objective:To investigate the activity of Egyptian propolis extracts(ethanol and water)on cryptosporidiosis in experimentally infected dexamethasone-immunosuppressed ***:A total of 180 male rats(190-220)g BWt were randomly divided into 9 equal groups(G1-G9).Groups of rats were kept as(G1):normal control,(G2-G9):immunosuppressed with dexamethasone and(G3-G9):infected with Cryptosporidium *** from(G4to G9)were given orally ethanol and water extract of propolis(at a dose of 50 mg/kg BWt)and nitazoxanide(standard anti-cryptosporidial drug at a dose of 100 mg/kg BWt)to infected rats with different *** pellets were collected from all groups to monitor oocysts shedding from the 2nd to the 15th day post *** the end of the experiment,blood was collected from all groups for determination of leukogram and serum *** specimens were also examined ***:The highest reduction of oocysts shedding in faecal samples was 88%in rats prophylactically treated with propolis ethanol extract at the 4th dpi,and in rats prophylactically treated with water extract of propolis,was91%at the 6th *** was a marked increase in neutrophils count and α_2-andβ-globulins levels in infected rats treated with both extracts,while a significant decrease was detected in lymphocytes compared to the infected non treated group.β-globulin level markedly increased in the rats administered *** changes were observed in the ileum of rats infected with ***:Egyptian propolis extracts have an activity on cryptosporidiosis in ***,propolis modulated the immunity in dexamethasone-immunosuppressed rats.

objective: The aim of our study was to evaluate the clinical features, to define the frequency of mutation type, to assess genotype-phenotype correlation and the response to colchicine in childhood-onset Familial Mediterranean fever (FMF) in Lebanon. Methods: The characteristics of 550 children, presenting with FMF symptoms between January 2003 and January 2013 and having a positive Mediterranean fever gene (MEFV gene) mutation, were prospectively investigated. The clinical and genetic characteristics as well as the response to colchicine and its side effects were studied in 321 FMF children. The mutations were correlated with clinical presentation and disease severity. Results: out of the 321 patients (183 males and 138 females), abdominal pain was the most common presenting feature documented in 84.7%. Mutational analysis detected simple heterozygotes, compound heterozygotes and homozygotes in 56.4%, 30.9% and 11.2% patients respectively. The most frequent mutation was M694V (37.2%), followed by E148Q mutation (27.4%). 71% patients received colchicine therapy;only 33.3% of them showed complete response. Genotype-phenotype correlation showed that M694V followed by E148Q was associated with moderate to severe disease form (71.6% and 62.7% respectively, P = 0.005). There was no association between mutation type and colchicine response. Conclusion: The most important features were the predominance of the M694V and E148Q. The M694V subgroup, followed by E148Q subgroup had a high disease severity score. our data indicate an enhanced expression of the disease with E148Q mutation.

Laryngeal cyst causing neonatal airway obstruction during labor is a very rare condition [1]. Congenital laryngeal cysts are a rare cause of neonatal airway obstruction. Traditionally, these cysts have been treated surgically by endoscopic excision or marsupialization. However, the cyst often extends beyond the larynx. We describe a case of a newborn that, during delivery, became cyanotic due to airway obstruction and respiratory distress. To the best of our knowledge this is the first report of a saccular cyst obstructing airway during birth prior to intubation. The immediate and late treatments together with a literature review are described.

The current work concerns the optimization process of phenolic compounds solid liquid extraction from grape byproducts at high temperatures and short incubation times. The effect of five experimental parameters (solidliquid ratio, particle size, time, temperature and solvent mixture) mostly believed to affect the extraction process was undertaken. A first response surface methodology experimental design was used to optimize the solid-liquid ratio and milling time parameters. A second design was used for the optimization of the quantitative and qualitative parameters. The quantitative parameters studied are: total phenolic compounds, flavonoid content, total monomeric anthocyanin composition and tannin concentration. The qualitative parameters analyzed are: antiradical activity and antioxidant capacity. The second design was based on the use of time, temperature and solvent mixture as optimization parameters. The assays were first conducted separately revealing the best experimental conditions for the maximization of each response variable alone. A simultaneous response surface methodology of all the responses taken together was then conducted, showing the optimal extraction conditions to be: 93 minutes at 94?C and in 66% ethanol/water solvent. The maximal response values obtained for each parameter are: Total Phenolic Compounds yield (5.5 g GAE/100g DM), Flavonoid Content (5.4 g GAE/100g DM), Total Monomeric Anthocyanin yield (70.3 mg/100g DM), Tannin Concentration (12.3 g/L), Antiradical Activity (67.3%) and Total Antioxidant Capacity (393 mgAAE/L). All of the optimal values were acquired at 3 mL/g solid-liquid ratio and 6.8 min milling time. The obtained extracts could be used as natural bioactive compounds in several industrial applications.

Between crisis and opportunity, old age has many faces in Lebanon. While old age was first seen as a time of wisdom, the current trend accentuated by migration and the economic crisis sets a challenge for the Lebanese elder;they may be left behind unless something is done in all communities at a national level. In this short communication, we attempt to broaden the scope of thinking about training programs to target population-shift and create a true change. We reflect on our results as an intermediate outcome of public health interventions. We suggest that it may be worthwhile to evaluate population-shifts further and base the success of an intervention on measures of Health Related Quality of Life through the response-shift created by such national public health programs.

