Preoperative Anemia Is Associated with Increased Mortality Following Primary Unilateral Total Joint Arthroplasty

Background: Total joint arthroplasty is a commonly performed procedure for end-stage osteoarthritis. Preoperative anemia is a well-characterized and potentially modifiable risk factor for morbidity and mortality in non-cardiac surgery. This retrospective cohort study identified the prevalence of anemia in our total joint arthroplasty population and investigated if there was an association with all-cause mortality. Study Design and Methods: Using an electronic medical record, we examined all patients who underwent a primary unilateral total joint arthroplasty at Dartmouth-Hitchcock Medical Center from January 1, 2011 through July 1, 2015. Preoperative anemia thresholds were defined according to the World Health Organization as 12 mg/dl for women and 13 mg/dl for men. Kaplan-Meier survival analyses were performed to examine the relationship between preoperative anemia and all-cause mortality. Cox proportional hazards were calculated for various models adjusting for potential confounders. Results: 439 of the 3247 patients (13.5%) that underwent total joint arthroplasty met the preoperative definition for anemia. 48 patients (1.48%) died during 6470 patient years of follow up, generating an incidence rate of 7.4 deaths per 1000 patient years. The crude hazard ratio for death in anemic patients was 3.42 (95% CI 1.89, 6.82). In multiple models adjusting for various health related confounders, preoperative anemia was associated with a roughly two-fold increase risk of death compared to non-anemic patients. Conclusion: Preoperative anemia is prevalent in our population and it is associated with increased postoperative mortality in total joint arthroplasty patients, even when adjustments are made for significant co-morbidities.

Species distribution models are increasing in popularity for mapping suitable habitat for species of management con- cern. Many investigators now recognize that extrapolations of these models with geographic information systems （GIS） might be sensitive to the environmental bounds of the data used in their development, yet there is no recommended best practice for ＂clamping＂ model extrapolations. We relied on two commonly used modeling approaches： classification and regression tree （CART） and maximum entropy （Maxent） models, and we tested a simple alteration of the model extrapolations, bounding ex- trapolations to the maximum and minimum values of primary environmental predictors, to provide a more realistic map of suit-able habitat of hybridized Africanized honey bees in the southwestern United States. Findings suggest that multiple models of bounding, and the most conservative bounding of species distribution models, like those presented here, should probably replace the unbounded or loosely bounded techniques currently used [Current Zoology 57 （5）： 642-647,2011].

Current updates on laboratory techniques for the diagnosis of male reproductive failure

The incidence of male reproductive failure leading to infertility, whether due to delayed parenthood, environmental issues, genetic factors, drugs, etc., is increasing throughout the world. The diagnosis and prognosis of male subfertility have become a challenge. While the basic semen assessment has been performed for many years, a number of studies question the value of the traditional semen characteristics. This is partly due to inadequate methods and standardization, limited knowledge of technical requirements for quality assurance, and an incomplete understanding of what clinical information a semen assessment can provide. Laboratories currently performing semen and endocrine assessment show great variability. The World Health Organization （WHO） manual for the evaluation of semen has been the core of andrology and fertility evaluation that has helped in further development of this field over many years. These include the physical appearance of the ejaculate, assessments of sperm count, motility, vitality, morphology, and functional aspects of the sperm and semen sample. These tests also include male endocrine profile, biochemical evaluation of the semen, detection of antisperm antibodies in serum, the use of computer-aided sperm analysis （CASA）, sperm DNA integrity, and its damage due to oxidative stress. Assisted reproductive techniques （e.g., IVF, ICSI） have shown great success but are too expensive. Further development in this field with newer techniques and extensive training/instructions can improve accuracy and reduce variability, thus maintaining the quality and standards of such an evaluation. There is an urgent need to have standardized training centers and increased awareness in this area of men＇s health for reproductive success.

Purinergic contraction of the rat vas deferens in L-NAMe-induced hypertension：effect of sildenafil

Hypertension （HTN） is a risk factor for erectile dysfunction, but its effect on vas deferens （VD） contractility and the ejaculatory response has not been delineated. NG-nitro-L-arginine methyl ester （L-NAMe）, a nitric oxide synthase inhibitor, was used for induction of nitric oxide （NO）-deficient HTN. Our aim was to evaluate the effects of L-NAMe-induced HTN on rat VD contractility and to determine whether sildenafil affects VD contractility. A total of 36 male rats were divided into （1） control, （2）L-NAMe-HTN, （3） sildenafil treated L-NAMe-HTN groups. Group 2 was treated with L-NAMe （40 mg kgI per day） in drinking water for 4 weeks. Group 3 received sildenafil （1.5 mg kg^-1 per day, by oral gavage） concomitantly with L-NAMe. The prostatic portion of the VD was subjected to electrical field stimulation （eFS, 1-20 Hz）, and the P2X1 agonist α,β-methylene ATP （α,β meATP, 100 μmol L^-1-1 μmol L-1） and the al-adrenoceptor agonist phenylephrine （Phe, 100 μmol L^-1-1 mmol L^-1） were used to construct concentration-response curves. These experiments were repeated in the presence of P2X receptor antagonist, pyridoxalphosphate-6-azophenyl-2＇,4＇-disulfonic acid （PPADS, 30 μmol L-1）. VD contractions in response to eFS, a,β-meATP and Phe were significantly enhanced by L-NAMe. Sildenafil treatment in the L-NAMe group improved the contractile response of VD to eFS （20 Hz）. In the presence of PPADS, the enhanced contractile response of VD to eFS and a,β-meATP in hypertensive rats was reversed. In the rat model of chronic NO depletion, the purinergic and adrenergic components and eFS affect VD contractility. The VD contractile response may be mediated more by the purinergic system than the adrenergic system, and sildenafil may alter the ejaculatory response in men with Pe.

