Surgical management of pituitary adenoma during pregnancy

BACKGROUND Although conservative treatment is typically recommended for pregnant patients with pituitary adenoma(PA),surgical treatment is occasionally necessary for those with acute symptoms.Currently,surgical interventions utilized among these patients is poorly studied.AIM To evaluate the surgical indications,timing,perioperative precautions and postoperative complications of PAs during pregnancy and to provide comprehensive guidance.METHODS Six patients with PAs who underwent surgical treatment during pregnancy at Peking Union Medical College Hospital between January 1990 and June 2021 were recruited for this study.Another 35 pregnant patients who were profiled in the literature were included in our analysis.RESULTS The 41 enrolled patients had acute symptoms including visual field defects,severe headaches or vision loss that required emergency pituitary surgeries.PA apoplexies were found in 23 patients.The majority of patients(55.9%)underwent surgery in the second trimester of pregnancy.A multidisciplinary team was involved in patient care from the preoperative period through the postpartum period.With the exception of 1 patient who underwent an induced abortion and 1 fetus that died due to a nuchal cord,39 patients delivered successfully.Among them,37 fetuses were healthy until the most recent follow-up.CONCLUSION PA surgery during pregnancy is effective and safe during the second and third trimesters.Pregnant patients requiring emergency PA surgery require multidisciplinary evaluation and healthcare management.

Integrative analysis of metabolome and transcriptome provides new insights into functional components of Lilii Bulbus

Objective:Lilium brownii var.viridulum(LB) and L.lancifolium(LL) are the main sources of medicinal lily(Lilii Bulbus,Baihe in Chinese) in China.However,the functional components of these two species responsible for the treatment efficacy are yet not clear.In order to explore the therapeutic material basis of Lilii Bulbus,we selected L.davidii var.willmottiae(LD) only used for food as the control group to analyze the differences between LD and the other two(LB and LL).Methods:Metabolome and transcriptome were carried out to investigate the differences of active components in LD vs LB and LD vs LL,Data of metabolome and transcriptome was analysed using various analysis methods,such as principal component analysis(PCA),hierarchical cluster analysis(HCA),and so on.Differentially expressed genes(DEGs) were enriched through KEGG and GO enrichment analysis.Results:The PCA and HCA of the metabolome indicated the metabolites were clearly separated and varied greatly in LL and LB contrasted with LD.There were 318 significantly differential metabolites(SDMs) in LD vs LB group and 298 SDMs in LD vs LL group.Compared with LD group,the significant up-regulation of steroidal saponins and steroidal alkaloids were detected both in LB and LL groups,especially in LB group.The HCA of transcriptome indicated that there was significant difference in LB vs LD group,while the difference between LL and LD varied slightly.Additionally,47 540 DEGs in LD vs LB group and 18 958 DEGs in LD vs LL group were identified.Notably,CYP450s involving in the biosynthesis of steroidal saponins and steroidal alkaloids were detected,and comparing with LD,CYP724,CYP710A,and CYP734A1 in LB and CYP90B in LL were all up-regulated.Conclusion:This study suggested that steroidal saponins and steroidal alkaloids maybe the representative functional components of Lilii Bulbus,which can provide new insights for Lilii Bulbus used in the research and development of classic famous formula.

Human-specific insertions play important roles in human phenotypes and diseases.Here we reported a 446-bp insertion(Insert-446)in intron 11 of the TBC1D8B gene,located on chromosome X,and traced its origin to a portion of intron 6 of the EBF1 gene on chromosome 5.Interestingly,Insert-446 was present in the human Neanderthal and Denisovans genomes,and was fixed in humans after human-chimpanzee divergence.We have demonstrated that Insert-446 acts as an enhancer through binding transcript factors that promotes a higher expression of human TBC1D8B gene as compared with orthologs in macaques.In addition,over-expression TBC1D8B promoted cell proliferation and migration through“a dual finger”catalytic mechanism(Arg538 and Gln573)in the TBC domain in vitro and knockdown of TBC1D8B attenuated tumorigenesis in vivo.Knockout of Insert-446 prevented cell proliferation and migration in cancer and normal cells.Our results reveal that the human-specific Insert-446 promotes cell proliferation and migration by upregulating the expression of TBC1D8B gene.These findings provide a significant insight into the effects of human-specific insertions on evolution.

Efficient electrocatalysts are vital to large-current hydrogen production in commercial water splitting for green energy generation.Herein,a novel heterophase engineering strategy is described to produce polymorphic CoSe2electrocatalysts.The composition of the electrocatalysts consisting of both cubic CoSe2（c-CoSe2） and orthorhombic CoSe2（o-CoSe2） phases can be controlled precisely.Our results demonstrate that junction-induced spin-state modulation of Co atoms enhances the adsorption of intermediates and accelerates charge transfer resulting in superior large-current hydrogen evolution reaction（HER）properties.Specifically,the CoSe2-based heterophase catalyst with the optimal c-CoSe2content requires an overpotential of merely 240 mV to achieve 1,000 mA·cm-2as well as a Tafel slope of 50.4 mV·dec-1.Furthermore,the electrocatalyst can maintain a large current density of 1,500 mA·cm-2for over 320 h without decay.The results reveal the advantages and potential of heterophase junction engineering pertaining to design and fabrication of low-cost transition metal catalysts for large-current water splitting.