In this study phenolic compounds extraction from grape byproducts was conducted using pure water as a solvent. High temperatures and low time incubation periods were used in the aim of reducing the cost of the process and heightening the phenolic compounds yield. Response surface methodology (RSM) was realized to study the effect of time and temperature on crushed and uncrushed grape pomace. The phenolic content was evaluated considering the quantity (total phenolics (TPC), flavonoids (FC), total monomeric anthocyanins (TMA) and tannins (TC)), and quality (antiradical activity (AA) and antioxidant capacity (AC)) of the extracts. High temperature low time extraction design used in this study was compared to the extraction process at moderate temperatures with relatively long periods of time. This was proved to ameliorate the quantitative extraction of phenolic compounds from grape pomace without affecting their bioactivity. Moreover, multiple response optimization showed the optimal extraction parameters to be 81?C and 140 minutes for the unmilled pomace samples, and 88?C and 5 minutes for the milled. TPC, FC, TMA, TC, AA and AC are almost the same for both optimums. Thus the possibility of replacing the milling process by the extraction time prolongation (for the unmilled pomace) of 135 minutes seems to be very plausible. HPLC analysis showed different quantity and diversity of extracted phenolics for the optimums. However this difference did not significantly affect the overall activity, showing that PC in the different extracts act in complete synergy all together leading to important biological properties. The obtained results using the extraction strategy adopted in this work could lead to several industrial applications.

ong>Background:ong> Polygonum multiflorum (brand name Fo-ti) is a Chinese herb used as an antioxidant, antiaging agent, and liver and kidney health. It can cause acute hepatotoxicity and even death. There is no published data on the effectiveness of N-Acetyl cysteine (NAC) as a sole effective therapy. ong>Case:ong> A 23-year-old, healthy male, presented with jaundice and malaise of one-month duration. He had been taking a Chinese herb named Fo-Ti for 2 months. Physical examination revealed jaundice, with no stigmata of chronic liver disease. His laboratory results showed transaminitis with markedly elevated transaminases. Testings for infectious and autoimmune causes were unremarkable. The patient was admitted to the hospital. Fo-Ti was stopped and he was started on oral NAC as peroral protocol. He was discharged home on esomeprazole and oral NAC. He returned on the second day with worsening jaundice. He was readmitted and esomeprazole was discontinued. NAC was continued and his liver enzymes eventually improved. The supplement was tested by gas-chromatography mass spectrometry and measured the presence of emodin. ong>Discussion:ong> This case highlights the potential benefit of NAC as treatment of Polygonum multiflorum induced hepatotoxicity. The proposed mechanism of emodin induced liver injury is through disturbing glutathione metabolism in liver cells. This could explain the favorable response as NAC has the ability to replenish intracellular glutathione. ong>Conclusion:ong> Polygonum multiflorum use has been associated with severe and prolonged hepatotoxicity. oral NAC should be considered in the treatment of these patients due to the favorable risk-benefit ratio.

在最近的病例报告中，牙周骨内袋缺损经釉基质提取物治疗后，对人牙周组织再生的组织学观察得到了不同的结果。本例为46岁女性，下颌第一磨牙计划拔除，其远中根存在一处深的骨内袋缺损，对其使用釉基质提取物进行治疗。术中在牙石最根方的牙根表面做凹槽标记。术后9个月获取包括远中根及其周围牙周组织的组织块。组织学分析证实在邻面和根分歧表面存在2种不同的愈合方式。在牙根的远中面可见到新生的细胞性牙骨质、骨和有功能性纤维排列的牙周膜形成，即有牙周组织再生，而在根分歧表面则出现了骨固连。本报告显示了牙周骨内袋缺损经过釉基质提取物治疗后，在同一缺损内的愈合中出现了生物学多样性。导致这种多样性的宿主特异的内源性和／或外源性因素有待研究。

BACKGRoUND The impact of type 2 diabetes mellitus(T2DM)on acute respiratory distress syndrome(ARDS)is debatable.T2DM was suspected to reduce the risk and complications of ***,during coronavirus disease 2019(CoVID-19),T2DM predisposed patients to ARDS,especially those who were on insulin at *** evaluate the impact of outpatient insulin use in T2DM patients on non-CoVID-19 ARDS *** We conducted a retrospective cohort analysis using the Nationwide Inpatient Sample *** patients diagnosed with ARDS were stratified into insulin-dependent diabetes mellitus(DM)(IDDM)and non-insulindependent DM(NIDDM)*** applying exclusion criteria and matching over 20 variables,we compared cohorts for mortality,duration of mechanical ventilation,incidence of acute kidney injury(AKI),length of stay(LoS),hospitalization costs,and other clinical *** Following 1:1 propensity score matching,the analysis included 274 patients in each ***,no statistically significant differences emerged between the IDDM and NIDDM groups in terms of mortality rates(32.8%vs 31.0%,P=0.520),median hospital LoS(10 d,P=0.537),requirement for mechanical ventilation,incidence rates of sepsis,pneumonia or AKI,median total hospitalization costs,or patient disposition upon *** Compared to alternative anti-diabetic medications,outpatient insulin treatment does not appear to exert an independent influence on in-hospital morbidity or mortality in diabetic patients with non-CoVID-19 ARDS.
