胰腺癌是生存率最低和预后最差的消化道肿瘤之一。大多数患者确诊时已失去手术根治切除的机会,并在应用一线化疗药物吉西他滨(GEM)治疗过程中出现耐药性,进一步降低治疗效果。近年来,铁死亡作为一种新型铁依赖型细胞程序性死亡形式,其调控机制也被广泛研究并应用于胰腺癌的治疗中。有实验研究表明诱导胰腺癌细胞铁死亡可有效降低其对化疗药物的耐药性。本文综述了铁死亡在胰腺癌吉西他滨耐药中的研究进展,旨在为研究铁死亡与胰腺癌治疗提供新的思路和方向。

Porta hepatis tuberculous lymphadenopathy(TL)is rare,and the clinical and imaging manifestations often mimic *** offer a bet-ter understanding of porta hepatis TL and thus improve its diagnosis and treatment,here,we retrospectively reviewed and analyzed 10 porta hepatis TL cases between May 2017 and November *** this study,porta hepatis TL occurred predominantly in women(70%),with a mean age of 43.1 years(range,16-70 years).Among the patients,60%were initially suspected of malignancy(metastatic lymphadenopathy and lymphoma).The clinical manifestations were nonspecific,such as fever and weight loss,without overt abdominal *** patients showed high concentrations of serum C-reactive protein,erythrocyte sedimentation rate,alkaline phosphatase,γ-glutamyl transpeptidase and carbohydrate antigen *** rim-like enhancement was a characteristic finding on contrast-enhanced CT for all the *** examination confirmed the diagnosis in 90%of patients with caseous granulo-matous inflammation and positive results of Xpert MTB/RIF,quantitative real-time PCR and/or metagenomic next-generation sequenc-ing *** patients underwent antituberculosis therapy with a median treatment duration of 13.5 months(range,1-43 months).They all showed satisfactory therapeutic responses with improved symptoms and resolution on imaging after *** examination by biopsy remains the primary diagnostic method.A combination of the characteristic epidemiologic,clinical,imaging fea-tures and lesion biopsy for histopathology are essential for the diagnosis and treatment of TL.

乡村振兴离不开乡镇企业的发展。当前,我国乡镇企业发展态势虽整体向好,却依然受制于融资难和融资贵等因素,与此同时,区块链技术在我国金融领域,尤其是普惠金融领域,逐步得到重视与应用。本文以区块链技术为切入点,分析其对乡镇企业融资的影响。通过对区块链技术影响乡镇企业融资的耦合机理的研究发现,区块链技术可通过共享机制“校准”信息不对称,共识机制“降频”实物抵押度,合约机制“稀释”歧视性因子以及溯源机制“侦测”交易链风险有效缓解乡镇企业融资困境,提出基于区块链技术下乡镇企业融资“一主两翼”策略的应用模式,并进一步就区块链技术如何有效助推乡镇企业融资有针对性地提出了建议。

The vaccine-induced innate immune response is essential for the generation of an antibody *** date,how Ad5-vectored vaccines are influenced by preexisting anti-Ad5 antibodies during activation of the early immune response remains ***,we investigated the specific alterations in GP1,2-specific IgG-related elements of the early immune response at the genetic,molecular,and cellular levels on days 0,1,3,and 7 after Ad5-EBOV *** a causal multiomics analysis,distinct early immune responses associated with GP1,2-specific IgG were observed in Ad5-EBOV recipients with a low level of preexisting anti-Ad5 *** study revealed the correlates of the Ad5-EBOV-induced IgG response and provided mechanistic evidence for overcoming preexisting Ad5 immunity during the administration of Ad5-vectored vaccines.

