AIM: To study the results for the treatment of symptomatic hemorrhoids using rubber band ligation (RBL) method. METHODS: A retrospective study for 750 patients who came to the colorectal unit from June, 1998 to September, 2006, data was retrieved from archived fi les. RBL was performed using the Mc Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 mo, 6 mo and through telephone call every 6 mo for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcome for second or third degree hemorrhoids (P = 0.31). Symptomatic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 complications from RBL which required no hospitalization. Complications were pain, rectal bleeding and vaso-vagal symptoms(4.13%, 4.13% and 1.33% of patients, respectively). At 1 mo there were a significant improvement in mean SF-36 scores over baseline in five items, while after 2 years there were improvement in all items over baseline, but not significant. No significant manometeric changes after band ligation. CONCLUSION: RBL is a simple, safe and effective method for treating symptomatic second and third degree hemorrhoids as an out patient procedure with signifi cant improvement in quality of life. RBL doesn't alter ano-rectal functions.

The Mediterranean Sea is home to an interesting biodiversity. The current cumulative and descriptive study aims to enumerate some relatively large marine mammals, reptiles, and fishes that have been spotted, caught, by-caught, or stranded on the coast of the Gaza Strip, Palestine, which extends about 42 km along the Mediterranean Sea. This 20-year study from 2003 to 2022 relied much on frequent field visits, meetings, and discussions with stakeholders, following local media sites and social media pages, and photography. The study recorded at least 26 giants or relatively large marine organisms on the Mediterranean coast of the Gaza Strip, Palestine. Certainly, the coming years may bring other marine organisms of relatively large sizes. Marine mammals included three species of cetaceans with the Fin Whale (Balaenoptera physalus) being the largest mammal and even the largest animal ever recorded in this study. Marine reptiles included three species of sea turtles, the largest of which is the Leatherback Sea Turtle (Dermochelys coriacea), which is in fact the world’s largest sea turtle. The bony fishes were represented by ten species, the largest of which was the Ocean Sunfish (Mola mola), which is the largest bony fish in the world. The cartilaginous fishes included ten species with the largest specimens encountered were the Shortfin Shark (Isurus oxyrinchus), Bluntnose Sixgill Shark (Hexanchus griseus), Scalloped Hammerhead Shark (Sphyrna lewini), and Giant Devil Ray (Mobula mobular). Seabirds were not included in the study. The alexandria Pompano (alectis alexandrinus) and the Silver-Cheeked Toadfish (Lagocephalus sceleratus), which are bony fishes, appear to be the relatively smallest marine organisms recorded here. In conclusion, the role of the various parties must be coordinated to ensure the sustainability of human activities and their compatibility with the task of conserving local marine biota, including the gigantic or relatively large ones.

Mortality is a well-established patient-important outcome in critical care *** contrast,morbidity is less uniformly reported(given the myriad of critical care illnesses and complications of each)but may have a common end-impact on a patient’s functional capacity and health-related quality-of-life(HRQoL).Survival with a poor quality-of-life may not be acceptable depending on individual patient values and ***,as mortality decreases within critical care,it becomes increasingly important to measure intensive care unit(ICU)survivor *** measurements with a preference-based scoring algorithm can be converted into health utilities on a scale anchored at 0(representing death)and 1(representing full health).They can be combined with survival to calculate quality-adjusted life-years(QalY),which are one of the most widely used methods of combining morbidity and mortality into a composite *** QalYs have been use for health-technology assessment decision-making,an emerging and novel role would be to inform clinical decision-making for patients,families and healthcare providers about what expected HRQoL may be during and after ICU *** care randomized control trials(RCTs)have not routinely measured or reported HRQoL(until more recently),likely due to incapacity of some patients to participate in patient-reported outcome *** differences in HRQoL measurement tools can lead to non-comparable *** this end,we propose the validation of a gold-standard HRQoL tool in critical care,specifically the *** combined health-utility and mortality(disaggregated)and QalYs(aggregated)can be reported,with disaggregation allowing for determination of which components are the main drivers of the QalY *** use of HRQoL,health-utility,and QalYs in critical care RCTs has the potential to:(1)Increase the likelihood of finding important effects if they exist;(2)improve research efficiency;and(3)help inform optimal m

随着城市化进程的发展,城市高楼日益增多,但城市高楼危机却日益突显。城市楼房在发生火灾时居民无法及时逃生;室内电梯封闭结构通风差、病毒等高危传播,封闭电梯结构还导致人身侵害犯罪频发,传统室内电梯的通风、照明日夜不停,浪费能源显著。城市高楼及电梯的上述问题,已经成为阻碍城市发展的最严峻技术瓶颈问题。为解决城市楼房及电梯结构设计存在的重大缺陷,提出了颠覆传统楼房结构、电梯及楼梯布局和结构的新型楼房,将每栋楼房建为至少具有两个在空间上分隔、且具有一定防火安全距离的单元,每个单元都通过连廊连接到室外电梯间,室外电梯间独立于楼房任何单元,电梯间内具有独立供电的电梯阵列、楼梯阵列以及可以通达任何一个楼房单元的廊道。室外电梯间及电梯采用通风防雨的通透结构。电梯阵列可以采用神经网络等智能优化控制策略。本设计的智慧城市绿色建筑电梯阵列突破了城市发展的技术瓶颈和危机,具有极其重要意义。
