR group was decreased and significantly different lipids mainly included phosphatidylcholine and lysophosphatidylcholine containing polyunsaturated fatty acids.Hepatic transcriptomics analysis indicated acutephase response pathways were upregulated in the SR group.Conclusions Our study suggests that RMT can shorten the transition process of the ruminal microbiota of postpartum dairy cows with no benefit on dry matter intake or feed efficiency.Inoculation with rumen fluid may not be a useful approach to promote the recovery of postpartum dairy cows.

Enhanced cyclic stability of partially disordered spinel cathodes through direct fluorination with gaseous fluorine

Spinel-type cathodes are considered an optimal substitute for conventional layered oxide cathodes owing to their use of inexpensive and earth-abundant manganese as the redox-active element.Moreover,the introduction of cation disorder can effectively suppress the detrimental two-phase reaction to realize high capacities in a wide voltage range.However,the continuous capacity decay during cycles has hindered the widespread application of these cathode materials.Inorganic fluorides exhibit excellent electrochemical stability at high voltage;therefore,in this study,the direct F2 gas reaction with a partially disordered spinel cathode(Li1.6Mn1.6O3.7F0.3,LMOF1.6) was initially applied to investigate the impacts of fluorination on the surface structure and electrochemical performances.The inorganic fluorinated layer,mainly containing LiF,was distributed uniformly on the surface of LMOF1.6nanoparticles after fluorination for an appropriate time without the turbulence caused by the valency of manganese cation,which improved the capacity retention and rate capability by the suppression of structural damage,parasitic reaction,and cation dissolution.The LMOF1.6cathode fluorinated for 0.5 h exhibited a capacity of283.6 mAh·g-1at 50 mA·g-1and an enhanced capacity retention of 29.6% after 50 cycles in the voltage range of1.5-4.8 V,as compared to the pristine LMOF1.6 with only27.9% capacity retention.

AIM:To evaluate dry eye disease(DED)symptomatology and mental health status in different COVID-19 patients.METHODS:A cross-sectional observational design was used.Totally 123 eligible adults(46.34%of men,age range,18-59y)with COVID-19 included in the study from August to November,2022.Ocular Surface Disease Index(OSDI),Five-item Dry Eye Questionnaire(DEQ-5),Hospital Anxiety and Depression Scale(HADS),and Pittsburgh Sleep Quality Index(PSQI)were used in this study.RESULTS:OSDI scores were 6.82(1.25,15.91)in asymptomatic carriers,7.35(2.50,18.38)in mild cases,and 16.67(4.43,28.04)in recurrent cases,with 30.00%,35.56%,and 57.89%,respectively evaluated as having DED symptoms(χ2=7.049,P=0.029).DEQ-5 score varied from 2.00(0,6.00)in asymptomatic carriers,3.00(0,8.00)in mild cases,and 8.00(5.00,10.00)in recurrent cases,with 27.50%,33.33%,and 55.26%,respectively assessed as having DED symptoms(χ2=8.532,P=0.014).The prevalence of clinical anxiety(50.00%)and depression(47.37%)symptoms were also significantly higher in patients with recurrent infection(χ2=24.541,P<0.001;χ2=30.871,P<0.001).Recurrent infection was a risk factor for high OSDI scores[odds ratio,2.562;95%confidence interval(CI),1.631-7.979;P=0.033]and DEQ-5 scores(odds ratio,3.353;95%CI,1.038-8.834;P=0.043),whereas having a fixed occupation was a protective factor for OSDI scores(odds ratio,0.088;95%CI,0.022-0.360;P=0.001)and DEQ-5 scores(odds ratio,0.126;95%CI,0.039-0.405;P=0.001).CONCLUSION:Patients with recurrent COVID-19 have more severe symptoms of DED,anxiety,and depression.

