Dear Editor, The current standard 12-core systematic transrectal ultrasound （TRUS）-guided biopsy in prostate cancer diagnosis has the disadvantages of overdetecting indolent tumors, while failing to identify clinically significant cancers in up to 35%.1 Advances in imaging techniques, notably the multiparametric magnetic resonance imaging （mpMRI）, allow clinically significant prostate cancers to be detected with more precise localization） Fusion of MRI and real-time TRUS enables targeted biopsy of suspicious lesions, overcoming human error, and inability of biopsy site tracking in cognitive fusion？ Multiple Western series have suggested a higher detection rate, higher specificity, and better risk stratification with fusion biopsy.

Objective:Reporting research is as important a part of a study as its design or *** guidelines(RGs)provide structured advice on how to report research studies clearly and *** study aimed to review the development of RGs of chinese medicine(CM)and to provide recommen-dations for ***:Through a systematic search of the Enhancing the QUAlity and Transparency Of health Research(EQUATOR)Network and electronic databases up to January 1,2022,we identified a total of 15 RGs of CM,and further summarized their characteristics and *** addition,we reviewed the development of international RGs and analyzed its impact for ***:Compared with the generic RGs,the reporting standards of CM have been rapidly developed over the last 10 years,of which 57%were issued in recent 3 years(2019-2021).Currently,the system of RGs of CM has been established,especially for clinical trials,including various CM interventions and covering the guidelines from trial registration,protocol,results publication to the evidence synthesis and clinical practice ***,the application of RGs of CM is ***:It is necessary to take further measures to promote practical application,improve journals’endorsement,and establish quality monitoring procedures for RGs of CM in the future.

BACkGROUND: It has been suggested that addition of obesity score to the APAchE-Ⅱ system can lead to more accurate prediction of severity of acute pancreatitis. However there is scanty information on the usefulness of the combined APAchE-O scoring system in Asian patients. This study aimed to compare the accuracy of Ranson, APAchE-Ⅱ and APAchE-O systems in assessing severity of acute pancreatitis in a local chinese population. METHODS: One hundred and one consecutive patients with acute pancreatitis were prospectively studied. Body mass index (BMI) was measured on admission. Ranson score, APAchE-Ⅱ and APAchE-O scores were recorded on admission and at 48 hours. By adopting the cut-off levels and definitions advocated in the Atlanta consensus for severe disease, the diagnostic accuracy of the three scoring systems was compared by the area under the curve (AUC) under the receiver operator characteristic curve. RESULTS: Of the 101 patients, 12 (11.9%) patients suffered from severe pancreatitis. Obesity was uncommon and only two patients (2.0%) had BMI >30. Eighty-two (81.2%) patients were normal weight (BMI≤25) whereas 17 (16.8%) were overweight ( BMI 25-30 ). Overweight or obesity (BMI >25) was not associated with severe pancreatitis (P= 0.40). The AUC for admission scores of Ranson, APAchE-Ⅱ, and APAchE-O systems was 0. 549, 0. 904 and 0. 904, respectively. The AUC for 48-hour scores of Ranson, APAchE-Ⅱ and APAchE-O systems was 0.808, 0.955 and 0.951, respectively. CONCLUSIONS: The APAchE-Ⅱ scoring system is more accurate than the Ranson scoring system of the prediction of severity in acute pancreatitis. Addition of obesity score does not significantly improve the predictive accuracy of the APAchE-Ⅱ system in our local population with a low prevalence of obesity.

We evaluated the efficacy and safety of as-needed tadalafil in a diverse clinical population （with varying patient demographics, disease severity, and comorbid medical conditions） of Asian men with erectile dysfunction （ED）. An integrated analysis of five double-blind, placebo-controlled trials （N = 1 046） was performed. Patients were randomly assigned to receive 10 mg tadalafil （N = 185）, 20 mg tadalafil （N = 510）, or placebo （N = 351）. Efficacy assessments included the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP） diary and Global Assessment Question （GAQ）. Patients receiving 10 mg or 20 mg tadalafil showed significant improvement from baseline-to-end point on the Erectile Function domain of the International Index of Erectile Function （IIEF-EF） domain score in all clinical sub-populations analyzed, compared with patients receiving placebo （P 〈 0.001）. The 10-mg and 20-mg tadalafil groups showed a mean success rate of 64.1% and 70.5% for sexual intercourse attempts （SEP3, successful intercourse）, respectively, compared with 33.4% in the placebo group （P 〈 0.001）, and 85.5% and 85.4% reported improved erections at end point GAQ, respectively, versus 43.5% in the placebo group （P 〈 0.001）. Tadalafil was well tolerated across all groups studied. Headache and back pain were the most frequently reported adverse events. Overall, tadalafil was effective and well tolerated across a diverse clinical spectrum of Asian men with ED.

