Critical analysis of feeding jejunostomy following resection of upper gastrointestinal malignancies

AIm To assess nutritional recovery,particularly regarding feeding jejunostomy tube(FJT)utilization,following upper gastrointestinal resection for malignancy. mETHODS A retrospective review was performed of a prospectively-maintained database of adult patients who underwent esophagectomy or gastrectomy(subtotal or total)for cancer with curative intent,from January 2001 to June 2014. Patient demographics,the approach to esophagectomy,the extent of gastrectomy,FJT placement and utilization at discharge,administration of parenteral nutrition(PN),and complications were evaluated. All patients were followed for at least ninety days or until *** The 287 patients underwent upper GI resection,comprised of 182 esophagectomy(n=107 transhiatal,58.7%; n=56 Ivor-Lewis,30.7%)and 105 gastrectomy [n=63 subtotal(SG),60.0%; n=42 total(TG),40.0%]. 181 of 182 esophagectomy patients underwent FJT,compared with 47 of 105 gastrectomy patients(99.5% vs 44.8%,P m most had undergone TG(n=39,92.9% vs n=8 SG,12.9%,P median length of stay was similar between esophagectomy and gastrectomy groups(14.7 d vs 17.1 d,P=0.076). Upon discharge,87 esophagectomy patients(48.1%)were taking enteral feeds,with 53(29.3%)fully and 34(18.8%)partially dependent. meanwhile,20 of 39 TG patients(51.3%)were either fully(n=3,7.7%)or partially(n=17,43.6%)dependent on tube feeds,compared with 5 of 8 SG patients(10.6%),all of whom were partially dependent. Gastrectomy patients were significantly less likely to be fully dependent on tube feeds at discharge compared to esophagectomy patients(6.4% vs 29.3%,P=0.0006). PN was administered despite FJT placement more often following gastrectomy than esophagectomy(n=11,23.4% vs n=7,3.9%,P=0.0001). FJT-specific complications requiring reoperation within 30 d of resection occurred more commonly in the gastrectomy group(n=6),all after TG,compared to 1 esophagectomy patient(12.8% vs 0.6%,P=0.0003). minimizing Weight by Optimizing Different Truss Parts Using Finite Element Analysis

This paper presents a study of minimizing weight by optimizing different truss parts using finite element analysis and comparing warren trusses with other trusses. The aim of the optimization is to find a light design. Existing structural steel trusses were initially optimized for minimum weight and constrained with allowable stresses and deflections. Applicable Eurocode 3 design conditions are presented, which provide the constraints for the problem. Steel truss is a preferred solution in large-span roof structures due to its good attributes, such as being lightweight and durable. Existing structural steel trusses were initially optimized for minimum weight and constrained with allowable stresses and deflections. Constant spans of the trusses have been considered, and each truss has been subjected to the same types of load cases. The top chord member load has been kept constant in each truss at 2 kN/m. Two sets of load conditions are taken as the self-weight of the truss and the snow load, but the structure is calculated by the load combination. The structural steel trusses were optimized using the design optimization tool as a first-order optimization method in RFEm, and it was extended to compare the most suitable truss geometry for the minimum weight. Finally, it is concluded that the warren truss has a higher stiffness-to-weight ratio than other trusses after optimization. The goal of this study was to analyze all trusses and ensure that the structural stress is less than the allowable stress and that the deflection is less than the allowable deflection. The span and height are constant in all cases because they have no impact on the weight increase;only the position of the rods and cross-section size affect the building’s ability to withstand loads and weight increases. 前颈部多余的组织非常适合做头颈部再造术中筋膜皮瓣的供体，这是冈为前颈部皮肤与头颈部皮肤质地柑似，且有很多穿支。然而，为避免血管危象，既往的尸体研究已限制了该部分皮肤作为较长或较大皮瓣的供体的使用，本研究的目的是确定此类皮瓣的血管基础，并修改之前的皮瓣设计，提供崩途广泛、可靠的甲状腺上动脉穿支皮瓣。本研究同顾了45例颈部CT血管造影片，评估前颈皮肤的血供情况。以上述发现为基础，8例患者接受头颈部再造术，术中使用的皮瓣以该区域主要穿支为基础。所有患者体内均找到了直径大于0．5mm的穿支，位置在胸锁乳突肌前缘中线处2cm半径范同内。作为筋膜穿支，该穿支存颈筋膜深层出现，且在颈部同侧穿过颈阔肌，邻近中线。90例患者中，89例发现甲状腺上动脉穿支，1例发现甲状腺下动脉穿支。8例患者均接受术前成像，以甲状腺上动脉穿支为基础的皮瓣设计均成功完成手术。本研究证明，甲状腺上动脉穿支皮瓣是可靠的血管解剖部位，适用于头颈部缺损广泛的再造术。（循证医学证据等级：Ⅳ级，疗法）

