Predictive factors for early aspiration in liver abscess

AIM: To determine the predictive factors for early aspiration in liver abscess. METHODS: A retrospective analysis of all patients with liver abscess from 1995 to 2004 was performed. Abscess was diagnosed as amebic in 661 (68%) patients, pyogenic in 200 (21%), indeterminate in 73 (8%) and mixed in 32 (3%). Multiple logistic regression analysis was performed to determine predictive factors for aspiration of liver abscess. RESULTS: A total of 966 patients, 738 (76%) male, mean age 43 ± 17 years, were evaluated: 540 patients responded to medical therapy while adjunctive percutaneous aspiration was performed in 426 patients. Predictive factors for aspiration of liver abscess were: age ≥ 55 years, size of abscess ≥ 5 cm, involvement of both lobes of the liver and duration of symptoms ≥ 7 d. Hospital stay in the aspiration group was relatively longer than in the non aspiration group. Twelve patients died in the aspiration group and this mortality was not statistically significant when compared to the non aspiration group. CONCLUSION: Patients with advanced age, abscess size > 5 cm, both lobes of the liver involvement and duration of symptoms > 7 d were likely to undergo aspiration of the liver abscess, regardless of etiology.

The high prevalence of gastroesophageal reflux disease （GERD） in Western societies has accelerated the need for new modalities of treatment. Currently, medical and surgical therapies are widely accepted among patients and physicians. New potent antisecretory drugs and the development of minimally invasive surgery for the management of GERD are at present the pivotal and largely accepted approaches to treatment. The minimally invasive treatment revolution, however, has stimulated several new endoscopic techniques for GERD. Up to now, the data is limited and further studies are necessary to compare the advantages and disadvantages of the various endoscopic techniques to medical and laparoscopic management of GERD. New journal articles and abstracts are continuously being published. The Food and Drug Administration has approved 3 modalities, thus gastroenterologists and surgeons are beginning to apply these techniques. Further trials and device refinements will assist clinicians. This article will present an overview of the various techniques that are currently on study. This review will report the efficacy and durability of various endoscopic therapies for gastroesophageal reflux disease （GERD）. The potential for widespread use of these techniques will also be discussed. Articles and abstracts published in English on this topic were retrieved from Pubmed. Due to limited number of studies and remarkable differences between various trials, strict criteria were not used for the pooled data presented, however, an effort was made to avoid bias by including only studies that used off-PPI scoring as baseline and intent to treat.

Biochar is a carbon-containing material prepared through thermal treatment of biomass in limited supply of oxygen,and used for an array of applications including waste management,climate change mitigation,soil fertility improvement,bio-energy production,and contaminant *** data related to biochar,its production,and the wide applicability were collected using Web of Science Core Collection Database(on 25/10/2022),while bibliometric network analysis was performed using VOSviewer software to analyse year-wise,author-wise,country-wise,and journal-wise publication trends,construct keyword co-occurrence maps,and identify research areas receiving greater ***,the applications of biochar were reviewed and mechanistic insights were *** of the findings include:>50%of documents(>13,000)getting published in the past 3 years,>90%of documents(>21,000)being research articles,~50%of publications(>10,000)being related to environmental sciences,pyrolysis being the most widely used(~40%articles)production technique(followed by carbonization,gasification,combustion,and torrefaction),China being the most active country in terms of publications(>11,000),and biochar being mostly used for removing contaminants(followed by soil improvement,waste management,energy production,and climate change mitigation).Various strengths,weaknesses,opportunities,and threats(SWOT analysis)of biochar production and wide-ranging applicability were ***,gaps were identified including the need for performing elaborate life cycle assessments,exploring machine learning and artificial intelligence for upgrading conversion technology and producing application-specific biochar,and investigating mechanistic aspects of soil-biochar interactions and nanoscale transformation of *** study covers a broad spectrum of biochar applicability to identify areas receiving lesser attention,which could guide the future researchers for augmenting biochar research.