Background Colorectal surgery was regarded as one of the high risk surgery for post-operative deep vein thrombosis （DVT） and pulmonary embolism. This study aimed at investigating the incidence of venous thromboembolism （VTE） after colorectal surgery for *** Data were collected from the prospective database of colorectal malignancy from 2000 to 2008. A total of 1421 colorectal （open and laparoscopic） operations were performed for the colorectal malignancy without DVT *** Only seven patients （0.5%） developed symptomatic DVT and one of them had complication of pulmonary embolism without mortality. Open operation for colorectal malignancy was identified as possible risk factor of DVT,however, risk factors like operative time, low anterior resection, sex, age etc. were not *** Risk of venous thromboembolism after colorectal operation is low in chinese of our locality and it might be safe to perform colorectal operation for malignancy without DVT prophylaxis.

Background Single incision laparoscopic colectomy has been performed in recent years, and has been shown to be feasible and safe. This study was to assess the feasibility of single incision laparoscopic right hemicolectomy and to compare the differences in different approaches. Methods This retrospective study included eighteen patients with carcinoma of caecum and ascending colon, undergoing single incision laparoscopic right hemicolectomy. This study also compared single incision laparoscopic right hemicolectomy using different approaches：（1） single incision multiport, （2） single access port and （3） glove port. Results There was no statistical difference in surgical outcomes. Concerning the surgeon＇s satisfaction toward three methods, overcrowding and durability were similar but the single incision multiport was associated with the highest gas-leak and the ＂glove＂ port was associated with poor durability. However, the method of single incision multiport has the lowest average cost of the special trocar or port in each operation. The operative time and blood loss of the operations in this study were comparable to previous publications. Conclusion There was no significant difference between different approaches of single incision laparoscopic right hemicolectomy for colonic cancer in right side colon.

Aim： To assess the efficacy and safety of tadalafil in comparison to a placebo, when taken on demand for 12 weeks by East/Southeast Asian men with erectile dysfunction （ED）, Methods： This multicenter, randomized, double-blind, parallel group, placebo-controlled study was conducted at 17 centers across East and Southeast Asia between August 2002 and February 2003. Men more than 18 years of age with mild to severe ED of various etiologies were randomized to receive a placebo or 20 mg of tadalafil taken as needed （maximum once daily）. Efficacy assessments included the International Index of Erectile Function, the Sexual Encounter Profile diary and Global Assessment Questions. Results： Tadalafil significantly improved erectile function as compared to the placebo （P 〈 0.001）. At the endpoint, the patients receiving 20 mg of tadalafil reported a greater mean per patient percentage of successful intercourse attempts （Sexual Encounter Profile question 3： 70.9% compared to 33.5% in the placebo） and a greater proportion of improved erections （Global Assessment Question： 86.2% compared to 30.1%）. Most （≥3%） treatment emergent adverse events were mild or moderate. The most common treatment emergent adverse events were headache, back pain, dizziness and dyspepsia. Conclusion： Tadalafil was an effective and well-tolerated treatment for ED in East and Southeast Asian men.

