Role of Two Dimension Strain and Strain Rate Echocardiography in Assessment of Rheumatic Mitral Valve Stenosis

Introduction: Current echocardiographic parameters have a limitation in assessing mitral valve (MV)apparatus in rheumatic mitral stenosis (MS) patient. In the current study, we use 2 dimensional (2D)longitudinal strain (S) and strain rate (Sr) imaging in evaluating the papillary muscle longitudinal strain (LS) as an objective and quantitative echocardiographic parameter with high reproducibility in the assessment of MV apparatus in patients with mild to moderate rheumatic MS with preserved ejection fraction (EF%).?Patients and Method: The study included 40 patients with established diagnosis of MS subdivided in to 31 patients with moderate MS (mean age: 32 ± 5) and 9 patients with mild MS (mean age: 31 ± 6). 20 healthy individuals (mean age 31 ± 6) as a control for cases. The mitral valve area (MVA) was estimated using planimetry and pressure half time (PHT) methods. 2D longitudinal systolic S and Sr imaging was carried out for all participants from the apical long axis (LAX), 4 chamber (4C), 2 chamber (2C) views. Global longitudinal systolic S and Sr were estimated by averaging the 3 apical views. Longitudinal myocardial strain of papillary muscle PMs was assessed by the use of the free strain method from apical 4 chamber view for the antrolateral papillary muscle (APM) and apical long axis view for postromedial papillary muscle (PPM).?Results: Patients with MS had significantly decreased longitudinal LV systolic S and Sr in comparison with control group (p Conclusion: Patients with MS and preserved EF% had decreased APM-LS & PPM-LS in comparison with control group, and had decreased longitudinal LV systolic S and Sr when compared with control group. 2D strain as well as Sr imaging might be a useful method for assessment of mitral valve apparatus in patients with MS & preserved EF%.

Association of the Monocyte to HDL Cholesterol Ratio with Thrombus Burden in Patients with Acute Coronary Syndrome

Background: Intracoronary thrombus burden is associated with some adverse events and poor prognosis in patients with Acute Coronary Syndrome. Identifying predictors of the intracoronary thrombus burden may contribute to the management of Acute Coronary Syndrome. Objective: To assess the correlation between the Monocyte to HDL Cholesterol ratio with thrombus burden in patients with Acute Coronary Syndrome. Patients and Methods: 138 patients with ACS who were admitted to CCU department in National Heart Institute Cairoand Menoufia University Hospitals, Menoufia, underwent primary percutaneous coronary intervention (PCI).Angiographic thrombus burden was classified based on thrombolysis in myocardial infarction (TIMI) thrombus grades. The patients were grouped into 2 categories of low thrombus burden (grades 0 - 3) (49.7%) and high thrombus burden (grades 4 and 5) (50.3%). Results: On analysis we found that the MHR was significantly higher in the high thrombus burden group compared with the low thrombus (0.052 ± 0.019 vs 0.014 ± 0.008);P 0.001. Conclusion: Among Acute Coronary Syndrome patients underwent Primary PCI, MHR was independent predictor of high thrombus burden in patients with Acute Coronary Syndrome.

Severity of Coronary Artery Disease by Coronary CT Angiography in Relation to Left Ventricular Diastolic Function

Objectives:?The aim of this study is to evaluate the relationship between severity of CAD by coronary mutlislice computed tomography scan and the degree of diastolic function Methods:?The study consisted of 80 patients who underwent both coronary CTA and TTE within 1 year with no identifiable intervening cardiovascular event in National?Heart?Institute and Menoufia?University?Hospitals. All patients were subjected to detailed medical history, physical examination, full electrocardiography study including tissue Doppler imaging on mitral annulus, MSCT coronary angiography and different laboratory investigation as CBC, creatinine, lipid profile. Results:?From 80 patients who have done both echocardiography examination and MSCT coronary angiography, the CT scan results were 38 patients with non-obstructive coronary artery disease and 42 patients with obstructive CAD, higher number of patients with diastolic dysfunction with higher values of E/E’, medial e’, lateral e’, LAVI (left atrial volume index) and TRV (tricuspid regurge velocity) were found in the group with obstructive coronary artery disease.?Conclusion:?Our study showed that diastolic dysfunction is more prevalent in patients with obstructive coronary artery disease with higher grade of diastolic dysfunction.

