Different lymph node staging systems for predicting the prognosis of colorectal neuroendocrine neoplasms

BACKGROUND Colorectal neuroendocrine neoplasms(NENs)are a rare malignancy that primarily arises from the diffuse distribution of neuroendocrine cells in the colon and *** studies have pointed out that the status of lymph node may be used to predict the *** To investigate the predictive values of lymph node ratio(LNR),positive lymph node(PLN),and log odds of PLNs(LODDS)staging systems on the prognosis of colorectal NENs treated surgically,and compare their predictive *** This cohort study included 895 patients with colorectal NENs treated surgically from the Surveillance,Epidemiology,and End Results *** endpoint was mortality of patients with colorectal NENs treated surgically.X-tile software was utilized to identify most suitable thresholds for categorizing the LNR,PLN,and *** were selected in a random manner to form training and testing *** prognosis of surgically treating colorectal NENs was examined using multivariate cox analysis to assess the associations of LNR,PLN,and LODDS with the prognosis of colorectal NENs.C-index was used for assessing the predictive *** conducted a subgroup analysis to explore the different lymph node staging systems’predictive *** After adjusting all confounding factors,PLN,LNR and LODDS staging systems were linked with mortality in patients with colorectal NENs treated surgically(P<0.05).We found that LODDS staging had a higher prognostic value for patients with colorectal NENs treated surgically than PLN and LNR staging *** results were obtained in the different G staging subgroup ***,the area under the receiver operating characteristic curve values for LODDS staging system remained consistently higher than those of PLN or LNR,even at the 1-,2-,3-,4-,5-and 6-year follow-up *** LNR,PLN,and LODDS were found to significantly predict the prognosis of patients with colorectal NENs treated surgically.

Transition metal dichalcogenides(TMDs),with the general formula MX_(2)(M=Mo/W/Fe/Co/Ni,etc.;X=S/Se/Te),have attracted extensive research interests for hydrogen evolution reaction(HER).Compared with numerous studies on noble-metal-free TMDs,the chalcogen-dependent HER catalytic properties of noble-metal-based TMDs are lack of sufficient research ***,a facile electrospinning-assisted synthetic strategy is proposed to synthesize ruthenium dichalcogenides(RuX_(2),X=S/Se/Te)nanoparticles decorated carbon nanofibers(CNFs).Benefiting from the identical nanofibrous morphology and exposed crystal planes of RuX_(2)(111),the catalytic activities of RuX_(2)@CNFs samples were investigated and compared in a fair and direct *** electrochemical measurements coupled with density functional theory calculations were carried out to probe their intrinsic HER catalytic activities,resulting in the catalytic activity order of RuS_(2)@CNFs>RuSe_(2)@CNFs>RuTe_(2)@CNFs in acidic media and that of RuS_(2)@CNFs>RuTe_(2)@CNFs>RuSe_(2)@CNFs in alkaline *** superior catalytic performance of RuS_(2)@CNFs mainly stems from the relative lower HER energy barriers and thereby the higher intrinsic catalytic activity of RuS_(2)(111),leading to ultralow overpotentials of 44 and 9 mV at 10 mA·cm^(-2) in acidic and alkaline media,***_(2)(111)is endowed with the more optimized Gibbs free energy of hydrogen adsorption(ΔGH*)than RuTe_(2)(111),but RuTe_(2)(111)shows enhanced catalytic property for H_(2)O dissociation and OH-desorption than RuSe_(2)(111),therefore,resulting in the altered catalytic activity sequences for RuSe_(2) and RuTe_(2) in acidic and alkaline media.

Intrahepatic portal venous systems in adult patients with cavernous transformation of portal vein: Imaging features and a new classification

Background: Cavernous transformation of the portal vein(CTPV) due to portal vein obstruction is a rare vascular anomaly defined as the formation of multiple collateral vessels in the hepatic hilum. This study aimed to investigate the imaging features of intrahepatic portal vein in adult patients with CTPV and establish the relationship between the manifestations of intrahepatic portal vein and the progression of CTPV. Methods: We retrospectively analyzed 14 CTPV patients in Beijing Tsinghua Changgung Hospital. All patients underwent both direct portal venography(DPV) and computed tomography angiography(CTA) to reveal the manifestations of the portal venous system. The vessels measured included the left portal vein(LPV), right portal vein(RPV), main portal vein(MPV) and the portal vein bifurcation(PVB). Results: Nine males and 5 females, with a median age of 40.5 years, were included in the study. No significant difference was found in the diameters of the LPV or RPV measured by DPV and CTA. The visualization in terms of LPV, RPV and PVB measured by DPV was higher than that by CTA. There was a significant association between LPV/RPV and PVB/MPV in term of visibility revealed with DPV( P = 0.01), while this association was not observed with CTA. According to the imaging features of the portal vein measured by DPV, CTPV was classified into three categories to facilitate the diagnosis and treatment. Conclusions: DPV was more accurate than CTA for revealing the course of the intrahepatic portal vein in patients with CTPV. The classification of CTPV, that originated from the imaging features of the portal vein revealed by DPV, may provide a new perspective for the diagnosis and treatment of CTPV.