Relationships among Sedentary Time,Electronic Product Addiction,and Depression in Adolescents during the COVID-19 Epidemic:A Cross-Lagged Study

Objective:This study was conducted to explore the relationships among sedentary behavior(SB),electronic product addiction(EPA),and depression(D)in adolescents during the COVID-19 epidemic.Methods:A total of 604 adolescents(including 309 girls and 295 boys aged 12-18)were selected from Qufu City,Shandong Province,China for three rounds of investigation.The model was constructed using AMOS 23.0 software,and cross-lagged analysis was conducted.Results:SB at T1 can significantly positively predict SB and EPA at T2(p<0.05).EPA at T1 can significantly positively predict SB and D at T2(p<0.05).Physical activity level and SB at T2 can significantly predict SB and EPA at T3(p<0.05).EPA at T2 can significantly predict SB,EPA,and D at T3(p<0.05).Conclusions:SB and EPA are predictive factors for D.Moreover,SB can significantly positively predict D and indirectly predict D through the mediating effect of EPA.

Effect of Frequency on Degradation in BOPP Films Under Repetitively Pulsed Voltage

Operating conditions of film capacitors are complex,and the problem of film insulation failure caused by repetitively pulsed voltage is becoming ever serious.Degradation of the film under repetitively pulsed voltage cannot be accurately evaluated by the average breakdown electric field.In this paper,the effects of pulsed electric field and pulse repetition frequency on the breakdown in biaxially oriented polypropylene(BOPP)films are investigated.Three phases of BOPP degradation are proposed based on the voltage amplitude,i.e.,maintenance(M),decline(D),and near-zero(N).Evolution of the BOPP film from degradation to breakdown at different frequencies is presented.Meanwhile,transition of discharge mode and elemental composition of the film are analyzed.Experimental results show continuous heat generation under repetitive microsecond pulses is the dominant factor for degradation of BOPP film.The number of applied pulses and the repetitive stressing time decrease exponentially with increase of frequency.This research can be contributed to the safe and reliable operation of capacitors.

Impacts of agri-food e-commerce on traditional wholesale industry:Evidence from China

Rapidly expanding studies investigate the effects of e-commerce on company operations in the retail market.However,the interaction between agri-food e-commerce(AEC)and the traditional agri-food wholesale industry(AWI)has not received enough attention in the existing literature.Based on the provincial panel data from 2013 to 2020 in China,this paper examines the effect of AEC on AWI,comprising three dimensions:digitalization(DIGITAL),agrifood e-commerce infrastructure and supporting services(AECI),and agri-food e-commerce economy(AECE).First,AWI and AEC are measured using an entropy-based combination of indicators.The results indicate that for China as a whole,AWI has remained practically unchanged,whereas AEC exhibits a significant rising trend.Second,the findings of the fixed-effect regression reveal that DIGITAL and AECE tend to raise AWI,whereas AECI negatively affects AWI.Third,threshold regression results indicate that AECI tends to diminish AWI with three-stage inhibitory intensity,which manifests as a first increase and then a drop in the inhibition degree.These results suggest that with the introduction of e-commerce for agricultural product circulation,digital development will have catfish effects that tend to stimulate the vitality of the conventional wholesale industry and promote technical progress.Furthermore,the traditional wholesale industry benefits financially from e-commerce even while it diverts part of the traditional wholesale circulation for agricultural products.

Due to its highly insidious and rapid progression,deep tissue pressure injury(DTPI)is a clinical challenge.Our previous study found that DTPI may be a skeletal muscle injury dominated by macrophage immune dysfunction due to excessive iron accu-mulation.Decellularized extracellular matrix(dECM)hydrogel promotes skeletal muscle injury repair.However,its role in po-larizing macrophages and regulating iron metabolism in DTPI remains unclear.Here,porcine dECM hydrogel was prepared,and its therapeutic function and mechanism in repairing DTPI were investigated.The stimulus of dECM hydrogel toward RAW264.7 cells resulted in a significantly higher percentage of CD2o6+macrophages and notably decreased intracellular divalent iron levels.In mice DTPI model,dECM hydrogel treatment promoted M1 to M2 macrophage conversion,improved iron metabolism and reduced oxidative stress in the early stage of DTPI.In the remodeling phase,the dECM hydrogel remarkably enhanced revascularization and accelerated skeletal muscle repair.Furthermore,the immunomodulation of dEcM hydrogels in vivo was mainly involved in the P13k/Akt signaling pathway,as revealed by GO and KEGG pathway analysis,which may ameliorate the iron deposition and promote the healing of DTPI.Our findings indicate that dECM hydrogel is promising in skeletal muscle repair,inflammation resolution and tissue injury healing by effectively restoring macrophage immune homeostasis and normalizing iron metabolism.
