Recently,nano-systems based on molecular communications via diffusion(mCvD)have been implemented in a variety of nanomedical applications,most notably in targeted drug delivery system(TDDS)scenarios.Furthermore,because the mCvD is unreliable and there exists molecular noise and inter symbol interference(ISI),cooperative nano-relays can acquire the reliability for drug delivery to targeted diseased cells,especially if the separation distance between the nano transmitter and nano receiver is increased.In this work,we propose an approach for optimizing the performance of the nano system using cooperative molecular communications with a nano relay scheme,while accounting for blood flow effects in terms of drift velocity.The fractions of the molecular drug that should be allocated to the nano transmitter and nano relay positioning are computed using a collaborative optimization problem solved by themodified Central Force Optimization(mCFO)algorithm.Unlike the previous work,the probability of bit error is expressed in a closed-form expression.It is used as an objective function to determine the optimal velocity of the drug molecules and the detection threshold at the nano receiver.The simulation results show that the probability of bit error can be dramatically reduced by optimizing the drift velocity,detection threshold,location of the nano-relay in the proposed nano system,and molecular drug budget.

Delorme's operation plus sphincteroplasty for complete rectal prolapse associated with traumatic fecal incontinence

Rectal prolapse associated with traumatic fecal incontinence is a rare clinical combination. This study was designed to assess Delorme＇s operation with sphincteroplasty as a surgical management of this combination in terms of recurrence and improvement of fecal incontinence. In this prospective study, we enrolled patients suffering from short, full-thickness rectal prolapse associated with traumatic fecal incontinence who had been admitted to Alexandria main University Hospital during the period of may 2010-January 2013. Preoperative data including cause of trauma, duration of symptoms, results of anal manometry, and degree of fecal incontinence using Wexner score were collected from all patients. Delorme＇s procedure with overlap sphincteroplasty was done in all patients. Recurrence of prolapse and improvement of fecal incontinence were assessed after 1, 3, 6 and 12 months. The study included 13 patients aged （32±8.7） years, 9 females and 4 males. Cause of sphincteric injury included previous anal surgery in 7 patients and normal labor in 6 patients. Duration between sphinctefic injury and operation was （8.08±2.47） months. Preoperative Wexner＇s mean score was 16.07±3.4. Early postoperative complications included superficial wound infection （69.2%）, minor wound dehiscence （61.5%）, and postoperative bleeding （7.6%）. Recurrence was detected in 1 patient at 6 month follow-up. Wexner＇s score showed significant improvement for all patients after 6 months （4.00±2.04）. In conclusion, combination of Delorme＇s procedure and sphincteroplasty for treatment of patients with short complete rectal prolapse associated with traumatic fecal incontinence is a safe, effective surgical management with satisfactory results regarding anatomical and functional outcomes.

moringa oleifera is well known as a highly nutritious plant and a water purifier in fish culture.Tilapia fish has many impressive qualities making them very suitable for aquaculture.Therefore,this study aimed to evaluate the effect of supplementation of moringa oleifera aqueous extract(mOAE)at varying concentrations(0,100,200,and 400 mg/kg diet for 90 days)on growth performance,hematological,and some biochemical parameters of Nile tilapia.Additionally,histological changes and immunohistochemical expression of caspase 3 in gills and livers were evaluated.Significant enhancements(Pells,hemoglobin,packed cell volume,white blood cells,and total serum protein were observed in fish fed 200 mg mOAE/kg diet compared with other groups.However,a significant reduction(Pmg mOAE/kg diet revealed disturbance in the corresponding parameters.Severe histological degenerative changes were detected in the gills and liver of fish fed 400 mg mOAE/kg diet.In comparison,fish fed 200 mg mOAE/kg diet showed the best histo-architectures of these organs compared with other groups.Concomitant immune expressions of caspase 3 were determined where gills and liver of fish fed 400 mg mOAE/kg diet revealed extensive number of positive caspase 3 cells indicating severe apoptotic features.Accordingly,a dietary mOAE at 200 mg/kg diet is recommended to improve the growth performance,hematological parameters,liver and kidney functions in Nile tilapia.meanwhile,caution must be considered when using mOAE at a concentration over 200 mg/kg diet.