BACKGROUND SMARCA4 is a component of chromatin remodeling of SWItch/sucrose-nonfermenting(SWI/SNF)complexes and plays an essential role in oncogenesis.SMARCA4-deficient malignancies arising from the gastrointestinal tract are rare and have a poor prognosis.There is no standard treatment for advanced and undifferentiated SMARCA4-deficient duodenal malignancies.Programmed death 1(PD-1)antibodies,known as immune checkpoint inhibitor antibodies,potentially play a role in treating gastrointestinal tract malignancies.CASE SUMMARY We present two patients with SMARCA4 deficiency and TP53 gene mutation in advanced undifferentiated carcinomas of the duodenum.For both patients,SMARCA4 deficiency was confirmed by immunohistochemical staining for the BRG1 protein,while TP53 gene mutations were observed via next-generation sequencing.Both patients were administered chemotherapy in combination with an anti-PD-1 antibody.The two patients exhibited completely different responses to treatment and had different prognoses.Case 1 experienced rapid progression after PD-1 infusion and chemotherapy,case 2 experienced a remarkable response after treatment,and the progression-free survival was more than 6 months.CONCLUSION This study described our clinical and pathological observations of SMARCA4-deficient advanced undifferentiated carcinoma of the duodenum.PD-1 combined with chemotherapy showed a certain efficacy in select patients,providing options for treating these highly malignant tumors.Patients with liver metastases had a worse prognosis than did those with only lymph node metastasis.

Development of the second version of Global Prediction System for Epidemiological Pandemic

Coronavirus disease 2019(COvID-19)is a severe global public health emergency that has caused a major cri-sis in the safety of human life,health,global economy,and social order.Moreover,CovID-19 poses significant challenges to healthcare systems worldwide.The prediction and early warning of infectious diseases on a global scale are the premise and basis for countries to jointly fight epidemics.However,because of the complexity of epidemics,predicting infectious diseases on a global scale faces significant challenges.In this study,we developed the second version of Global Prediction System for Epidemiological Pandemic(GPEP-2),which combines statis-tical methods with a modified epidemiological model.The GPEP-2 introduces various parameterization schemes for both impacts of natural factors(seasonal variations in weather and environmental impacts)and human so-cial behaviors(government control and isolation,personnel gathered,indoor propagation,virus mutation,and vaccination).The GPEP-2 successfully predicted the COVID-19 pandemic in over 180 countries with an average accuracy rate of 82.7%.It also provided prediction and decision-making bases for several regional-scale CovID-19 pandemic outbreaks in China,with an average accuracy rate of 89.3%.Results showed that both anthropogenic and natural factors can affect virus spread and control measures in the early stages of an epidemic can effectively control the spread.The predicted results could serve as a reference for public health planning and policymaking.

“文化书写”是指小说对一个社会空间中的文化形态进行呈现,在许多小说中体现为对民俗风情的展示,这种传统书写往往将文化表现为一种恒定的状态,而忽视了文化在时代、生活中的变迁,尤其会低估个体对置身其中的文化的能动性乃至反向塑形能力。这容易令人把“文化”理解为一种教条化的知识结论,但事实上文化跟人的命运具有不可分割性。“书写文化”就是对写作者精神主体自觉的强调,让写作结合个体的生命体验,反思自我置身其中的文化模式,把地方性经验置放到全人类的视野中去考量与思辨,这样才更能“深描”出文化的内质。由此,小说可以被看成是生命主体借助虚构去介入社会文化的一种精神实践,这种实践让文化获得生机并不断生成新的可能性。

A Cementing Technology for Shale Oil Horizontal Wells

Organic rich dark shale of Q Formation can be found in many areas(e.g.,in the North of S Basin).The shale target stratum is easy to hydrate and often undergoes spallation.Therefore,centering the casing in the horizontal section of the irregular borehole is relatively difficult.Similarly,achieving a good cementflushing efficiency under complex borehole conditions is a complex task.Through technologies such as centralizer,efficient preflushing,multi-stageflushing and ductile cement slurry,better performances can be achieved.In this study,it is shown that the cementing rate in the DY2H horizontal section is 97.8%,which is more than 34%higher than that of adjacent wells.This cementing matching technology for sidetracking horizontal wells can be used to improve the cementing quality of continental shale and provides a reference for future applications in thisfield.

快手类短视频平台的兴起,给农村女性的主体意识、主体行动、人际交往、日常生活等带来多种改变,为农村女性重新建构自己的主体性提供了机会。从实践主体性的视角出发,通过对农村女性的访谈和参与式观察,研究发现:线上,农村女性的实践主体性主要表现为自我呈现与自我消费;线下,农村女性的实践主体性主要表现为自我组织与关系重建。线上与线下围绕快手,既内外有别,又相互影响,互相强化。但是,农村女性的主体性依然面临社区情理难以改变的问题,在公共领域的呈现依然十分有限,对乡村公共生活的参与也明显不足。乡村振兴、乡土重建需要重建女性群体与村庄公共生活的关联。

模块化多电平铁路功率调节器作为一个耦合的多变量非线性系统,传统PI控制的直接功率控制难以实现对系统的精确解耦.本文提出了一种基于线性扩张状态观测器的反馈线性化直接功率控制方法,根据Lie导数构建了模块化多电平铁路功率调节器(MMC-RPC)两输入/两输出功率仿射模型,设计了精确反馈线性化功率解耦控制器.针对不确定因素等扰动对精确反馈线性化控制效果的影响,设计了线性扩张状态观测器对扰动进行观测和补偿,实现了功率的精确跟踪控制.最后,通过MATLAB/Simulink平台搭建仿真模型对所提控制方法进行了验证.
