Endoscopic ultrasound-guided vs endoscopic retrograde cholangiopancreatography biliary drainage for obstructed distal malignant biliary strictures: A systematic review and meta-analysis

BACKGROUND For palliation of malignant biliary obstruction(mBO), the gold-standard method of biliary drainage is endoscopic retrograde cholangiopancreatography(ERCP)with the placement of metallic stents. Endoscopic ultrasound(EUS)-guided drainage is an alternative that is typically reserved for cases of ERCP ***, however, there have been robust randomized clinical trials(RCTs)comparing EUS-guided drainage and ERCP as primary approaches to *** To compare EUS guidance and ERCP in terms of their effectiveness and safety in palliative biliary drainage for *** This was a systematic review and meta-analysis, in which we searched the mEDLINE, Excerpta medica, and Cochrane Central Register of Controlled Trials databases. Only RCTs comparing EUS and ERCP for primary drainage of mBO were eligible. All of the studies selected provided data regarding the rates of technical and clinical success, as well as the duration of the procedure, adverse events, and stent patency. We assessed the risk of biases using the Jadad score and the quality of evidence using the Grading of Recommendations Assessment,Development and Evaluation *** The database searches yielded 5920 records, from which we selected 3 RCTs involving a total of 222 patients(112 submitted to EUS and 110 submitted to ERCP). In the EUS and ERCP groups, the rate of technical success was 91.96%n and 91.81%, respectively, with a risk difference(RD) of 0.00%(95%CI:-0.07, 0.07;P = 0.97; I^2 = 0%). The clinical success was 84.81% and 85.53% in the EUS and ERCP groups, respectively, with an RD of-0.01%(95%CI:-0.12, 0.10; P = 0.90; I^2 =0%). The mean difference(mD) for the duration of the procedure was-0.12%(95%CI:-8.20, 7.97; P = 0.98; I^2 = 84%). Psidium guajava (guava) leaf is a phytotherapic used in folk medicine to treat gastrointestinal and respiratory dis-turbances and is used as anti-inflammatory medicine. In nuclear medicine, blood constituents (BC) are labelled with techne-tium-99m (99mTc) and used to image procedures. However, data have demonstrated that synthetic or natural drugs could modify the labelling of BC with 99mTc. The aim of this work was to evaluate the effects of aqueous extract of guava leaves on the labelling of BC with 99mTc. Blood samples of Wistar rats were incubated with different concentrations of guava extract and labelled with 99mTc after the percentage of incorporated radioactivity (%ATI) in BC was determined. The results suggest that aqueous guava extract could present antioxidant action and/or alters the membrane structures involved in ion transport into cells, thus decreasing the radiolabelling of BC with 99mTc. Failure Analysis of Austenitic Stainless Steel Implant Screws and Prospection of Chemical Composition Using Artificial Intelligence

In this work, austenitic stainless steel screws employed in a locking compression plate for veterinarian use were investigated. These types of implants are widely utilized in bone fractures healing. Two surgical screws were extracted due to the observation of slight superficial red rust colorizing on one of the screw implants, visual evidence of probable screw rusting. From the same implant, another screw was extracted simultaneously without visual evidence of rusting. In order to characterize and analyze the different behavior of both screws, the chemical composition was characterized by atomic absorption and energy dispersive X-ray spectroscopy (EDS) coupled to a scanning electron microscope (SEm). Also, the screws were studied by metallography, optical microscopy (Om), Vickers microhardness tests, and SEm analysis. On the other hand, a prospection for alloy chemical composition limits of these types of implants was performed based on the Schaeffler-Delong diagram and the ASTm F-138 standard. To analyze the effect of the chemical composition, heat treatment, microstructure, pitting resistance equivalent number (PRE) and stacking fault energy (SFE), a genetic algorithm (GA) and an artificial neural network (ANN) were used. In accordance with the elemental analysis, the surgical screws do not fulfill the ranges of the chemical composition established by the ASTm F-138 standard. Furthermore, there were found differences between the microstructures of the screws. In regard to the prospection, the results of GA and ANN support the proposed chemical composition region on the Schaeffler-Delong diagram. The corrosion failure was associated with severe plastic deformation and the presence of precipitates. The proposal can minimize the cause of failures in these types of austenitic stainless steel implants.

Role of rumination in the relationship between metacognition and shyness

AIm To explore the association between metacognitive beliefs, rumination and shyness in a non-clinical sample of adults. mETHODS One hundred and three healthy subjects from the general population were enrolled in the study. Shyness was evaluated using the Revised Cheek and Buss Shyness Scale, rumination was assessed using the Ruminative Response Scale, metacognition was evaluated using the metaCognitions Questionnaire 30, and anxiety levels were measured using the State Trait Anxiety Inventory form Y. Correlation analyses, mediation models and 95% bias-corrected and accelerated(BCaCI) bootstrapped analyses were performed. mediation analyses were adjusted for sex and anxiety. RESULTS Shyness, rumination and metacognition were significantly correlated(P metacognition and shyness was fully mediated by rumination(Indirect effect: 0.20; 95% BCaCI: 0.08-0.33).CONCLUSION These findings suggest an association between metacognition and shyness. Rumination mediated the relationship between metacognition and shyness, suggesting that rumination could be a cognitive strategy for shy people. Future research should explore the relationship between these constructs in more depth.

Effect of initial microstructure on austenite formation kinetics in high-strength experimental microalloyed steels

Austenite formation kinetics in two high-strength experimental microalloyed steels with different initial microstructures comprising bainite-martensite and ferrite-martensite/austenite microconstituents was studied during continuous heating by dilatometric analysis. Austenite formation occurred in two steps：（1） carbide dissolution and precipitation and （2） transformation of residual ferrite to austenite. Dilatometric analysis was used to determine the critical temperatures of austenite formation and continuous heating transformation diagrams for heating rates ranging from 0.03°C.s^-1 to 0.67°C.s^-1. The austenite volume fraction was fitted using the Johnson-mehl-Avrami-Kolmogorov equation to determine the kinetic parameters k and n as functions of the heating rate. Both n and k parameters increased with increasing heat- ing rate, which suggests an increase in the nucleation and growth rates of austenite. The activation energy of austenite formation was determined by the Kissinger method. Two activation energies were associated with each of the two austenite formation steps. In the first step, the austenite growth rate was controlled by carbon diffusion from carbide dissolution and precipitation; in the second step, it was controlled by the dissolution of residual ferrite to austenite.