在高速工业化和国际贸易快速发展的进程中,以农产品进口换取工业品出口的需要和外来廉价农产品的涌入导致德意志帝国出现了农产品价格与利润双双下跌的状况,使农业保护问题凸显。尽管农业界发出了强烈的保护呼声,但自俾斯麦离职后,新任帝国宰相卡普里维却改变了自1879年以来所实行的农业保护政策。原本与农业界紧密合作的德国保守党,此时也并未坚决支持农业保护政策。在此背景下,德国农业界在1893年创立了农民协会。凭借出色的竞选动员能力和贵族特权地位,农民协会迅速在政治舞台上占据优势地位,并基于农业至上和政府干预理念,提出了以提高关税为核心的一系列农业保护建议。农业危机的不断加剧与农民协会所施加的巨大压力,共同构成了帝国当局决心加强农业保护的主要驱动力。帝国当局不顾工业界反对所推行的农业保护政策,如提高粮食进口关税、禁止粮食期货交易、强化进口畜产品检疫,以及严格控制人造黄油的生产与销售等,有效促进了农业的复苏与进步,有利于德国工农业的协调发展。

18世纪普鲁士盛行农奴制,国家既与领主结盟,又限制领主过度压榨农奴。19世纪上半叶王权下探,领主交出部分权力,乡镇自治获得法律承认,但领主在县长任命、乡村治安等领域仍保有特权,东部、北部乡镇也未获得自治权。1871年后,普鲁士王家政府意识到乡村治理随着农奴解放必须调整,制定了针对东部、北部省份的县级和乡镇条例。这些条例通过警察国家化、县长专业化、扩大资产阶级和农民参与权,在规范乡村自治的同时确立了国家的权威,促进了乡村治理体系的现代转型。县镇条例实施后,以容克地主为代表的大地主仍在乡村治理中占有明显优势。

作为非线性发展方程的一种特殊局域解,呼吸子具有包络振荡结构,且这种振荡呈现周期性变化.根据呼吸子在分布方向和演化方向的周期性,呼吸子主要有3种类型,即Kuznetsov-Ma呼吸子(Kuznetsov-Ma breather,KMB)、Akhmediev呼吸子(Akhmediev breather,AB)和一般呼吸子(general breather,GB).近年来,周期背景下的呼吸子现象在许多非线性物理领域被观察到,比如在非线性光纤光学、流体力学等.研究表明背景周期波的调制不稳定性可以激发呼吸子的产生,且周期背景下的呼吸子具有非常丰富的物理性质和相互作用.因此,最近在周期背景下呼吸子的时空结构和相互作用引起了广泛关注.Gerdjikov-Ivanov方程可以被用来描述在量子场理论、弱非线性色散水波、非线性光学等领域中的非线性物理现象.构造该模型的各种类型的解是非常有意义的工作.据了解,在椭圆函数背景下的多呼吸子之前还未被研究过.本文首先利用修正的平方波(modified squared wave,MSW)函数法和行波变换法获得该方程的椭圆函数解.然后,在椭圆函数解初始条件下得到该方程Lax对的通解.基于椭圆函数的转换公式以及积分公式,将势函数周期解化简为只含有weierstrass椭圆函数.然后,利用达布变换构造出在椭圆函数背景下呼吸子的具体表达形式.在椭圆函数背景下,推导出3种不同类型的呼吸子,包括GB,KMB和AB.最后,给出3种呼吸子的时空结构三维图,并且展示它们之间相互作用的过程.

Clinical effects of detailed nursing management interventions on medication adherence and disease perception in patients with drugresistant tuberculosis

BACKGROUND Tuberculosis(TB)is a chronic respiratory infectious disease that considerably jeopardizes human health,and there is no effective vaccine suitable for its prevention in the entire *** To investigate the promotion of medication adherence and disease cognition in patients with drug-resistant(DR-)TB using detailed nursing *** In total,114 patients with DR-TB who were diagnosed and treated at our hospital between January 2019 and January 2023 were included in this *** in the control group(n=57)were managed with conventional nursing care,while those in the observation group(n=57)were managed with detailed nursing *** adherence,disease awareness scores,medication safety,and nursing satisfaction were compared between the two groups after the *** The post-intervention medication compliance rate was 91.23%in the observation group and 75.44%in the control group,with the former being 15.79%higher than the latter(P<0.05).There was no statistically significant difference in the disease awareness scores between the two groups before the intervention;the disease awareness scores of the observation group were significantly higher than those of the control group after the intervention(P<0.05).The incidence of gastrointestinal reactions,joint swelling and pain,hearing loss,electrolyte disorders,and liver and kidney function abnormalities were lower in the observation group than those in the control *** total nursing satisfaction of the observation group was higher than that of the control group(P<0.05).CONCLUSION Implementation of detailed nursing management for patients with DR-TB can effectively improve medication adherence,enhance awareness of the disease,ensure safety of medication,and improve satisfaction with nursing care.