Sex-dependent resilience to extreme drought events: implications for climate change adaptation of a South American endangered tree species

Background Recent changes in climatic trends are resulting in an increased frequency and intensity of extreme events, with unknown effect on ecosystem dynamics in the near future. Extreme drought episodes are recognized as disturbance factors capable of modifying forest dynamics and tree growth. Within this context, dioecious tree species may be impacted by climatic extremes, affecting male/female proportions and, consequently, reproductive processes and species persistence. Therefore, there is an urgent need for species-specific assessments of growth tolerance to extreme dry spells in dioecious tree species, to establish effective conservation strategies for these particular natural resources. Araucaria araucana(araucaria), an endangered dioecious Patagonian tree species, has recently undergone decay and mortality episodes in response to increasing dry climatic conditions. While sex-dependent tolerance to extreme drought episodes has been assessed in the species' humid distribution range, there is still a lack of information on the gender-based resilience of trees growing in the drier environments of the species' *** We reconstructed, through dendrochronological methods, the sex-dependent response of 105 araucaria individuals(55 female and 50 male trees) to five regional extreme dry spells employing a set of different indices. Resistance, recovery period, and average growth reduction of standardized tree-ring growth were examined, analysing the effect of biotic(sex, pre-drought stem tree growth) and abiotic(local climatic conditions before, during, and after extreme climatic episodes) factors on tree *** Sex influences only the species resistance to climatic disturbance, with male individuals showing lower tolerance to extreme drought events. Influence of anodization conditions on deposition of Hydroxyapatite on α/β Ti alloys for Osseointegration: Atomic Force Microscopy Analysis

Integrating titanium-based implants with the surrounding bone tissue remains challenging. This study aims to explore the impact of different anodization voltages （20-80V） on the surface topography of two-phase （α/β） Ti alloys and to produce TiO2films with enhanced bone formation abilities. Scanning electron microscopy coupled with energy dispersive spectroscopy （SEM-EDS） and atomic force microscopy （AFM） were applied to investigate the morphological， chemical， and surface topography of the prepared alloys and to confirm the growth of hydroxyapatite （HA） on their surfaces. Results disclosed that the surface roughness of TiO2films formed on Ti-6Al-7Nb alloys was superior to that of Ti–6Al–4V alloys. Ti-6Al-7Nb alloy anodized at 80 V had the highest yields of HA after immersion in SBF with enhanced HA surface coverage. The developed HA layer had a mean thickness of 128.38 ± 18.13 μm， suggesting its potential use as an orthopedic implantable material due to its promising bone integration and， hence， remarkable stability inside the human body.

Electrochemical anodization of cast titanium alloys in oxalic acid for biomedical applications

Titanium and its alloys have numerous biomedical applications thanks to the composition and morphology of their oxide *** this study,the colorful oxide films were formed by anodizing cast Ti-6Al-4V and Ti-6Al-7Nb alloys in a 10%oxalic acid solution for 30 s at different voltages(20–80 V)of a direct current power *** force microscopy was used as an accurate tool to measure the surface roughness of thin films on the nanometer *** electron microscopy and X-ray diffraction were performed to analyze surface morphology and phase *** to the results,the produced titanium oxide layer showed high surface roughness,which increased with increasing anodizing *** impact of anodizing voltages on the color and roughness of anodized layers was *** corrosion resistance of the anodized samples was studied in simulated body fluid at pH 7.4 and a temperature of 37℃ utilizing electrochemical impedance spectroscopy and the potentiodynamic polarization *** anodized samples for both alloys at 40 V were at the optimal voltage,leading to a TiO_(2) layer formation with the best compromise between oxide thickness and corrosion ***,findings showed that TiO_(2) films produced on Ti-6Al-7Nb alloys had superior surface roughness properties compared to those of Ti-6Al-4V alloys,making them more appropriate for orthopedic *** the obtained data and the fruitful discussion,it was found that the utilized procedure is simple,low-cost,and ***,anodization in 10%oxalic acid proved a viable alternative for the surface finishing of titanium alloys for biomedical applications.