Cellular energy status is often believed to be crucial in the patho physiology of severe acute diseases involving sepsis or trauma such as multiorgan failure, mu ltiorgan dysfunction syndrome, systemic inflammatory response system, etc. Wit h this in mind increased blood lactate, mainly considered only as a marker or t issue hypoxia is a classical tool used to assess the severity of the illness an d to predict the pejorative outcome. However, during the past years the actual r ole of hypoxia in the pathophysiology of such acute diseases with an increased b lood lactate has been questioned. The association between increased blood lactate concentration and poor prognosi s in ICU patients is well documented but it is clearly not a cause-effect relat ionship. It is similar to what occurs in the case of increased blood ketone I n severe insulin deficient state and it would be a major understanding to be lieve that lactate is a toxic compound in acute states. In acute illness, incre ased lactate concentration or lactate metabolism can be due to the combination of various origin in which the role of splanchnic bed is prominent. In the case of strong reduction in oxygen delivery, such as severe shock, it is quite clear that there is a relationship between cellular energy defect and increased lact ate production. But even in these cases a decrease in lactate metabolism (eithe r oxidative or recycling routes) may also be involved. In septic states, the ro le of blood lactate is a matter of controversy and several reports have e mphasiz ed the fact that hyperlactatemia in sepsis was not due to the oxygen d eficit. It has been shown that mild hyperlactatemia in stable septic patients was due to an impaired lactate clearance rather than overproduction. Besides the main conc lusion that any therapeutic attempts to increase oxygen delivery in such situa tion may not be adapted, another point can be raised concerning the meaning of s uch lactate increase. In order to assess whether salt tolerance could be Improved In spring wheat （Triticum aestivum L.）, the present study was performed by soaking the seeds of two cultlvars, namely MH-97 （salt sensitive） and Inqlab-91 （salt tolerant）, for 12 h In distilled water or 100 mol/m^3 CaCl2, KCI, or NaCI. Primed seeds from each treatment group and non-primed seeds were sown In a field In which NaCI salinity of 15 dS/m was developed. Priming of seeds with CaCl2, followed by priming with KCI and NaCI, was found to be effective In alleviating the adverse effects of salt stress on both wheat cultivars In terms of shoot fresh and dry weights and grain yield. Priming with CaCl2 alleviated the adverse effects of salt stress on hormonal balance In plants of both cultlvars. In MH-97 plants, CaCl2 pretreatment considerably reduced leaf absclslc acid （ABA） concentrations and Increased leaf free salicylic acid （SA） concentrations under both saline and non-saline conditions. In contrast, In the Inqlab-91 plant, CaCl2 Increased free Indoleacetic acid （IAA） and indolebutyrlc acid （IBA） content. However, priming of seeds with CaCl2 did not alter free polyamlne levels in either cultlvar, although spermldlne levels were considerably lower In plants raised from seeds treated with CaCl2 for both cultlvars under saline conditions. Priming with KCI Increased growth In Inqlab-91 plants, but not In MH-97 plants, under saline conditions. The salinity Induced reducUon In auxins （IAA and IBA） was alleviated by NaCI priming In both cultlvars under saline conditions. However, NaCI Increased leaf free ABA content and lowered leaf SA and putresclne levels In Inqlab-91 plants under saline conditions. In conclusion, although all three priming agents （I.e. CaCl2, KCI, and NaCI） were effective In alleviating the adverse effects of salt stress on wheat plants, their effects on altering the levels of different plant hormones were different In the two cuItlvars.

Hindukush is an active subduction zone where at least one earthquake occurs on daily *** seismic hazard studies,it is important to develop a local magnitude scale using the data of local seismic *** have computed local magnitude scale for Hindukush earthquakes using data from local network belonging to Center for Earthquake Studies(CES)for a period of three years,i.e.2015–2017.A total of 26,365 seismic records pertaining to 2,683 earthquakes with magnitude 2.0 and greater,was used with hypocentral distance less than 600 *** scale developed by using this data comes to be M_(L)=logA+0.929logr+*** magnitude determined through formulated relation was compared with that of standard relation for Southern California and relation developed by the same authors for local network for Northern *** was observed that Hindukush region has high attenuation as compared to that of Southern California and Northern Punjab which implies that Hindukush is tectonically more disturbed as compared to the said regions,hence,seismically more active as *** have calculated station correction factors for our *** correction factors do not show any pattern which probably owes to the geological and tectonic complexity of this *** deviation and variance of magnitude residuals for CES network determined using Hutton and Boore scale and scale developed in this study were compared,it showed that a variance reduction of 44.1%was *** of magnitude residuals for different distance ranges was almost zero which showed that our magnitude scale was stable for all distances up to 600 *** developed magnitude scale will help in homogenization of earthquake *** has been observed that b-value of CES catalog decreases when magnitude is calculated by using newly developed magnitude scale.

Smart home devices are vulnerable to a variety of *** matter gets more complicated when a number of devices collaborate to launch a colluding attack(e.g.,Distributed-Denial-of-Service(DDoS))in a network(e.g.,Smart home).To handle these attacks,most studies have hitherto proposed authentication protocols that cannot necessarily be implemented in devices,especially during Device-to-Device(D2D)*** into the potential of Ethereum blockchain and smart contracts,this work proposes a lightweight authentication mechanism that enables safe D2D interactions in a smart *** Ethereum blockchain enables the implementation of a decentralized prototype as well as a peer-to-peer distributed ledger *** work also uses a single server queuing system model and the authentication mechanism to curtail DDoS attacks by controlling the number of service requests in the *** simulation was conducted twenty times,each with varying number of devices chosen at random(ranging from 1 to 30).Each requester device sends an arbitrary request with a unique resource requirement at a *** is done to measure the system's consistency across a variety of device *** experimental results show that the proposed protocol not only prevents colluding attacks,but also outperforms the benchmark protocols in terms of computational cost,message processing,and response times.

A numerical radius inequality due to Shebrawi and Albadawi says that: If Ai, Bi, Xi are bounded operators in Hilbert space, i = 1,2,..., n , and f,g be nonnegative continuous functions on [0, ∞) satisfying the relation f(t)g(t) = t (t∈[0, ∞)), then for all r≥1. We give sharper numerical radius inequality which states that: If Ai, Bi, Xi are bounded operators in Hilbert space, i = 1,2,..., n , and f,g be nonnegative continuous functions on [0, ∞) satisfying the relation f(t)g(t) = t (t∈[0, ∞)), then ?where . Moreover, we give many numerical radius inequalities which are sharper than related inequalities proved recently, and several applications are given.