The efficient transmission of images,which plays a large role inwireless communication systems,poses a significant challenge in the growth of multimedia technology.High-quality images require well-tuned communication standards.The Single Carrier Frequency Division multiple Access(SC-FDmA)is adopted for broadband wireless communications,because of its low sensitivity to carrier frequency offsets and low Peak-to-Average Power Ratio(PAPR).Data transmission through open-channel networks requires much concentration on security,reliability,and integrity.The data need a space away fromunauthorized access,modification,or deletion.These requirements are to be fulfilled by digital image watermarking and encryption.This paper ismainly concerned with secure image communication over the wireless SC-FDmA systemas an adopted communication standard.It introduces a robust image communication framework over SC-FDmA that comprises digital image watermarking and encryption to improve image security,while maintaining a high-quality reconstruction of images at the receiver side.The proposed framework allows image watermarking based on the Discrete Cosine Transform(DCT)merged with the Singular Value Decomposition(SVD)in the so-called DCT-SVD watermarking.In addition,image encryption is implemented based on chaos and DNA encoding.The encrypted watermarked images are then transmitted through the wireless SC-FDmA system.The linearminimummean Square Error(mmSE)equalizer is investigated in this paper to mitigate the effect of channel fading and noise on the transmitted images.Two subcarrier mapping schemes,namely localized and interleaved schemes,are compared in this paper.The study depends on different channelmodels,namely PedestrianAandVehicularA,with a modulation technique namedQuadratureAmplitude modulation(QAm).Extensive simulation experiments are conducted and introduced in this paper for efficient transmission of encrypted watermarked images.In addition,different variants of SC-FDmA based on the Discrete Wavelet Transform(DWT),Discre

Abscess of the caudate lobe of the liver, a rare disease with a challenging management: a case report

We reported a rare case of abscess of the caudate lobe of the liver in a 60-year old man. We first tried computed tomography （CT） guided percutaneous drainage of the abscess but failed to eradicate the infection. Deterioration of the general condition of the patient necessitated open surgical drainage, which resulted in cure of the abscess. The peculiar anatomical location of caudate lobe abscess introduces a great challenge for the surgeon in plan- ning the appropriate management and paucity of patients with caudate lobe abscess has led to lack of guidelines for management. The non-operative interventional radiology approach has become the therapeutic choice for pyo- genic liver abscess, but is it applicable also for caudate lobe abscess？

Developing a biomaterial for wound healing applications is still a challenge.Herein,dysprosium oxide（Dy2O3）,aluminum oxide（Al2O3）,and graphene oxide（GO） were embedded in cast films based on polycaprolactone（PCL） to be examined for wound dressing usage.Different techniques were used to characterize the fabricated films including X-ray diffraction（XRD）,Fourier transform infrared spectroscopy（FTIR）,Raman,and scanning electron microscopy（SEm）,besides their biological activity.The thermogravimetric analysis（TGA） exhibits high thermal stability as the scaffold weight decreases slightly to 98.6% after raising the temperature from room temperature to 280℃.The cell viability was investigated and it is shown that the viable cells grow up to approximately 93% at 75 μg/mL.meanwhile,the cell attachment shows excellent behavior as the cells attach on most of the surface of the modified PCL which shows high biocompatibility.

Late onset ilio-psoas abscess due to stump appendicitis： a case report

We describe a case of right ilio-psoas abscess caused by stump appendicitis 14 years after open appendectomy.Stump appendicitis is a rare complication of appendectomy. Right ilio-psoas abscess was diagnosed in an immunecompetent patient and treated by ultrasound guided percutaneous drainage twice without identifying the cause of the abscess. The patient did not improve until diagnostic laparoscopy was performed revealing a long stump appendicitis to be the origin of infection. It was treated by completion appendectomy. Surgical exploration may be necessary in persistent or recurrent ilio-psoas abscesses. We identified 4 reported cases of post-appendectomy ilio-psoas abscess but without recognizing the cause of the abscess and its relation to appendectomy. This is the first reported case of ilio-psoas abscess that developed as a complication of stump appendicitis.

