Right Ventricular Dysfunction in Patients with Acute STEMI, Radionuclide-Based Assessment

Background: An acute myocardial infarction, particularly one that is large and transmural, can produce alterations in the topography of both the infarcted and noninfarcted regions of the ventricle. Cardiac remodeling after acute myocardial infarction (AMI) is characterized by molecular and cellular mechanisms involving both left and right ventricles. Objective: To study the right ventricular affection in patients with acute myocardial infarction of the left ventricle and treated with primary PCI. Methods: The study was done in the critical care department, Cairo University (in which 2,500 primary percutaneous coronary interventions (PCIs) are done annually). Forty patients with acute ST-elevation myocardial infarction (STEMI) were subjected to primary (PCI), we excluded all patients with RV infarction and patients with pulmonary hypertension. RV function was assessed by first pass radionuclide angiocardiography within 48 hours of admission. Results: Regarding the incidence of right ventricular dysfunction in patients with acute left ventricular STEMI who treated with 1ry PCI, there were 21 patients (52.5%) with RV EF% patients (47.5%) with RV time to peak filling rate (TPFR) > 180 msec. There was a significant relationship between right ventricular dysfunction and duration of ICU stay, impairment of LV systolic function, failure of complete resolution of ST segment elevation, failure of early peaking of cardiac enzymes, occurrence of in-hospital complications and one year mortality. Conclusion: The incidence of right ventricular dysfunction in patients with acute left ventricular STEMI is higher than expected and has a negative impact on their outcome.

Recently,much interest has been given tomulti-granulation rough sets (MGRS), and various types ofMGRSmodelshave been developed from different viewpoints. In this paper, we introduce two techniques for the classificationof MGRS. Firstly, we generate multi-topologies from multi-relations defined in the universe. Hence, a novelapproximation space is established by leveraging the underlying topological structure. The road construction industry aims to contribute to the protection of already compromised *** mix asphalt(CMA)is a measure initiated by the road industry to protect the environment and preserve *** having additional benefits,CMA has attracted little attention due to its inferior ***'s performance is enhanced using a sustainable binder bio-modifier,natural cup lump rubber(CLR)is one of *** study evaluated the tensile properties,rutting,moisture susceptibility,and adhesion properties of CLR-modified CMA(CMA-CR).The tensile property was enhanced by 26%due to CLR ***-CR had excellent rutting resistance of less than 2 mm rut depth at 10,000 load cycles,showing 70%improvement compared with conventional *** susceptibility evaluation indicated that CMA-CR had tensile strength ratio(TSR)value of 104%,satisfying the minimum 80%requirement of AASHTO *** also retained more than 96%bitumen *** moisture damage resistance was improved by 12%and 10%in terms of TSR and stripping,*** durability results revealed that the CMA-CR mixture prevented higher mass loss,representing 14%improvement compared with conventional CMA.

Minimally Invasive Total Arterial Coronary Artery Bypass Grafting in Left Main Stem Disease

Objective: The aim was to show that Minimally Invasive total arterial revascularization for left main stem coronary artery disease, via a left anterior Mini thoracotomy using bilateral internal thoracic arteries is feasible, reproducible and safe. Further, there has been no exclusive data or experience with minimally invasive coronary artery bypass grafting in left main stem disease. Methods: From April 2019 to March 2024, 41 patients with left main stem stenosis, left main equivalent disease and unprotected left main with triple vessel disease underwent off pump minimally invasive multivessel coronary artery bypass grafting using either in situ pedicled Bilateral Internal Thoracic arteries or Left and Right Internal Thoracic artery Y composite conduits at three centers. Bilateral Internal Thoracic arteries were harvested under direct vision. All patients had an Intra-Aortic Balloon Pump inserted via the femoral artery prior to induction of anesthesia, to prevent any hemodynamic instability, arrhythmias, and was removed following completion of the procedure in the operating room without any complications. Efficacy and outcomes were evaluated by i) Primary (MACCE)-Major Adverse Cardiac and Cardiovascular events and ii) Secondary outcome measures including total length of stay, return to full physical activity and quality of life. Mean follow-up was 1.4 years (Maximum was 2.5 years). Results: 41 patients with left main stem coronary artery stenosis, underwent total arterial revascularization using bilateral internal thoracic arteries. Left main stem stenosis was present in 29 patients, Unprotected left main stem stenosis with triple vessel disease in 7 and left main equivalence in 5 patients. In this cohort, 29 patients with only left main stem stenosis had 2 grafts each, 7 patients with left main and triple vessel disease had 3 grafts and 5 patients with left main equivalent disease had 2 grafts respectively. Machine learning and data-driven prediction of pore pressure from geophysical logs:A case study for the Mangahewa gas field,New Zealand

