Efficacy,safety,and cost-effectiveness of triple therapy in preventing relapse in rheumatoid arthritis:A randomized controlled trial(ESCoRT study)

Background:Biological agents,such as tumor necrosis factor inhibitors(TNFi),have been widely used in rheumatoid arthritis(RA)patients and greatly improved goal achievement.The aim of this study was to investigate whether conventional synthetic diseasemodifying anti-rheumatic drugs(csDMARDs)combination was better in reducing relapse than methotrexate(MTX)monotherapy,and more cost-effective than continuing TNFi plus MTX in RA patients who achieved low disease activity(LDA)with TNFi and MTX therapy.Methods:RA patients who failed to csDMARDs received an induction therapy of MTX plus TNFi for maximally 12 weeks.Those achieving LDA in 12 weeks were randomly assigned at a 1:1:1 ratio into three groups:(A)adding hydroxychloroquine and sulfasalazine for the first 12 weeks and then discontinuing TNFi for the following 48 weeks;(B)maintaining TNFi and MTX for 60 weeks;and(C)maintaining TNFi and MTX for the first 12 weeks and then discontinuing TNFi for the following 48 weeks.The primary outcome was relapse.Results:A total of 117 patients were enrolled for induction therapy and 67 patients who achieved LDA within 12 weeks were randomized,with 24,21,and 22 patients in groups A,B,and C,respectively.The relapse rates of groups A and B during the entire 60 weeks were comparable[10/22(45.5%)vs.7/20(35.0%),χ^(2)=0.475,P=0.491],however,significantly lower than that of group C[10/22(45.5%)vs.17/20(85.0%),χ^(2)=5.517,P=0.019;7/20(35.0%)vs.17/20(85.0%),χ^(2)=11.035,P=0.004,respectively].Taking RMB 100,000 Yuan as the threshold of willingness to pay,compared to MTX monotherapy(group C),both TNFi maintenance and triple csDMARDs therapies were cost-effective,but triple csDMARDs therapy was better.Conclusion:For RA patients who have achieved LDA with TNFi and MTX,csDMARDs triple therapy was a cost-effective option in favor of reducing relapse.Trial registration:ClinicalTrials.gov,NCT02320630.

Covalent organic frameworks(COFs) have been attracting growing concerns since the first report in2005. With the well-defined and ordered structures, COFs express big potential in mass transport, storage/separation and energy conversion applications. From the perspective of both theory and application,the construction of crystalline COFs with high quality and variety is highly worth to be devoted to. To give insight into the crystalline process of COFs and deeply understand the factors of COFs crystallization,this review was concentrated on the recent progress in construction of crystalline COFs. Accordingly, the types and crystallization process of COFs were summarized firstly. And then the factors on crystallinity and the measures for improving the crystallinity of COFs were classified and discussed in detail. Finally,the perspectives for the development of COFs in further was given at the end of this review.

For localized fires, it is necessary to consider the thermal and mechanical responses of building elements subject to uneven heating under the influence of wind. In this paper, the thermomechanical phenomena experienced by a ceiling jet and I-beam in a structural fire were simulated. Instead of applying the concept of adiabatic surface temperature (AST) to achieve fluid–structure coupling, this paper proposes a new computational fluid dynamics–finite element method numerical simulation that combines wind, fire, thermal, and structural analyses. First, to analyze the velocity and temperature distributions, the results of the numerical model and experiment were compared in windless conditions, showing good agreement. Vortices were found in the local area formed by the upper and lower flanges of the I-beam and the web, generating a local high-temperature zone and enhancing the heat transfer of convection. In an incoming-flow scenario, the flame was blown askew significantly;the wall temperature was bimodally distributed in the axial direction. The first temperature peak was mainly caused by radiative heat transfer, while the second resulted from convective heat transfer. In terms of mechanical response, the yield strength degradation in the highest-temperature region in windless conditions was found to be significant, thus explaining the stress distribution of steel beams in the fire field. The mechanical response of the overall elements considering the incoming flows was essentially elastic.