Predictive Value of Neutrophil-to-Lymphocyte Ratio in Outcomes of Patients with Acute Coronary Syndrome

Objectives: Previous studies have demonstrated the role of inflammation in acute coronary syndrome (ACS). The neutrophil-to-lymphocyte ratio (NLR) was found to be a useful inflammatory marker for predicting adverse outcomes. We hypothesized that an elevated neutrophil-to-lymphocyte ratio would be associated with increased mortality in patients with acute coronary syndrome. Methods: The study consisted of 40 patients with acute coronary syndrome who were admitted to Cardiology Department—Menoufia University Hospitals. The primary endpoint was all-cause in-hospital as well as 30-day mortality, and the patients were divided into three tertiles according to their admission NLR results. Results: All-cause 30-day mortality in the three groups based on NLR was 0.0%, 7.7% and 28.6%, in the low-, middle- and high-NLR groups, respectively (P χ2 test). In a logistic regression analysis, including baseline demographic, clinical, and biochemical covariates, the neutrophil-to-lymphocyte ratio was an independent predictor of mortality (OR = 2.44, 95% CI = 1.185 - 5.007, P Conclusion: An elevated neutrophil-to-lymphocyte ratio (NLR), a simple, relatively inexpensive and universally available inflammatory marker, seems to be a predictor of 30-day mortality in patients with acute coronary syndrome.

Risk factors for an atherothrombotic event in patients with diabetic macular edema treated with intravitreal injections of bevacizumab

AIM:To identify risk factors for an atherothrombotic event(ATE)among patients who were treated for diabetic macular edema(DME)with intravitreal bevacizumab ***:This retrospective study enrolled all consecutive patients with DME who were treated by intravitreal bevacizumab from 2009 through 2016 in a single *** were divided into one group treated by bevacizumab and subsequently had an ATE and a second group also treated by bevacizumab and did not have an ***:A total of 455 patients with DME were ***-two of the patients had an ATE.A multivariate model adjusted for age,gender,smoking,body mass index,hemoglobin A1c(HbA1c),duration of diabetes,creatinine,and blood pressure revealed an increased risk for ATE in the patients with diabetic duration of more than 13 y,a systolic blood pressure over 153.5 mm Hg at first treatment,or having been treated by more than 4 intravitreal bevacizumab ***,patients that had an ATE within 3 mo from the last intravitreal treatment underwent more bevacizumab injections(5.2±3.4 vs 3.07±1.86;P153.5 mm Hg,a history of diabetic mellitus for more than 13 y,and treatment with more than 4 intravitreal bevacizumab *** factors need to be borne in mind when bevacizumab is being considered in the management of patients with DME.

Early Results of Coronary Artery Bypass Graft (CABG) in Patients with Low Ejection Fraction

Background: Patients with ischemic heart disease and reduced ejection fraction have increased risk for postoperative complications and mortality. Our aim was to evaluate the effect of low EF (on the early outcomes after CABG and identify the predictors of mortality. Methods: From August 2018 to November 2019, 170 consecutive patients underwent CABG. Group 1 included 120 patients with EF ( EF (>40;57.90% ± 2.27%), 41 were men (82.0%), and the mean age was 54.30 ± 7.01 years and used as a control group. Results: Overall 30-day mortality was 10/120 patients (8.3%). Factors associated with higher mortality were females (70.0% vs. 17.3%, P Conclusion: CABG in EF < 40% is associated with more complications compared with those had EF higher than 40%;however, the clinical and echocardiographic parameters improved over time.

Microstructure characteristics and tensile property of ultrasonic treated-thixocast A356 alloy

Billets of A356 aluminum alloy were treated using ultrasonic vibrations during *** billets were reheated to the semisolid state at different routes to optimize the *** were,then,thixocast using a die casting *** results showed that the ultrasonic-treated billets exhibited finely distributed α（Al） globules after reheating and *** thixocast ultrasonic-treated billets showed higher ultimate tensile strength and elongation compared with the untreated ***,the thixocast parts showed a tendency to ductile fracture under tension when made from ultrasonic-treated billets,while those made of untreated billets showed brittle fracture with obvious straight *** results revealed the feasibility and competence of ultrasonic melt treatment as a potential route for preparing billets for thixocasting.
