We evaluated the visual and refractive outcomes after phakic visian toric implantable collamer lens(ICL) insertion in stable keratoconus(KC). This retrospective study investigated toric ICL implantation in 14 eyes of 8 patients with stable KC. After 6 mo, the mean uncorrected distance visual acuity improved significantly from 0.77 to 0.15 logMAR. The mean best corrected distance visual acuity(BCDVA) improved from 0.18±0.1 to 0.15±0.1 logMAR. Fifty percent of eyes maintained their preoperative BCDVA; 42.8% gained one line. There was no statistical difference in high order or coma aberration. The mean refractive manifest spherical equivalent(MSE), mean refractive manifest spherical error, mean manifest astigmatism decreased significantly postoperatively. At 6 mo postoperatively, our achieved mean spherical equivalent was approximately 74%. no intraoperative or postoperative complications occurred. Toric ICL implantation was effective, predictable and safe to correct refractive error and improve visual acuity in patients with stable KC.

Although gastrointestinal stromal tumors(GISTs)are rare,with an incidence of 1/100000 per year,they are the most common sarcomas in the peritoneal *** considerable progress in the diagnosis and treatment of GIST,about half of all patients are estimated to experience *** only two drugs,sunitinib and regorafenib,approved by the Food and Drug Administration,selecting treatment options after imatinib failure and coordinating multidisciplinary care remain *** addition,physicians across the Middle East face some additional and unique challenges such as lack of published local data from clinical trials,national disease registries and regional scientific research,limited access to treatment,lack of standardization of care,and limited access to mutational *** global guidelines set a framework for the management of GIST,there are no standard local guidelines to guide clinical practice in a resource-limited ***,a group of 11 experienced medical oncologists from across the Gulf and Levant region,part of the Rare Tumors Gastrointestinal Group,met over a period of one year to conduct a narrative review of the management of GIST and to describe regional challenges and gaps in patient management as an essential step to proposing local clinical practice recommendations.

When it comes to water and energy, it is hard to obtain one without the other. Water is required to produce energy and energy is necessary in water production and management. As demands for water are escalating due to rapid population growth and urbanization, understanding and quantification of the interdependency between water and energy, along with analyzing nexus interactions, trade-offs and risks are a pre-requisite for effective and integrated planning and management of these two key sectors. This paper performs an assessment of the water-energy nexus in the municipal sector of the Eastern Province of Saudi Arabia, where the electric energy footprint in the water value chain (groundwater, desalination and wastewater treatment) and the water footprint in.electric energy generation (thermal power plants) are quantified using data for the year 2013. The results confirmed the high and strong dependency on energy for the municipal water cycle in the Eastern Province and revealed that energy generation dependency on freshwater resources is also major and evident, especially at farther distances from the coastal areas. Thermal desalination is by far the most energy intensive stage among the entire Eastern Province water cycle. In 2013, it was estimated 13% of the Eastern Province energy generation capacity goes for desalination, that’s a 5% of the Kingdom capacity. Substantial energy input for desalination in the Eastern Province is attributed to the production and conveyance of water to the Capital Riyadh (48.9 kWh/m3 and 4.2 kWh/m3 respectively). As for groundwater pumping, it was estimated that 206.2 GWH was used for pumping 268 MCM in 2013 (0.764 kWh/m3). Energy requirement for primary, secondary and tertiary wastewater treatment was found to be the least (2 - 108 GWH) and was equivalent to an average of 0.4 kWh/m3. The purpose of this paper is to present a numerical approach based on the artificial neural networks(Anns)for solving a novel fractional chaotic financial model that represents the effect of memory and chaos in the presented *** method is constructed with the combination of the Anns along with the Levenberg-Marquardt backpropagation(LMB),named the *** technique is tested for solving the novel problem for three cases of the fractional-order values and the obtained results are compared with the reference *** numbers neurons have been used to solve the fractional-order chaotic financial *** selection of the data to solve the fractional-order chaotic financial model are selected as 75%for training,10%for testing,and 15%for *** results indicate that the presented approximate solutions fit exactly with the reference solution and the method is effective and *** obtained results are testified to reduce the mean square error(MSE)for solving the fractional model and verified through the various measures including correlation,MSE,regression histogram of the errors,and state transition(ST).

