Real time endoscopic ultrasound elastography and strain ratio in the diagnosis of solid pancreatic lesions

AIM To evaluate the accuracy of the elastography score combined to the strain ratio in the diagnosis of solid pancreatic lesions(SPL). METHODS A total of 172 patients with SPL identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and strain ratio in differentiating malignant from benign lesions. The semi quantitative score of elastography was represented by the strain ratio method. Two areas were selected, area(A) representing the region of interest and area(B) representing the normal area. Area(B) was then divided by area(A). Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing diagnoses made by elastography, strain ratio and final *** SPL were shown to be benign in 49 patients and malignant in 123 patients. elastography alone had a sensitivity of 99%, a specificity of 63%, and an accuracy of 88%, a PPV of 87% and an NPV of 96%. The best cut-off level of strain ratio to obtain the maximal area under the curve was 7.8 with a sensitivity of 92%, specificity of 77%, PPV of 91%, NPV of 80% and an accuracy of 88%. Another estimated cut off strain ratio level of 3.8 had a higher sensitivity of 99% and NPV of 96%, but with less specificity, PPV and accuracy 53%, 84% and 86%, respectively. Adding both elastography to strain ratio resulted in a sensitivity of 98%, specificity of 77%, PPV of 91%, NPV of 95% and accuracy of 92% for the diagnosis of SPL. CONCLUSION Combining elastography to strain ratio increases the accuracy of the differentiation of benign from malignant SPL.

BACKGROUND The current coronavirus disease 2019(COVID-19)pandemic has affected routine endoscopy service across the gastroenterology *** led to the suspension of service provision for elective *** To assess the potential barriers for resuming the endoscopy service in *** A national online survey,four domains,was disseminated over a period of 4 wk in August *** primary outcome of the survey was to determine the impact of the COVID-19 pandemic on the endoscopy service and barriers to the full resumption of a disabled center(s).RESULTS A hundred and thirteen Egyptian endoscopy centers participated in the *** waiting list was increased by≥50% in 44.9% of areas with clusters of COVID-19 cases(n=49)and in 35.5% of areas with sporadic cases(n=62).Thirty nine(34.8%)centers suffered from staff shortage,which was considered a barrier against service resumption by 86.4% of centers in per-protocol *** multivariate analysis,the burden of cases in the unit locality,staff shortage/recovery and the availability of separate designated rooms for COVID-19 cases could markedly affect the resumption of endoscopy practice(P=0.029,ely)and Odd’s ratio(0.15,1.8 and 0.16,respectively).CONCLUSION The COVID-19 pandemic has led to restrictions in endoscopic *** staff shortage/recovery and the availability of COVID-19 designed rooms are the most important barriers against *** working hours and dividing endoscopy staff into teams may help to overcome the current situation.

Smart prediction of liquefaction-induced lateral spreading

The prediction of liquefaction-induced lateral spreading/displacement(Dh)is a challenging task for civil/geotechnical *** this study,a new approach is proposed to predict Dh using gene expression programming(GEP).Based on statistical reasoning,individual models were developed for two topographies:free-face and gently sloping *** with a comparison with conventional approaches for predicting the Dh,four additional regression-based soft computing models,*** process regression(GPR),relevance vector machine(RVM),sequential minimal optimization regression(SMOR),and M5-tree,were developed and compared with the GEP *** results indicate that the GEP models predict Dh with less bias,as evidenced by the root mean square error(RMSE)and mean absolute error(MAE)for training(i.e.1.092 and 0.815;and 0.643 and 0.526)and for testing(i.e.0.89 and 0.705;and 0.773 and 0.573)in free-face and gently sloping ground topographies,*** overall performance for the free-face topology was ranked as follows:GEP>RVM>M5-tree>GPR>SMOR,with a total score of 40,32,24,15,and 10,*** the gently sloping condition,the performance was ranked as follows:GEP>RVM>GPR>M5-tree>SMOR with a total score of 40,32,21,19,and 8,***,the results of the sensitivity analysis showed that for both free-face and gently sloping ground,the liquefiable layer thickness(T_(15))was the major parameter with percentage deterioration(%D)value of 99.15 and 90.72,respectively.

Lunar soil simulants(LSS)have been pivotal in the success of past and current lunar *** have enhanced the design of lander and rover wheels through interaction studies on the lunar testbed under simulated environmental *** have also been used to evaluate the geotechnical,geomechanical,and dynamic characteristics of the lunar soil(LS),which is vital for future lunar *** most simulants have focused on either the chemical and mineralogical or geotechnical properties,developing a comprehensive LSS that mimics the lunar soil is of paramount importance for lander and rover soft-landing and future lunar *** article presents the significant development and characterization of the new LSS NYUAD-1,the first regional soil simulant tailored for the planned Emirates lunar *** material and geotechnical properties of NYUAD-1,including chemical composition,mineralogy,particle size,morphology,specific gravity,density,shear strength,and compressibility behaviors,were assessed through laboratory tests conducted per ASTM *** analysis with authentic lunar regolith and various regolith simulants confirms the significance and applicability of NYUAD-1 for lunar-based research,offering a promising step toward future lunar habitation.