Depositional environment， petrophysical evaluation and electrical properties of Zeit Formation， Northwestern shore of Gulf of Suez， Egypt

The Zeit sand reservoir is one of the most prolific formations at Northwestern side of the Gulf of Suez. In this research we will try to coordinate between electrical， petrophysical properties， depositional environment and facies discrimination in order to evaluate the hydrocarbon potentiality of studied Zeit formation. The statistical parameters for potassium (K)， thorium (Th) and Th/U ratio contents have a general increase towards northwestern parts， whereas uranium (U) content has a general increase towards southeastern parts. The sandstone facies is distinguished from the other facies by its thorium content >4 ppm. U has high carbonate content (U ≥1 ppm). Rocks’ electrical properties vary greatly depending on a number of factors. electrical measurements were taken at frequencies range of (5×10-4Hz–100 kHz) for fully saturated samples (clayey sandstone) with NaCl (20 gm/Littre). As salinity， clay content， and frequency increase， consequently does the electrical properties. The continental condition are present in northwestern part (back-sea) which is distinguished by high K percent， high Th， high Th/U ratio， and low U contents. Low K， Th， and Th/U ratio contents， with high U contents， characterize the marine depositional environment that existed around the east and southeastern parts (fore-sea coincide with the dipping of strata). Furthermore， the studied Zeit Formation has good petrophysical properties that coincide with marine conditions. The middle and eastern parts (around ISS-94 and CSS-288) is a good reservoir (porosity 36%–39%， shale content <15%， hydrocarbon saturation 71%–92%， and net pay thickness 17–63 feet).

The post-resuscitation period is recognized as the main predictor of cardiopul-monary resuscitation(CPR)outcomes.The first description of post-resuscitation syndrome and stony heart was published over 50 years ago.major manifestations may include but are not limited to,persistent precipitating pathology,systemic ischemia/reperfusion response,post-cardiac arrest brain injury,and finally,post-cardiac arrest myocardial dysfunction(PAmD)after successful resuscitation.Why do some patients initially survive successful resuscitation,and others do not?Also,why does the myocardium response vary after resuscitation?These ques-tions have kept scientists busy for several decades since the first successful resuscitation was described.By modifying the conventional modalities of resu-scitation together with new promising agents,rescuers will be able to salvage the jeopardized post-resuscitation myocardium and prevent its progression to a dismal,stony heart.Community awareness and staff education are crucial for shortening the resuscitation time and improving short-and long-term outcomes.Awareness of these components before and early after the restoration of circulation will enhance the resuscitation outcomes.This review extensively addresses the underlying pathophysiology,management,and outcomes of post-resuscitation syndrome.The pattern,management,and outcome of PAmD and post-cardiac arrest shock are different based on many factors,including in-hospital cardiac arrest vs out-of-hospital cardiac arrest(OHCA),witnessed vs unwitnessed cardiac arrest,the underlying cause of arrest,the duration,and protocol used for CPR.Although restoring spontaneous circulation is a vital sign,it should not be the end of the game or lone primary outcome;it calls for better understanding and aggressive multi-disciplinary interventions and care.The development of stony heart post-CPR and OHCA remain the main challenges in emergency and critical care medicine.

Pyronaridine combined with diminazene aceturate inhibits Babesia in vitro and in vivo

Objective:To evaluate the combination therapy of pyronaridine tetraphosphate and diminazene aceturate against Babesia in vitro and in vivo.methods:Bioinformatic analysis was performed using atom pair fingerprints.An in vitro combination test was performed against Babesia bovis and Theileria equi.moreover,the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminazene aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory assay.Results:Pyronaridine tetraphosphate and diminazene aceturate exhibited nearly similar molecular weights.The in vitro combination of pyronaridine tetraphosphate and diminazene aceturate was synergistic on Babesia bovis and additive on Theileria equi.In addition,5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminazene aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminazene aceturate alone from day 6 post treatment to day 12 post treatment.The combination therapy also normalized the hematological parameters of infected mice.Conclusions:An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminazene aceturate inhibits Babesia in vitro and in mice,suggesting it might be a new paradigm for the treatment of babesiosis.