Viral diseases cause serious economic loss in farmed animals industry.However,the efficacy of remedies for viral infection in farmed animals is limited,and treatment strategies are generally lacking for aquatic animals.Interactions of commensal microbiota and viral infection have been studied in recent years,demonstrating a third player in the interaction between hosts and viruses.Here,we discuss recent developments in the research of interactions between commensal bacteria and viral infection,including both promotion and inhibition effect of commensal bacteria on viral pathogenesis,as well as the impact of viral infection on commensal microbiota.The antiviral effect of commensal bacteria is mostly achieved through priming or regulation of the host immune responses,involving differential microbial components and host signaling pathways,and gives rise to various antiviral probiotics.Moreover,we summarize studies related to the interaction between commensal bacteria and viral infection in farmed animals,including pigs,chickens,fish and invertebrate species.Further studies in this area will deepen our understanding of antiviral immunity of farmed animals in the context of commensal microbiota,and promote the development of novel strategies for treatment of viral diseases in farmed animals.

Hierarchically porous nitrogen-doped carbon(HPNC) with interconnected micro-,meso-,and macropores was generated from KOH and urea etching as a carbon scaffold to infuse Se.In this structure,micropores confine Se as single-chain molecules,preventing the generation of higher-order polyselenides,the mesopores provide effective pathways for electrolyte diffusion and ion transport and offer sufficient void space to accommodate the volume change of Se upon lithiation/delithiation,and the doped nitrogen improves the electrical conductivity of HPNC and increases the utilization of Se.When applied as a scaffold for Se in a lithium-selenium battery,the Se/HPNC cathode with a large Se content of 67.6 wt% maintains a high discharge capacity of 410 mAh·g^(-1) at the 500 th cycle at1.0 C,and it demonstrates a discharge capacity as high as371 mAh·g^(-1) even at a high current density of 5.0 C.Moreover,when the current density returns to 0.2 C,the discharge capacity recovers to 527 mAh·g^(-1) with a capacity retention rate of 90.5% for the first 0.2 C,indicating the excellent structural stability of HPNC.HPNC has significant potential for application in energy storage systems.

Heat Transfer Performance of a Novel Microchannel Embedded with Connected Grooves

To improve the heat transfer performance of microchannels,a novel microchannel embedded with connected grooves crossing two sidewalls and the bottom surface(type A)was designed.A comparative study of heat transfer was conducted regarding the performances of type A microchannels,microchannels embedded with grooves on their bottom(including types B and C),or on the sidewalls(type D)as well as smooth rectangular microchannels(type E)via a three-dimensional numerical simulation and experimental validation(at Reynolds numbers from 118 to 430).Numerical results suggested that the average Nusselt number of types A,B,C,and D microchannels were 106,73.4,50.1,and 12.6%higher than that of type E microchannel,respectively.The smallest synergy angle β and entropy generation number Ns,a were determined for type A microchannels based on field synergy and nondimensional entropy analysis,which indicated that type A exhibited the best heat transfer performance.Numerical flow analysis indicated that connected grooves induced fluid to flow along two different temperature gradients,which contributed to enhanced heat transfer performance.

The coronavirus disease 2019(COVID-19)epidemic has triggered a huge impact on healthcare,socioeconomics,and other aspects of the world over the past three years.An increasing number of studies have identified a complex relationship between COVID-19 and stroke,although active measures are being implemented to prevent disease transmission.Severe COVID-19 may be associated with an increased risk of stroke and increase the rates of disability and mortality,posing a serious challenge to acute stroke diagnosis,treatment,and care.This review aims to provide an update on the influence of COVID-19 itself or vaccines on stroke,including arterial stroke(ischemic stroke and hemorrhagic stroke)and venous stroke(cerebral venous thrombosis).Additionally,the neurovascular mechanisms involved in SARS-CoV-2 infection and the clinical characteristics of stroke in the COVID-19 setting are presented.Evidence on vaccinations,potential therapeutic approaches,and effective strategies for stroke management has been highlighted.

A Comparative Study on Asperity Peak Modeling Methods

The peak identification scheme based method(three-point definition)and the spectral moments based method(spectral moment approach)are both widely used for asperity peak modeling in tribology.To discover the differences between the two methods,a great number of rough surface profile samples with various statistical distributions are first randomly generated using FFT.Then the distribution parameters of asperity peaks are calculated for the generated samples with both methods.The obtained results are compared and verified by experiment.The variation rules of the differences between the two methods with statistical characteristics of rough surfaces are investigated.To explain for the discovered differences,the assumptions by spectral moment approach that the joint distribution of surface height,slope and curvature is normal and that the height distribution of asperities is Gaussian,are examined.The results show that it is unreasonable to assume a joint normal distribution without inspecting the correlation pattern of[z],[z′]and[z′′],and that the height distribution of asperities is not exactly Gaussian before correlation length of rough surface increases to a certain extent,20 for instance.