Detection of virulent strains of Helicobacter pylori and its relation to symptoms and signs in children

Objective:To detect the prevalence of Helicobacter polori(***) in a group of pediatric patients complaining of gastrointestinal disorders,and to identify the virulent strains of H. pylori and its relation to peptic ***:The study included 60 patients(41 males and 19 females),aged from 6 months to 14 years,with complaining of different gastrointestinal *** IgG antibodies were *** examination of the oesophagus, stomach and duodenum was done in all patients using a fibreoptic *** addition, gastric antral biopsies were taken for diagnosis of *** infection in the following test: rapid urease test,histopathologic examination and *** cytotoxin assay of virulent strains was *** infection was diagnosed if 3 or more tests for diagnosis were found to be ***:33 out of 60 patients(55%) was positive.12(20%) patients had positive toxin producing strains(virulent strains).Virulent strain had a significantly closer relationship with recurrent abdominal pain,while no difference was found in other symptoms. Age of patients showed a highly significant positive correlation with virulent *** strain. Histopathological finding revealed that oesophagitis was significandy common in patients with negative *** and histopathological findings revealed that virulent strain had a significandy higher gastritis and oesophagitis ***:Infection by virulent strains of *** in children is not uncommon:It is associated with endoscopic and histopathologic *** cases infected with virulent strain showed positive results by using rapid urease test,culture and IgG antibodies.

Preparation and characterization of (CeO2)x-(Fe2O3)1-x nanocomposites:reduction kinetics and hydrogen storage

A series of nanosized CeO2-Fe2O3 mixed-oxide nanocomposites with different Ce4+/Fe3+molar ratios were successfully prepared by a co-precipitation *** surface area increased with Fe2O3 content increasing up to 60 wt%in the ***,with further increase in Fe2O3 content,the surface area began to *** reduction processes of the CeO2-Fe2O3 nanocomposites were studied in a hydrogen atmosphere at 300-600℃.The reduction rates increased by increasing both the temperature and Fe2O3 content in the *** microstructure of the reduced composites at 500℃illustrated the presence of a considerable number of macro-and *** activation energy values were calculated which were in the range of 3.56-5.37 kJ mol-1 at the initial stages(up to 35%reduction)and 5.21-10.2 kJ·mol-1 at the final stages(up to 80%reduction)of *** rate-controlling mechanisms in both the initial and final reduction stages were determined,and the initial reaction stage was controlled by combined gaseous diffusion and interfacial chemical reaction mechanisms for all the composites except for pure CeO2,which was controlled by a chemical reaction *** final reaction stage was controlled by a gaseous diffusion mechanism for some composites,while for the others it was controlled by combined gaseous diffusion and interfacial chemical reaction *** hydrogen sorption properties of the nanocomposites were studied by pressure composition isotherms using a volumetric *** storage in the nanocomposites increased by increasing the temperature because of the formation of oxygen vacancies which enhance atomic H adsorption and function as strong adsorption sites forming more metal hydride covalent bonds.

Outcomes of pancreaticoduodenectomy in elderly patients

BACKGROUND： Although the mortality and morbidity of pancreaticoduodenectomy（PD） have improved significantly over the past years, the concerns for elderly patients undergoing PD are still present. Furthermore, the frequency of PD is increasing because of the increasing proportion of elderly patients and the increasing incidence of periampullary tumors. This study aimed to analyze the outcomes of PD in elderly ***： We studied all patients who had undergone PD in our center between January 1995 and February 2015. The patients were divided into three groups based on age： group I（patients aged 〈60 years）, group II（those aged 60 to 69 years） and group III（those aged ≥70 years）. The primary outcome was the rate of total postoperative complications. Secondary endpoint included total operative time, hospital mortality, length of postoperative hospital stay, delayed gastric emptying, re-exploration, and survival ***： A total of 828 patients who had undergone PD for resection of periampullary tumor were included in this study. There were 579（69.9%） patients in group I, 201（24.3%） in group II, and 48（5.8%） in group III. The overall incidence of complications was higher in elderly patients（25.9% in group I, 36.8% in group II, and 37.5% in group III; P=0.006）. There were more patients complicated with delayed gastric emptying in group II compared with the other two groups. There was no significant difference in the incidence of postoperative pancreatic fistula, biliary leakage, pancreatitis, pulmonary complications and hospital ***： PD can be performed safely in selected elderly patients. Advanced age alone should not be a contraindication for PD. The outcome of elderly patients who have undergone PD is similar to that of younger patients, and the increased rate of complications is due to the presence of associated comorbidities.

Biliary leakage following pancreaticoduodenectomy:Prevalence,risk factors and management

Background: Few studies investigated biliary leakage after pancreaticoduodenectomy(PD) especially when compared to postoperative pancreatic fistula(POPF). This study was to determine the incidence of biliary leakage after PD, predisposing factors of biliary leakage, and its management. Methods: We retrospectively studied all patients who underwent PD from January 2008 to December 2017 at Gastrointestinal Surgery Center, Mansoura University, Egypt. According to occurrence of postoperative biliary leakage, patients were divided into two groups. Group(1) included patients who developed biliary leakage and group(2) included patients without identified biliary leakage. The preoperative data, operative details, and postoperative morbidity and mortality were analyzed. Results: The study included 555 patients. Forty-four patients(7.9%) developed biliary leakage. Ten patients(1.8%) had concomitant POPF. Multivariate analysis identified obesity and time needed for hepaticojejunostomy reconstruction as independent risk factors of biliary leakage, and no history of preoperative endoscopic retrograde cholangiopancreatiography(ERCP) as protective factor. Biliary leakage from hepaticojejunostomy after PD leads to a significant increase in development of delayed gastric emptying, and wound infection. The median hospital stay and time to resume oral intake were significantly greater in the biliary leakage group. Non-surgical management was needed in 40 patients(90.9%). Only 4 patients(9.1%) required re-exploration due to biliary peritonitis and associated POPF. The mortality rate in the biliary leakage group was significantly higher than that of the non-biliary leakage group(6.8% vs 3.9%, P = 0.05). Conclusions: Obesity and time needed for hepaticojejunostomy reconstruction are independent risk factors of biliary leakage, and no history of preoperative ERCP is protective factor.