Generating timeline summaries with social media attention

Timeline generation is an important research task which can help users to have a quick understanding of the overall evolution of one given topic. Previous methods simply split the time span into fixed, equal time intervals without studying the role of the evolutionary patterns of the underlying topic in timeline generation. In addition, few of these methods take users＇ collective interests into considerations to generate timelines. We consider utilizing social media attention to address these two problems due to the facts： 1） social media is an important pool of real users＇ collective interests; 2） the information cascades generated in it might be good indicators for boundaries of topic phases. employing Twitter as a basis, we propose to incorporate topic phases and user＇s collective interests which are learnt from social media into a unified timeline generation algorithm. We construct both one informativeness-oriented and three interestingness-oriented evaluation sets over five topics. We demonstrate that it is very effective to generate both informative and interesting timelines. In addition, our idea naturally leads to a novel presen- tation of timelines, i.e., phase based timelines, which can potentially improve user experience.

Impacts of Land Process on the Onset and evolution of Asian Summer Monsoon in the NCeP Climate Forecast System

Impacts of land models and initial land conditions （ICs） on the Asian summer monsoon, especially its onset, were investigated using the NCeP Climate Forecast System （CFS）. Two land models, the Oregon State University （OSU） land model and the NCeP, OSU, Air Force, and Hydrologic Research Laboratory （Noah） land model, were used to get parallel experiments NCeP/Department of energy （DOe） Global Reanalysis 2 System （GLDAS）. The experiments also used land ICs from the （GR2） and the Global Land Data Assimilation Previous studies have demonstrated that, a systematic weak bias appears in the modeled monsoon, and this bias may be related to a cold bias over the Asian land mass. Results of the current study show that replacement of the OSU land model by the Noah land model improved the model＇s cold bias and produced improved monsoon precipitation and circulation patterns. The CFS predicted monsoon with greater proficiency in e1 Nifio years, compared to La Nifia years model in monsoon predictions for individual years. and the Noah model performed better than the OSU These improvements occurred not only in relation to monsoon onset in late spring but also to monsoon intensity in summer. Our analysis of the monsoon features over the India peninsula, the Indo-China peninsula, and the South Chinese Sea indicates different degrees of improvement. Furthermore, a change in the land models led to more remarkable improvement in monsoon prediction than did a change from the GR2 land ICs to the GLDAS land ICs.

Characterization of multi-scale ionospheric irregularities using ground-based and space-based GNSS observations

Ionospheric irregularities can adversely affect the performance of Global Navigation Satellite System (GNSS). How-ever, this opens the possibility of using GNSS as an effective ionospheric remote sensing tool. Despite ionospheric monitoring has been undertaken for decades, these irregularities in multiple spatial and temporal scales are still not fully understood. This paper reviews Virginia Tech’s recent studies on multi-scale ionospheric irregularities using ground-based and space-based GNSS observations. First, the relevant background of ionospheric irregularities and their impact on GNSS signals is reviewed. Next, three topics of ground-based observations of ionospheric irregulari-ties for which GNSS and other ground-based techniques are used simultaneously are reviewed. Both passive and active measurements in high-latitude regions are covered. Modelling and observations in mid-latitude regions are considered as well. emphasis is placed on the increased capability of assessing the multi-scale nature of ionospheric irregularities using other traditional techniques (e.g., radar, magnetometer, high frequency receivers) as well as GNSS observations (e.g., Total-electron-Content or TeC, scintillation). Besides ground-based observations, recent advances in GNSS space-based ionospheric measurements are briefly reviewed. Finally, a new space-based ionospheric observa-tion technique using GNSS-based spacecraft formation flying and a differential TeC method is demonstrated using the newly developed Virginia Tech Formation Flying Testbed (VTFFTB). Based on multi-constellation multi-band GNSS, the VTFFTB has been developed into a hardware-in-the-loop simulation testbed with external high-fidelity global ionospheric model(s) for 3-satellite formation flying, which can potentially be used for new multi-scale ionospheric measurement mission design.

Drought Tolerance and Recovery of the Sedge Carex planostachys (Cyperaceae) from Central Texas Woodlands

Carex planostachys Kunze (Cyperaceae, Cedar sedge) is an herbaceous species in a genus commonly inhabiting mesic or hydric habitats. Carex planostachys is found in arid and semi-arid Juniperus woodlands. Arid conditions impose survival challenges to plants in dry areas. Some plants have plasticity responses to soil water content and continued normal though reduced functions through droughts, but most herbaceous plants do not survive. Limited previous studies have suggested C. planostachys is tolerant of drought. Physiological responses of C. planostachys from Juniperus woodlands was examined is this study to determine how long plants could survive without water and if they are capable of recovery from very negative water potentials beyond what is considered the permanent wilting point for most herbaceous plants. Plants were placed in pots in partial shade in this experiment. Water loss from the soil with plants was an inverse 2nd order polynomial function with soil water decreasing from 32% to 8% by day 28 of the study. Leaf water potential was also an inverse 2nd order polynomial function but did not decline significantly until 14 days without watering. Leaf water potential was −10.0 MPa after 35 days without watering. Non-watered plants Anet, (photosynthetic rate) was significantly lower compared to the water treatment by day 21 as was stomatal conductance and transpiration. When non-watered plants were watered after 21, 28 or 35 days, full recovery of physiological responses occurred within 7 days. The length of time that C. planostachys was able to withstand drought was greater than the annual trends in lack of precipitation during springtime in this area. Carex planostachys can photosynthesize at water stress between −8 and −10 MPa. Carex planostachys drought and shade tolerance enables it to occupy an understory niche devoid of other herbaceous plants.