Importance: It remained unclear that the efficacy comparison between low-dose immune tolerance induction (LD-ITI) incorporating immunosuppressants (IS) when severe hemophilia A (SHA) patients had inhibitor-titer ≥200 Bethesda Units (BU)/mL (LD-ITI-IS200 regimen) and LD-ITI combining with IS when SHA patients had inhibitor-titer ≥40 BU/mL (LD-ITI-IS40 regimen).Objective: To compare the efficacy of the LD-ITI-IS200 regimen with that of the LD-ITI-IS40 regimen for SHA patients with high-titer ***: A prospective cohort study on patients receiving LD-ITI-IS200 compared to those receiving LD-ITI-IS40 from January 2021 to December 2023. Both received LD-ITI [FVIII 50 IU/kg every other day]. IS (rituximab+ prednisone) was added when peak inhibitor tier ≥200 BU/mL in the LD-ITI-IS200 regimen and ≥40 BU/mL in the LD-ITI-IS40 regimen. Success is defined as a negative inhibitor plus FVIII recovery ≥66% of the ***: We enrolled 30 patients on LD-ITI-IS200 and 64 patients on LD-ITI-IS40, with similar baseline clinical characteristics. A lower IS-use rate was discovered in the LD-ITI-IS200 regimen compared to the LD-ITI-IS40 regimen (30.0% vs. 62.5%). The two regimens (LD-ITI-IS200 vs. LD-ITI-IS40) had similar success rate (70.0% vs. 79.7%), median time to success (9.4 vs. 10.6 months), and annualized bleeding rate during ITI (3.7 vs. 2.8). The cost to success was lower for LD-ITI-IS200 than for LD-ITI-IS40 (2107 vs. 3256 US Dollar/kg). Among patients with peak inhibitor-titer 40-199 BU/mL, 10 non-IS-using (on LD-ITI-IS200 regimen) and 28 IS-using (on LD-ITI-IS40 regimen) had similar success rates (70.0% vs. 78.6%) and time to success (9.0 vs. 8.8 months).Interpretation: In LD-ITI, IS are not necessary for inhibitor titer <200 BU/mL.

The superconducting ground state of kagome metals AV_(3)Sb_(5)(where A stands for K,Rb,or Cs)emerges from an exotic charge density wave(CDW)state that potentially breaks both rotational and time reversal ***,the specifics of the Cooper pairing mechanism,and the nature of the interplay between these two states remain elusive,largely due to the lack of momentum-space(k-space)superconducting energy gap *** implementing Bogoliubov quasiparticle interference(B QPI)imaging,we obtain k-space information on the multiband superconducting gap structureΔ_(SC)^(i)(k)in pristine CsV_(3)Sb_(5).We show that the estimated energy gap on the vanadium d_(xy/x^(2)-y^(2))orbital is anisotropic but nodeless,with a minimal value located near the M ***,a comparison ofΔ_(SC)^(i)(k)with the CDW gapΔ_(CDW)^(i)(k)obtained by angle-re solved photoemission spectro scopy(ARPES)reveals direct k-space competition between the se two order parameters,i.e.,the opening of a large(small)CDW gap at a given momentum corresponds to a small(large)superconducting *** the long-range CDW order is suppressed by replacing vanadium with titanium,we find a nearly isotropic energy gap on both the V and Sb *** information will be critical for identifying the microscopic pairing mechanism and its interplay with intertwined electro nic orders in this kagome superconductor family.

目的分析支气管镜检标本微小核糖核酸(microRNA,miRNA)-21、miRNA-431对肺结核病情发展及预后评估价值。方法纳入陕西省结核病防治院2022年5月—2023年7月临床收治43例确诊肺结核患者为研究组,纳入同期43例肺炎患者为对照组,研究组中根据肺部病灶肺野数量分级轻度14例、中度19例、重度10例,根据预后情况分为预后不良和预后良好,比较研究组与对照组、研究组内不同病情严重程度以及研究组内不同预后患者miRNA-21、miRNA-431水平,采用logistic多因素分析影响肺结核疾病发展及预后的因素,建立影响肺结核病情发展及预后的预测模型,通过受试者工作特征曲线(receiver operating characteristic curve,ROC)分析模型预测价值。结果研究组miRNA-21、miRNA-431水平均显著高于对照组,组间差异统计学意义成立(P<0.05);研究组内,病情严重程度与患者miRNA-21、miRNA-431水平成正比,且重度肺结核患者miRNA-21、miRNA-431水平显著高于轻度、中度肺结核患者,组间差异有统计学意义(P<0.05);研究组内,预后不良肺结核患者miRNA-21、miRNA-431水平均显著高于预后良好肺结核患者,组间差异有统计学意义(P<0.05);以miRNA-21、miRNA-431表达为自变量,连续变量;将肺结核病情发展及预后作为因变量进行logistic回归分析。结果显示,miRNA-21、miRNA-431高表达均为肺结核病情发展及预后的影响因素(P<0.05);绘制ROC曲线结果显示,预测模型最佳临界值为0.0895,miRNA-21、miRNA-431和两项联合的ROC曲线下面积(area under curve,AUC)为0.614(0.651~0.732)、0.671(0.686~0.811)、0.891(0.787~0.993)。结论肺结核患者miRNA-21、miRNA-431水平高表达,与病情发展密切相关,在一定程度上可预测肺结核患者预后,联合诊断综合应用价值高,建议临床推广使用。