Outcomes of emergency and interval hepatectomy for ruptured resectable hepatocellular carcinoma: a single tertiary referral centre experience

Aim: The short and long term outcomes of patients who underwent emergency and interval hepatectomy for ruptured and resectable hepatocellular carcinoma (HCC) were analysed. Methods: The data of patients with ruptured HCC presenting between April 2004 and October 2015 were analysed. Emergency hepatectomy was defined as hepatectomy within 48 h of the clinico-radiological diagnosis of HCC rupture. Results: Thirty patients underwent hepatectomy for ruptured HCC. Nine (30%) patients underwent emergency hepatectomy. The median age was 56 and 54 years (P = 0.13) with a similar gender distribution. The mean HCC size (10.5 vs. 8.3 cm, P = 0.17), total blood loss (3,000 vs. 850 mL, P = 0.002) and total units of red blood cell transfusion (1.9 vs. 0.5 units, P = 0.27) were greater in the emergency hepatectomy group. The complication rate was 44% and 38% (P = 0.53), with median length of hospital stay of 10 and 12 days (P = 0.07) in the emergency and interval hepatectomy groups, respectively, and no 30-day mortality in both groups. The median overall survival was 29 and 15.7 months (P = 0.25), with survival rates of 78%, 45%, 0% and 85%, 43% and 5% at 1, 3 and 5 years in the emergency and interval hepatectomy groups, respectively. Conclusion: Hepatectomy should be considered for ruptured HCC provided the patient could tolerate curative resection.

This paper examined the prioritized climate-smart agricultural practices by smallholder farmers,the motivations of adopting climate-smart agricultural practices,the enablers to the successful adoption of climate-smart agricultural practices,and the barriers to the successful adoption of climate-smart agricultural practices in the transitional and savannah agroecological zones of ***,we employed ethnographic research using participatory approaches,including two stakeholder workshops and household surveys with 1061 households in the transitional and savannah agroecological zones of *** weighted average index(wai)and problem confrontation index(PCI)were used to rank smallholder farmers’perceived enablers to the adoption of climate-smart agricultural practices and the barriers affecting climate-smart agricultural practices,*** suggest that the majority of the respondents used a suite of climate-smart agricultural practices,including the timely harvesting of produce and storage,emergency seed banking,appropriate and timely weed and pest control,and early planting as practices to build climate *** majority of smallholder farmers primarily employed climate-smart agricultural practices to improve household food security(96.2%),reduce pests and diseases(95.6%),and obtain higher yields and greater farm income(93.2%).Findings also show that secured land tenure system arrangement,understanding the effects of climate ch.nge,and access to sustainable agricultural technologies were ranked the first,second,and third most important enablers to the adoption of climate-smart agricultural practices with the wai values of 2.86,2.75,and 2.70,*** barriers to the successful adoption of climate-smart agricultural practices included incidences of pests and diseases(PCI=2530),inadequate access to agricultural credit(PCI=2502),high cost of improved crop varieties(PCI=2334),and limited government support with farm inputs(PCI=2296).Sm

The mammalian epididymis not only plays a fun dame ntal role in the maturati on of spermatozoa,but also provides protecti on agai nst various *** foremost among these is the threat posed by oxidative stress,which arises from an imbalance in reactive oxygen species and can elicit damage to cellular lipids,proteins,and nucleic *** mice,the risk of oxidative damage to spermatozoa is mitigated through the expression and secretion of glutathione peroxidase 5(GPX5)as a major luminal scavenger in the proximal caput epididymidal ***,the loss of GPX5^-/-mediated protection leads to impaired DNA integrity in the spermatozoa of aged Gpx57 *** explore the underlying mechanism,we have conducted transcriptomic analysis of caput epididymidal epithelial cells from aged(13 months old)Gpx5^-/-m *** analysis revealed the dysregulation of several thousand epididymal mRNA transcripts,in eluding the downregulation of a subgroup of piRNA pathway gen es,in aged Gpx5^-/-*** agreeme nt with these fin dings,we also observed the loss of piRNAs,which potentially bind to the P-element-induced wimpy testis(PlWI)-like proteins PIWIL1 and *** absence of these piRNAs was correlated with the elevated mRNA levels of their putative gene targets in the caput epididymidis of Gpx5^-/-***,the oxidative stress response genes tend to have more targeting piRNAs,and many of them were among the top increased genes upon the loss of GPX5^-/-.Taken together,our findings suggest the existence of a previously uncharacterized somatic piRNA pathway in the mammalian epididymis and its possible invoIvement in the aging and oxidative stress-mediated responses.