Distinct and Additive Effects of Alcohol and Thiamine Deficiency in the Developing Brain: Relevance to Fetal Alcohol Spectrum Disorder

Background: Neurodevelopmental abnormalities in fetal alcohol spectrum disorder (FASD) are linked to brain insulin resistance and oxidative stress. However, the role of thiamine deficiency as a distinct or additive factor in the pathogenesis of the neurodevelopmental and metabolic derangements in FASD has not been determined. methods: Control and ethanol-exposed human PNET2 cerebellar neuronal cells and rat cerebellar slice cultures were treated with vehicle or pyrithiamine (Pyr) to assess independent and additive effects of thiamine deficiency on ethanol-mediated neurotoxicity, mitochondrial dysfunction, insulin resistance, inhibition of neuronal and glial genes, and oxidative stress. Results: Pyr treatments (0 - 200 µm) caused dose-dependent cell loss (Crystal Violet assay) and reduced mitochondrial function (mTT assay) in PNET2 neuronal cultures. Ethanol alone (100 mm) significantly reduced PNET2 neuronal viability, mTT activity, and ATP production. Over the broad dose range of Pyr treatment, ethanol significantly reduced ATP content and cell number and increased mitochondrial mass (mitoTracker Green). Ex vivo cerebellar slice culture studies revealed ethanol-induced developmental architectural disruption that was substantially worsened by Pyr. The adverse effects of ethanol were linked to increased lipid peroxidation and inhibition of asparatyl-asparaginyl-β-hydroxylase (ASPH) expression. The independent and additive effects of Pyr were associated with increased cytotoxicity, lipid peroxidation, Caspase 3 activation, and Tau accumulation. Conclusions: During development, alcohol exposure and thiamine deficiency exert distinct but overlapping molecular pathologies that ultimately impair the structure and function of cerebellar neurons. While both insults drive cell loss and mitochondrial dysfunction with increased lipid peroxidation, ethanol’s additional inhibitory effects on ASPH reflect impairments in insulin and IGF signaling. Background: The pathologic substrates of frontotemporal dementia (FTD) are difficult to predict in vivo. Objective: To determine whether different pathologic substrates of FTD have distinct patterns of regional atrophy on magnetic resonance imaging (mRI). Design: Retrospective case study. Setting: The Institute of Neurology, University College London, and the Institute of Psychiatry, King's College London. Patients: Twenty-one cases of FTD selected on pathologic grounds (9 with ubiquitin-positive [tau-and α-synuclein-negative] inclusions [FTD-U], 7 with Pick disease [PiD], and 5 with familial FTD with tau exon 10+16 mutations [tau exon 10+16]) and 20 healthy controls were studied. main Outcome measu res: Patterns of gray matter atrophy in each group as assessed by voxel-based morphometry (VBm) and a blinded visual assessment of each mRI study. Results: All pathologic substrates were associated with atrophy that involved the inferior and medial temporal and inferior frontal lobes. Additionally, specific VBm signatures were identified for each subgroup: FTD-U was associated with asymmetric (left > right) temporal lobe atrophy, PiD was associated with severe dorsolateral bifrontal atrophy, and tau exon 10+16 was associated with asymmetric (right > left) medial temporal lobe atrophy. The VBm findings were supported by blinded visual assessment. Conclusion: These findings suggest that mRI patterns of regional gray matter atrophy constitute signatures of tissue pathology in FTD.
