BackgroundTo compare the arithmetic mean (M-SIA) and the summated vector mean of surgically induced astigmatism (SVM-SIA) according to the incision site after phakic intraocular lens (Visian implantable collamer lens (ICL), STAAR Surgical) implantation.MethodsThis study comprised 121 eyes of 121 consecutive patients undergoing ICL surgery through a 3.0-mm temporal or superior clear corneal incision. The magnitude and the axis of corneal astigmatism preoperatively and 3 months postoperatively were measured using an automated keratometer. The M-SIA and the SVM-SIA were determined according to the incision site.ResultsThe magnitude of corneal astigmatism significantly increased from 1.23 ± 0.59 D preoperatively to 1.46 ± 0.72 D postoperatively in the temporal incision group (Wilcoxon signed-rank test, P < 0.001), but it significantly decreased from 1.09 ± 0.36 D preoperatively to 0.86 ± 0.41 D postoperatively in the superior incision group (P < 0.001). The M-SIA was 0.48 ± 0.30 D, and the SVM-SIA was 0.23 ± 0.52 D at a meridian of 82° in the temporal incision group. The M-SIA was 0.57 ± 0.30 D, and the SVM-SIA was 0.47 ± 0.45 D at a meridian of 1° in the superior incision group.ConclusionsICL implantation induces the M-SIA by approximately 0.5 D, but the SVM-SIA decreased to 50% and 80% of the M-SIA in magnitude through temporal and superior incisions, respectively. The direction of the SVM-SIA showed a tendency toward corneal flattening to the incisional site. It should be noted that the M-SIA is somewhat different from the SVM-SIA according to the incision site.

N-methyl-D-aspartate receptor(NMDAR)encephalitis is a rare type of autoimmune limbic encephalitis.Herein,we report an autopsy case of highly suspected anti‑NMDAR encephalitis,based on the patient’s past psychiatric history and autopsy findings of bilateral hippocampal sclerosis and ovarian teratoma.We found that it can be difficult to accurately diagnose autoimmune encephalitis at postmortem forensic autopsy without a case history and background information.

Effect of Reinforcement Clustering on Crack Initiation Mechanism in a Cast Hybrid Metal Matrix Composite during Low Cycle Fatigue

The reinforcement distribution of metal matrix composites (MMCs) plays an important role in low cycle fatigue. Thus, it is essential to study the effect of reinforcement clustering on the crack initiation mechanism of MMCs. In this study, the effect of reinforcement clustering on the microcrack initiation mechanism in a cast hybrid MMC reinforced with SiC particles and Al2O3 whiskers was investigated experimentally and numerically. Experimental results showed that microcracks always initiated in the particle-matrix interface, located in the cluster of the reinforcements. The interface debonding occurred in the fracture which created additional secondary microcracks due to continued fatigue cycling. The microcrack coalesced with other nearby microcracks caused the final fracture. To validate the experimental results on the microcrack initiation, three dimensional unit cell models using finite element method (FEM) were developed. The stress distribution in both the reinforcement clustering and non-clustering regions was analyzed. The numerical analysis showed that high stresses were developed on the reinforcements located in the clustering region and stress concentration occurred on the particle-matrix interface. The high volume fraction reinforced hybrid clustering region experienced greater stresses than that of the SiC particulate reinforced clustering region and low volume fraction reinforced hybrid clustering region. Besides, the stresses developed on the non-clustering region with particle-whisker series orientation were reasonably higher than that of the non-clustering region with particle-whisker parallel orientation. The high volume fraction reinforced hybrid clustering region is found to be highly vulnerable to initiate crack in cast hybrid MMC during low cycle fatigue.

研究了一种SiCp及Al2O3w增强铸态混杂金属基复合材料(MMC)的疲劳裂纹扩展(FCG)机理,同时对比研究了Al2O3w增强铸态金属基复合材料和铸态铝合金的疲劳裂纹扩展机理。在研究近临界和裂纹稳定扩展区域的疲劳裂纹扩展(FCG)机理时,发现混杂MMC的临界应力强度因子?Kth值高于其他两种材料的?Kth值,说明应力强度因子?K值较低时混杂MMC可以更好地抵抗裂纹扩展。随着?K值的降低,两种MMC在近临界区域显示出相似的FCG机理,即主要由增强相–基体界面的剥离控制,随后由铝基体中空隙的形核与合并控制;在裂纹稳定或中等扩展区域,?K值较高时FCG除了受界面上周期性裂纹扩展引起的增强相–基体界面剥离的影响之外,还显著受到铝基体中疲劳条带的影响。此外,在高?K值下,因为局部失稳断裂机制,可见铝基体中空隙的形核与合并以及SiCp和Al2O3w中的穿晶断裂。对于铸态铝合金,在低?K值下,FCG主要受空隙的形核与合并所控制;在高?K值下,FCG主要受铝晶粒的疲劳条带控制,随后受Si团簇中空隙的形核与合并控制。

