Objective: The management of traumatic facial nerve paralysis(FNP) has remained a controversial issue with conflicting findings arguing between surgical decompression and conservative ***, recent advances in endoscopic surgery may consolidate the management plan for this ***: This prospective clinical study included patients with posttraumatic FNP at a tertiary referral center. Patients were categorized in two main groups: surgical and conservative. Indications for surgery included patients with immediate and complete FNP, no improvement in facial function on medical treatment, with electroneurography showing >90% degeneration or electromyography showing fibrillation potential. Patients who did not satisfy this criterion received the conservative approach. The transcanal endoscopic approach(TEA) or endoscopic assisted transmastoid approach was performed for facial nerve decompression in the surgical ***: The main outcome was facial function improvement, assessed using the House Brackmann grading scale(HBGS) 6 months after surgery, and hearing state assessed using the air bone gap(ABG).Results: The study included 38 patients, of whom 15 underwent had surgical decompression and 23underwent conservative therapy. A significant improvement in facial nerve function from a mean of4.66 ± 0.97 to 1.71 ± 0.69(P = 0.001) and ABG from a median of 30(10-40) to 20(10-25)(P = 0.002)was ***: Decision-making in cases of traumatic FNP is critical. The geniculate ganglion and tympanic segment were the most commonly affected areas in FNP cases. The TEA represents the most direct and least invasive approach for this area.

An angiosarcomatous component in gliosarcoma may be associated with an increased intraoperative hemorrhagic risk and preoperative diagnostic challenge. We report a unique case of gliosarcoma with an angiosarcomatous component in a 61-year-old man. His brain MRI demonstrated a well-demarcated right occipital tumor with multiple flow voids and rim-like enhancement as well as intratumoral strip and nodular enhancements. He underwent a craniotomy for tumor resection. Intraoperatively, significant tumor hemorrhage required greater efforts to control intraoperative bleeding and to maintain hemostasis. Pathological examination of the tumor revealed alternating gliomatous and sarcomatous/angiosarcomatous components with intratumoral hemorrhage. He was postoperatively treated with chemoradiation. The tumor recurred at 9 months, for which the second resection was performed with similarly greater efforts to achieve hemostasis. The recurrent tumor was pathologically similar despite treatment-associated changes. Awareness of this angiosarcomatous component in gliosarcoma with the hemorrhagic risk is important for both the preoperative diagnosis and surgical management.

本文研究了甘蔗渣灰部分替代普通硅酸盐水泥(OPC)对膨胀土岩土性能的影响。研究了水泥处理膨胀土的压实特性、微观结构、张拉强度、抗压强度、CBR值、剪切波速和小应变剪切模量。按不同比例设计制备土样,在7天、14天和28天的不同养护时间进行了标准击实试验、无侧限抗压强度试验、CBR试验、巴西劈裂试验和弯曲元试验。并进行了X射线衍射和扫描电镜测试。X射线衍射分析表明,甘蔗渣灰的主要矿物为石英和白晶石。扫描电镜分析表明,蔗渣灰中存在不同粒径的颗粒,有许多纤维状颗粒。结果表明,用蔗渣灰代替OPC可以改善处理后土壤的压实特性,使其最大干密度降低,最佳含水量提高。用蔗渣灰部分替代OPC还可以提高处理膨胀土的早期强度。结果表明,不同置换比例的试样的拉伸强度均有所提高,这可能是由于蔗渣灰纤维结构长,抗拉伸破坏能力强。当置换率不大于20%时,部分置换试件的CBR值甚至高于单独用水泥处理的试件。经处理后土体的弯曲单元试验表明,刚度存在明显的各向异性,但随着固化时间的延长和置换率的增加,刚度逐渐降低。试验结果表明,蔗渣灰是一种有效的矿物掺合料,CBA20(20%)是水泥的最佳替代率。。

The Impact of Human Parvovirus B19 Co-Infection on Liver Function of HCV Infected Patients

Background: Human Parvovirus B19 is most known for causing disease in the pediatric population but can also affect adults. Human co-infection with Parvovirus B19 could deteriorate the prognosis of patient with chronic ill-ness. Objectives: This paper attempts to determine the prevalence of Parvovirus B19 in HCV infected patients and to evaluate the impact of Parvovirus B19 on liver enzymes activity of Hepatitis C patients. Study Design: The study population includes 74 chronic HCV (patient group) and 49 cases without viral hepatitis (control group). Nucleic acid of Parvovirus B19 was detected in Serum samples by nested polymerase chain reaction (nested-PCR) method. Results: Parvovirus B19-DNA infection was detected in 28.0% of chronic HCV patients. Parvovirus B19-HCV co-infection caused increasing in alanine aminotransferase (alT) and Aspartate aminotransferase (AST) activity than in B19-negative HCV patients. Conclusion: We conclude that Parvovirus B19 acted synergistically with HCV by increasing the levels alanine ami-notransferase (alT) and Aspartate aminotransferase (AST).

Underreporting of Energy Intake in Iraqi People Living in the UK According to Body Weight

Previous studies have shown that underreporting of energy intake is common when assessing dietary intake, particularly in the overweight and obese. The aim of this study was to obtain an estimate of energy and macronutrient intake according to weight status and to investigate underreporting among Iraqi people living in the UK. Twenty eight adult volunteers (n = 16 males) were recruited. Dietary intake was estimated 3 times using the 24 hour dietary recall tech-nique. Portion size was estimated using household measures and the Photographic Atlas of Food Portion Sizes. Energy and macronutrients estimations were calculated, and weight and height were measured to calculate Body Mass Index (BMI). Energy intake was compared to estimated basal metabolic rate (BMR) and the Goldberg equation was used to assess underreporting. Of the overall sample, 57.1% (n = 16) and 21.4% (n = 6) subjects were overweight and obese, respectively. The mean energy intakes were low for males (1977 kcal/d) and females (1586 kcal/d). The percentage of under-reporters was high (57.1%) and this was higher in overweight and obese subjects (68.1%) compared to those of normal weight (4.5%). Overweight and obesity are common among well-educated Iraqi people. Underreporting of energy intake is common especially among overweight and obese Iraqi individuals.

We evaluated the visual and refractive outcomes after phakic visian toric implantable collamer lens(ICL) insertion in stable keratoconus(KC). This retrospective study investigated toric ICL implantation in 14 eyes of 8 patients with stable KC. After 6 mo, the mean uncorrected distance visual acuity improved significantly from 0.77 to 0.15 logMAR. The mean best corrected distance visual acuity(BCDVA) improved from 0.18±0.1 to 0.15±0.1 logMAR. Fifty percent of eyes maintained their preoperative BCDVA; 42.8% gained one line. There was no statistical difference in high order or coma aberration. The mean refractive manifest spherical equivalent(MSE), mean refractive manifest spherical error, mean manifest astigmatism decreased significantly postoperatively. At 6 mo postoperatively, our achieved mean spherical equivalent was approximately 74%. No intraoperative or postoperative complications occurred. Toric ICL implantation was effective, predictable and safe to correct refractive error and improve visual acuity in patients with stable KC.