Pore pressure is an essential parameter for establishing reservoir conditions,geological interpretation and drilling *** pressure prediction depends on information from various geophysical logs,seismic,and direct down-hole pressure ***,a level of uncertainty accompanies the prediction of pore pressure because insufficient information is usually recorded in many *** machine learning(ML)algorithms can decrease the level of uncertainty of pore pressure prediction uncertainty in cases where available information is *** this research,several ML techniques are applied to predict pore pressure through the over-pressured Eocene reservoir section penetrated by four wells in the Mangahewa gas field,New *** predictions substantially outperform,in terms of prediction performance,those generated using a multiple linear regression(MLR)*** geophysical logs used as input variables are sonic,temperature and density logs,and some direct pore pressure measurements were available at the reservoir level to calibrate the predictions.A total of 25,935 data records involving six well-log input variables were evaluated across the four *** ML methods achieved credible levels of pore pressure prediction *** most accurate models for predicting pore pressure in individual wells on a supervised basis are decision tree(DT),adaboost(ADA),random forest(RF)and transparent open box(TOB).The DT achieved root mean square error(RMSE)ranging from 0.25 psi to 14.71 psi for the four *** trained models were less accurate when deployed on a semi-supervised basis to predict pore pressure in the other *** two wells(Mangahewa-03 and Mangahewa-06),semi-supervised prediction achieved acceptable prediction performance of RMSE of 130—140 psi;while for the other wells,semi-supervised prediction performance was reduced to RMSE>300 *** results suggest that these models can be used to predict pore pressure in nearby locat

A New Hiatus within the Lutetian of the El Basatin Section,Gebel Mokattam, Egypt:Field and Sedimentological Observations, with Special Emphasis on Nummulites

The Eocene succession of the El Basatin Section in Gebel Mokattam, east of Cairo, consists, from base to top, of two main units; the Mokattam and Maadi Formations. The Mokattam Formation consists of two Members, the Building Stone Member and the Giushi Member. The Upper Building Stone Member yielded six species of Nummulites belonging to the Upper Lutetian. These species are: Nummulites farisi Hussein et al., 2004; Nummulites cf. praegizehensis Boukhary and Hussein-Kamel, 1993; Nummulites cf. gizehensis(Forsk?l, 1775); Nummulites discorbinus(Schlotheim 1820) and Arxina schwageri(Silvestri, 1928) emended by Boukhary et al. 2012 and Nummulites crassichordatus Boukhary et al., 2010. The Giushi Member yielded three species that indicate a Bartonian age. These species, which continued from their first appearance in the Upper Building Stone Members, are N. discorbinus, A. schwageri and N. crassichordatus. The Maadi Formation, which has been previously considered to be of Bartonian–Priabonian age, is devoid of fossils in the study section. The two members of the Mokattam Formation represent a carbonate platform facies. The deposition of the Upper Building Stone Member was disturbed during the Lutetian by slumping and a convolute-bedding interval, indicating a short hiatus. The subsequent regression resulted in a very shallow marine to near-shore facies in the above Maadi Formation.

Formation of AlN from Secondary Al Resources

Synthesis of AlN by NH4Cl-assisted direct nitridation of secondary Al resources was studied. Aluminum alloy scrap contained Mg and Zn element was used. Milled Al scrap （-1 mm） was mixed with NH4Cl and heated at 1000℃ for 1 h in presence of 1 L/min N2 flow gas. The nitrided product was evaluated by X-ray diffraction semi-quantitative analysis and scanning electron microscopy （SEM）. Aluminium nitride of 90.5% concentration was obtained with a hexagonal crystalline form. AlN is mostly formed in nano-whisker morphology （50～150 nm） which is homogenously distributed.