Regression of hepatocellular carcinoma during vitamin K administration

An 85-year-old man with HCV infection and diabetes mellitus was diagnosed as having hepatocellular carcinoma (HCC, 13 cm in diameter) based on high serum alpha-fetoprotein (AFP),AFP-L3,and des-γ-carboxy prothrombin levels as well as typical enhancement pattern on contrast-enhanced CT. The patient did not receive any interventional treatments because of advanced age and the advanced stage of HCC.He chose to take vitamin K,which was reported to suppress the growth of HCC in vitro. Three months after starting vitamin K, all three tumor markers were normalized and HCC was markedly regressed, showing no enhancement in the early arterial phase on CT. Here we present the report describing the regression of HCC during the administration of vitamin K.

Differential Mechanisms of the Effect of Peroxisome Proliferator-Activated Receptor Gamma Agonists on Bleomycin-Induced Lung Fibrosis

Background and Objectives: Peroxisome proliferator-activated receptor-g (PPAR-g) is a nuclear receptor whose activation regulates inflammation and fibrosis in various organs. We aimed to investigate the effect of two PPAR-g ligands, telmisartan and rosiglitazone, on lung injury and fibrosis induced by intratracheal bleomycin (BLM). Methods: Lung injury and fibrosis was induced in female C57Bl/6 mice by intratracheal instillation of 1.0 mg/kg of BLM. Some of the animals received rosiglitazone intraperitoneally or telmisartan in drinking water. Bronchoalveolar lavage (BAL) was performed 2, 7, 14 or 21 days after BLM instillation for cell counting and measurement of mediators in the lung. In a separate series, the lungs were sampled for collagen assay and histopathological evaluation. Results: Treatment with rosiglitazone or telmisartan significantly attenuated the BLM-induced increases in lung collagen content, pathological score, and inflammatory cells in BAL fluid. Rosiglitazone significantly suppressed BLM-induced elevation of TGF-b1, MCP-1, and IL-6 levels in the lung. In contrast, telmisartan made no changes in these cytokines, whereas it mitigated the BLM-induced increase in prostaglandin F2a in the lung. Higher concentrations of rosiglitazone and telmisartan attenuated proliferation of lung fibroblasts in vitro. Conclusions: Two PPAR-g ligands, rosiglitazone and telmisartan, exert protective effects on BLM-induced lung fibrosis through the suppression of different profibrotic mediators.

Objective: The present study sought to determine changes in plasma levels of catecholamine metabolites and serum levels of brain-derived neurotorophic factor (BDNF) among smokers with schizophrenia who were treated with varenicline. Methods: We compared plasma homovanillic acid (HVA) and 3-methoxy-4-hydroxyphenylglycol (MHPG) levels and serum BDNF levels across 4 groups, divided as follows: smokers with schizophrenia (S-Sc), nonsmokers with schizophrenia (NS-Sc), smokers who were otherwise healthy (S-HC) and nonsmokers who were otherwise healthy (NS-HC). In addition, we also examined plasma HVA and MHPG levels and serum BDNF levels prior to study initiation (T0) and 8 weeks after varenicline treatment (T8) in the S-Sc and S-HC groups. Results: Plasma HVA levels in the S-Sc group at T0 were significantly higher than in the NS-Sc, S-HC and NS-HC groups at the same time point (S-Sc at T0;7.6 ± 1.6 ng/mL, NS-Sc;4.8 ± 1.0 ng/mL, S-HC;4.3 ± 0.9 ng/mL, NS-HC;3.4 ± 1.4 ng/mL, p < 0.0001). Plasma MHPG levels in the S-Sc group at T0 were significantly higher than in the NS-Sc and S-HC groups;in addition, plasma MHPG levels in the NS-HC group were significantly higher than in the S-HC group (S-Sc at T0;5.2 ± 0.7 ng/mL, NS-Sc;2.8 ± 1.2 ng/mL, S-HC;1.9 ± 0.8 ng/mL, NS-HC;3.6 ± 1.3 ng/mL, p < 0.0002). Serum BDNF levels did not differ between the S-Sc and NS-Sc groups. In contrast, no difference in plasma HVA levels, plasma MHPG levels or serum BDNF levels was observed between T0 and T8. Conclusions: In conclusion, plasma levels of HVA and MHPG in the S-Sc group were significantly higher than in the NS-Sc group. Serum BDNF levels did not differ between the S-Sc and NS-Sc groups. Plasma HVA and MHPG levels and serum BDNF levels did not change after treatment with varenicline.