First Cycle neutrophil Count and Subsequent Chemotherapy Dose Delivery in Breast Cancer Patients Receiving Adjuvant Chemotherapy

Background: Chemotherapy-induced neutropenia is a major dose-limiting toxicity of systemic cancer chemotherapy, and it is associated with substantial morbidity, mortality, and costs. The ability to predict the group of patients who are at high risk for neutropenic complications might help to reduce the morbidity due to febrile neutropenia and increase the likelihood of delivering full chemotherapy dose on time. In addition, hematopoietic growth factors could be administered to patients who need them and allow more efficient use of medical resources. Methods: The study included 120 female patients with breast cancer, treated in adjuvant setting with FAC chemotherapy regimen. Chemotherapy was delayed if the AnC was less than 1500 cells/μL on the planned first day of the cycle. Dose delays would be for 1 week. The chemotherapy dose was reduced if the patient experienced an episode of febrile neutropenia. Dose reductions usually consist of a 20% reduction in dosage of all chemotherapy drugs. Results: Out of the 120 patients in the study, 58 patients experienced neutropenic complications (febrile neutropenia or dose delayed) accounting for 48.3% of total sample size. In univariate analysis, there was statistically significant association between first cycle AnC nadir and neutropenic complication (p = 0.0001). There was also statistically significant association between pretreatment first cycle AnC (p = 0.001), pretreatment platelets count (p = 0.054), first cycle platelets count nadir (p = 0.041), number of involved lymph nodes (p = 0.031) and the occurrence of varies neutropenic complications. However, in multivariate analysis, AnC nadir was only the independent risk factor for occurrence of neutropenic. Conclusions: The first cycle absolute neutrophil counts nadir provides useful information on the risk of future neutropenia, febrile neutropenia and dose delay or reduction with estimated sensitivity and specificity, which were 64.5% and 77.6%, respectively. A Schiff base L [2-thiophenecarboxylic acid, 2-（2-pyridinylmethylene）hydrazide] with its lanthanide metal complexes was synthesized. These complexes were characterized by elemental analysis, molar conductivity measurements, spectral analysis（nMR, FT-IR, and UV-Vis）, luminescence and thermal gravimetric analysis. The Schiff base ligand was a tridentate chelate and coordinates to the central lanthanide ion with 1：2 metal：ligand ratio. The conductivity data showed a 1：1 electrolytic nature with a general formula [LnL_2（nO_3）_2]nO_3. The luminescence emission properties for Sm, Tb, and Eu complexes were observed and showed that the ligand L could absorb and transfer energy to Sm（III）, Tb（III） and Eu（III） ions. The complexes possessed a good antibacterial activity against different bacterial strains. In addition, the scavenging activity of the Ln（III） complexes on DPPH was concentration dependant and the complexes were significantly more efficient in quenching DPPH than the free Schiff base ligand.

AIM To review impacts of interventions involving self-management education, health coaching, and motivational interviewing for type 2 diabetes. METHODS A thorough review of the scientific literature on diabetes care and management was executed by a research team. RESULTS This article summarizes important findings in regard to the validity of developing a comprehensive behavioral system as a framework for empirical investigation. The behavioral system framework consists of patients' knowledge(K), motivation(M), attitude(A), and practice(P) as predictor variables for diabetes care outcomes(O). Care management strategies or health education programs serve as the intervention variable that directly influences K, M, A, and P and then indirectly affects the variability in patient care outcomes of patients with type 2 diabetes. COnCLUSIOn This review contributes to the understanding of the KMAP-O framework and how it can guide the care management of patients with type 2 diabetes. It will allow the tailoring of interventions to be more effective through knowledge enhancement, increased motivation, attitudinal changes, and improved preventive practice to reduce the progression of type 2 diabetes and comorbidities. Furthermore, the use of health information technology for enhancing changes in KMAP and communications is advocated in health promotion and development.
